An Exploits Valley Air Services Cessna 172N (C-GVKT), in the circuit for Runway 31 at Gander, NL (CYQX), was instructed to proceed straight out on departure after a touch-and-go. C-GVKT was observed starting its left crosswind turn towards traffic that was inbound for the left downwind of Runway 31. Air traffic control (ATC) intervened and instructed the aircraft to make a right turn and resume the runway heading to de-conflict with an Exploits Valley Air Services Cessna 152 (C-FPXA) from Gander, NL (CYQX) to Gander, NL (CYQX).

Shortly after departure, a Phoenix Air Gulfstream G-1159A (N163PA/PHA162) on a flight from St. Johns, NL (CYYT) indicated that the aircrafts flaps would not fully retract, the same issue that had been reported on the previous flight. PHA162 requested to divert to Gander, NL (CYQX) and did not declare an emergency, nor requested any assistance. There was no impact to operations and the flight landed at 1514Z.

Shortly after departure, a Phoenix Air Gulfstream G-1159A (N163PA/PHA162) on a flight from Gander, NL (CYQX) indicated that the aircraft flaps would not fully retract. The flight requested to divert to St. Johns, NL (CYYT) and did not declare an emergency, nor requested any assistance. There was no impact to operations and the flight landed at 0052Z.

Incident at GANDER INTL NL (CYQX)

An Airmed International Raytheon Hawker 800XP (N823AM) from Gander, NL (CYQX) to Charlotte/Douglas, NC (KCLT) was issued a takeoff clearance for Runway 31. The aircraft began its roll, but aborted the takeoff due to an indication. The aircraft taxied back to the main apron and requested taxi instructions to depart again, stating that the issue was resolved. The aircraft was instructed to backtrack and line up, and subsequently was issued another takeoff clearance. Moments after the takeoff clearance was given, the aircraft advised that it needed to taxi back to the main apron due to another indication. No impact on operations.

Incident at GANDER INTL NL (CYQX)

A Newfoundland and Labrador Air Ambulance Service Beech B300C (C-GNLO) from Deer Lake, NL (CYDF) to Gander, NL (CYQX) executed a missed approach due to a landing gear issue. The aircraft advised that the issue was resolved and requested a diversion to St. Johns, NL (CYYT). Subsequently, the aircraft declared an emergency and landed without incident in CYYT. No impact on operations.
UPDATE: TSB Report #A21A0003: C-GNLO, a Government of Newfoundland and Labrador Beech B300C, was conducting a flight from Deer Lake, NL (CYDF) to Gander, NL (CYQX). On approach to CYQX, the landing gear was selected "down" but the crew did not receive a "left-hand main landing gear down" green indication. The crew cancelled their IFR clearance, carried out the checklist items, and consulted with their maintenance department. It was decided to divert to St. Johns, NL (CYYT), where an emergency was declared in order to have emergency services standing by as a precaution. The aircraft landed at CYYT without further incident. A faulty "left-hand main landing gear down" indication lamp was replaced, and the aircraft was returned to service following operational checks.

Beech 1900D (C-FFCU/VSL613) from St-Augustin, QC (CYIF) to Lourdes-de-Blanc-Sablon, QC (CYBX) made their initial contact when in the left downwind circuit for Runway 23, while a Provincial Airlines de Havilland DHC-8-311 (C-GHRJ/PVL413) from Lourdes-de-Blanc-Sablon, QC (CYBX) to Gander Intl, NL (CYQX) was taxied for a departure from Runway 23. Consequently, VSL613 had to extend their left downwind to allow PVL413 to depart.

At 0521Z, an American, corporately registered Bombardier BD-700-1A10 (N151QS) from Wichita, KA (KICT) to Madrid–Barajas, Spain (LEMD) advised of an issue with a window in the cockpit. The aircraft requested to turn around and land at Gander, NL (CYQX). Traffic was considered and re-routed to accommodate the turn-back. Clearance for the turn-back was issued at F430. N151QS advised it was not declaring an emergency. The aircraft landed without incident at CYQX at 0742Z.

The pilot of an American, corporately registered Gulfstream G280 (N776AG) from Shannon, Ireland (EINN) to Burlington, VT (KBTV) declared an emergency due to an engine failure, and requested clearance to Gander, NL (CYQX). N776AG was cleared as requested. The aircraft landed safely at 1826Z. No impact on operations.
UPDATE: TSB Report #A20A0040: N776AG, a privately operated Gulfstream G280 aircraft was conducting a flight from Shannon Intl (EINN), Ireland, to Burlington Intl (KBTV), Vermont. At about 45°49 North and 052°52 West, the number 02 engine (Honeywell HTF7250G) automatically shut down without warning. The crew declared an emergency, reviewed weather, and coordinated with ATC for a diversion to Gander Intl (CYQX), NL. The crew completed the engine fail checklists and decided not to attempt a restart of the engine. The APU was started below FL350 to facilitate electrical, environmental, and bleed air sources, and the flight landed at CYQX without further incident. Subsequent troubleshooting determined that false number 02 engine N2 overspeed logic caused the automatic shutdown of the engine. Gulfstream and Honeywell are collaborating on an investigation into the engine shutdown. The engine control units are with Honeywell for further evaluation.

Incident at GANDER INTL NL (CYQX)

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: The pilot of a Jazz de Havilland DHC-8-402 (C-GGBF/JZA8863) from Gander, NL (CYQX) to Halifax/Stanfield, NS (CYHZ) departed from Runway 03 at 2120Z and, shortly after take-off, contacted the tower to ask if any rubber had been left on the runway after their departure, because they had experienced some shaking with the nose landing gear during take-off. The field foreman for the airport inspected the runway and found no evidence of any debris. The tower contacted the aircraft and relayed the ground crews report. No impact to operations.

At 2000Z, an Exploits Valley Air Services Ltd. Beech 1900D (C-GLHO/EV123T) from Gander, NL (CYQX) to Gander, NL (CYQX) declared an emergency due to an issue with the aircrafts equipment. The Emergency Response Services (ERS) unit was notified. The aircraft landed safely on Runway 31 at 2013Z. No impact on operations.
Update: TSB Report #A20A0032: C-GLHO, a Beechcraft 1900D aircraft operated by Exploits Valley Air Service (EVAS), was conducting a training flight at Gander Intl (CYQX), NL, with 3 people onboard. While conducting a simulated single engine approach, and manipulating the power and propeller levers, the number 1 engine went into an uncommanded auto-feather. The crew could not unfeather the engine, and decided to shut down the engine as a precautionary measure. The crew declared an emergency and requested an Aircraft Rescue and Fire Fighting (ARFF) unit to be on standby. The aircraft landed without further incident. Maintenance determined that with the autofeather system armed and either power lever brought back to idle, the low power setting engine would feather intermittently. The autofeather arming switch was replaced and the aircraft was returned to service.

A Jazz de Havilland DHC-8-402 (C-GGNW/JZA8862) from Halifax/Stanfield, NS (CYHZ) to Gander, NL (CYQX) requested to return to CYHZ due to a flight control problem. No emergency was declared but the pilot requested trucks to be on standby and delaying vectors. No operational impact. JZA8862 landed safely at 1616Z.
Update TSB Report #A20A0029: C-GGNW, a de Havilland DHC-8-402 aircraft operated by Jazz Aviation LP, was conducting flight JZA8862 from Halifax/Stanfield Intl (CYHZ), NS, to Gander Intl (CYQX), NL. Shortly after the departure from CYHZ, the No. 2 roll spoiler switch-light SPLR 2 PUSH OFF illuminated in-flight. The crew pushed the switch-light to select the system to off, followed QRH checklist instructions, and then discussed conditions with company maintenance control personnel. The crew then contacted ATC, requested return to CYHZ, and requested that airport emergency services stand by for the landing. The aircraft landed and taxied to the gate without further incident. Company maintenance carried out testing and inspection of the No. 2 spoiler system in accordance with fault isolation manual instructions and no faults were observed.
