Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX)

A United Parcel Service Airbus A300-600F (UPS463) from Montreal/Mirabel, QC (CYMX) to Louisville, KY (KSDF) departed from CYMX Runway 06 at 1314Z. On climb, at about 1316Z, the flight crew reported that they had an issue, wanted to run a few checklists and return to CYMX. When queried as to the source of the issue, the flight crew indicated that they had a cargo door warning light that was on. The flight landed safely without incident at 1337Z. No emergency was declared and there was no impact on other traffic.

A Morningstar Air Express AT72 (MAL7069) from Quebec/Jean Lesage, QC (CYQB) to Montreal/Mirabel, QC (CYMX) indicated that it had seen a fox on Runway 06/24 near B6. Aéroports de Montréal (ADM) was notified. A Cargair Cessna 172S (C-FDDC) conducted a low-altitude approach and then pulled up while a vehicle (RED16) was conducting an inspection and attempting to scare away the fox, which was not seen again by RED16.

At departure from Runway 06, a Nolinor Aviation Boeing 737-200 (NRL942) from Montreal/Mirabel, QC (CYMX) to Val-dOr, QC (CYVO) declared a PAN PAN due to a landing gear problem and requested to return. The PAN PAN was cancelled once the aircraft was established on final for Runway 06. Aircraft Rescue and Fire Fighting (ARFF) services were notified as a precaution. No impact on operations.

The aircraft was observed departing Runway 06 instead while an Académie Aéronautique Cessna 172N (C-GYBV) on a flight from Montreal/Mirabel Intl, QC (CYMX) to Montreal/Mirabel Intl, QC (CYMX) was turning base to final Runway 24. Impact: C-GYBV was asked to alter its circuit to avoid conflict with the departing C-GIGW.
UPDATE AOR 260767-V2: An Académie Aéronautique Cessna 172N (C-GIGW) on a flight from Montreal/Mirabel Intl, QC (CYMX) to Alexandria, ON (CNS4) had received advisory for Runway 24 but taxied short of Runway 06 on KILO. The aircraft called for departure Runway 06 which was not caught by air traffic services (ATS) while an Académie Aéronautique Cessna 172N (C-GYBV) on a flight from Montreal/Mirabel Intl, QC (CYMX) to Montreal/Mirabel Intl, QC (CYMX) was turning base to final Runway 24. Impact: C-GYBV asked to alter circuit to avoid conflict with the departing C-GIGW.

A Morningstar Air Express Boeing 757-2B7 (C-FMOC/MAL7054) from Greater Moncton/Romeo LeBlanc, NB (CYQM) to Montreal/Mirabel, QC (CYMX) had to reject its approach and fly holding patterns, and a Nolinor Aviation Boeing 737-200 (NRL55) from Montreal/Trudeau, QC (CYUL) to Montreal/Mirabel, QC (CYMX) flew holding patterns while the animal was being removed. Runway 06/24 was closed from 0143Z to 0158Z.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX)

During ground-air transfer (from 121.8 to 119.1), an Académie aéronautique Cessna 172N (C-GIGW) was instructed to hold short of Runway 24 on B6. There was a loss of communication. The aircraft crossed the stopline. The flight service station (FSS) was not able to communicate with it. Aéroports de Montréal (ADM) and the terminal were notified. Air traffic services (ATS) instructed C-GIGW by telephone to clear the runway after a 10-minute delay. A Bombardier aircraft (CDR626), an Académie aéronautique Piper PA-34-200 (C-GTXG), and several Cessnas were delayed. Operations returned to normal afterwards.

There was a power outage followed by a generator failure starting at 0032Z. The technical operations coordinator (TOC), the terminal, and Aéroports de Montréal (ADM) were notified. The TOC specified that the batteries would work for 2 hours. At 0232Z, there was a total failure of equipment in the tower. The TOC indicated that all the necessary NOTAMs had been issued. The flight information centre (FIC) was advised; partial weather forecasts via telephone. Power was restored thanks to the generator at 1100Z on November 4, 2019.

Due to the use of Roberval, QC (CYRJ) frequency 122.2, a private pilot operating a Hughes 369D (C-GSZN) on a flight from Montreal, QC (CYMX) to Alma, QC (CYTF) crossed the CYTF mandatory frequency (MF) zone on the wrong frequency. Unknown impact at CYTF.
UPDATE: AOR-256847-V4: The private pilot of a Hughes 369D (C-GSZN) on a flight from Montreal, QC (CYMX) to Alma, QC (CYTF) was unreachable 5 minutes after it had called again on final. The flight service station (FSS) contacted airport operations at Roberval, QC (CYRJ) so that it could verify that the aircraft was on the ground. Airport operations did not find it. The FSS notified the La Tuque sector of the Montreal area control centre (CZUL ACC), the technical operations coordinator (TOC), the FSS manager, the CYRJ weather observer, the police, and the shift manager in order to locate the aircraft. The aircraft was found on the ground at CYTF. When the recording was checked, it revealed that the pilot said Alma universal communications (UNICOM) during initial contact and that the FSS responded Rouyn radio. The supervisor listened to the audio and the pilot had in fact communicated with Alma UNICOM during initial contact.
