The aircraft cancelled its IFR at 1641Z, directly above the airport while it was heading to Runway 06. The aircraft landed at Rimouski, QC (CYXK) instead of CYYY. The Montreal Area Control Centre (CZUL) was notified. The separation between a Jazz de Havilland DHC-8-300 (JZA8907) from Moncton, NB (CYQM) to Montreal/Trudeau, QC (CYUL) and an Air Transat Airbus A321-211 (TSC811) from Varadero, Cuba (MUVR) to Montreal/Trudeau, QC (CYUL) on final for Runway 24R was reduced to 2.6 NM when it should have been 3 NM. Impact on operations: TSC811 decided to conduct a go-around on Runway 24R.
Update TSB #A19Q0112: C-FACT, a Jazz Aviation LP Dash 8 100-300 (JZA907), was conducting an IFR flight from Moncton/Greater Moncton International (CYQM), NB, to Montreal/Pierre Elliott Trudeau International (CYUL) QC. C-GEZO, an Air Transat A.T. Airbus A321-211 (TSC811), was conducting an IFR flight from Varadero (MUVR), Cuba, to Montreal/Pierre Elliott Trudeau International (CYUL), QC. JZA907 was inbound for landing on Runway 24R, followed by TSC811. ATC asked TSC811 to reduce its approach speed to maintain the required 3 NM distance between the 2 aircraft. After JZA907 landed on the runway, ATC advised TSC811 that JZA907 was headed towards B2 to clear the runway. TSC811 acknowledged the advisory and initiated an ascent. Spacing at this time was 2.6 NM.

Incident at 97NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

While inbound, a Jazz de Havilland DHC-8-300 (JZA8907) from Moncton, NB (CYQM) to Montreal/Trudeau, QC (CYUL) declared PAN PAN due to cabin depressurization. JZA8907 descended to 8 000 ft, then cancelled its PAN PAN. JZA8907 landed without incident at 0519Z. No impact on operations.
TSB Update #A19Q0097: C-FACT, a Jazz Aviation LP de Havilland DHC-8-300, was on instrument flight rules (IFR) flight JZA8907 from Greater Moncton/Roméo Leblanc Intl, NB (CYQM) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 3 crew members and 12 passengers on board. While the aircraft was in cruising flight at FL240, there was a pressurization problem. The crew declared a PAN PAN and conducted a rapid descent to 8 000 ft, but the passenger oxygen masks, however, were not deployed. The crew then cancelled the PAN PAN, and the aircraft landed without incident at CYUL.

Update TSB #A19Q0052: C-GKUK, a Bombardier DHC-8-400 aircraft operated by Jazz Aviation LP, was conducting flight JZA8909 from Greater Moncton Roméo Leblanc Intl (CYQM), NB to Montreal/Pierre Elliott Trudeau Intl (CYUL), QC with 4 crew members and 61 passengers on board. During cruise flight at 16 000 feet, the aircraft experienced a loss of cabin pressure, accompanied by a loud popping sound heard from the back of the aircraft. The flight crew donned their oxygen masks, initiated an emergency descent to 10 000 feet, and declared a PAN PAN. The flight continued to destination without further incident. The operator’s maintenance identified a leak from a faulty cargo door seal.

Fifty seconds passed before the beginning of the turn. The result was a loss of separation with a Jazz Bombardier CL-600-2B19 (series 200) (JZA8851) from Moncton/Roméo LeBlanc, NB (CYQM) to Ottawa/Macdonald–Cartier, ON (CYOW), which was abeam and descending to FL210. The paths were therefore divergent. No impact on operations.
Update - TSB Report #A19Q0013: C-GFAJ, an Air Canada Airbus A330-300, was on instrument flight rules (IFR) flight ACA419 from Montreal/Pierre Elliot Trudeau Intl., QC (CYUL) to Toronto/Lester B. Pearson Intl., ON (CYYZ). While it was climbing to FL200, the flight crew was instructed to proceed direct MELTI. 50 seconds passed before the beginning of the turn. Consequently, its separation with C-GGJA, a Jazz Aviaton LP Bombardier CRJ-200 on flight JZA851 to Ottawa/MacDonald-Cartier Intl., ON (CYOM), was reduced to 4.7 miles laterally, while the standard is 5 miles. The paths of the aircraft were divergent.

TSB Report #A19O0125: C-GENK, a de Havilland DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN3436 from Toronto / Lester B. Pearson Intl (CYYZ) ON to Moncton / Greater Moncton Intl (CYQM), NB. During climb out from CYYZ, the windshield on the captains side cracked. The flight crew declared a PAN PAN and returned to CYYZ. The aircraft landed uneventfully with ARFF standing by. Maintenance replaced the cracked windshield and the aircraft returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Pilot windshield cracked during climb through 8000ft out of YYZ, approximately 1-2 minutes after windshield heat was selected to norm. The aircraft returned to YYZ. The windshield was replaced and the aircraft was returned to service.

An ABC Aerolineas Airbus A320 (AIJ2830) from Mexico City, Mexico (MMMX) to Toronto/Pearson, ON (CYYZ) was cleared for the instrument landing system (ILS) Runway 24R and flew through the localizer. An Air Canada Rouge Airbus A319 (ROU1501) from Moncton, NB (CYQM) to Toronto/Pearson, ON (CYYZ) was established on the visual for Runway 23. The controller had pointed out traffic to both flights. AIJ2830 was instructed to turn left immediately and ROU1501 was instructed to turn right and climb. Nearest approach was 1.5NM laterally and 400 feet vertically. ROU1501 landed on Runway 23 at 1107Z and AIJ2830 landed on Runway 24R at 1102Z. Visual meteorological conditions prevailed. No other operational impact.

A WestJet Encore de Havilland DHC-8-402 (WEN8987) from Moncton, NB (CYQM) to Toronto/Pearson, ON (CYYZ) declared an emergency due to an electrical failure and requested a second approach with a flapless landing. Aircraft landed without further incident (WOFI) at 1134Z.
Update TSB #A19O0082: C-GKWE, a de Havilland DHC-8-400 aircraft operated by WestJet Encore, was conducting flight WEN8987 from Greater Moncton/Romeo Leblanc Intl (CYQM), NB to Toronto/Lester B. Pearson Intl (CYYZ), ON. During the extension of the flaps on the approach to CYYZ, a Flap Power caution light illuminated and the flaps stopped at the 5° position. A PAN PAN was declared, and the aircraft landed uneventfully with ARFF standing by. The operator’s maintenance replaced the main, auxiliary and standby batteries. A test flight was conducted and, following ground checks, no further faults were found. The aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. In cruise, L LWR CB N10 released, 20 minutes later J10 and K10 released. Upon flap selection L7 released (with associated Flap Power Caution). After shutdown RH N10 released. The previous day, prior to putting power on the aircraft following run, found numerous CBs released. Electrical Power Control Unit (EPCU) was replaced at that time but defect occurred on subsequent flight as described above. Standby Battery replaced, Main Battery replaced, LH N10 Circuit breaker replaced and operational test of all affected systems accomplished. Additional troubleshooting carried out in cooperation with DeHavilland tech support. Test flight satisfactory.

The aircraft were then followed by a Moncton Flight College Diamond DA 20-C1 (C-FXJD) on a flight from Greater Moncton/Romeo Leblanc Intl, NB (CYQM) to Greater Moncton/Romeo Leblanc Intl, NB (CYQM) on a stop and go 28 and a Moncton Flight College Beech C90A (C-GDHS) on a flight from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) on instrument landing system (ILS) 03. C-GDHS on instrument landing system (ILS) 03 executed a missed approach due to traffic conflicts, and C-FXJD terminated stop and go 28 due to traffic conflicts.

A Moncton Flight College Diamond DA 20-C1 (C-GMFE) on a flight from Greater Moncton/Romeo Leblanc Intl, NB (CYQM) to Greater Moncton/Romeo Leblanc Intl, NB (CYQM) reported a red laser strike 1/4-1/2NM off its 9 oclock at approximately 1.5NM SW of CYQM along the centreline of Runway 29 at 0306Z. 2 strikes lasting a few seconds each. Pilot not affected. The flight returned to land CYQM.
