Incident at Kelowna (Aerodrome, runway or taxiway shutdown, FOD (foreign object debris))

A WestJet Boeing 737-6CT (C-GWJT/WJA253) from Calgary, AB (CYYC) to Kelowna, BC (CYLW) reported a bird strike when short final for Runway 16. A Government Of Canada, Royal Canadian Mounted Police Pilatus PC-12 (C-GMPW) from Prince Albert (Glass Field), SK (CYPA) to Kelowna, BC (CYLW) was slowed and a Southern Interior Flight Centre Cessna 172S (C-GICG) from Kelowna, BC (CYLW) to Kelowna, BC (CYLW) was put in a hold for spacing to allow for a runway inspection. One bald eagle was found intact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The flight crew of WJA253 reported the bird strike on log page AK65711. Maintenance performed a bird strike inspection in accordance with (IAW) Aircraft Maintenance Manual (AMM) 05-51-18. No faults or damage found. Aircraft returned to service on log page AK65711.

At Prince Albert (Glass Field), SK (CYPA), an emergency locator transmitter (ELT) activation occurred for a duration of 5 minutes. The Joint Rescue Coordination Centre (JRCC) Trenton was notified. The ELT was traced to a Transwest Air Beech 200 (C-GFSG) that was undergoing maintenance at CYPA.

A Saskatchewan Public Safety Agency (Air Operations) Convair 340 (C-GSKQ/TKR475) on a flight from La Ronge (Barber Field), SK (CYVC) to La Ronge (Barber Field), SK (CYVC) reported an engine failure. The aircraft declared an emergency, and returned to CYVC. TKR475 landed safely at 0026Z with emergency services standing by. No impact on operations.
UPDATE TSB Report #A20C0031: C-GYSK, a Convair CV340/580 aircraft operated by the Province of Saskatchewan/Northern Air Operations, was conducting a local flight as TKR475 from Prince Albert/Glass Field (CYPA), SK with 2 crew members on board. While joining downwind in the circuit, the flight crew noted an issue with the right engine propeller. The flight crew actioned the checklist, shut down and secured the right engine, declared an emergency and returned to the airport. The aircraft landed safely with emergency services standing by. No impact on operations. The operators maintenance identified that the control valve of the propeller controller unit had failed. The control valve was replaced and the aircraft was returned to service.

A West Wind Aviation Beech 1900D (C-GWWK/WEW801) from Saskatoon/Diefenbaker, SK (CYXE) to Prince Albert (Glass Field), SK (CYPA) extended its downwind to allow time for C-GFZX to taxi off the runway. WEW801 was delayed by 3 minutes on their arrival. Foreign object debris (FOD) check completed by airport vehicle (ST31).

Three aircraft were being sequenced for Runway 27 at CYXE. A loss of separation occurred when less than required wake turbulence minima existed between the first aircraft, a Westjet Encore Ltd. de Havilland DHC-8-402 (C-GEEN/WEN3266) from Edmonton, AB (CYEG) to Saskatoon/Diefenbaker, SK (CYXE), and the second aircraft, a 1867829 Alberta Ltd. Pilatus PC-12/47 (C-FIPN) from Regina, SK (CYQR) to Saskatoon/Diefenbaker, SK (CYXE). The second aircraft was then vectored to follow the third aircraft, a Transwest Air Ltd. Saab 340B (C-GTJX) from Prince Albert (Glass Field), SK (CYPA) to Saskatoon/Diefenbaker, SK (CYXE), and a subsequent loss of surveillance separation occurred between the second and third aircraft.
UPDATE TSB Report #A19C0150: C-GEEN, a de Havilland DHC-8-402 operated by WestJet Encore as flight WEN3226 was conducting a flight from Edmonton Intl (CYEG), AB to Saskatoon/John G. Diefenbaker Intl (CYXE), SK. WEN3226 was cleared for the RNAV Y Runway 27. C-FIPN, a Pilatus PC 12/47 operated by 1867829 Alberta Ltd., was conducting a flight from Regina Intl (CYQR) SK to CYXE. C-FIPN was sequenced behind C-GEEN and was cleared for RNAV Z Runway 27 by Winnipeg Centre. Both aircraft were cleared for the approach and switched to CYXE tower frequency. C-GTJX, a Saab 340B operated by Transwest Air Ltd, was conducting a flight from Prince Albert/Glass Field (CYPA), SK to CYXE. C-GTJX was sequenced behind C-GEEN and C-FIPN for the RNAV Z Runway 27. During the approach, a loss of separation for wake turbulence occurred between C-GEEN and C-FIPN. The required separation was reduced to 2.9 nautical miles when the required separation is 4 nautical miles. C-FIPNs approach clearance was cancelled and C-FIPN was asked to contact Winnipeg Center. The controller cleared C-FIPN to turn right to a heading of 360 degrees, bringing the aircraft north of the approach path for Runway 27. The controller subsequently cleared C-FIPN to turn further right to a heading of 270 degrees. During the vectoring of C-FIPN, lateral separation between C-FIPN and C-GTJX was reduced to 2 nautical miles when 3 nautical miles in required. The controller cleared C-FIPN to turn to a heading of 360 degrees again, followed by another clearance to turn right to a heading of 270 degrees. The crew of C-FIPN asked for confirmation of the last clearance and the controller corrected the clearance to a right turn to a heading of 090 degrees. C-FIPN subsequently conducted the RNAV Z Runway 27 uneventfully. All aircraft landed safely in CYXE.

Assistance was offered and declined. One aircraft was affected and delayed approximately 3 minutes while a vehicle (ST31) completed a foreign object debris (FOD) check.

There was no bird strike, but they rejected the takeoff as a precaution. Clearance was cancelled and re-issued a few minutes later. C-GKCY departed with no other impact on operations a few minutes later.

The pilot of a J.A. Steyn Medical Professional Corporation Piper PA-28-180 (C-FZEQ) from Morden, MB (CJA3) to Prince Albert (Glass Field), SK (CYPA) contacted the flight service station (FSS) at 0405Z via cell phone to report they lost their radio and would be "no radio" (NORDO) for their arrival into CYPA. They arrived at 0425Z after completing an overhead right downwind onto Runway 26. A Transwest Airlines aircraft (TW281) was holding short on Taxiway Delta for 2 minutes during C-FZEQs arrival. C-FZEQ arrived safely, and TW281 departed without further delay.

Accident at PRINCE ALBERT (GLASS FIELD) SK (CYPA) (Aerodrome, runway or taxiway shutdown, FOD (foreign object debris))

The nose gear of a Province of Saskatchewan Gulfstream 695A (C-GOVT/BD167) from La Ronge (Barber Field), SK (CYVC) to Prince Albert (Glass Field), SK (CYPA) collapsed after exiting Runway 26 onto Taxiway D. NAO aircraft maintenance engineers (AMEs) removed BD167 from Taxiway D at 2238Z. A foreign object debris (FOD) inspection was completed and FOD was removed from the intersection of Runway 08/26 and Taxiway D. No impact on operations.
Update TSB #A19C0093: C-GOVT, a Rockwell Commander 695A operated by the Province of Saskatchewan as flight BD167, landed on runway 26 at Prince Albert/Glass Field (CYPA), SK with only the pilot on board. After turning off the runway onto taxi way Delta, the nose gear collapsed. The aircraft came to a sudden stop and the engines were then shut down. There were no injuries and the aircraft sustained substantial damage. The aircraft was removed from the taxi way and taken to the tanker base. The operator will advise of their findings.

Maintenance staff was notified but was unable to locate the animal. No impact to operations.

Another vehicle (ST31) was notified and chased the coyote east. BD162 landed safely.

C-GVCU entered Runway 08 to backtrack without contacting the tower and with an aircraft, a privately registered Cessna A185F from Prince Albert (Glass Field), SK (CYPA) to Regina, SK (CYQR), one mile final with landing clearance. Air traffic control (ATC) instructed the aircraft on final to pull up and go around. C-GVCU departed safely. No other impact on operations.

The Winnipeg, MB (YWG) flight information centre (FIC) phoned to advise of an inbound aircraft that was "no radio" (NORDO). The aircraft, a Skylines Advertising Inc. Cessna 182B (C-FKWV) from origin unknown to Prince Albert (Glass Field), SK (CYPA), landed on Runway 26 at 1926Z NORDO.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. It was reported that the aircrafts radio failed on a flight to Prince Albert. The pilot contacted the flight service station (FSS) on cell phone and they notified Prince Albert. Maintenance found faulty alternator which was replaced and the aircraft returned to service.

A Transwest Air Beech B200 (C-GTGP/TW02-M) from Prince Albert (Glass Field), SK (CYPA) to La Ronge (Barber Field), SK (CYVC) MEDEVAC taxiing for departure reported seeing a coyote near the threshold of Runway 26. The airport staff was notified. No impact to operations.

Air traffic control (ATC) was advised that a Transwest Air Ltd. Saab 340B (C-FJVW/TW102) from Prince Albert (Glass Field), SK (CYPA) to Prince Albert (Glass Field), SK (CYPA) was returning with an indication, no assistance required. TW102 landed safely on Runway 26.

Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA)

A Transwest Air Saab 340B (C-GTJX/TW280) from Saskatoon, SK (CYXE) to Prince Albert, SK (CYPA) executed a missed approach for Runway 26 due to geese in the approach path. TW280 joined left hand circuit for Runway 26 and landed safely at 2229Z. No impact on operations.

Incident at YORKTON MUNI SK (CYQV)

A Miccar Aerial Ltd Beech B200 (C-GGGQ/GT1128) from Prince Albert, SK (CYPA) to Yorkton, SK (CYQV) on area navigation (RNAV) 04 approach reported a precautionary engine shutdown 4 NM north of KEGTU at 2349Z. Emergency services notified. Aircraft landed without further incident at 2357Z and cleared the runway without assistance.
Update TSB #A19C0029: C-GGGQ, a B200 Beech King Air aircraft operated by Miccar Aerial (dba Good Spirit Air Service), was conducting flight GT1128 from Prince Albert (Glass Field) (CYPA), SK to Yorkton Muni (CYQV), SK with only the 2 pilots on board. During the descent through 12 000 feet approximately 40 nm north of CYQV, the flight crew noticed fluctuations in the fuel flow and gas generator speed (ng) for the left engine (Pratt & Whitney Canada PT6A-42). As the flight continued in the descent, the left engine parameters returned to normal, then followed by a dramatic engine rollback. The flight crew feathered the engine, noticed an increase in ITT, and performed a precautionary engine shutdown. The aircraft landed without further incident in CYQV with emergency services responding. The operator’s maintenance replaced the fuel control unit.

A Transwest Air Limited Saab 340B (C-FJVW/TW503) from Prince Albert, SK (CYPA) to Saskatoon, SK (CYXE) reported a dead rodent 700 ft from the threshold of Runway 26. The airport staff removed the remains. No impact to operations.

Incident at PRINCE ALBERT (GLASS FIELD) SK (CYPA) (Aerodrome, runway or taxiway shutdown)

A Province of Saskatchewan Convair 340 (C-GYSK/TKR474) from La Ronge, SK (CYVC) to Prince Albert, SK (CYPA) inbound for Runway 26 declared an emergency and requested standby due to engine failure following a touch and go at 1725Z. 911 and all pertinent parties notified. Aircraft elected to circle to the east and wait for emergency services arrival. Aircraft landed safely at 1742Z and exited the runway with standby on site. A Transwest Air Limited Partnership Beech 1900D (C-GTWG) delayed approximately 5 minutes for a foreign object debris (FOD) check by ST31.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The Person Responsible for Maintenance (PRM) stated that the aircraft experienced a propeller overspeed on the left-hand (L/H) engine. The propeller was feathered/engine shut down and the aircraft returned to Prince Albert (Glass Field), SK (CYPA). Maintenance crews investigated the problem and found the propeller dome seals had twisted and were cut by the retaining groove for the seals. Propeller fluid was bypassing in the dome, thus the propeller was unable to govern properly. Maintenance replaced the propeller dome seals and the aircraft was returned to service. Service Difficulty Report (SDR) was submitted on April 22, 2019.
Update TSB #A19C0037: C-GYSK, a Convair CV340/580 aircraft operated by the Province of Saskatchewan/Northern Air Operations, was conducting a local flight as TKR474 from Prince Albert (Glass Field) (CYPA), SK with 3 crew members on board. During the initial climb after the takeoff, the flight crew noted an issue with the number 1 engine propeller (Hamilton Standard 54H60-77), and decided to do a precautionary engine (Allison 501-D22G) shutdown. The flight crew declared an emergency, and returned to CYPA where the aircraft landed without further incident. An inspection of the propeller system by the operators maintenance revealed efective O-rings inside the propeller hub. The operator has filed a Service Difficulty Report (SDR) with Transport Canada.
