Incident at 244NM NE TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

TSB Report #A21O0003: C-GJZS, a Bombardier CL-600-2D24 (Series 900) operated by Jazz, was conducting flight JZA8918 from Toronto/Lester B. Pearson, ON (CYYZ) to Moncton/Romeo LeBlanc, NB (CYQM) with 4 crew members and 16 passengers onboard. Shortly after takeoff, the flight crew noticed a small engine number 1 fan vibration of 1.4 Mils, which is still within acceptable limits. As the aircraft continued climbing, the vibration slowly increased until the aircraft levelled off at FL390, where the vibration reached the yellow arc, with an indication from 3.7 to a maximum of 4.0 Mils. The crew actioned the QRH and conducted the applicable trouble shooting. The captain requested a lower altitude from ATC to reduce power and attempt to stabilize the vibration. As the indication was still around 3.3 Mils, the crew declared a "PAN PAN", turned back towards CYYZ and shut down the number 1 engine, as directed by the QRH. The aircraft landed safely in CYYZ, where emergency vehicles were standing by. Engine number 2 fan vibration remained steady, around .04 and 0.5 Mils. Company maintenance conducted an inspection, with no defects found. The number one fan blade retaining pins were replaced, and the engine tested. Results were within acceptable limits.

Accident at 41NM SSE GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT (CYQM)

The pilot of a privately registered Piper PA-28-140 from Moncton/Romeo Leblanc, NB (CYQM) to Moncton/Romeo Leblanc, NB (CYQM) landed the aircraft in a field due to a rough running engine. No injuries. Damage was sustained to the wing and propeller.
UPDATE: TSB Report #A21A0004: A privately operated Piper Cherokee PA-28-140 was conducting a local flight from Moncton/Romeo Leblanc, NB (CYQM). While in cruise, the engine experienced a fuel starvation for undetermined reasons. The pilot conducted a forced approach into a field approximately 41NM south southeast of CYQM. The aircraft sustained substantial damage and the pilot reported no injuries. Two weeks prior to this occurrence, the aircraft had a power loss, resulting in a forced landing, and the aircraft was not damaged. Subsequently, maintenance had inspected and replaced a carburettor heat hose before returning the aircraft to service.

At Saint John, NB (CYSJ), at approximately 1455Z, a vehicle (ST21) informed the Flight Service Station (FSS) that a person was shovelling snow near the lights on Taxiway I. A Moncton Flight College Diamond DA 20-C1 (C-FNHO) from Moncton/Romeo LeBlanc, QC (CYQM) to Moncton/Romeo LeBlanc, QC (CYQM) was on approach for Runway 32. The FSS observed a vehicle and person on Taxiway I. C-FNHO proceeded with a stop-and-go. After the aircraft was airborne, the FSS observed the person shovelling snow across the hold short line and a second person walking from the Irving ramp onto Taxiway I to join the first person. The vehicle stayed on Taxiway I. The FSS notified the maintenance garage. While a vehicle (ST26) was en route to Taxiway I, the two individuals and the initial vehicle (ST21) returned to the Irving ramp. After determining that the aircraft was a Moncton Flight College Diamond DA 20-C1 (C-GPUX) from Moncton/Romeo LeBlanc, NB (CYQM) to Moncton/Romeo LeBlanc, NB (CYQM), the aircraft was asked to squawk its identification and was determined to be receiver only (RONLY). The aircraft then squawked 7600. Once inbound, the aircraft was then heard calling and was asked to change back to the previous code. After landing, the aircraft failed to vacate the runway by stopping on the wrong side of the hold short line.
