Incident at INUVIK / MIKE ZUBKO NT (CYEV) (ATM - NAVAIDS/radar)

A Kenn Borek Air Ltd. Beech B200GT (C-FKBI/KBA200) on a MEDEVAC flight from Inuvik, NT (CYEV) to Paulatuk, NT (CYPC) reported that the very high frequency omnidirectional range (VOR) indicator not working for CYEV at 0925Z. The Technical Operations Coordinator (TOC) advised it had failed and will be reset. VOR indicator was serviceable and confirmed with aircraft at 0933Z. No impact to operations.

Incident at YELLOWKNIFE NT (CYZF) (Aerodrome - visual aids)

A Canadian North Inc. Boeing 737-200 (MPE245) from Inuvik, NT (CYEV) to Yellowknife, NT (CYZF) was inbound, and Runway 16 edge lights were set to brightness 5 with precision approach path indicators (PAPIs) on. Runway identification (ID) lights did not appear to activate. Controller attempted to toggle the edge lights from 4 to 5 in attempt to activate the ID lights, with no success. After landing, MPE245 confirmed that the Runway 16 ID lights were not on. Impact was that the runway identification lights (RILS) were not operating as published.

The Aklavik/Freddie Carmichael, NT (CYKD) Airport Manager (APM) called at 2135Z to issue a NOTAM to close Runway 13/31 due to a disabled aircraft, a Kenn Borek Air Beech B200GT (C-FKBI/KBA200) from Inuvik/Mike Zubko, NT (CYEV) to Aklavik/Freddie Carmichael, NT (CYKD). The aircraft was stuck in the soft shoulder. NOTAM issued. The runway re-opened at 2208Z. No known impact to operations.

A Kenn Borek Air Beech B200GT (C-FKBI/KBA200M) on a MEDEVAC flight from Inuvik/Mike Zubko, NT (CYEV) departed Runway 06 at 1009Z, and reported leaving the zone. Area control centre (ACC) advised that the aircraft was returning with a gear indication and that no assistance was required. KBA200M landed and exited Runway 06 without further incident at 1039Z. No impact to operations.

At Inuvik, NT (CYEV), a vehicle (BAR1) was stuck in runway protected area (RPA) on the south side of Runway 06/24. A Kenn Borek Air Ltd. Beech B200GT (C-FKBI/ KBA200) from Yellowknife, NT (CYZF) to Inuvik, NT (CYEV) was held for 5 minutes. Vehicle was able to free itself and vacate runway. No other impact to the operation.

Incident at INUVIK / MIKE ZUBKO NT (CYEV) (ATM - NAVAIDS/radar)

A Canadian Helicopters Limited Bell 212 (C-FBHF) from Inuvik/Mike Zubko, NT (CYEV) to Inuvik/Mike Zubko, NT (CYEV) advised not receiving identification from YEV very high frequency omnidirectional range (VOR). The distance measuring equipment (DME) and direction had been received. The pilot advised that the same issue was noted earlier from a Canadian Helicopters Limited Bell 202 (C-FOKV) that did not report to the Flight Service Station at that time. A CTC Ken Borek Air Beech 200 (C-FEKB/KBA203) inbound from Paulatuk, NT (CYPC) to Inuvik/Mike Zubko, NT (CYEV) confirmed the same issue. The Technical Operations Coordinator (TOC) was advised.

Incident at 6301N / 13439W

An Air North Charter and Training Ltd. Aerospatiale ATR 42-320 (C-FVGF/ANT409) from Whitehorse/Eric Nielsen, YT (CYXY) to Inuvik, NT (CYEV) had a voltage issue in an electrical system and returned to CYXY. Aircraft landed at CYXY at 1641Z. No impact on operations.
UPDATE from Aircraft Maintenance & Manufacturing: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After departure from Whitehorse, YT (CYXY), the crew of ANT409 received an "under voltage" light indication. Several minutes later, the captain’s Electronic Attitude Direction Indicator (EADI), Electronic Horizontal Situation Indicator (EHSI), and UNS failed and the crew elected to return to CYXY. During the returning flight, systems recovered and became operational and the aircraft landed without further incident. Since the systems had recovered and were operating normally, Air North maintenance was unable to find a hard fault, however a review of the systems affected determined that the standby buss had likely lost power and then recovered. Air North maintenance identified relay 4PD as the most likely cause of this intermittent power loss. Relay 4PD was replaced and aircraft returned to service with no further reported incidents.

Incident at INUVIK / MIKE ZUBKO NT (CYEV) (ATM - inaccurate aeronautical information, ATM - weather observation systems, ATM - ILS irregularity, ATM - NAVAIDS/radar)

At Inuvik, NT (CYEV), a failure of communication of the Navnet, phone lines, the Extended Computer Display System (EXCDS), the Human Weather Observation System (HWOS), and the weather data entry (WDE) occurred from approximately 0630Z until 0807Z. The Technical Operations Coordinator (TOC) was contacted and advised by satellite phone, then later by cell phone. TOC confirmed loss of the peripheral air-ground link (PAL), remote communications outlet (RCO) frequencies, and loss of monitoring for the very high frequency omnidirectional range (VOR), instrument landing system (ILS), and runway visual range (RVR). Weather was sent by contingency over cell phone to Whitehorse Flight Information Centre (FIC).

An inbound Canadian Helicopters Limited Bell 412EP (C-GZEY) from origin unknown to Inuvik, NT (CYEV) reported being unable to receive the Inuvik (YEV) very high frequency omnidirectional range (VOR), however, the associated distance measuring equipment (DME) was serviceable. Confirmed correct frequency 112.5 with pilot and advised the Technical Operations Coordinator (TOC) of the issue. TOC advised they would look into it and issue a NOTAM if unable to return VOR to service. At 2311Z, the TOC issued a NOTAM for the VOR being unserviceable (U/S) until 08/16 at 2359Z.

DCFFU landed at 2007Z. Impact on operations involved a First Air Aerospatiale ATR-42-300 (FAB892) from Inuvik, NT (CYEV) to Yellowknife, NT (CYZF), taxiing for a departure from Runway 06, having to vacate the runway to accommodate DCFFU.

Update TSB Report #A19W0026: C-GSKB, a Beech 1900D aircraft operated by Kenn Borek Air, was conducting flight KBA901 from Paulatuk (Nora Aliqatchialuk Ruben) (CYPC), NT, to Inuvik (Mike Zubko) (CYEV), NT with two flight crew members on board. Upon selection of landing gear down on arrival at CYEV, the flight crew observed that there was no green light for the nose landing gear. The appropriate checklist was completed, and a manual landing gear extension was performed. The flight crew declared an emergency, and landed without further incident.

The vehicles were off the runway prior to the arrival of C-GALF. No impact on operations.

The controller issued a clearance that had the pilot of Kenn Borek Air Ltd. Beech 200 (KBA200) from Inuvik/Mike Zubko, NT (CYEV) to Yellowknife, NT (CYZF) report on the 113 degree radial. The flight service station (FSS) correctly read the radial to the pilot, but the pilot read back a degree radial of 130. The FSS did not catch the error.

Incident at SACHS HARBOUR (David Nasogaluak Jr.Saaryuag) NT (CYSY) (False warning - smoke or fire)

A Kenn Borek Air Beech 200 (KBA203) from Sachs Harbour, NT (CYSY) to Inuvik, NT (CYEV) departed Sachs Harbour enroute to Inuvik. When the aircraft was approximately 60 nautical miles southwest of Sachs Harbour, the pilot declared an emergency due to an engine failure and was returning to Sachs Harbour. The aircraft landed safely at 1935Z. No impact to operations.
Update TSB report #A19W0016: C-FEKB, a Beech 200 King Air aircraft operated by Kenn Borek Air, was conducting flight KBA203 from Sachs Harbour (David Nasogaluak Jr Saaryuaq) (CYSY), NT to Inuvik (Mike Zubko) (CYEV), NT with 2 crew members on board. Shortly after take off from CYSY, the flight crew received a left engine (Pratt & Whitney Canada PT6A-41) fire indication. Before the flight crew could address the problem, the indication went out. A scan of the engine instruments revealed no anomalies, so the flight crew elected to continue the flight. Approximately 60 nm south west of CYSY, a second left engine fire warning indication was received. There was no physical indication of fire, and troubleshooting failed to correct the problem. The flight crew elected to shut the engine down, and activated the fire bottle. An emergency was declared, and the flight crew elected to return to CYSY where an uneventful landing was completed. Subsequent investigation by the operator’s maintenance determined that there was an unserviceable fire detector for the left hand engine, which was providing an erroneous fire indication.

At 0730Z, an unmanned air vehicle (UAV) with red and green flashing lights was observed at the Inuvik, NT (CYEV) aerodrome, south of the runway, by the flight service station (FSS). A Kenn Borek Air Ltd. Beech 200 (KBA203-M) MEDEVAC flight from Yellowknife, NT (CYZF) to Inuvik, NT (CYEV) that was VFR inbound from the North was advised. On the final approach, the pilot of the aircraft confirmed that the UAV was approximately 0.5NM South of the runway. The Royal Canadian Mounted Police (RCMP) were advised. The RCMP followed up, advising that they were unable to find the source.

Incident at INUVIK / MIKE ZUBKO NT (CYEV) (ATM - inaccurate aeronautical information)

Frequency 122.6 was unserviceable (U/S) at 1528Z with one vehicle on the runway, a North-Wright Airways Cessna 207 (C-FBAX) on a flight from Inuvik/Mike Zubko, NT (CYEV) to Aklavik/Freddie Carmichael, NT (CYKD), that was taxiing for departure Runway 24. Runway edge lights (REDL) flashed and vehicle exited the Runway. C-FBAX experienced an approximate 5 minute delay departing. Hand-held radio (NICAN) deployed at 1535Z. Contractor working at transmit (TX)/receive (RX) site advised a cable was cut at 1535Z, frequencies 132.4 and 126.7 also affected. Regular service on 122.6 was restored at 1916Z.

At Inuvik, NT (CYEV) the main transmission 122.3 transmit (TX)/receive (RX) function was briefly unserviceable, hand-held radio (NICAN) was deployed to 122.3 from 122.6. Outage was due to contractor work at the TX/RX site. The flight service station (FSS) werent informed of the planned outage in advance. 122.3 main service restored after approximately 5 min outage. Two aircraft were operating in the control zone at the time: A North-Wright Airways Ltd. Cessna 207A (C-MOK) from Aklavik/Freddie Carmichael, NT (CYKD) to Inuvik, NT (CYEV) and an Air North Charter and Training Ltd. (ANT208) Aerospatiale-Alenia ATR-42-300 from Inuvik, NT (CYEV) to Dawson City, YT (CYDA). No operational impact.

A Kenn Borek Air Beech B200GT (C-FKBI/KBA200-M) from Paulatuk, NT (CYPC) to Inuvik/Mike Zubko, NT (CYEV) was unable to establish contact with the Inuvik flight service station (FSS) on the 122.3 frequency after multiple attempts, beginning at 0733Z. The Edmonton area control centre (ACC) was unable to establish communications via interphone or landline. Contact with KBA200-M and the Inuvik FSS was re-established at 0758Z. KBA200-M experienced approximately 15 minutes of delay in landing due to lack of runway lights.

Incident at INUVIK / MIKE ZUBKO NT (CYEV) (Aerodrome - power failure, Aerodrome - visual aids, ATM - ILS irregularity, ATM - inaccurate aeronautical information)

At 0015Z, there was a commercial power failure at the Inuvik/Mike Zubko, NT (CYEV) aerodrome. The backup generator failed to start. All aerodrome lighting was unserviceable (U/S). The runway visual range (RVR) ILS/DME 06 was unserviceable (U/S). The backup power was started by the electrician at 0046Z. A vehicle advised that the frequency 122.6 was also unserviceable until the backup was started.

A vehicle (RED9) exited Runway 06/24 onto Taxiway D without authorization and once off Taxiway D was observed re-entering Taxiway D. No impact on operations.

Incident at INUVIK / MIKE ZUBKO NT (CYEV)

After departure, a North-Wright Airways Ltd Beech 1900D (NWL205) from Inuvik/Mike Zubko, NT (CYEV) to Norman Wells, NT (CYVQ) was given a restriction to cross 10 nautical miles from CYEV at A060 or below. Two minutes later, NWL205 reports that it is 9.5 nautical miles from CYEV and climbing through A080. The aircraft was re-cleared to FL210. Separation not assured with inbound aircraft, a Buffalo Airways Ltd. Lockheed L 188 (BFL1426) from Cambridge Bay, NU (CYCB) to Inuvik/Mike Zubko, NT (CYEV).

Incident at NORMAN WELLS FSS / FSS DE NORMAN WELLS (ATM - inaccurate aeronautical information)

A Canadian North Inc Boeing 737 300 (MPE444) from Norman Wells, NT (CYVQ) to Inuvik, NT (CYEV) was unable to contact Edmonton Center via 134.825 or 132.8. The flight service station (FSS) tried to forward position report to Edmonton and found the interphone and long distance telephone services not working.
