A Pascan British Aerospace Jetstream 3212 (C-FPSC/PSC582) from Quebec/Jean Lesage, QC (CYQB) to Montreal/Saint-Hubert, QC (CYHU) was 2 NM on final for Runway 06L. A Cargair Cessna 152 (C-FRUC) from Montreal/Saint-Hubert, QC (CYHU) to Montreal/Saint-Hubert, QC (CYHU) was on base for Runway 06R. Air traffic services (ATS) passed the traffic information along to the two aircraft. Despite this, PSC582 received a traffic alert and collision avoidance system resolution advisory (TCAS RA) and decided to pull up. No loss of separation and no other impact on operations.

Incident at 7NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Pascan British Aerospace Jetstream Model, 3212 (PSC837) from Montreal/Saint-Hubert, QC (CYHU) to Rouyn-Noranda, QC (CYUY) received and read back instruction to climb to 7 000 ft, but was seen at 7 400 ft. When questioned by air traffic services (ATS), PSC837 indicated it was at 7 140 ft. No impact on operations.

Incident at ST-JEAN QC (CYJN)

A Cargair Cessna 172N (C-GVHE) from Montreal/Saint-Hubert, QC (CYHU) to Montreal/Saint-Hubert, QC (CYHU) declared MAYDAY after a cabin fire at 1928Z. Emergency services notified. C-GVHE landed without incident on Runway 11 at 1931Z and cleared the runway at the threshold of Runway 02.

A 9206-7685 Québec Inc. Robinson R66 (C-GZJD) on a flight to Montreal/St-Hubert Heli (Héli), QC (CTG2) received and read back instruction to cross the two Runway 24s, mid-runway at 600 ft. The aircraft was seen crossing lower than 600 ft. When questioned, the aircraft confirmed that its altitude was 400 ft. No impact on operations.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

An Orizon Aviation Québec Cessna 152 (C-GJBF) from Quebec/Jean Lesage, QC (CYQB) to Quebec/Jean Lesage, QC (CYQB) on final for Runway 24 stopped responding to communications. The aircraft conducted a pull-up despite a green light sent by the tower. C-GJBF then squawked 7600, a green light was sent to it, and it landed. A Cargair Ltd. Piper PA-23-250 (C-GARI) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) on area navigation (RNAV) approach 24 had to conduct a pull-up to avoid conflict with C-GJBF.

Incident at MONTRÉAL / ST-HUBERT QC (CYHU) (Declared emergency/priority)

The pilot of a Cargair Ltd. Cessna 152 (C-GAUB) from Montreal/Saint-Hubert, QC (CYHU) to Montreal/Saint-Hubert, QC (CYHU) in the Runway 24R circuit notified the tower controller that it was experiencing difficulties with its engine and that it was declaring an emergency. The tower thus gave it priority and the aircraft landed without incident on Runway 24R. It was then able to taxi to the club on its own. Emergency services were notified and cancelled during the same communication due to the short window between the beginning and end of the emergency. The tower had to extend an aircraft in the Runway 24R circuit and delay a departure from Runway 24R.

A Heli Source Ltd. Hughes 369D (C-GTNM) from Montreal/St-Hubert, QC (CYHU) to Quebec/Jean Lesage, QC (CYQB) taking off from Runway 06L on standard instrument departure (SID) 6 made a left turn instead of maintaining the runway heading and levelled off at 1 100 ft instead of climbing to 3 000 ft. Incident at MONTRÉAL / ST-HUBERT QC (CYHU) (Aerodrome - power failure, ATM - inaccurate aeronautical information)

A vehicle (ST23) reported a power outage at Montreal/Saint-Hubert, QC (CYHU). At 0425Z, the generator was "lit up". The flight service station (FSS) at CYUY could not transmit on frequency 118.4 for a few seconds. ST23 then reported that during subsequent attempts, power was intermittent. Impact unknown.

An emergency was not declared and no assistance was requested. The aircraft landed without incident at 0434Z. No impact on operations.

A WestJet Encore de Havilland DHC-8-402 (C-GWRE/WEN3513) from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Toronto/Lester B. Pearson Intl, ON (CYYZ) declared an emergency due to a landing gear issue and requested to return to CYUL. No impact on operations. Emergency vehicles were deployed. The aircraft landed without incident at 1220Z. A runway inspection was performed, and nothing unusual was found. Impact on operations: a Pascan MEDEVAC flight (PSC366) was delayed for 6 minutes at departure from Montreal/St-Hubert, QC (CYHU).
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Nose Landing Gear (NLG) doors not closing after. Nose gear door lock proximity sensor rigged in accordance with (IAW) Aircraft Maintenance Manual (AMM) 32-61-06. Returned to service.

Update: TSB Report #A19Q0189: C-GARI, a Cargair Ltd. Piper PA-23-250, was on a local VFR training flight from Montreal/St-Hubert, QC (CYHU) with an instructor and a student on board. While in cruising flight, a vibration was felt and smoke appeared behind the left engine, followed by an oil leak on the engine cowling. The engine (Avco Lycoming IO-540-C4B5) then made a very loud noise, and the instructor turned it off and feathered the propeller. The pilot declared a state of emergency, went back to CYHU, and landed on Runway 24R without incident. There were no injuries or damage.

The terminal control unit (TCU) indicated that a Chrono Jet Boeing 737-2T4 (C-FYPN/MBK791) from Iqaluit, NU (CYFB) to Montreal/Saint Hubert, QC (CYHU) had one of its tires explode at takeoff from CYFB and requested the presence of firefighters at the airport. Longueuil Police were notified. The aircraft landed without incident at 0724Z. Impact: A NOTAM was issued concerning the closure of Runway 06L/24R until 0930Z. (reference: AOR 258514-V1)
Update: TSB Report #A19Q0188: C-FYPN, a Chrono Aviation Inc. Boeing 737-200, was on an IFR flight from Iqaluit, NU (CYFB) to Montreal/St-Hubert, QC (CYHU) with 88 passengers and 5 crew members. During takeoff, the aircraft blew one of the 2 tires in the left main gear. The crew decided to continue the flight to its destination and during the approach, it requested the presence of Aircraft Rescue and Fire Fighting (ARFF) services. The aircraft landed on Runway 24R without incident; minor damage was noted, and there were no injuries.

While landing at CYVO, the aircraft made an excursion on the side of the runway after blowing the right tire. The aircraft had minor damage. No one was injured.

Aviation incident report #16354: The pilot of a 9394-2985 Québec Inc. Beech G36 (C-GLDI) from Montreal/Saint-Hubert, QC (CYHU) conducted a direct approach on long final Runway 02 at St-Hyacinthe, QC (CSU3), despite the fact that another pilot was already flying in the circuit: an Académie de lAviation Saint-Hyacinthe Cessna 172M (C-GAXD), which had mentioned that it was an aerodrome traffic frequency (ATF) area. Two aircraft, C-GAXD and an Aéroclub de Montréal Cessna 150M (C-GLVV) were instructed to extend the downwind to eliminate any chance of a conflict.

Incident at BROMONT QC (CZBM)

Aviation Incident Report #16328: The pilot of an Aéroclub de Montréal Cessna 150M (C-GXBR) from Montreal/Saint-Hubert, QC (CYHU) to Bromont, QC (CZBM) communicated with CZBM universal communications (UNICOM) to ask if it could come and conduct touch-and-go landings. The aircraft confirmed that the runway in use was the right-hand circuit of Runway 23. However, the aircraft entered the circuit the wrong way, which created a probable risk of collision. An Eid Air Aviation Cessna 172K (C-FESL) from Bromont, QC (CZBM) to Bromont, QC (CZBM) attempted to communicate, but there was no response. After a few attempts to communicate with C-GXBR, the aircraft confirmed that it knew that the runway in use was Runway 23, but it was still going the wrong way. I told it again that it was not flying in the right direction but was met with silence on the radio. It changed its heading to pass over Runway 05 (still going the wrong way in the circuit) and to change back to a northbound heading and come back 10 minutes later on long final for Runway 23 for touch-and-go landings. Fearing for his safety, the pilot of a Cessna 172M (C-FEID) landed and ended his touch-and-go training. C-GXBRs operator was contacted.
