Incident at REGINA INTL SK (CYQR) (ATM - inaccurate aeronautical information)

The Winnipeg Flight Information Centre (FIC) issued a NOTAM for the closure of Taxiway C and Taxiway M at Regina, SK (CYQR), which was incorrect. The closure should have been for Taxiway P and Taxiway M.

Incident at REGINA INTL SK (CYQR) (Aerodrome, runway or taxiway shutdown)

At 1630Z, a Lipsett Holdings Ltd. Piper PA-31T (C-GZGL) from Regina, SK (CYQR) to Regina, SK (CYQR) became disabled on Runway 13 after landing with its landing gear retracted. Emergency response services (ERS) were notified and responded. Runway 13 was closed and several aircraft were delayed. The disabled aircraft was removed from Runway 13 at 1902Z.

Incident at CALGARY INTL AB (CYYC)

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3271) from Regina, SK (CYQR) to Calgary, AB (CYYC) conducted a missed approach for Runway 29 due to wind shear. The aircraft landed safely. No operational impact.

A departing Shock Trauma Air Rescue Service Messerschmitt MBB-BK117 B-2 (C-GDGP/STAR9) on a MEDEVAC flight from Regina, SK (CYQR) to Regina, SK (CYQR) reported a door issue and requested clearance back to the airport. The pilot elected not to declare an emergency and landed safely at 1626Z. No operational impact.

Incident at VANCOUVER INTL BC (CYVR)

A WestJet Boeing 737-7CT (C-GWBF/WJA302) on a flight from Vancouver Intl, BC (CYVR) to Regina Intl, SK (CYQR) reported a bird strike while departing Runway 26L.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew on WestJet flight WJA302 departing CYVR to CYQR on March 22, 2020 reported a bird strike on the Captains side, near the pitot and Angle of Attack sensor, on take-off. The defect was recorded in log book. A WestJet ACA met the aircraft in Regina and performed a bird strike inspection in accordance with Boeing Aircraft Maitenance Manual (AMM) with no damage or bird remains identified. The aircraft was returned to service and has flown 3 legs and 12 hours 46 minutes since the bird strike with no related defects.

A privately registered Diamond DA40 from Dryden, ON (CYHD) to Regina, SK (CYQR) was issued IFR departure clearance by the Sioux Lookout, ON (CYXL) flight service station (FSS) with routing CYHD YBR KEMKA. The clearance was read back correctly. En route, the controller queried the aircraft as to why they appeared to be off course. The pilot believed the cleared route to be CYHD KEMKA. En route, the controller corrected the clearance, no impact to operations.

Needham Field, SK (CYYO) informed the tower on initial climb-out that an aircraft door was left ajar and that the aircraft required a stop-and-go to latch it effectively. The aircraft joined the circuit, were cleared for a stop-and-go, and departed without further incident.

A Government of Canada, Royal Canadian Mounted Police Pilatus PC-12/47E (C-GMPW) from Prince Albert (Glass Field), SK (CYPA) to Regina, SK (CYQR) advised the flight service station (FSS) that they were taxiing via Alpha to hold short of Runway 08 while a West Wind Aviation Aerospatiale ATR 42-300 (C-GWWC/WEW481) from Points North Landing, SK (CYNL) to Prince Albert (Glass Field), SK (CYPA) was commencing the area navigation (RNAV) 08 approach. FSS noticed C-GMPW taxi past the hold line and alerted the pilot. C-GMPW turned around and taxied behind the hold line. WEW481 was advised and was approximately 4 miles final when C-GMPW was behind hold line and holding short.

The aircraft was observed departing the zone to the north. Phone calls later confirmed that the aircraft arrived safely at CAG2. No impact on operations.

Incident at REGINA INTL SK (CYQR)

A taxiing 1847698 Ontario Inc. Cessna 172G (C-FUEX) from Regina, SK (CYQR) to Regina, SK (CYQR) informed ground control that the left main landing gear of a departing Lipsett Holdings Ltd. Piper PA-31T (C-GZGL) from Regina, SK (CYQR) to Regina, SK (CYQR) did not appear to be retracted. C-GZGL elected to join the circuit and conduct a low approach for a visual inspection by the tower. On the second approach, C-GZGL declared an emergency, joined the right circuit and subsequently landed and taxied to the apron under its own power.
Update TSB Report #A20C0101: C-GZGL, a privately operated Piper PA-31T Cheyenne, was conducting a local flight from Regina International, SK (CYQR). After takeoff, another pilot reported that the left main landing gear was not retracted. The pilot flew to the west of CYQR to conduct troubleshooting. The Regina tower asked if an emergency was being declared. The pilot responded "negative" and asked the tower for permission to conduct a "low-and-over" to allow for a landing gear inspection. After the fly-past, the tower reported that the landing gear appeared to be down. The pilot requested a full-stop landing on the next approach. While on the base leg, the tower asked again if an emergency was being declared, to which the pilot responded "negative". On short final, the pilot declared an emergency and the subsequent landing occurred without further incident. Maintenance personnel replaced the hydraulic power pack due to internal leakage and conducted successful landing gear swings.

After confirming that an Allied Wings Bell 412CF (C-FYZL) from Portage La Prairie/Southport, MB (CYPG) to Regina, SK (CYQR) was landing at the KREOS hangar, air traffic control (ATC) instructed C-FYZL to remain northwest of the control tower, in order to remain clear of the approach to Runway 26 to allow for a Skylink Express Inc. Beech 1900C (C-GKGA/SLQ720) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Regina, SK (CYQR) to arrive. Once over the airport parking lot, C-FYZL turned south of the control tower. When queried by ATC, C-FYZL indicated they were landing on Apron 02, with SLQ720 on very short final for Runway 26. ATC had both aircraft in sight and ascertained they were not in conflict. SLQ720 and C-FYZL both landed safely, with no impact on operations.

At Regina, SK (CYQR), a privately registered Cessna 172H departed from Runway 26 to join the circuit. Shortly after departure, the pilot requested to return for landing due to a vibration. The Cessna 172H landed on Runway 26 at 1730Z and was able to taxi to the apron without issue. A WestJet Boeing 737-6CT (WJA294) from Calgary, AB (CYYC) to Regina, SK (CYQR), on approach to Runway 31, was extended to allow the Cessna 172H to land first.

A Carson Air Ltd. Fairchild SA227-AC (C-GKLK/CRN8059) from Calgary, AB (CYYC) to Regina, SK (CYQR) was initially on Taxiway Juliet and holding short of Bravo. The ground controller then issued instructions to taxi via Bravo and Delta to Runway 17L. The aircraft instead proceeded via Juliet, then Delta. A WestJet Boeing 737-7CT (C-GWSQ/WJA110) from Vancouver, BC (CYVR) to Calgary, AB (CYYC) that had landed on Runway 17L and was exiting via Delta 3 had to give way to C-GKLK as it taxied via the wrong route.

Incident at CALGARY INTL AB (CYYC)

TSB Report #A20W0065: C-GKLK, a Carson Air Ltd. Fairchild SA227-AC, was conducting an IFR flight from Regina (CYQR), SK, to Calgary (CYYC), AB, with 2 flight crew on board. During the initial portion of the approach, the flight crew observed the left hand hydraulic pressure warning light begin to flicker and then go solid, followed 30 seconds later by the right hand hydraulic pressure light. The flight crew actioned the appropriate checklists, manually extended the landing gear and advised the tower that a long landing would be required for the flap 0 landing. The aircraft landed and taxied clear without issue. Maintenance found a chaffed hydraulic line in the right hand engine bay. A Service Difficulty Report (SDR) was submitted to Transport Canada.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On approach into Calgary, aircraft C-GKLK experienced a "hydraulic fail" indication. The landing gear was lowered on emergency, "3 green" indicators were obtained and a successful landing was made. Maintenance found a hydraulic line in the left-hand (L/H) engine bay that had been chaffed through. The area is difficult to inspect due to the number of clamps and protective cord wrapping in the area, however, the line had been clamped in accordance with the maintenance manual requirements. The line was replaced and the rest of the company fleet of SA227 were inspected for similar wear, yielding one other aircraft with a minor in-limits chafe in the same area. An ongoing detailed inspection will be implemented to supplement the general visual inspection specified by the manufacturers maintenance program. Service difficulty report (SDR) #20200821006 has been submitted.

The DA40 crossed the hold short line at Taxiway K without air traffic control (ATC) clearance. A Regina Flying Club Cessna 172N (C-GYQR) from Regina, SK (CYQR) to Regina, SK (CYQR) on final with a touch-and-go clearance was instructed to go around. The DA40 departed without further incident.

At 2206Z, the pilot of a 629503 Saskatchewan Ltd. Piper PA-32R-300 (C-GULZ) from Assiniboia, SK (CJN4) to Regina, SK (CYQR), on final, indicated they did not have 3 green on their landing gear and was pulling up to go around. The tower controller confirmed the gear did not appear to be fully extended. At 2230Z, after talking to support at Cardinal Aviation, the pilot of C-GULZ declared an emergency and advised they had 2 green on the mains but no green on the nose gear. At 2235Z, a subsequent low approach was made, and the tower was able to advise that all three gears appeared to be down. C-GULZ landed safely at 2239Z. 2 VFR aircraft departures were delayed for the emergency landing.

The pilot appeared to experience difficulty controlling the aircraft and executed a go-around. The pilot was asked if they wished to cancel IFR and remain in the circuit. They agreed, and landed VFR at 1924Z. No impact on operations.

Incident at SASKATOON / JOHN G. DIEFENBAKER INTL SK (CYXE)

After departure, the pilot of a Saskatchewan Air Ambulance Beech B200 (C-GSAH/SLG2) from Saskatoon/John G. Diefenbaker, SK (CYXE) to Saskatoon/John G. Diefenbaker, SK (CYXE) reported a gear issue to the area control centre (ACC) controller. The pilot declared an emergency, manually locked the gear in down position, returned to CYXE and landed safely at 0007Z. No other impact to operations.
Update: TSB Report #A20C0084: C-GSAH, a Beech B200 aircraft operated by Saskatchewan Air Ambulance, was conducting flight SLG2 from Saskatoon/John G. Diefenbaker Intl (CYXE), SK, to Regina Intl (CYQR), SK. Shortly after takeoff, there was a loud bang during the landing gear retraction cycle. The master caution light illuminated and the red light in the landing gear handle remained illuminated. The "Hyd Oil Low" annunciator illuminated and the landing gear circuit breaker tripped. The flight crew notified air traffic control (ATC) and consulted the Emergency Checklist (QRH). The flight crew conducted a manual landing gear extension and obtained three green, "down-and-locked" lights. The flight crew elected to return to CYXE and declared an emergency. The subsequent landing was uneventful and, after consultation with maintenance, the aircraft taxied to the apron. The operators maintenance personnel discovered that the right main landing gear actuator body had ruptured. The operator filed a Service Difficulty Report.

A Jazz Bombardier CL-600-2D24 (C-FCJZ/JZA7944) from Regina Intl, SK (CYQR) to Toronto/Lester B. Pearson Intl, ON (CYYZ) levelled off at FL240 while deviating for weather and declared a PAN PAN due to icing. After deviating and remaining clear of cloud, the aircraft was able to resume on course at 1506Z.
Update: TSB Report #A20C0080: C-FCJZ, a Bombardier CL-600-2D24 operated by Jazz Aviation as flight JZA7944, was enroute from Regina Intl (CYQR), SK, to Toronto/Lester B. Pearson Intl (CYYZ), ON, with a crew of 2 and 35 passengers onboard. During the climb out of CYQR, at approximately 22000 feet above sea level (ASL), the crew received a left anti-ice duct warning message. The crew decided to level off the aircraft at 24000 feet ASL to stay clear of cloud and then declared a PAN PAN due to icing. After remaining clear of clouds, the flight was able to resume to its destination without further incident.
