A Pascan British Aerospace Jetstream Model 3212 (PSC485) from Montreal/St-Hubert, QC (CYHU) to Quebec/Jean Lesage, QC (CYQB) was cleared to 6000 feet and was observed descending through 5400 feet. Air traffic services (ATS) instructed the aircraft to stop its descent at 5000 feet and inquired. PSC485 concurred they were cleared to 6000 feet only. No impact on operations.

A Sky Jet M.G. Beech 200 (C-GCLQ/VSL447) from Quebec/Jean Lesage, QC (CYQB) to Quebec/Jean Lesage, QC (CYQB) made initial contact with the tower before entering the control zone (CZ). The tower tried again to communicate with the aircraft to give it landing clearance, but it did not receive a response. VSL447 was cleared to land through use of a continuous flashing green light. The aircraft cleared Runway 24 via Runway 11 instead of continuing straigh and taking Taxiway Hotel. Ground control flashed a green light and VSL447 taxied on Runway 11 up to Charlie.

Traffic information was transmitted to PSC582. Despite this, PSC582 initiated its descent. No impact on operations.

En route to CYZV, the aircraft made an emergency landing using its parachute: 510072N/0670097W. The emergency locator transmitter (ELT) signal went off. Joint Rescue Coordination Centre (JRCC) Trenton was notified. Thanks to the help of aircraft in the area, the crash site was located. There was one person on board, who was safe and sound.

The pilot changed the aircrafts heading for a few moments to avoid being blinded. According to him, the act seemed to be intentional. The shift manager and the air traffic operations specialist (ATOS) were notified at 0237Z, and the Sûrete du Québec (SQ) du Bas-Saint-Laurent was informed at 0242Z.

Accident at TROIS-RIVIÈRES QC (CYRQ) (Aerodrome, runway or taxiway shutdown)

The manager of the Trois-Rivières, QC (CYRQ) airport informed the Quebec flight information centre (FIC) that there had been a crash at the side of the runway. A Cargair Ltd. Piper PA-23-250 (C-GDUL) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) conducted a training approach followed by a runway excursion and caught fire. Firefighters were at the scene and intervened quickly. The airport was closed by NOTAM until 2000Z, and a Visajet Ltd. aircraft (VJT744) was redirected to Montreal/Trudeau, QC (CYUL).
TSB Update #A19Q0091: a Cargair Piper PA-23-250 Aztec (C-GDUL) was on a visual flight rules (VFR) night flight from Montreal/Saint-Hubert, QC (CYHU) with 1 pilot and 1 passenger on board. According to the flight plan that was filed, an approach with a pull-up was expected at Quebec/Jean Lesage, QC (CYQB) and Trois-Rivières, QC (CYRQ) before returning to CYHU. During the approach for CYRQ, the pilot modified his intentions and conducted a touch-and-go. At takeoff,the aircraft skidded to the left and crashed at the edge of the runway. The occupants were injured, but they managed to exit the aircraft quickly before being looked after by emergency services that had already arrived at the scene, at the request of another aircraft. A post-impact fire destroyed the aircraft.

En route from Québec/Jean Lesage, QC (CYQB) to Punta Cana, Dominican Republic (MDPC), an Air Transat Airbus A310 (TSC406) reported hydraulic problems to the Federal Aviation Administration (FAA) and asked to divert to Montreal/Pierre Elliott Trudeau, QC (CYUL) . No emergency declared; however, TSC406 had to be towed away to clear the runway. TSC406 landed on Runway 24R at 1803Z. At landing, TSC406 had a hydraulic fluid leak. Impact: Runway 06L/24R was unavailable from 1803Z to 1826Z. The National Operations Centre (NOC) was notified.

The controller saw that JZA8727 was initiating a left turn when he cleared SLQ739 for takeoff, but JZA8727 then turned right. The controller reminded JZA8727 to turn left as instructed.

A privately registered Piper PA-28R-200 from an unknown location to Quebec/Jean Lesage, QC (CYQB) tried to communicate with the tower twice with a transmission that the tower could not make out. After a scan for the final on Runway 29, the aircraft was seen 3 NM on final without the landing gear down, and it pulled up towards the northwest (NW). The pilot then left the zone and contacted, via cellphone, the CYQB flight information centre (FIC), which was passing information between the tower and the pilot for a return to CYQB and a landing on Runway 24 without any issues.
