A Synergy Aviation Ltd. Guimbal Cabri G2 (C-GUBA) from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU) called at 1818Z and advised that it would be returning to the airport due to a possible radio issue. The aircraft was given advisory and traffic and responded. C-GUBA then entered the control zone and began squawking 7600. The flight landed without incident at 1822Z. A Cessna in the circuit for Runway 30 extended to accommodate. Minimal impact on operations.

An American, corporately registered Cirrus SR22 (N787ZE) from High Prairie, AB (CZHP) to Grande Prairie, AB (CYQU) reported hearing an Emergency Locator Transmitter (ELT) signal for approximately 10 seconds, at 5500ft. The area control centre (ACC) shift manager (SM) and the air traffic operations specialist (ATOS) were advised.

A Bailey Helicopters Ltd. Aerospatiale AS-350-BA (C-FBHD) on a flight to Fort St. John, BC (CYXJ) was transitioning from East to West at 050, and a Sunwest Aviation Ltd. Beech 1900D (CNK77) from Dawson Creek, BC (CYDQ) to Grande Prairie, AB (CYQU) was inbound from the West. In passing C-FBHD, the CNK77 pilot reported receiving a traffic alert and collision avoidance system (TCAS) resolution advisory (RA). Traffic was passed to both C-FBHD and CNK77 by the flight service station (FSS).

At 2023Z, air traffic control (ATC) observed PBR635 off course, proceeding through the runway centreline. A Can-West Corporate Air Charters Ltd. Beech 200 (CWA924) from Grande Prairie, AB (CYQU) to Edmonton, AB (CYEG) on the approach for Runway 30 was instructed to stop descent at 6000 feet to ensure separation. A privately registered Cessna 172 from Grande Prairie, AB (CYQU) to Fairview, AB (CEB5) was observed crossing the hold short markers at threshold Runway 25 on Taxiway C with 2 pieces of traffic on final for Runway 07: an Adventure Aviation Inc. Cessna 172N (C-GXRM) from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU), and an Adventure Aviation Inc. Cessna 172S (C-GHOU) from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU). Traffic and updates were previously provided and the Flight Service Station (FSS) had requested that the Cessna 172 hold short of Runway 25. The aircraft stopped just beyond the hold short markers. Both inbound aircraft were advised and elected to continue landing. No further operational impact.

A Canadian North Inc. Boeing 737-36N (MPE1740) from Calgary, AB (CYYC) to Fort Mackay/Horizon, AB (CYNR) departed Runway 35L, heading 360 degrees, followed by a Jazz de Havilland DHC-8-402 (JZA8477) from Calgary, AB (CYYC) to Grande Prairie, AB (CYQU), heading 335. There was only 25 degrees separation and 1.9 miles in trail. Required separation is 30 degrees or 3 miles in trail.
UPDATE TSB #A20W0020: C-GPNL, a Canadian North Boeing B737-36N operating as flight MPE1740 from Calgary Intl. (CYYC), AB to Fort Mackay/Horizon (CYNR), AB with 5 crew and 129 passengers onboard departed RWY 35L on a heading of 360. Following MPE1740 was C-GJZF, a Jazz Aviation de Havilland DHC-8-402, operating as JZA8477 from Calgary Intl. (CYYC), AB to Saskatoon (CYXE), SK with 3 crew and 34 passengers onboard took off and climbed on a heading of 335. A loss of separation occurred as JZA8477 closed the distance on MPE1740. Separation deteriorated to 25 degrees and 1.9 NM laterally where 30 degrees and 3 NM is required. Separation was regained without controller intervention when JZA8477 turned to a northeast heading.

The FSS reported the animals position to a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3195) from Calgary, AB (CYYC) to Grande Prairie, AB (CYQU), a Surerus Construction & Development Ltd. Cessna A185F (C-GSPK) from Edmonton/Villeneuve, AB (CZVL) to Grande Prairie, AB (CYQU), and a Shock Trauma Air Rescue Service Messerschmitt MBB-BK117 A-4D (STAR5-T) from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU) while they continued their operations and landed. The controller cleared SDE563 to climb to FL280 on the YOJ 200 degree radial. SDE563 was observed on radar at FL222 on the 180 degree radial. The controller stopped the climb of SDE563 at FL230 until the aircraft was past a Can-West Corporate Air Charters Ltd. Several attempts were made to contact C-GECD before, during, and after the aircraft exited the zone with no response. No impact to operations.

A WestJet Encore de Havilland DHC-8-402 (C-FOWE/WEN3101) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ) was on the approach for Runway 30. The CYXJ flight service station (FSS) passed WEN3101 traffic to a privately registered Piper PA-28R-201T from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU) and YXJ FSS requested that the Piper PA-28R-201T remain north of the approach. The pilot of the Piper PA-28R-201T reported traffic in sight. At 0106Z, the Piper PA-28R-201T was observed flying south through the approach. WEN3101 reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) with the VFR traffic and elected to climb. No other impact to the operation.

Incident at FORT ST. JOHN BC (CYXJ)

A Kirsch-Air Ltd. Cessna T182T (C-GHEC) from Grande Prairie, AB (CYQU) to Fort St. John, BC (CYXJ) received an arrival advisory for Runway 30 at CYXJ. A Northern Youth Programs Inc. Cessna U206D (C-FZNS) from Fort St. John, BC (CYXJ) to Fort St. John, BC (CYXJ) was established in the circuit for Runway 30. After a wind check (350 at 10 gusting 17), the pilot of C-GHEC advised they were changing to Runway 03. When asked how they planned to conform to or avoid the established traffic pattern, the pilot of C-GHEC advised that they would join straight in for Runway 03 over the Peace River. Shortly thereafter, they turned right base for Runway 03 (circuits for Runway 03 are left-hand), in line with the left downwind for Runway 30, and requested that C-FZNS extend its departure leg. C-FZNS extended its departure leg, and C-GHEC arrived on Runway 03, then C-FNZS resumed normal circuits.

An American privately registered Cessna 170B from Grande Prairie, AB (CYQU) to Fort St. John, BC (CXYJ) made the initial call to CYXJ flight service station (FSS) 6 miles southeast on the Runway 30 approach at 4,900ft with an estimated time of arrival (ETA) of 4 minutes. A medevac, was on a straight in approach for Runway 30 and had to deviate around the Cessna 170B passing over the right hand side of them with 1,000ft vertical separation. Both landed without issue. No impact to operations.

Incident at MUNCHO LAKE / MILE 462 BC (Water) (CBF8)

An American, corporately registered Kaman K-1200 (N805RA) from Grande Prairie, AB (CYQU) to Muncho Lake, BC (CBF8), on a VFR flight plan, became overdue. A review indicated that the flight had departed an enroute point of Fort Nelson, BC (YYE) at 2140Z and had not updated the estimated time of arrival (ETA), which was 2127Z. The flight service station (FSS) advised the Joint Rescue Coordination Centre (JRCC) Victoria of the situation and a new ETA was agreed upon. The aircraft landed safely at 2253Z. JRCC Victoria was advised.

An advisory was provided by the Peace River flight service station (FSS). No further radio calls were received from CNK432 until the pilot reported down and clear.

A Sunwest Aviation Ltd. Beech 1900D (C-GSLX/CNK448) from Grande Prairie, AB (CYQU) to Dawson Creek, BC (CYDQ), inbound into CYDQ, reported at 2NM final for Runway 24 at 1604Z. The Peace River, AB (CYPE) flight service station (FSS) requested that the pilot report when the aircraft is off the runway. No further radio call was received from the pilot. The CYPE FSS had no visual reference in CYDQ and could not determine if the aircraft landed safely. A search was conducted by an airport maintenance truck and, at 1611Z, it was confirmed that the aircraft was on the main apron, unloading.

Aviation Incident Report #16080: At Grande Prairie, AB (CYQU), a Beechcraft T-6 Texan II landed. While taxiing to a fixed-base operator (FBO), the flight crew smelled hot electrical wires and asked for Aircraft Rescue and Fire Fighting (ARFF) response. While ARFF responded, the crew was deplaned, aircraft engine was off, and there was no sign of smoke or fire. No further actions required for ARFF. No other operations impacted.

An American corporately registered Cessna 150H (N22594) from Grande Prairie, AB (CYQU) to Fort Nelson, BC (CYYE) declared an emergency 7NM NW of CYQU. Aircraft completed a controlled landing in a field 6NM NW of CYQU. Emergency services dispatched. No injuries, no impact on operations.
UPDATE TSB Report #A19W0173: N22594, a privately operated Cessna 150H had departed Grande Prairie (CYQU), AB to Fort Nelson (CYYE), BC. Shortly after take-off from CYQU, the engine (Teledyne Continental O-200) experienced a reduction in power. A precautionary landing was conducted to an open field approximately 7 nm to the NW of CYQU. There were no injuries to the occupants or damage to the aircraft. The aircraft later took off for CYYE with no further issues.

A privately registered Beech C90 from Grande Prairie, AB (CYQU) to Grande Prairie, AB (CYQU) had departed CYQU and was climbing enroute when the pilot reported a mechanical issue and advised returning to CYQU. No assistance was requested and the pilot landed safely at CYQU at 1709Z, no impact to operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After takeoff the pilot of a privately registered Beech C90 had a Fuel Transfer Pump indication. They returned to base and had maintenance carry out required repairs prior to departure.

After a safe arrival at Grande Prairie, AB (CYQU), a WestJet Encore Bombardier DHC-8-402 (C-GTWE/WEN3187) from Calgary, AB (CYYC) to Grande Prairie, AB (CYQU) reported what appeared to be an intentional laser strike. The strike occurred when the aircraft was 5 miles final for Runway 30 and came from the direction of the city. No injuries were reported. The Royal Canadian Mounted Police (RCMP) and the Shift Manager were notified.

The flight plan of a Sikorsky S-70 (H-60 Black Hawk) military aircraft (ICY35) enroute from Grande Prairie, AB (CYQU) to Whitehorse/Erik Nielsen, YT (CYXY) entered into the "information request" (INREQ) stage. Aircraft was located 10 minutes south of CYXY by the Trenton Joint Rescue Coordination Council (JRCC) via the Elmendorf Air Force Base, AK (PAED) National Guard flight tracking.
