Incident at YORKTON MUNI SK (CYQV)

The pilot of a Good Spirit Air Service Ayres S2R-T34 (C-FSPZ) was conducting circuits at Yorkton, SK (CYQV) when they reported a Cessna overhead the field. The flight service station (FSS) had no contact with the unknown Cessna. The aircraft joined the circuit and landed. The pilot of a Crowsnest Properties and Developments Ltd. Cessna 172M (C-GGTB) on a flight to Yorkton, SK (CYQV) contacted the FSS to advise that their radio had failed.

A Good Spirit Air Service Piper PA-36-285 (C-FPMD) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) landed on the road, approximately 2NM Southeast (SE) from CYQV.
UPDATE from Aviation Operations: The aircraft (C-FPMD) landed on an unknown road due to running out of fuel. The pilot refuelled the aircraft and returned to CYQV. No emergency declared. No injuries or damage to the aircraft.

Incident at 35NM YORKTON SK VOR/DME (YQV)

Several overflying aircraft reported an emergency locator transmitter (ELT) signal of varying strength and intensity.
Update AOR 265295-V2: The emergency locator transmitter (ELT) signal was transmitted by a Good Spirit Air Service Air Tractor AT-401 (C-FSPD) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV). The Joint Rescue Coordination Centre (JRCC) Trenton conducted an aerial search and found C-FSPD undamaged with a malfunctioning ELT.

Incident at 2NM WINNIPEG / ST. ANDREWS MB (CYAV)

Shortly after departure, a Harvs Air Service Ltd. Piper PA-44-180 (C-GKPY) from Winnipeg/St. Andrews, MB (CYAV) to Yorkton, SK (CYQV) was observed making a turn on the CYAV standard instrument departure (SID) 5 without authorization. The controller turned C-GKPY back on course. No impact to operations.

Incident at YORKTON MUNI SK (CYQV) (Aerodrome, runway or taxiway shutdown)

Upon landing on Runway 04, the engine of a Good Spirit Air Service Kubassek DK Cooper Hawk (C-GDNK) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) stopped. The aircraft was disabled on Runway 04 for two minutes until the engine re-started. The aircraft then exited the runway safely at 1608Z. A Leading Edge Aviation Ltd. Cessna 172M (C-GJKR) changed from Runway 04 to Runway 31 for landing. No impact on operations.

Incident at YORKTON MUNI SK (CYQV) (ATM - NAVAIDS/radar)

The Yorkton (YQV) very high frequency omnidirectional range (VOR) 115.8 was already NOTAMd as unmonitored. A NAV CANADA Bombardier CL-600-2B19 (C-GFIO/NVC101) from Saskatoon/John G. Diefenbaker, SK (CYXE) to Prince Albert (Glass Field), SK (CYPA) arrived at Yorkton at 1750Z for a flight check of the area navigation (RNAV) and the Yorkton VOR approaches. NVC101 could not detect any signal from the VOR. NVC101 completed the check of the RNAV approaches at 1825Z and still there was no signal from the VOR. Winnipeg Technical Operations Coordinator (TOC) advised. Winnipeg TOC issued a NOTAM for the Yorkton VOR, which noted that the VOR was unserviceable until May 29th, 2020 at 21359Z.

A Leading Edge Aviation Ltd. Cessna 172M (C-GJKR) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) advised the Regina (YQR) flight service station (FSS) that the runway edge lights for Runway 04/22 at CYQV were unserviceable (U/S). All other airport lighting appeared to be serviceable. C-GJKR arrived on Runway 13 at 0315Z without incident. Airport authority advised. No adverse effect upon flight safety or impact on operations.

Airport staff advised that Sask Power needed to cut power at the airport from 1645Z to 1700Z which would result in the loss of Regina remote communications outlet (RCO) mandatory frequency (MF) 119.65. Shutdown was coordinated with 1 training aircraft in the area. Power and service resumed by 1700Z.

While a vehicle was authorized to be on Yorkton, SK (CYQV) Runway 04/22 with permission to cross Runway 13/31 until further advised, a Leading Edge Aviation Ltd. Cessna 172M (C-GJKR) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) departed Runway 31 without first contacting the flight service station (FSS) for a departure advisory. C-GJKR had taxied to the apron after doing circuits in order to drop someone off before resuming circuits.

It was a strong signal with an abrupt start and stop from 0357Z until 0411Z. Signal stopped at Yorkton, SK (CYQV) 101/109. The Joint Rescue Coordination Centre (JRCC) Trenton was notified at 0420Z.

SNAP2 had to deviate west of final to avoid coming too close to the C-GECQ.

An American corporately registered Cessna 208B (N516NC) on a flight from Regina Intl, SK (CYQR) to Yorkton, SK (CYQV) was given taxi instructions for Runway 08 via Taxiway K including a hold short of both runways. The controller observed N516NC cross the hold line and enter the intersection of both runways at K. The controller instructed N516NC to switch to tower frequency and to backtrack to line up Runway 08. N516NC departed without further incident and no impact on operations.

Accident at NORTH BATTLEFORD (CAMERON McINTOSH) SK (CYQW)

The Winnipeg (WG) Flight Information Centre (FIC) relayed a pilot report that an unknown high wing Cessna had crashed in the vicinity of the threshold of Runway 30 at North Battleford, SK (CYQW). Emergency locator transmitter (ELT) reports were subsequently received from overflying aircraft. The Aviation Operations Centre (AVOPS), the Transportation Safety Board of Canada (TSB), and the Trenton Joint Rescue Coordination Centre (JRCC) were advised. No operational impact.
Update TSB #A19C0103: C-FQLO, a privately registered Cessna 150H, was conducting a VFR flight from Yorkton Muni (CYQV), SK, to North Battleford (CYQW), SK. On final approach for Runway 30 at CYQW, the engine (Teledyne Continental O-200-A) stopped. The pilot conducted a forced landing in a canola field approximately 1/4 mile prior to the threshold of Runway 30. On landing, the aircraft sustained substantial damage to the propeller and wings and the ELT activated. The pilot was uninjured. It was reported that a subsequent inspection of the fuel tanks revealed there was no fuel remaining.

At 0320Z, a Carson Shymanski Farms Ltd. Weatherly 620B (C-FNDK) from Choiceland, SK, to Yorkton, SK (CYQV) advised inbound to CYQV and planning to land on Runway 31. Pilot was advised that all runways at Yorkton were closed by NOTAM for construction. Pilot requested to land alongside Runway 04/22 in the grass. The flight service station (FSS) informed the pilot that they would be doing so at their own risk and requested that they wait until the construction crew could be notified of the pilots intentions. Pilot landed alongside runway without reporting final at 0330Z and before crew was notified.

A Mitchinson Flying Service Limited Cessna 172S (C-FARC) was on a VFR flight plan from Saskatoon/Diefenbaker, SK (CYXE) to from Roblin, MB (CKB7) to Okotoks, AB (CFX2). While westbound from CKB7, C-FARC diverted to Yorkton, SK (CYQV) for fuel. C-FARC made radio calls but could not hear the flight service station (FSS) reply or any transmissions from other aircraft while inbound to CYQV. C-FARC made broadcast of intentions for a low pass on Runway 04 then a full stop landing on Runway 04. C-FARC was observed by a privately registered, amateur built Cub J3C-65 from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) doing a low pass on Runway 31 at 1746Z, then landing on Runway 31 at 1751Z. Operational impact: the Cub J3C-65 aircraft had a delayed departure from Runway 04, southwest bound, until intentions of C-FARC could be visually ascertained.

An Air Canada Rouge Airbus A319-114 (C-FYJG/ROU1539) from Toronto/Pearson, ON (CYYZ) to Saskatoon/Diefenbaker, SK (CYXE) was cleared for the approach into CYXE. ROU1539 reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and advised levelling at 6000 ft. VFR traffic, a Mitchinson Flying Service Limited (C-GLVO) from Saskatoon/Diefenbaker, SK (CYXE) to Yorkton, SK (CYQV), was at 5500 ft.
Update TSB #A19C0079: C-FYJG, an Airbus 319 operated by Air Canada Rouge, was conducting flight ROU1539 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Saskatoon/John G. Diefenbaker Intl (CYXE), SK. On descent into CYXE, in the vicinity of DUNEM waypoint, the flight crew observed a TCAS traffic advisory, which was quickly followed by the TCAS aural resolution to level off. The crew leveled off at approximately 5000 feet and observed VFR traffic (a Prairie Aviation Training Centre Cessna 172) approximately 500 feet below them. The flight was being controlled by the NAV CANADAs Winnipeg North Low Specialty controller, who had relieved the previous controller only 2 minutes before. NAV CANADA procedures require that VFR traffic be passed to IFR traffic as time and workload permits. NAV CANADA is conducting a safety investigation into the occurrence.

After landing, the pilot of a Good Spirit Air Service Beech B200 (C-FVCC/GT967) from Yorkton, SK (CYQV) to Thompson, MB (CYTH) indicated that they had received a traffic alert and collision avoidance system (TCAS) warning while on final approach. The only reported traffic was a Perimeter Aviation de Havilland DHC-8-106 (C-FUQY/PAG401) from Thompson, MB (CYTH) to Gods Lake Narrows, MB (CYGO) that was waiting for departure on Apron 1. PAG401 said it may have been them.

A Miccar Aerial Ltd Beech B200 (C-GGGQ/GT1128) from Prince Albert, SK (CYPA) to Yorkton, SK (CYQV) on area navigation (RNAV) 04 approach reported a precautionary engine shutdown 4 NM north of KEGTU at 2349Z. Emergency services notified. Aircraft landed without further incident at 2357Z and cleared the runway without assistance.
Update TSB #A19C0029: C-GGGQ, a B200 Beech King Air aircraft operated by Miccar Aerial (dba Good Spirit Air Service), was conducting flight GT1128 from Prince Albert (Glass Field) (CYPA), SK to Yorkton Muni (CYQV), SK with only the 2 pilots on board. During the descent through 12 000 feet approximately 40 nm north of CYQV, the flight crew noticed fluctuations in the fuel flow and gas generator speed (ng) for the left engine (Pratt & Whitney Canada PT6A-42). As the flight continued in the descent, the left engine parameters returned to normal, then followed by a dramatic engine rollback. The flight crew feathered the engine, noticed an increase in ITT, and performed a precautionary engine shutdown. The aircraft landed without further incident in CYQV with emergency services responding. The operator’s maintenance replaced the fuel control unit.

Incident at YORKTON MUNI SK (CYQV) (Aerodrome - visual aids)

At 0126Z, a Leading Edge Aviation Cessna 172M (C-GJKR) from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) was conducting circuits and returned to the hangar due to being unable to activate the runway edge lights (REDL) or taxiway (TWY) lights at CYQV. The flight service station (FSS) advised the airport operator. At 0157Z, the airport operator advised the FSS that the lighting problem was resolved and that the runway edge lights and taxiway lights were serviceable (SVCBL).

The pilot established cell phone contact with the Winnipeg, MB (YWG) flight information centre (FIC). The aircraft landed on CYQV Runway 22 at 0351Z.

At 1836Z, with a privately registered Cessna 182A from Yorkton, SK (CYQV) to Yorkton, SK (CYQV) on 1.5NM final for Runway 22 and a vehicle (BW122) on Runway 13/31 holding short of Runway 22, a Mitchinson Flying Service Limited Cessna 172RG (C-GIQJ) from Saskatoon/Diefenbaker Intl, SK (CYXE) to Yorkton, SK (CYQV), that was not in contact with the Regina (YQR) flight service station (FSS), underflew the Cessna 182A inbound to the airport. C-GIQJ made a low pass over the airport while the Cessna 182A executed a "360" for spacing. While C-GIQJ joined the circuit for Runway 22, the Cessna 182A landed at 1838Z. C-GIQJ executed another low pass over Runway 22 at 1839Z, then departed the circuit. The vehicle was removed from Runway 13/31 via Taxiway B at 1840Z, as the intentions of C-GIQJ were now unknown. C-GIQJ was then reported approximately 5NM North of the airport, inbound. After being queried by the YQR FSS, the aircraft reported that it had been on the old remote aerodrome advisory service (RAAS) frequency. C-GIQJ landed without further incident at 1851Z.

The pilot of a privately registered Cessna A185F from Russell, MB (CJW5) to Yorkton, SK (CYQV) attempted to file a VFR flight plan with a third party Internet application and inadvertently filed an IFR flight plan. After arrival, the pilot called the Winnipeg (WPG) flight information centre (FIC) to close flight plan and was informed that no VFR flight plan was on file. The Winnipeg (WPG) Air Traffic Operations Specialist (ATOS) was holding an IFR flight plan (FP) for the aircraft, for which an overdue departure search had started.

An Allied Wings Aviation Limited Bell 412CF (C-FYZQ/OTLW78) from Portage La Prairie/Southport, MB (CYPG) to Yorkton, SK (CYQV) declared a possible mechanical issue, but declared no emergency, at 1727Z. The aircraft landed safely at 1741Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. A 90 degree chip light illuminated on the caution panel. No cats eye tripped. A small metal sliver was found on the 90 degree detector. The detector was cleaned and reinstalled per Airspace Management Manual (AMM). A ground run was carried out with no faults found. A review of the records revealed no reoccurrences with this fault.
