Incident at 5NM N FLEUR intersection

Despite having been authorized and correctly reading back FLEUR direct CYGV, a Gouvernement du Québec Bombardier CL-600-2B16 (QUE10) on a MEDEVAC flight from Quebec/Jean Lesage Intl, QC (CYQB) to Havre St-Pierre, QC (CYGV) flew FLEUR direct ML. Air traffic services (ATS) corrected the situation. No impact to operations.

While 50NM East of Rouyn-Noranda, QC (CYUY), a Sky Jet MG Inc. Beech 200 (C-FSKQ/VSL443) from Quebec/Jean Lesage, QC (CYQB) to Rouyn-Noranda, QC (CYUY) declared an emergency due to a landing gear problem. Emergency vehicles were deployed. The aircraft landed at CYUY at 1339Z. The landing gear collapsed during the landing. The National Operations Centre (NOC), the Transportation Safety Board of Canada (TSB) and the Joint Rescue Coordination Center (JRCC) Trenton were notified. No injuries. Impact: Runway closed by NOTAM, 3 aircraft were diverted to another airport (an Exact Air Inc. aircraft (flight ET839), a Hydro-Quebec de Havilland DHC-8 (HYD207), and a Pascan aircraft (flight PSC366)). A privately registered Beech C-23 flew over the facilities instead of conducting a touch-and-go.

There was an error when entering a flight plan from a Canadian Helicopters Aerospatiale AS 350 B-2 (C-GLNK) from Mary River, NU (CMR2) to Mary River, NU (CMR2). The end date was supposed to be July 09, but it was June 29 that was entered. The flight plan was closed on June 29 during a call to the companys dispatcher, since the pilot was unable to be reached. It was on July 09, when the pilot contacted the Flight Information Centre (FIC) at Quebec/Jean Lesage, QC (CYQB) to close their flight plan, that it was noticed that there had been a loss of alert service.

Incident at ST-GEORGES QC (CYSG)

Aviation Incident Report # 16687: A privately registered Aero Commander 112TC took off from Runway 24 at Saint-Georges, QC (CYSG) on a flight to Quebec/Jean Lesage Intl, QC (CYQB). At 500 from ground level, while in the climb, the pilot noticed a privately registered, amateur-built, Vans RV-10 that was heading straight toward them. The pilot of the Aero Commander had to conduct an abrupt maneuver and turn left. The Vans RV-10 was not in communication and did not overfly the airport. Runway 24 was the active runway that day, and many aircraft were landing and taking off. The Vans RV-10 did not communicate their intentions and landed Runway 06. The pilot of the Vans RV-10 informed the Aero Commanders mechanic that they were on the wrong frequency.

A Glencore Canada Corporation Boeing 737-2R8C (C-FFAL/RAG100) from Quebec/Jean Lesage, QC (CYQB) to Kattiniq/Donaldson, QC (CTP9) experienced a problem with its #2 engine. The crew shut down the engine as a precaution and requested a return to CYQB airport and that emergency services be deployed. No impact on operations.
Update: TSB Report A20Q0070: C-FFAL, a Boeing 737-2R8C operated by Glencore Canada Corporation under the code RAG100, was flying under instrument flight rules from Quebec/Jean Lesage, QC (CYQB) to Kattiniq/Donaldson, QC (CTP9). After about 1.5 hours of In flight, the crew noticed a low indication of the oil pressure from engine #2. As a precaution, engine #2 was shut down and the crew advised the air traffic controller that they were making a U-turn to return to CYQB. Aircraft Rescue and Fire Fighting (ARFF) services were deployed at the request of the crew. The aircraft landed without incident. The maintenance department discovered that an oil line was broken.

Incident at 6NM QUÉBEC / JEAN LESAGE INTL QC (CYQB)

Following takeoff from Quebec/Jean Lesage, QC (CYQB), the pilot of a Government of Quebec Canadair CL600-2B16 (C-GURG/QUE10) on a MEDEVAC flight from Quebec/Jean Lesage, QC (CYQB) to Val-dOr, QC (CYVO) reported a problem with the landing gear system. The aircraft made the decision to return to CYQB, where vehicles had been deployed as a precaution. QUE10 landed at 0248Z without incident, no impact on operations.
UPDATE TSB Report #A20Q0069: C-GURG, a Canadair CL60 operated by the Quebec Government Air Service, which operated flight QUE10 under instrument flight rules, had just taken off from Quebec City/Jean-Lesage, QC (CYQB) for a flight to Val dOr, QC (CYVO). When selecting the landing gear in the retracted position, the nose wheel did not retract. The Quick Reference Manual (QRH) checklist had been completed and the landing gear was successfully deployed. The crew decided to return to CYQB and, as a precaution, requested that the Aircraft Rescue and Fire Fighting Services (ARFF) unit be deployed. The aircraft landed on the runway without incident. An inspection revealed the presence of a white cloth, which masked the gear position sensors.

Despite having been authorized and correctly re-reading 7000ft, an American, privately registered Cirrus SR22 from Quebec/Jean Lesage, QC (CYQB) to Kuujjuaq, QC (CYVP) continued to climb up to 7400ft before the intervention of air traffic control (ATC). Aircraft indicated autopilot trouble. No impact.

Diamond DA 42 M-NG (C-FMJM) from Quebec/Jean Lesage, QC (CYQB) to Chicoutimi/St-Honoré, QC (CYRC) advised of the loss of the right engine and declared an emergency. C-FMJM requested to be redirected to CYQB. Coordination performed. C-FMJM landed without further issue at 1901Z. The National Operations Center (NOC) and the Aviation Operations Center (AVOPS) were notified.
Update TSB Report #A20Q0063: C-FMJM, a Diamond DA42 operated by Rans Capital Air Inc., was flying according to the rules of instrument flight from Quebec/Jean Lesage, QC (CYQB) to Chicoutimi/St-Honoré, QC (CYRC) with 2 crew members on board. In cruise flight, at an altitude of 7000 feet, the crew heard two "bangs", immediately followed by the indications "ECU AFAIL" and "ECU B FAIL" for the right engine. The indicated temperature and pressure of the engine remained within operational limits. The crew declared an emergency and asked to return to CYQB. The aircraft landed safely at 1901 UTC. There were no injuries.

An Exact Air Beech A100 (C-FLTS/ET410) from Quebec/Jean Lesage Intl, QC (CYQB) to Val-dOr, QC (CYVO) requested to return to CYQB due to a fuel leak. During the descent toward Runway 06, ET410 declared PAN PAN and requested emergency services on the side of the runway. ET410 landed on Runway 06 at 1719Z without issue. No impact to operations.
Update TSB Report #A20Q0062: C-FLTS, a Beech A100 King Air type aircraft operated by Exact Air under the designator ET410, was flying under instrument flight rules from Quebec/Jean Lesage, QC (CYQB) to Val-dOr, QC (CYVO). While the aircraft was at an altitude of 8,000 feet uphill after departure, the crew observed a slight fuel leak from the left engine nacelle. The crew requested a return to CYQB due to a fuel leak. 15 nautical miles west of CYQB, during the descent for Runway 06, the crew declared a PAN PAN. The aircraft landed without problems on Runway 06 at 1719 UTC with the presence of the aircraft fire fighting and rescue unit (ARFF). The maintenance department discovered a leak via the nacelle probe.

An iFly College Inc. Diamond DA 20-C1 (C-FMIS) from Quebec/Jean Lesage Intl, QC (CYQB) to Quebec/Jean Lesage Intl, QC (CYQB) was conducting circuits and reported seeing fireworks in the left downwind for Runway 29. Police advised via the operations coordination centre (OCC) at the airport. The 3 aircraft that were conducting training were changed sides in the circuit. No other impact.
