A privately registered Canadian Ultralight Chinook Plus 2, origin and destination unknown, reported an engine failure and landed at a lake 6NM west northwest (WNW) of Kenora, ON (CYQK). The pilot reported having landed safely 5 minutes later. No emergency was declared or assistance requested. No impact to operations.

Incident at In the vicinity of: KENORA ON (CYQK)

The pilot of a Government of Canada, Department of National Defence Lockheed C-130 (Hercules) (ATLS339) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Winnipeg/James Armstrong Richardson, MB (CYWG) reported to the Kenora flight service station (FSS) that they had an engine issue and declared an emergency. The engine was shut down and the aircraft returned, VFR, to CYWG. Emergency services were activated and the aircraft landed at 1749Z without further issue. No impact to operations.

Incident at KENORA ON (CYQK)

The pilot of a Lockheed C-130 (Hercules) aircraft reported an emergency locator transmitter (ELT) signal north of Kenora, ON (CYQK). The ELT source was identified as a privately registered Aeronca 7CCM float plane that was operating outside the CYQK control zone (CZ). The C-130 aircraft followed the float plane until it landed. The pilot of the float plane then called the flight service station (FSS) via cell phone and confirmed that they were safe. The Joint Rescue Coordination Centre (JRCC) Trenton was notified by the C-130 aircraft.

TSB Report #A20C0034: C-FITU, a Boeing 777-300 aircraft operated by Air Canada as flight ACA2236, was conducting a flight from Tokyo/Narita Intl. (RJAA), Japan to Toronto/Lester B. Pearson Intl. (CYYZ), ON. During cruise, the flight crew received EICAS message “TIRE PRESSURE”. The flight continued to CYYZ and a PAN PAN was declared. The aircraft landed without incident. TSB #A19C0027: C-GYNF, an Agusta AW139 helicopter operated by 7506406 Canada Inc. (ORNGE Rotor Wing Operation), was conducting a flight from Kenora (CYQK), ON to White Dog, ON. During the taxi to the active runway for the departure at CYQK, the flight crew received a #2 ENGINE OIL TEMP warning illumination from the Crew Alerting System (CAS). While still on the ground, the flight crew confirmed the high oil temperature, initiated the QRH, and carried out a precautionary shut down of the number 2 engine (PWC PT6C-67C). The aircraft was taxied back to the apron. The operators maintenance replaced the fuel oil cooler.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-FYAG had a brake dragging issue on landing into Kenora, ON (CYQK). Maintenance found the 3 brake dragging. Brake assembly replaced and aircraft returned to service.

Incident at THUNDER BAY ON (CYQT)

The pilot of a Thunder Airlines Limited Mitsubishi MU-2B-60 (C-GZNS/THU870) from Kenora, ON (CYQK) to Thunder Bay, ON (CYQT) called at 0551Z and said they noticed some damage on the aircraft after landing. C-GZNS landed at 0517Z. The pilot advised they had hit a windrow on the right side of Runway 07 and damaged both left and right landing gear fairings. Two other aircraft, a Jazz de Havilland DHC-8-311 (C-GABP/JZA8298) from Winnipeg/Richardson Intl, MB (CYWG) to Thunder Bay, ON (CYQT) and a WestJet Encore Ltd. de Havilland DHC-8-402 (C-GENM/WEN3219) from Toronto/Pearson Intl, ON (CYYZ) to Thunder Bay, ON (CYQT), landed before the received phone call, and a Jazz de Havilland DHC-8-402 (C-GKUK/JZA8547) from Toronto/Pearson Intl, ON (CYYZ) to Thunder Bay, ON (CYQT) advised of possible debris on the runway and elected to land without a runway inspection. A WestJet Encore Ltd. de Havilland DHC-8-402 (C-GJEN/WEN3204) from Winnipeg/Richardson Intl, MB (CYWG) to Thunder Bay, ON (CYQT) had its landing delayed due to a runway inspection. No debris was found. No other operational impact.
Update TSB Report #A19C0012: C-GZNS, a Mitsubishi MU-2B-60 aircraft operated by Thunder Airlines, was conducting flight THU860 from Kenora (CYQK), ON to Thunder Bay (CYQT), ON with 2 crew members and 2 passengers on board. During the landing roll on Runway 07 at CYQT, the aircraft collided with a snow drift, and directional control of the aircraft was lost. The aircraft veered to the right, then to the left, however remained on the runway. There were no injuries, and the aircraft sustained minor damage to the landing gear fairings and doors.

A Bearskin Airlines Fairchild SA227-DC (C-GJVW/BLS345) from Thunder Bay, ON (CYQT) to Kenora, ON (CYQK) reported a gear indication and orbited the field while they ran through checklists and diagnostics. They declared an emergency after troubleshooting. The aircraft decided to continue to Winnipeg Intl, MB (CYWG) for landing. No further impact to operations.
Update TSB Report #A19C0014: C-GJVW, a Fairchild SA227-DC aircraft operated by Bearskin Airlines, was conducting flight BLS345 from Thunder Bay (CYQT), ON to Kenora (CYQK), ON with 2 crew members and 4 passengers on board. During the final approach into CYQK, the flight crew selected landing gear down, however the landing gear would not extend. The flight crew decided to divert the aircraft to Winnipeg/James Armstrong Richardson Intl (CYWG), MB, where a successful emergency/manual landing gear extension was completed. An emergency was declared, and the aircraft landed without further incident. An inspection of the system by the operators maintenance revealed a broken wire to the landing gear control valve.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Wiring was inspected at landing gear control valve at power pack assy, found broken wire to terminal strip #311. Repaired broken terminal ring at wire termination on pin 6 IAW WM 32-30-00. Landing gear functional checks carried out per AMM 32-00-00. No further faults found.

A North Central Aviation Pilatus PC-12/47 (NCJ96) from Minneapolis, MN (KANE) to Kenora, ON (CYQK) was handed off by the MSP controller at FL200 with descent to 8,000 at the pilots discretion. The Trout Lake Controller did not recognize that the MSP controller stated the aircraft was descending to 8,000 and did not enter it in Canadian Automated Air Traffic System (CAATS). The Trout Lake Controller handed off the position to the second Trout Lake Controller. The pilot commenced descent and was immediately queried by the second Trout Lake Controller. The aircraft was recleared to 8,000. No other aircraft involved.

No other impact to operations.

Incident at AMOS / MAGNY QC (CYEY)

Aviation Incident Report No. 15568: A privately registered Cessna R172K aircraft that was flying from Waskaganish (CYQK), QC to Amos/Magny (CYEY), QC, and which was executing touch-and-go landings on runway 32, executed a touch-and-go landing on Lake Beauchamp, located 0.6 nm west of the runway. While executing this manoeuvre, it cut off from its final approach to runway 32 a Beech 100 aircraft operated by Propair Inc. (C FWRM/PRO4450). The C-FWRM aircraft landed nonetheless, while avoiding the other aircraft. The Cessna R172K aircraft signalled its intentions, but did not receive clearance and realized that it was going to intercept the final approach of the C-FWRM aircraft; however, the manoeuvre appeared to be safe.

Incident at POPLAR HILL ON (CPV7) (Aerodrome, runway or taxiway shutdown)

A Superior Airways Cessna 208B (C-FYMK) called the Flight Information Centre (FIC) on Red Lake, ON (CYRL) remote communications outlet (RCO) to say that the company had an aircraft with a flat tire stuck on the runway at Poplar Hill, ON (CPV7). No contact made with airport staff or Ministry of Transportation. Superior Airways, the aircraft operator, was called and asked to provide updates as they happened, and a NOTAM closing the runway was issued with a until approximate time of 0200Z. Transport Canada (TC) operator from CPV7 called regarding NOTAM for runway closure. They said the aircraft was mostly off the runway, but the wing was still sticking out near the end of the runway. They wanted to cancel the runway closure so they could have the other superior airways aircraft land to bring the disable aircraft a spare tire. They did not want any other aircraft to land. Area Control Centre (ACC) was contacted to enquire as to what could be done since the runway could not be open for only one aircraft to land. ACC shift manager told the airport manager to contact Transport Canada. Spoke with Kenora, ON (CYQK) Flight Service Station (FSS) who informed that a Superior Airways Cessna 208 (C-GTBY) was inbound to CPV7 with an estimated time of arrival (ETA) of 2315Z. The pilot said they were aware of the runway closure, but that they were going to land to bring the aircraft a spare tire.
