The pilot of a Chrono Jet Inc. de Havilland DHC-8-101 (MBK810) from Thunder Bay, ON (CYQT) to Roberval, QC (CYRJ) did not report that it was on the ground and off the runway at CYRJ. Confirmation of arrival was obtained by the weather observer.

An American corporately operated Piper PA-30 (N7131Y) from Thunder Bay, ON (CYQT), to Val-dOr, QC (CYVO) experienced a failure after it had landed and cleared the runway via Taxiway C. A privately registered Cessna 177RG from La Tuque, QC (CYLQ) to Val-dOr, QC (CYVO) landed at 1853Z and cleared the runway via C less than 200 ft from Runway 18/36.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

After it had received and correctly read back standard instrument departure (SID) YQB6 for Runway 24, a privately registered Cessna 510 from Quebec/Jean Lesage, QC (CYQB) to Thunder Bay, ON (CYQT) was seen in a right turn on a heading of 290 even though the SID calls for a climb to 4 000 ft on the runway centreline. No impact on operations.

Shortly after departure, a North Star Air Pilatus PC-12 (BF708) on a flight from Thunder Bay, ON (CYQT) to Thunder Bay, ON (CYQT) reported an aircraft issue and advised returning to CYQT. The aircraft landed at 1725Z with no impact to operations.

A privately registered Bellanca 17-30A from Thunder Bay, ON (CYQT) to Thunder Bay, ON (CYQT) did not establish contact with the tower prior to entering the zone. First contact with the tower occurred when the aircraft was 3 miles final for Runway 25. The aircraft was 1 mile in trail of an IFR Pilatus PC-12 when communication was established.

A small dog was observed on Taxiway E. Emergency Response Operations Centre (EROC) advised and airport vehicle removed the dog, no operational impact.

A Porter Airlines de Havilland DHC-8-402 (C-GLQJ/POE256) on a flight from Toronto/Billy Bishop, ON (CYTZ) to Thunder Bay, ON (CYQT) was cleared to FL240 which was read back correctly. POE256 was subsequently observed to climb above FL240 and was cleared again to FL240. The aircraft was observed to climb to FL249 before descending to FL240. No impact on operations.

The Thunder Bay, ON (CYQT) remote communications outlet (RCO) 122.2 at Pickle Lake, ON (CYPL) cut out mid-advisory to an Air Bravo Pilatus PC-12/45 (C-GBJV/AB1237) from Pickle Lake, ON (CYPL) to Thunder Bay, ON (CYQT) at 1759Z. An Air Bravo Pilatus PC-12/45 (C-GFKA/AB1182) from Lansdowne House, ON (CYLH) to Pickle Lake, ON (CYPL) was on approach at the time. RCO came back briefly from 1807Z to 1808Z. Technical Operations Coordinator (TOC) was advised. RCO came back at around 1813Z, and TOC was advised that RCO was serviceable.

A Swiss registered Pilatus PC-12 (PCH915) on a flight from Iqaluit, NU (CYFB) to Thunder Bay, ON (CYQT) filed YER direct YTL as part of their flight plan. When the initial clearance was given at CYFB, the pilot read back YPL instead of YTL, was corrected and read back YTL. After passing YER, the aircraft turned on course for YPL. When queried the pilot stated they had YER to YPL. No impact to operations.

Incident at 15NM N From CYAG - FORT FRANCES MUNI - FORT

TSB Report #A19C0123: C-GYUA, a Mitsubishi MU-2B-60 aircraft, operated by Thunder Airlines Limited, was conducting flight THU890 from Fort Frances Muni (CYAG), ON to Red Lake (CYRL), ON. At approximately 15 nautical miles North of CYAG, climbing through 7000 feet, the right hand fire warning light illuminated. The flight crew stopped the climb and carried out the engine fire checklist. The flight crew did not observe fire or smoke from the engine and a decided to return to CYAG with both engines operating. During the descent, the fire warning light extinguished and a flight medic reported smoke from the right hand engine. An emergency was declared and the right hand engine was shutdown. The aircraft landed safely in CYAG. The operators maintenance inspected the aircraft in CYAG and a ferry flight was subsequently completed to Thunder Bay (CYQT), ON. The operators maintenance then replaced the right hand fire detector control unit.

Immediately after departure, a Porter Airlines de Havilland DHC-8-402 (POE467) on a flight from Thunder Bay, ON (CYQT) to Toronto/Billy Bishop, ON (CYTZ) reported a gear malfunction and advised they would remain close to CYQT to diagnose the issue. The pilot subsequently advised they could not retract the gear and would land at CYQT but may need assistance to exit. The aircraft landed at 1341Z and was able to exit the runway. ROU1510 was delayed 13 minutes on arrival.

Incident at 240NM THUNDER BAY ON (CYQT) (Communication navigation surveillance/air traffic system)

A Goldcorp Canada de Havilland DHC-8-314 (C-GMWT) on a flight from Thunder Bay, ON (CYQT) to Opapimiskan Lake, ON (CKM8) initiated a missed approach in Opapimiskan Lake, ON (CKM8) and requested clearance back to Thunder Bay, ON (CYQT) with the East low Specialty controller. The pilot was given clearance and readback the transponder code incorrectly which the controller did not detect. C-GMWTs target remained uncorrelated and therefore did not produce a strip in Extended Computer Display System (EXCDS). The uncorrelated target was noticed later by another controller in East Low Specialty and the correct transponder was given to C-GMWT.

A Summit Air Ltd. de Havilland DHC-8-102 (C-GASB/SU1811) on a flight from Thunder Bay, ON (CYQT) to Sioux Lookout, ON (CYXL) departed CYQT and reported a gear indication issue and advised returning to CYQT. Issue resolved and the aircraft landed safely. An Ornge Global Air PC-12 (PUL103) on a flight from Thunder Bay, ON (CYQT) to Sioux Lookout, ON (CYXL) had its arrival delayed 5 minutes to accommodate SU1811. Duplicate AOR #257242-V1 deleted.
UPDATE from Aircraft Maintenance & Manufacturing: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After take-off from Thunder Bay, ON when the landing gear was selected UP the landing gear handle light and the left-hand (L/H) and right-hand (R/H) MAINS lights remained amber, with the NOSE GEAR light being red. The crew followed procedures and selected the landing gear DOWN, three green lights were confirmed and the aircraft returned for landing without incident. Company maintenance bled the #2 hydraulic system, cleared the Proximity Sensing Electronics Unit (PSEU) codes and conducted Bite check operation of the Nose Landing Gear (NLG) Solenoid Sequence Valve (SSV), all operating normally. Manual landing gear release was also confirmed as operating normally. The aircraft was released pending a satisfactory test flight which resulted in all systems operating normally. There were no further report incidents.

A Perimeter Aviation Fairchild SA227-AC (C-GYXL/BLS312) on a flight from Sioux Lookout, ON (CYXL) to Thunder Bay, ON (CYQT) reported a bird strike landing Runway 25. Bird remains found on inspection.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 3 C-GYXL incurred a bird strike on the #2 engine on landing into CYQT. Aircraft inspected, and it was confirmed a bird strike did occur, requiring the #2 engine to be replaced.
