A Moncton Flight College (MFC) Diamond DA 20-C1 (C-FEGL) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC) advised of an engine failure on departure (the aircraft had not become airborne). Airport authority advised and the aircraft was towed back to MFC flight school at 1250Z. Multiple MFC flight school aircraft delayed for departure.

A Moncton Flight College Diamond DA-20-C1 (C-FEFG) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) advised ground control that they were stuck on Taxiway D. Aircraft was removed from snow bank and towed back to flight school by a tug vehicle (MFC325). Multiple Diamond DV-20s from flight school were denied taxi until C-FEFG was removed from Taxiway D at 1337Z.

Incident at 30NM NW SHERBROOKE QC (YSC) VOR/DME

An Airsprint Inc. Cessna 525A (C-FASR/ASP524) on a flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Fredericton Intl, NB (CYFC) reported having lost reduced vertical separation minimum (RVSM) capability during enroute climb due to autopilot malfunction. Initially having filed FL330 as a requested cruising altitude, ASP524 revised the requested altitude to FL270. The aircraft was subsequently cleared as requested. No impact on operations.

Incident at 4529N / 07313W

A Jazz de Havilland DHC-8-402 (C-GGNY/JZA8500) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Fredericton Intl, NB (CYFC) declared an emergency due to a problem with the cargo door and returned to CYUL to land. The aircraft landed without incident at 1245Z. No impact on operations.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During initial climb, the Flight Crew received a ‘Fuselage Doors’ warning light associated with the aircraft baggage door. An emergency was declared and the flight executed an air turn back without further incident. Upon arrival the aircraft cargo door external handle was found extended, not in the stowed position. The cargo door was inspected and the aircraft returned to service. No Service Difficulty Report (SDR) required.
Update: TSB report No. A19Q0181: C-GGNY, a Dash 8-400 operated by Jazz Aviation LP as JZA500 with four crew members and 54 passengers on board, was flying under instrument flight rules from Montréal/Pierre Elliott Trudeau International Airport (CYUL), QC, to Fredericton (CYFC), NB. During the initial climb, the fuselage door warning light illuminated. Crew members followed the appropriate checklist and noted that the rear cargo door light was on. A PAN PAN was declared and the aircraft returned to land without incident at CYUL. There were no injuries. Following a check, it was determined that the handle on the rear cargo door had not been properly secured during closing.

The BODPO transition specifies 4 000 ft or more between BODPO and IKDEB. C-GDHS was observed to be descending at 3 000 ft. The aircraft was then cleared for a visual approach. Incident at 20NM SE BEAUCE (ST-GEORGES) QC VOR/DME (VLV)

A Porter Airlines de Havilland DHC-8-402 (C-GKQA/POE242) from Fredericton, NB (CYFC) to Ottawa/Macdonald-Cartier, ON (CYOW) declared "PAN PAN PAN" and reported an erroneous fuel reading. The aircraft decided to divert to Montreal/Trudeau, QC (CYUL). No equipment was requested on the ground and the pilot reported no fuel loss before landing. No impact on operations.
Update TSB #A19Q0064: C-GKQA, a Bombardier DHC-8-400 aircraft operated by Porter Airlines, was conducting flight POE242 from Halifax/Stanfield Intl (CYHZ), NS to Ottawa/MacDonald-Cartier Intl (CYOW), ON with 4 crew members and 72 passengers on board. During cruise flight, the flight crew noticed a fuel imbalance and initiated a fuel transfer from the right main tank, to the left one. Subsequently, the flight crew stopped the fuel transfer when it was realized that the left wing fuel quantity was indicating 200 lbs, and decreasing. No fuel leak could be observed by the cabin crew. A PAN PAN was declared, and the flight diverted to Montreal/Pierre Elliott Trudeau Intl (CYUL), QC. During the approach, fuel quantity indications returned to normal; consequently, the emergency was cancelled. The aircraft landed without further incident. The operator’s maintenance identified the left wing fuel indication system to be faulty.

The preceding arrival, a SkyWest Bombardier CL-600-2B19 (N902EV/SKW3130) from Chicago/OHare, IL (KORD) to Montreal/Trudeau, QC (CYUL) had missed exit A4, and a Jazz de Havilland DHC-8-402 (C-GGAH/JZA7959) from Montreal/Trudeau, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ) was lined up for departure. JZA8503 landed on Runway 24L at 1534Z.

Aviation Incident Report #16496: Passenger became aggressive during a WestJet Encore Ltd. Bombardier DHC-8-402 (C-GENV/WEN3404) flight from Toronto/Lester B. Pearson, ON (CYYZ) to Fredericton, NB (CYFC). The flight diverted to Ottawa/Macdonald-Cartier, ON (CYOW). The passenger was removed from the aircraft by police in CYOW. All other guests were offloaded so that the flight crew could debrief. After debrief it was deemed that the crew was not fit to continue operating so a new crew had to be activated.

Incident at 55NM OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

En route to Fredericton Intl, NB (CYFC), a WestJet Encore de Havilland DHC-8-402 (WEN3404) from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Fredericton Intl, NB (CYFC) reported an unruly passenger and requested to be diverted to Ottawa/MacDonald-Cartier Intl, ON (CYOW). WEN3404 requested the presence of police upon its arrival. WEN3404 landed on Runway 07 at 1904Z without incident. No impact. The National Operations Centre (NOC), the Aviation Operations (AVOPS), and North American Aerospace Defense Command (NORAD) were notified.

A 677276 N.B. Inc. Cessna 340A (C-GBLJ) on a flight from Toronto/Buttonville, ON (CYKZ) to Fredericton, NB (CYFC) checked in climbing to 3000 feet and the controller advised the pilot to maintain 3000 feet which was read back. The mode C disappeared momentarily, but C-GBLJ was next observed climbing through 3400 feet. When queried by the controller the aircraft descended immediately back down to 3000 feet.

Pearson Intl, ON (CYYZ) to Nassau, Bahamas (MYNN) on Taxiway V was instructed to follow an American corporately registered Bombardier BD-700-1A11 (N880ZJ) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Palm Beach Intl, FL (KPBI) from Taxiway F and D. An Air Canada Rouge Airbus A319 (ROU1524) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Fredericton Intl, NB (CYFC) on Taxiway D was instructed to follow N880ZJ and to give way to company on Taxiway D. As N880ZJ approached the intersection of Taxiways F and D, ROU1524 was instructed to follow N880ZJ and that "company" is going to follow you. ROU1816 had not been instructed to follow ROU1524 but held short of D waiting for ROU1524 to cross. There was no incursion and operations were not impacted.

Incident at 250NM N MUCC - Jardines del Rey Intl - Cayo Coco

TSB Report #A19F0054: C-GPVS, a Boeing 737-800 aircraft operated by Sunwing Airlines, was conducting flight SWG158 from Fredericton Intl (CYFC), NB to Cayo Coco/Jardines del Rey Intl (MUCC), Cuba with 6 crew members and 181 passengers on board. During cruise flight at FL360, approximately 250 nm north of MUCC, one of the flight crew members began to feel unwell after consuming a meal. Within the next 10 minutes, the flight crew member started to feel ok and was able to resume duties of Pilot Monitoring (PM). The aircraft landed at MUCC without further incident. In case of a re-occurrence during the return portion (SWG159) from MUCC to CYFC, it was decided that the affected flight crew member would carry out the same duties (PM). The affected flight crew member felt unwell again for a brief period during the return flight, which resulted in a state of fatigue. Controlled rest was used, in conjunction with careful hydration. Prior to the arrival at CYFC, the affected flight crew member assumed PM duties, and the flight landed without further incident.

The aircraft was instructed and read back to extend downwind and tower would call their base. C-FUYV was observed turning base flying towards a Moncton Flight College Piper PA-44-180 (C-FSUQ) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC) who was established on left base Runway 33. C-FUYV was instructed to do a left 360. C-FSUQ was instructed to turn final and traffic passed. C-FSUQ reported traffic in sight. No impact on operations.

At Fredericton, NB (CYFC), generator turned off and commercial power failed to power airfield. Both 119.0 and 121.7 frequencies unserviceable (U/S). Backup frequencies serviceable. All airfield lighting U/S. Tower ran on uninterruptible power system (UPS) for 10 minutes. Technical Operations Coordinator (TOC) and NAV CANADA Manager (NCM) advised. Commercial power restored at 0006Z. All equipment serviceable except glide path (G/P) and localizer (LOC) for instrument landing system (ILS) 09.

Incident at FREDERICTON INTERNATIONAL AIRPORT

On initial climb out, a Moncton Flight College Diamond DA 20-C1 (C-FSXA) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) indicated a rough running engine. No assistance required. Aircraft Rescue and Fire Fighting (ARFF) was advised. The aircraft landed safely at 1649Z. No impact on operations.

A Moncton Flight College Piper PA-44-180 (C-FHFS) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was conducting an instrument landing system (ILS) approach to Runway 09. Aircraft was issued an altitude restriction of not below 1600 ft due to circuit traffic on Runway 27. The altitude restriction was communicated 2 more times and read back each time. The aircraft was observed on radar descending to 1100 ft. Controller turned the aircraft and instructed them to climb to 1600 ft. No impact on operations.

Incident at 33 km N of Blackville, NB on Highway 8

The pilot of a Moncton Flight College Diamond DA 20-C1 (C-FDYW) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) called the London Flight Information Centre (FIC) to report that they did a forced landing on Highway 8, approximately 33 km North of Blackville, NB, due to the engine quitting. There were 2 persons on board (POB), no injuries or apparent aircraft damage. Aircraft pushed off of the road onto the shoulder. The Moncton (ZQM) Area Control Centre (ACC) was advised, and the Halifax (YHZ) Flight Information Centre (FIC) was advised. The YHZ FIC advised that they would notify the Royal Canadian Mounted Police (RCMP).

Incident at 4NM W J.A. DOUGLAS MCCURDY SYDNEY AIRPORT (CYQY)

A privately registered Robinson R44 II on a flight from Fredericton Intl, NB (CYFC) to Sydney/J.A. Douglas McCurdy, NS (CYQY) landed approximately 4NM W CYQY due to low fuel, unsure if they could make it to CYQY. The aircraft had extra fuel onboard. Due to unclear communications, CYQY Aircraft Rescue and Fire Fighting (ARFF) was activated. While vehicle (RED1) was conducting a search toward the threshold of Runway 06, the helicopter departed approximately 4NM W of CYQY inbound for fuel, and landed safely at 1444Z.

A WestJet Encore de Havilland DHC-8-402 (WEN3404) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Fredericton Intl, NB (CYFC) reported a possible bird strike on landing Runway 09. Runway inspection completed with no remains found. No impact on operations.

A Moncton Flight College Piper PA-44-180 (C-FHFS) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC) advised they hit a skunk on landing. No assistance required and runway inspection completed following the strike. No impact on operations.

Incident at FREDERICTON INTERNATIONAL AIRPORT

A Leading Edge Enterprises Ltd. Piper PA-31 (C-GKCN) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was instructed and read back to fly runway heading until 2100 feet prior to a right turn out. A Moncton Flight College Diamond DA 20-C1 (C-FEFZ) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) was 6NM NW and cleared for right downwind Runway 27. C-GKCN was observed turning through 1500 feet. C-GKCN was told to continue the climb and traffic passed to both aircraft. No impact on operations.

A Moncton Flight College Beech C90A (C-GDHS) on a flight from Fredericton Intl, NB (CYFC) to Fredericton Intl, NB (CYFC) was cleared to 6000ft and descended to 5000ft. Less than the prescribed minimum existed between a Korean Airlines Boeing 777-200 (HL8252/KAL8288) on a flight form Chicago/OHare Intl, IL (KORD) to Halifax/Stanfield Intl, NS (CYHZ) and C-GDHS. No other impact operations.

Incident at FREDERICTON INTERNATIONAL AIRPORT

A vehicle (ST38) was instructed to proceed via Taxiway Alpha and hold short Runway 27. A Moncton Flight College Diamond DA 20-C1 (C-FJZC) from Fredericton, NB (CYFC) to Fredericton, NB (CYFC) read this clearance back. Although C-FJZC read back to proceed Taxiway Alpha and hold short Runway 27, they continued out into the intersection to line up for Runway 27 for departure. C-FJZC was subsequently cleared for take-off. No impact on operations.
