A Confederation College of Applied Arts & Technology Cessna 172S (C-FNYX) from Thunder Bay, ON (CYQT) to Thunder Bay, ON (CYQT) experienced a tail strike while conducting a stop-and-go on Runway 25. The aircraft did not continue the departure and exited the runway. A Skycare Air Ambulance Swearingen SA226-TC (C-GKPX) from Red Lake, ON (CYRL) to Thunder Bay, ON (CYQT), on final for Runway 25, elected to conduct a go-around until the runway was inspected for foreign object debris (FOD).

Incident at THUNDER BAY ON (CYQT) (Aerodrome, runway or taxiway shutdown)

An Air Bravo Corp. Pilatus PC-12/47 (C-FWAL/AB1776) from Bearskin Lake, ON (CNE3) to Thunder Bay, ON (CYQT) reported a gear indication issue when 4NM final for Runway 25. The aircraft did a fly by to confirm that the nose wheel was not extended. After some troubleshooting, the aircraft was unable to extend the nose wheel. The airport authority opened Runway 12/30 at the aircrafts request. Aircraft landed on Runway 12 and the nose wheel collapsed. The aircraft came to a controlled stop on Runway 12, south of Runway 07/25. Runway 07/25 was available as soon as a runway inspection was completed. No injuries reported, one aircraft was delayed by 5 minutes.

The reporting aircraft included a Jazz de Havilland DHC-8-402 (JZA8289) from Toronto/Lester B. Pearson, ON (CYYZ) to Timmins, ON (CYTS), an Ornge Global Air Inc. Pilatus PC-12/47E (PUL105) from Toronto/Billy Bishop, ON (CYTZ) to Thunder Bay, ON (CYQT), and an Air Bravo Pilatus PC-12/45 (AB1250). The Joint Rescue Coordination Centre (JRCC) Trenton was advised. No operational impact.

A Bearskin Airlines Swearingen SW4 (BLS318) from Sioux Lookout, ON (CYXL) to Thunder Bay, ON (CYQT) departed CYXL as VFR, on the runway heading, and on initial contact accepted a clearance that included flying a heading of 100. Pilot then contacted the Centre and offered to continue a VFR climb. The controller restated the clearance to "fly 100" and subsequently stopped the aircrafts climb due to traffic. The pilot stated that they were still VFR, prompting the Centre to explain that they had accepted the clearance.

UPDATE: TSB Report #A20C0100: C-FWAF, a Pilatus PC-12/45 aircraft operated by Wasaya Airways LP, was conducting flight WSG5514 from Big Trout Lake, ON (CYTL) to Thunder Bay, ON (CYQT). While en route, the flight crew noticed a fuel quantity imbalance, with the right side indicating high. The flight crew selected the right hand fuel boost pump "on" and, after monitoring, noted that there was no improvement to the imbalance. The checklist was reviewed and the flight crew noted that there was no CAWS (Central Advisory and Warning System) light for the right boost pump. The flight crew checked the lamps and continued to monitor the imbalance. The flight crew consulted with maintenance. When the left quantity indication dropped by another LED bar, the flight crew declared PAN PAN, and elected to divert to Pickle Lake, ON (CYPL). The passengers were briefed and the subsequent landing was uneventful. The operators maintenance personnel replaced the right hand fuel boost pump and submitted an SDR.

Incident at THUNDER BAY ON (CYQT) (Aerodrome, runway or taxiway shutdown)

A privately registered Piper PA-28-180 from Winnipeg/St. Andrews, MB (CYAV) to Thunder Bay, ON (CYQT) was inbound with a fuel shortage. The aircraft landed without fuel and was disabled on Runway 25 for 20 minutes. Multiple aircraft were rerouted to Runway 30 for arrival and departure.
UPDATE: TSB Report #A20C0094: A privately registered Piper PA28-180 was enroute from Winnipeg/St. Andrews, MB (CYAV) to Thunder Bay, ON (CYQT) with the pilot and 1 passenger on board. While on approach to CYQT, the pilot noticed a low fuel indication and switched fuel tanks. During the base leg, the engine (Lycoming IO-360) lost complete power. The pilot declared an emergency and then successfully completed a forced landing on Runway 25. The aircraft was able to roll off the runway onto the taxiway under its own momentum and was then towed to the ramp. It was reported that both fuel tanks were empty. The aircraft was refueled and then departed a short time later. The pilot had just purchased the aircraft.

Incident at 20NM TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

At 1403Z, a Jazz Bombardier CL-600-2D24 (JZA7925) from Thunder Bay, ON (CYQT) to Toronto/Lester B. Pearson Intl, ON (CYYZ) performed a missed approach to Runway 05 due to a flap issue. JZA7925 troubleshot the issue and declared a PAN PAN at 1413Z due to a flap failure. Due to tailwinds on final, the configuration was changed and JZA7925 was set up for a landing on Runway 23. Emergency equipment was on standby and JZA7925 landed uneventfully at 1433Z. No operational impact.
UPDATE: TSB Report #A20O0088: C-FCJZ, a CRJ 900, was operated by Jazz Aviation LP as flight JZA7925 from Thunder Bay International, ON (CYQT) airport to Toronto/Lester B. Pearson International, ON (CYYZ) airport. When the flight crew selected the flaps from 30 to 45 degrees on final approach to Runway 05, a "Flaps Fail" caution message illuminated and the flaps remained at the 30 degrees indicated position. A go-around was initiated, the flaps were retracted to 0 degrees without incident and the caution message extinguished. ATC was advised and the flight crew prepared for an abnormal landing. Passengers, dispatch and maintenance were also notified. The flight crew elected to land as per the emergency checklist with flaps selected at 1 (slats at 20 degrees), and the captain upgraded the status to a "PAN PAN" with ATC. ATC redirected the aircraft to land on Runway 23 due to shifting winds. Emergency equipment was on standby and the aircraft landed without incident on Runway 23; emergency vehicles followed the aircraft to the gate. An inspection of the flaps and slats was conducted, with no faults found. There were no faults logged in the Maintenance Diagnostic Computer (MDC). The slat/flap control lever has been replaced and the system was tested serviceable.

Thunder Bay police called the Thunder Bay, ON (CYQT) tower to advise they had received a report that a remotely piloted aircraft system (RPAS) had been flying earlier in the evening in the vicinity of Arthur/Mountdale (1.0NM from the threshold of Runway 25 in the approach) at approximately 400ft above ground level (AGL). Steady arrivals for Runway 25, with circuits, were in progress, and no aircraft had reported RPAS activity.
