Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

After an ASL Airlines Belgium Boeing 747-400 (OEIFD/TAY011) from New York/John F. Kennedy, NY (KJFK) to Halifax/Stanfield, NS (CYHZ) landed on runway, the field foreman advised there was debris on the runway and that it was closed until it could be swept. An Exploits Valley Air Service Beech 1900D (C-GUPW/EV778) from Fredericton, NB (CYFC) to Halifax/Stanfield, NS (CYHZ) on approach to Runway 05 had to divert to land on Runway 32 instead. A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FENO/WEN3484) from Halifax/Stanfield, NS (CYHZ) to Sydney, NS (CYQY) delayed taxi for a few minutes as well. No other operational impact.

The tower initiated an overshoot for an Exploits Valley Air Services Beech 1900D (C-GLHO/EV768) on a flight from Saint John, NB (CYSJ) to Halifax/Stanfield Intl, NS (CYHZ) at approximately 1.5NM final Runway 05 due to a Jazz de Havilland DHC-8-402 (C-GYJZ/JZA8809) on a flight from Sydney/J.A. Douglas McCurdy, NS (CYQY) to Halifax/Stanfield Intl, NS (CYHZ) still on short final Runway 32. EV768 subsequently landed Runway 32. No operational impact.

At 1736Z a United Kingdom Royal Air Force Transport 9402 Airbus A330-200 (ZZ337/RRR9402) from Gander, NL (CYQX) to Halifax/Stanfield, NS (CYHZ) requested course change to make "gross weight adjustment". Once clear of other traffic, aircraft started dumping fuel at 1742Z on south-west track ending at 1814Z. No emergency, and no impact on other traffic.

A Jazz de Havilland DHC-8-402 (C-GGFJ/JZA7997) from Sydney, NS (CYQY) to Montreal/Trudeau, QC (CYUL) reported a bird strike (with a family of ducks on the runway) while landing on Runway 24R. A Jazz de Havilland DHC-8-301 (C-GMON/JZA8791) from Saint John, NB (CYSJ) to Montreal/Trudeau, QC (CYUL) decided to conduct a go-around. Several arrivals were redirected to Runway 24L. Several departures from Runway 24L were therefore delayed. A duck was found during the inspection of the runway. JZA8791 landed on Runway 24L at 1107Z.

Pearson, ON (CYYZ) reports sighting a drone on 4NM final approach to Runway 24R at approximately 2000ft. The following 2 aircraft also reported sighting the drone in the same location. The following arrivals were moved onto runway 23. At 1755Z, 2 aircraft elected to do the approach to Runway 24R and no drone activity reported. At 1805Z, Runway 24R operations resumed.

A privately registered Robinson R44 II on a flight from Fredericton Intl, NB (CYFC) to Sydney/J.A. Douglas McCurdy, NS (CYQY) landed approximately 4NM W CYQY due to low fuel, unsure if they could make it to CYQY. The aircraft had extra fuel onboard. Due to unclear communications, CYQY Aircraft Rescue and Fire Fighting (ARFF) was activated. While vehicle (RED1) was conducting a search toward the threshold of Runway 06, the helicopter departed approximately 4NM W of CYQY inbound for fuel, and landed safely at 1444Z.

The departure of a WestJet Encore de Havilland DHC-8-402 (WEN3483) on a flight from Sydney/J.A. Douglas McCurdy, NS (CYQY) to Halifax/Stanfield Intl, NS (CYHZ) was delayed approximately 5 minutes. SKV7722 landed at CYHZ at 1956Z.

TSB Report #A19A0075: C-FRJA, a Bombardier CL-600-2D24 (CRJ-900LR) aircraft operated by Jazz Aviation LP, was conducting flight JZA 8207 from Sydney/J.A. Douglas McCurdy (CYQY), NS to Toronto/Lester B. Pearson Intl (CYYZ), ON. Prior to boarding passengers, one of the flight attendants felt nauseous and vomited. Emergency medical services were called and the crewmember was transported to hospital. The flight reduced the amount of passengers to operate with one flight attendant and departed for destination without further incident.

CYYG Radio attempted to contact the aircraft prior to it entering the MF with no response, aircraft working with Moncton area control centre (CZQM). Initial contact with TSK902 was made at 3NM N CYYG at A029 transiting through the control zone.

Halifax joint rescue coordination centre (JRCC) advised and they advised there was a beacon in the area and to disregard ELT reports 100NM East of Sydney/J.A. Douglas McCurdy, NS (CYQY). No impact on operations and safety was not jeopardized.

TSB #A19A0068: C-GGNY, a Dash 8 Q400 aircraft operated by Jazz Aviation LP, was conducting flight JZA8862 from Halifax Stanfield International Airport, NS (CYHZ) to Gander International Airport, NL (CYQX). During cruise the aircraft encountered turbulence that caused one of cabin members to sustain a minor injury and was unable to continue their duties. The other cabin crew members assumed their duties for the remainder of the flight.

Strong emergency locator transmitter (ELT) signal heard on Sydney, NS (CYQY) frequency 243.0 by the Charlottetown, PE (YYG) flight service station (FSS) at 2048Z for less than 5 seconds. The pilot of a Breton Air Inc. Bell 206L (C-FJTO) from Sydney, NS (CYQY) to Halifax/Stanfield, NS (CYHZ) reported to the YYG FSS that their aircraft was responsible for the ELT signal. The Moncton Control Center (CZQM) was advised. The CZQM shift manager (SM) was also advised.

Update TSB #A19A0041: C-FLJZ, a Bombardier CL-600-2D24 CRJ900 aircraft operated by Jazz Aviation LP, was conducting flight JZA8206 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Sydney/J.A. Douglas McCurdy (CYQY), NS with 4 crew members and 78 passengers on board. Upon flaps selection during the approach into CYQY, the flight crew received a flap fault caution message. The flaps stopped at 8 degrees and the slats at 20 degrees. No emergency was declared, however ARFF was on standby for the landing. The aircraft landed without further incident.

A Jazz Bombardier CL-600-2D24 (series 900) (C-FLJZ/JZA8206) from Toronto/Lester B. Pearson, ON (CYYZ) to J.A. Douglas McCurdy Sydney, NS (CYQY) requested Aircraft Rescue and Fire Fighting (ARFF) on standby while on approach to Runway 24. No emergency declared. The aircraft landed safely at 1509Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The Operator has reported that there were a total of four (4) Flap Events for C-FLJZ in the month of June 2019. Several attempts to troubleshoot and rectify the defect were unsuccessful. After the 4th and final event, Company Maintenance initiated an action plan which included: Wiring checks, visual inspections, rigging checks, part replacement, Operational and Functional checks, and a test flight. The aircraft was returned to service with no recurrence. Service Difficulty Report No. 20190618014 was submitted.

An RCMP Air Services Branch Pilatus PC-12/47E (C-FGMQ) from Quebec/Jean Lesage, QC (CYQB) to Yarmouth, NS (CYQI) was observed on radar not following the area navigation (RNAV) course for the RNAV33 approach as cleared at CYQI. Aircraft landed and closed its flight plan at 1428Z. No impact on operations.

Incident at J.A. DOUGLAS MCCURDY SYDNEY AIRPORT (ATM - inaccurate aeronautical information, ATM - ILS irregularity)

At Sydney, NS (CYQY), the Web Query (IQY), localizer (LOC) and glide path (GP) failed, and were unable to be reset. The Technical Operations Coordinator (TOC) was advised. The TOC will issue a NOTAM.
