Incident at LONDON ON (CYXU)

A privately registered Cessna P210N, on a flight from London, ON (CYXU) to Geraldton (Greenstone Regional), ON (CYGQ) was observed turning right after departure Runway 33. Tower instructed the aircraft to rerun to Runway heading as per the CYXU standard instrument departure (SID). The aircraft complied and contacted Toronto (YYZ) Area Control Centre (ACC) leaving 2500 as instructed by tower.

Incident at WINDSOR ON (CYQG)

A privately registered Mooney M20J from Windsor, ON (CYGQ) to Windsor, ON (CYGQ) on final for Runway 07 reported possible gear issue. The aircraft did a low pass over Runway 12 to allow the tower to view the gear. The tower advised the gear appeared to be down. The pilot requested emergency vehicles on standby. Vehicles red1 and red2 responded. The aircraft landed without incident. No impact to operations.

Incident at OTTAWA / ROCKCLIFFE ON (CYRO)

A privately registered Vans RV 7 from Ottawa/Rockcliffe, ON (CYOO) to Geraldton, ON (CYGQ) set off its emergency locator transmitter (ELT) by mistake. Joint Rescue Coordination Centre (JRCC) Trenton was notified.

The pilot of an American, corporately registered Piper PA-18-150 (N744CC) on a transborder flight from origin unknown to Geraldton/Greenstone, ON (CYGQ) called the London (XU) flight information centre (FIC) on the Gore Bay-Manitoulin, ON (CYZE) remote communications outlet (RCO) while inbound and asked to close their VFR flight plan. No information on file. Checked with the United States Flight Service, and they did not have any information.

The pilot of a Chrono Aviation Inc. Beech 1900D (C-GXCA/NDL191) filed two flight plans. The first indicated a departure from Timmins/Victor M. Power, ON (CYTS) to Geraldton (Greestone Regional), ON (CYGQ), and the second indicated a departure from Geraldton (Greenstrone), ON (CYGQ) to Dryden Regional, ON (CYHD). The specialist who took the information wrote CYGK as the destination for the first flight plan and CYGK as the departure for the second. No impact on operations.

The pilot contacted London's Flight Information Centre (FIC) (CYXU), ON, to close his flight plan once on the ground at CYGQ.

The air traffic controller (ATC) gave a heading to C-GQNJ and the aircraft continued its flight without further incident (WFI). No impact on operations.

Incident at SIOUX LOOKOUT ON (CYXL)

Update TSB Report #A18C0053: C-GOJI, an Air Tractor AT402B aircraft operated by Fly On Ag Services Inc., was conducting a flight from Geraldton, ON (CYGQ) to Sioux Lookout, ON (CYXL). During the landing on Runway 34 directional control was lost. The aircraft exited the runway to the west, north of Taxiway A. The aircraft re-entered the runway and taxied to the apron. The winds were reported to be 260 degrees at 14 knots gusting to 27 knots. The tailwheel alignment required adjustment after a subsequent flight.
A Fly On AG Services Inc. Air Tractor AT -402B (C-GOJI) from Geraldton/Greenstone Regional, ON (CYGQ) to Sioux Lookout, ON (CYXL) landed Runway 34, with wind direction indicator at 260 degrees at 14G27. During landing roll, the aircraft exited runway on west side north of Taxiway "A". The aircraft was able to re-enter runway under its own power. No damage to aircraft. Staff Foreign object debris (FOD) check at 1830Z. No objects found. No impact to operations.

The crew on a Porter Airlines de Havilland DHC-8-402 (C-GKQA/POE220) from Toronto/Billy Bishop, ON (CYTZ) to Windsor, ON (CYGQ) reported possibly hitting a bird while landing Runway 25. No remains found. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The destination's aerodrome identification was corrected in the narrative from "Windsor, ON (CYGQ)" to "Windsor, ON (CYQG)".

Accident at GERALDTON / HUTCHISON LAKE ON (Water) (CNE6)

Update TSB Report #A18O0075: C-GVBI, a privately operated, float equipped Maule M-5-210C aircraft, was landing at Geraldton/Hutchison Lake, ON (CNE6). While on final approach to the glassy water surface, the pilot found the throttle to be stuck when power was needed to arrest the high rate of descent. The aircraft landed hard, bounced and dug in a float, causing the aircraft to overturn. The pilot, who was wearing a 4 point safety belt and an inflatable PFD, was able exit the aircraft and received minor injuries.
The Greenstone Fire Dispatch called to report an aircraft accident on Hutchison Lake. Geraldton (Greenstone Regional), ON (CYGQ) Airport Manager (APM) advised the aircraft is a privately registered Maule M-5-210C. Ontario Provincial Police (OPP), fire and ambulance on scene, and pilot was being tended to.

An American privately registered Aeronca 65-CA, on a flight from Sioux Lookout, ON (CYXL) to Geraldton, ON (CYGQ) was in position to depart. The aircraft then moved off to the side of the runway as the engine failed and was unable to restart. It took about 5 minutes to restart. The aircraft taxied back onto Runway and departed at 1331Z without incident. A Northern Skies Air Services Piper PA-31-350 (C-GBDN/GS103) on a flight from Sioux Lookout, ON (CYXL) to Wunnumin Lake, ON (CKL3), a 2080061 Ontario Inc. Swearingen SA226-AT (C-GCPX) on a flight from Sioux Lookout, ON (CYXL) to Fort Frances, ON (CYAG), a Wasaya Airways Beech 1900D (C-GZVJ/WSG909) on a flight from Sioux Lookout, ON (CYXL) to Pikangikum, ON (CYPM), and a Superior Airways Piper PA-31-350 (C-GAJW) on a flight from Red Lake, ON (CYRL) to Sioux Lookout, ON (CYXL) were delayed between 3-5 minutes.

Accident at GERALDTON (GREENSTONE REGIONAL) ON (CYGQ)

TSB Report#A17O0165: a privately operated Piper PA-22-150 aircraft, was conducting a flight from Wawa, ON (CYXZ) to Geraldton (Greenstone Regional), ON (CYGQ). When the aircraft touched down on the runway during the landing at CYGQ, the right main landing gear tire failed. The aircraft veered off the runway and on to the infield. The right wing tip and the right horizontal stabilizer contacted the ground and received substantial damage. The right main landing gear strut was also damaged. The pilot was not injured during the event.

An ORNGE Global Pilatus PC-12 (PUL103) from Geraldton, ON (CYGQ) to Sioux Lookout, ON (CYXL) was departing Geraldton and requested clearance to destination Sioux Lookout. The London Flight Service Station (FSS) relayed the request and the Area Control Centre (ACC) Controller issued clearance to destination Thunder Bay. The London FSS read back the clearance destination as destination Sioux Lookout. The ACC Controller did not detect the fact that the destination was not Thunder Bay. The error was detected after the aircraft was airborne and made initial contact with the ACC Controller enroute to Sioux Lookout. There was no traffic.

AOR 210239-V1: An Ornge Global Air Pilatus PC-12/47E (C-GRXD/ PUL103 Medevac) from Geraldton, ON (CYGQ) to London, ON (CYXU) was too high on approach Runway 15 and requested a 360. Toronto Area Control Centre (ACC) vectored PUL103 back onto approach. The aircraft landed without incident.

Incident at WINNIPEG / JAMES ARMSTRONG RICHARDSON INTL MB (CYWG)

A Perimeter Aviation Fairchild SA227-AC (C-FFJM /PAG204) from Geraldton, ON (CYGQ) to Winnipeg, MB (CYWG) while on final Runway 31 encountered a drone when approximately 4 mile final 2000' ASL. The drone passed above the aircraft by 100ft, no action taken by the pilot. No other operational impact.
UPDATE: TSB Report#A17C0013: C-FFJM, a Perimeter Aviation LP Fairchild SA-227-AC aircraft was conducting flight PAG204 from Gods Narrows, MB (CYGO) to Winnipeg/James Armstrong Richardson Intl, (CYWG). While at approximately 2000 ft. asl, on a 4 nm final approach to Runway 31 at CYWG, the aircraft passed approximately 100 feet below an unknown airborne object. The object was octagonal in shape and bright red in colour. The flight crew did not make an evasive manoeuvre but reported the event to ATC. The aircraft landed without further incident.

Incident at TIMMINS / VICTOR M. POWER ON (CYTS)

UPDATE: TSB Report#A16O0159: C-GASI, a Beech A100 aircraft operated by Thunder Airlines, was approximately 7 nautical miles on final approach for Runway 21 at Timmins, ON (CYTS) from Moosonee, ON (CYMO). At the same time, C-GKPX, a Swearingen SA226-TC aircraft operated by Skycare Air Ambulance, was taxiing for departure on Runway 28, backtracking first on Runway 21, then on Runway 28. The Flight Service Station (FSS) Specialist observed C-GKPX beginning its takeoff roll on Runway 28 while C-GASI was on very short final for Runway 21. FSS requested C-GKPX to abort its takeoff. C-GASI executed a missed approach while C-GKPX came to a stop within the intersection of the two runways.
A Thunder Airlines Beech A100 (C-GASI / THU700) from Moosonee, ON (CYMO) to Timmins, ON (CYTS) was approximately 7NM final Runway 21 when a 2080061 Ontario Inc Swearingen SA226-TC (C-GKPX) from CYTS to Geraldton, ON (CYGQ) began backtracking for departure Runway 28. The flight service station (FSS) noticed that when C-GKPX began their takeoff roll that THU700 was very short final and requested C-GKPX to abort takeoff. THU700 initiated a missed approach when C-GKPX came to a stop within the intersection. C-GKPX departed 3 minutes later and THU700 landed 2 minutes after that without further incident.

No separation was planned with a privately registered Cessna 180K from Timmins, ON (CYTS) to Red Lake, ON (CYRL), over flying the area at 80. There were no other aircraft in the vicinity and no other impact to operations.

An American privately registered Cessna 185D from Two Harbors, MN (KTWM) to Geraldton, ON (CYGQ) failed to report arrival at CYGQ within 1 hour of estimated time of arrival (ETA). Normal communication search procedures were implemented and aircraft was determined to have been substantially delayed upon arrival in Thunder Bay, ON (CYQT). The pilot eventually contacted London, ON (CYXU) flight information centre (FIC) and advised that their intended destination was Isobel Lake near CYGQ. Communication search was terminated and all agencies were advised.

A 2080061 Ontario Inc Piper PA-31-350 (C-FAQR) from Sioux Lookout, ON (CYXL) to Hornepayne, ON (CYHN) at 9000 ft indicated they would be requesting the very high frequency omnidirectional range / distance measuring equipment (VOR/ DME) Runway 08 approach at Geraldton, ON (CYGQ). The Winnipeg Area Control Centre (ACC) controller cleared the aircraft direct the initial approach fix GOTUK. The aircraft was observed descending through 8600 ft without approach clearance. The Winnipeg ACC controller cleared the aircraft for the approach. No other aircraft in the vicinity and no impact to operations.

Incident at SIOUX LOOKOUT ON (CYXL)

A 2080061 Ontario Inc Swearingen SA226-TC (C-GKPX) from Sioux Lookout, ON (CYXL) to Geraldton, ON (CYGQ) was in position Runway 16 and was observed starting takeoff role while an Air Bravo Pilatus PC-12/45 (C-FPCN/ AB258) from Keewaywin, ON (CPV8) to Sioux Lookout, ON (CYXL) was still on the runway. Flight Service Station (FSS) advised C-GKPX to stop and they did. AB258 cleared the runway about a minute later and C-GKPX departed without further incident. The south end of the runway was fogged in so C-GKPX could not see the full length of the runway.

Incident at DRYDEN REGIONAL ON (CYHD)

Update: TSB Report #A15C0107: The Discovery Air Fire Services Inc. Cessna 337G, registration C-GFSC, was conducting flight training at Geraldton, ON (CYGQ). On approach, the flight crew received an unsafe gear indication. After referencing the QRH and completing a manual gear extension, the flight crew diverted to Dryden, ON (CYHD) with an unsafe gear indication. While completing several low approaches, the flight crew and maintenance personnel were able to diagnose and correct the problem. The flight crew reset the circuit breaker and received a safe gear indication. The aircraft landed safely with ARFF on standby. Maintenance personnel have replaced the hydraulic power pack.
A Discovery Air Fire Services Cessna 337G (C-GFSC) inbound to Dryden, ON (CYHD), reported a gear indication problem. The pilot performed multiple flybys in front of a maintenance crew prior to landing. The flight landed without incident at 2255Z. No impact on airport operations.
Update: Follow-up information received from Flight Operations ? West [2015-09-16]: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The CASI contacted the operator for more information and was advised that this occurrence was being managed through the operator?s safety management system (SMS). The CASI was advised that Maintenance staff determined that the right main landing gear door actuator clip had dislodged, allowing the piston to overextend, allowing hydraulic fluid to circulate without building pressure. The actuator was inspected, re-built and a new clip installed. The system tested serviceable and the aircraft was returned to service. The CASI was satisfied with the maintenance actions taken and advised that no further follow-up was required at this time.

Incident at GERALDTON (GREENSTONE REGIONAL) ON (CYGQ)

A Confederation College Cessna 172N (C-GGXK) from Thunder Bay, ON (CYQT) to Geraldton, ON (CYGQ), inbound at 5500 feet, reported being unsure of position and was unable to locate the airport at 1430Z. A flight service station (FSS) specialist helped the pilot tune in to the Geraldton, ON (YGQ) very high frequency omnidirectional range / distance measuring equipment (VOR/DME) and get on track to the airport. Pilot reported the airport in sight at 1440Z.
