Shortly after departure, a Cargojet Airways Boeing 767-323 (C-GXAJ/CJT980) from Hamilton, ON (CYHM) to Covington/Cincinnati, KY (KCVG) advised of a nose wheel issue which required a return to CYHM, and declared "PAN PAN". Incident at 90NM HAMILTON ON (CYHM)

A 2166715 Ontario Inc. Aerostar 601P (C-FDBN) on a flight to Hamilton, ON (CYHM) advised air traffic control (ATC) that it was having electrical issues. C-FDBN then squawked 7600 due to a radio failure. The aircrafts communication and transponder were both intermittent. When communication and radar identification were established, C-FBDN declared an emergency. C-FBDN landed without further incident at CYHM at 2156Z. No impact to operations.

The Cleveland Air Route Traffic Control Centre (ZOB) advised that an American, corporately registered Learjet 60 (N711SE) from White Plains/Westchester, NY (KHPN) to Green Bay, WI (KGRB) was "no radio" (NORDO) and was not responding to radio calls. The aircraft transited, NORDO, through the Toronto (YZ) flight information region (FIR). Attempts to contact the aircraft were unsuccessful, and the aircraft was subsequently handed off to ZOB. The aircraft remained on the flight planned route. No impact on operations.

Incident at HAMILTON ON (CYHM) (Incursion - runway - animal, Aerodrome, runway or taxiway shutdown)

As a Skylink Express Inc. Beech 1900C (C-GSKM/SLQ782) from Hamilton, ON (CYHM) to Kingston, ON (CYGK) lined up at the threshold of Runway 24 for departure, 2 coyotes were spotted by the tower at the intersection of Runway 24 and Runway 30. The coyotes were crossing to the northwest side of the field. Maintenance was called to chase the coyotes away. The runway was then clear for the aircrafts departure. The total delay was about 05 minutes. No other operational impacts.

Incident at HAMILTON ON (CYHM) (ATM - inaccurate aeronautical information)

At Hamilton, ON (CYHM), upon coming out of the CAT II configuration, after turning the generator off, the tower lost the ability to transmit on both the ground and tower frequencies. The receiving function was still operational. Backup frequency was operational for the tower and ground frequency was operational via portable radio. No operational impact.

At approximately 1300Z, after landing, the ground crew of a Federal Express Corporation Airbus A300-600F (N664FE/FDX152) from Indianapolis, IN (KIND) to Montreal/Mirabel, QC (CYMX) called the tower to advise that an access panel door was missing. Runway 06/24 was closed for inspection. A "safety pin" was found on the runway. A Cargojet Airways Boeing 757-223 (C-GCJT/CJT592) from Montreal/Mirabel, QC (CYMX) to Hamilton, ON (CYHM) had a departure delay of approximately 10 minutes. 3 aircraft landed between the landing of FDX152 and the closure of the runway following the report of possible foreign object debris (FOD).

Smoke was observed from the left engine of a Sunwing Airlines Boeing 737-86J (C-GOWG/SWG9012) on departure from Montreal/Pierre Elliott Trudeau, QC (CYUL) for Hamilton, ON (CYHM). The pilot was advised by the terminal control unit (TCU). Twenty minutes later, a fuel spill was observed on Taxiway A up to Runway 24L. An inspection confirmed that the spill was from SWG9012 at the gate. The Montreal shift manager (GDQ) was notified. No impact to operations.

At takeoff, there was a loss of separation for wake turbulence between an Air Canada Airbus A320-211 (C-FPWE/ACA2339) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Hamilton, ON (CYHM) and an Air Transat Airbus A330-342 (C-GKTS/TSC398) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Punta Cana, Dominican Republic (MDPC). Separation was reduced to 4.1 NM, while the standard was 5 NM. No other impact on operations.
UPDATE: TSB Report #A20Q0010: C-FPWE, an Air Canada Airbus A320 (ACA2339), was on an instrument flight rules (IFR) flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Hamilton/John C. Munro Int’l Airport, ON (CYHM). It conducted its takeoff from Runway 24L after C-GKTS, an Air Transat Airbus 330 (TSC398), which was on an IFR flight from CYUL to Punta Cana International Airport, Dominican Republic (MDPC). A loss of wake turbulence separation occurred during the initial climb, when ACA2339 was 4.1 nautical miles (NM) behind TSC398, even though the required separation was 5 NM. No wake turbulence was reported, and the 2 aircraft continued their flights to their respective destinations without incident.

A Swoop Inc. Boeing 737-8CT (WSW107) from Hamilton, ON (CYHM) to Abbotsford, BC (CYXX) hit a bird on landing.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. A bird strike occurred during touchdown of WSW107. No engine parameter fluctuations. Engine #1 affected by bird debris. Bird strike inspection carried out, no faults found (NFF). Debris in Engine #1 was cleaned. A borescope inspection was carried out, with no faults found.

Incident at VANCOUVER INTL BC (CYVR)

A Cargojet Airways Ltd. Boeing 767-316 (C-GOCJ/CJT584) from Vancouver, BC (CYVR) to Hamilton, ON (CYHM) reported a hydraulic issue and returned to CYVR.
UPDATE: TSB Report #A20P0104: C-GOCJ, a Cargojet Airways Ltd. Boeing 767-316, was operating as flight CJT584 from Vancouver, BC (CYVR) to Hamilton, ON (CYHM), with 2 crew members on board. Shortly after takeoff, an engine-indicating and crew-alerting system (EICAS) caution alert "R HYD SYS PRESS" occurred, with the associated "SYS PRESS" light illuminated. An en route climb was established, the appropriate Quick Reference Handbook (QRH) checklist was actioned and the "SYS PRESS" light remained illuminated. The QRH checklist required the right hydraulic system to be disabled. The crew declared a "PAN PAN" and requested a return to CYVR. No fuel dump was required, as the aircraft was projected to be within 2000 pounds of the maximum certified landing weight. The aircraft landed without further incident. Emergency vehicles inspected the aircraft after landing and no hydraulic leaks were found. Maintenance tested the hydraulic system and verified that the defect was only an indication issue. The applicable hydraulic pressure switch was replaced, an overweight landing inspection was completed, and the aircraft was returned to service.

The pilot of a Great Lakes Helicopter Corp. Robinson R44 II (C-GPEV) from Buffalo, NY (KBUF) to Hamilton, ON (CYHM) called to close its transborder VFR flight plan. No flight plan was on file. A call to the Leidos flight service station (FSS) revealed that the pilot did file the flight plan, but it was never activated, despite a request from the pilot to assume the flight plan was open at the proposed time.

Incident at WINDSOR ON (CYQG) (ATM - operations)

While the control tower was closed by NOTAM for a mandatory break, a controller witnessed a Journey Air Diamond DA 20-C1 (C-GDME) from Windsor, ON (CYQG) to Windsor, ON (CYQG) taxiing and departing from Runway 25, with a Skylink Express Beech 1900C (SLQ734) from Hamilton, ON (CYHM) to Windsor, ON (CYQG) on final. C-GDME became airborne when the inbound traffic was about 4 miles final. No communications were made by C-GDME on the mandatory frequency. When the tower opened at 1330Z, the controller attempted to make contact with the aircraft twice while the aircraft was in the circuit for Runway 25. Communication was finally established with C-GDME when the aircraft was on final.

The Global Positioning System (GPS) unit (Garmin 430W) indicated that a Near North Flight Academy Cessna 172M (C-GDXE) from Hamilton, ON (CYHM) to Hamilton, ON (CYHM) was over the OMTED waypoint when the aircraft was conducting a hold on the T-Handle. The Centre noted that the aircraft was 4 miles North of the waypoint. The aircraft requested vectors for the instrument landing system (ILS) and continued the training flight using traditional means of navigation.

A Waterloo-Wellington Flying Club Cessna 172S (C-FATP) from Kitchener/Waterloo, ON (CYKF) to Kitchener/Waterloo, ON (CYKF) was cleared for a mid-right downwind for Runway 26, but instead joined the base leg, cutting off simulated approach traffic, a Brampton Flying Club Cessna 172S (C-GBNG) from Brampton-Caledon, ON (CNC3) to Hamilton, ON (CYHM), that was on final. C-GBNG had to execute evasive manoeuvres to avoid C-FATP.
UPDATE: Aviation Incident Report #16841: The Kitchener/Waterloo, ON (CYKF) tower cleared a Waterloo-Wellington Flying Club Cessna 172S (C-FATP) for a right downwind for Runway 26. Once established, C-FATP called when right downwind for Runway 26, and the tower told C-FATP to remain on the downwind, as the tower would call the base. C-FATP acknowledged the transmission from the tower and then proceeded to conduct a right turn base without clearance. The tower contacted C-FATP and asked if they had "the Cessna." C-FATP responded that they were looking for the traffic. C-FATP turned final very close in front of another aircraft, a Brampton Flying Club Cessna 172S (C-GBNG) on final. C-GBNG reported to the tower that they had conducted a collision avoidance turn to avoid C-FATP. C-GBNG was observed executing a significant pitch up and roll to the left to avoid C-FATP. C-GBNG requested and was then cleared to the south, out of the control zone. C-FATP continued on final and was cleared to land. Operational impact: occurrence of a near miss between the 2 aircraft.
