Incident at TORONTO / OSHAWA EXECUTIVE AIRPORT ON (CYOO)

A privately registered Cessna 150M from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) that was "transmit only" attempted to contact the tower multiple times, but was unable to receive. The pilot assumed that mandatory frequency (MF) procedures were in place despite the automatic terminal information service (ATIS). The aircraft proceeded to take-off from Runway 23 without clearance. The tower attempted to communicate with light gun signals, with no success.

At Toronto/Oshawa, ON (CYOO), a privately registered Cessna 150K from Toronto/Oshawa, ON (CYOO) to St-Jean, QC (CYJN) was observed taxiing on Taxiway A without clearance. The aircraft was instructed to stop. After obtaining the aircrafts intentions, the ground control instructed the aircraft to give way to a taxiing Toronto Airways Inc. Cessna 172S (C-GIST) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO). The aircraft acknowledged the traffic and proceeded to taxi towards the Cessna 172S. The aircraft was instructed to stop again and then indicated that they did not understand.

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: An aircraft on a flight from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) experienced a rough-running engine upon exiting the active runway. The pilot shutdown the engine and, while waiting for a tow, noticed a small fuel leak from the engine. CYOO staff were advised and the fuel was removed. No operational impact.

A privately registered Cirrus SR22 from Toronto/Oshawa, ON (CYOO) to Kitchener/Waterloo ON (CYKF) departed Runway 30 and reported a bird strike on climb out. The aircraft circled for the circuit and came back in to land. The aircraft taxied in with no issues. A foreign object debris (FOD) check of the runway was completed, and the bird (sparrow) was retrieved. No impact on operations.

Two Toronto Airways Inc. Cessna 172S aircraft, C-GBLQ and C-FGAR, both on a flight from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO), conducted a go-around on approach to Runway 23 due to birds in the vicinity of the threshold. The birds had moved on by the time a Cessna 182 landed shortly after. Both C-GBLQ and C-FGAR joined the circuit and landed soon after, without incident. No other impact to operations.

A Toronto Airways Inc. Cessna 172S (C-GFFQ) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) entered the CYOO control zone without communication at 2400ft, proceeding through the arrival path for Runway 23 at approximately 4.5NM final. A privately registered Beech S35 from Kingston, ON (CYGK) to Toronto/Oshawa, ON (CYOO), 6NM final at 2700ft, was advised to stop its descent until visual separation was established. C-GFFQ turned back Northbound, vacating the zone without communication. The Beech S35 landed without further incident.

A Durham Flight Centre Inc. Piper PA-23-250 (C-FHLI) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) was given a right turn out departing from Runway 12 due an aircraft (PTS2) conducting aerobatic work 6 miles straight out. The tower controller observed C-FHLI not making the turn, and when queried, the pilot said they were "turning now". C-FHLI did not turn and continued on the runway heading, flying through the area PTS2 was operating in. Visual separation was established and the aircraft switched to en-route frequency.

A Bell 206 helicopter (AIR1) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) was orbiting 3NM Southwest (SW) from CYOO at 2000ft above sea level (ASL). An Island Air Flight School & Charters Inc. Cessna 150M (C-GFDH) from Toronto/Billy Bishop, ON (CYTZ) to Toronto/Oshawa, ON (CYOO) was inbound from the SW to land at CYOO, and was instructed to maintain 2500ft ASL due to AIR1. Traffic was passed to both aircraft. Once they both had each other in sight, C-GFDH was instructed to wait until they were past AIR1 before descending to circuit altitude. A short time later, AIR1 complained that C-GFDH had not passed them yet, but had begun its descent. C-GFDH was then instructed to climb back up to 2500ft ASL.

A 2652662 Ontario Limited Diamond DA 40NG (C-GDBE) from Toronto/Oshawa, ON (CYOO) to Barrie-Orillia/Lake Simcoe, ON (CYLS) was departing from Runway 12, Northbound. Mutual traffic information was passed. FSY705 reported traffic on the traffic collision and avoidance system (TCAS). The DA40 reported traffic in sight and was instructed to pass behind. FSY705 climbed in response to a resolution advisory (RA) at approximately the same time that the DA40 was passing behind them.

An American, corporately registered Cessna 525A (N21RA) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) reported a fuel pressure issue shortly after departure. The pilot switched off the right engine (1 of 2) and requested an emergency landing. Fire trucks were called, and arrived approximately 1 minute before the aircrafts safe landing. Aircraft taxied in under its own power without issue. Fire trucks stood down. All appropriate agencies were notified. Minimal impact on operations.
Update TSB Report #A20O0051: The pilot of a corporately registered Cessna 525A (registration N21RA) on route from Oshawa, ON (CYOO) to Sault Ste Marie, ON (CYAM) declared an emergency shortly after departure due to a fuel pressure issue. The pilot shutdown the right engine (Williams, FJ-44), returned to Oshawa and landed without incident.

Pilot indicated they filed through the collaborative Flight Planning Services (CFPS), but perhaps spotty cell service was the reason it did not process. Earlier FP from CYOO to CNK4 had been entered in the system without issue.

The pilot of a Durham Flight Centre Inc. Cessna 152 (C-GGFX) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) contacted the Toronto/Billy Bishop, ON (CYTZ) tower indicating that they had entered the CYTZ control zone from the East to fly over the downtown but did not have a transponder that worked. The pilot was told to leave the control zone as they had not been given authorization to enter the control zone. The pilot then reported that they were still flying towards the downtown and wanted to sightsee, at which point the pilot was again told to exit the zone. No impact to operations. East satellite handed off a Brampton Flying Club Cessna 172R (C-GDEQ) that was inbound to Toronto/Oshawa, ON (CYOO) with oil issues. Aircraft Rescue and Fire Fighting (ARFF) called. Aircraft landed on Runway 30 and taxied in safely. ARFF released. Three aircraft were delayed for 5 minutes for taxi until aircraft landed safely. Three aircraft departed, and three aircraft taxied to active at their discretion prior to runway inspection.
