An automatic dependent surveillance - contract (ADS-C) report indicated that a French Blue Airbus A350-900 (FHREU/FBU713) from San Francisco, CA (KFSO) to Paris/Orly, France (LFPO) followed route TEALS PRAWN, while the cleared route and the route in the Canadian Automated Air Traffic System (CAATS) was TEALS Nain, NL (CYDP) PRAWN. The aircraft was cleared again for TEALS YDP PRAWN. No impact on operations.

The pilot of a Canadian Helicopters Limited Bell 212 (C-FBHF) on a flight from Nain, NL (CYDP) to Saglek, NL (CYSV) filed a VFR flight plan at 1153Z with the London Flight Information Centre (FIC) under the aircraft identification (ACID) C-GFHF. The estimated time of departure (ETD) was 1300Z with an estimated elapsed time (EET) of 0130Z for an estimated time of arrival (ETA) of 1430Z. The Halifax Flight Information Centre (FIC) queried dispatch for an arrival and it was discovered that the aircraft identification (ACID) should have been a Canadian Helicopters Limited Bell 212 (C-FBHF). At 1600Z, the pilot called the London Flight Information Centre (FIC) to close the VFR flight plan and confirmed they had filed under the wrong aircraft identification (ACID) in error.

Air traffic control (ATC) and Quebec, QC (CYQB) FIC contacted the flight service station (FSS) for flight information and update to Gander. No other known operational impact.

The pilot of LAL501, a DHC6 reported that actual conditions in Nain (CYDP) did not match the Mmeteorological Aerodrome Report (METAR). The 2200Z METAR indicated 15 Statute Miles (SM) Light Rain Showers ( -SHRA) a Few Clouds at 900 feet (FEW009)an Overcast condition at 7000 feet (OVC070) with no Special Weather Observations (SPECIS) having been issued. A Pilot Information Report (PIREP) during approach 2247Z indicated conditions 2SM OVC005.

The METAR at 1200Z reported a few clouds at 1600 ft and overcast at 2400 feet, the pilot reported at 1202Z an overcast condition at 400 feet which topped out at 1700 feet.

Incident at KUUJJUAQ QC (CYVP) (Communication navigation surveillance/air traffic system)

Update#1: The category "Canadian Aviation Regulations (CARs) infraction - alleged" was removed. According to the information received from the Commercial and Business Aviation division - Atlantic: The pilot reported on frequency 126.7 MHz, 15 minutes before the estimated time of arrival (ETA). Due to the high traffic on frequency 126.7 MHz, the pilot accidentally selected frequency 122.0 MHz instead of frequency 122.2 MHz, on radio #1. The aircraft therefore landed without having received confirmation from the Kuujjuaq (CYVP) flight service specialist. After landing, the specialist confirmed that he did not receive notice on frequency 122.2 MHz but heard the intention to land on Runway 25 on frequency 126.7 MHz.
Provincial Airlines De Havilland Dash 6 C-GJDE was VFR from Nain (CYDP) to Kuujjuaq (CYVP). The aircraft entered the Kuujjuaq mandatory frequency (MF) zone and landed on runway 25 without establishing radio contact with the flight service specialist (FSS).

Incident at GOOSE BAY NL (CYYR)

UPDATE TSB: A09A0087: SPR955, a Provincial Airlines De Havilland DH6 Twin Otter, (GIED), was en route from Goose Bay to Nain on a cargo flight when the right engine propeller moved toward the feather position. The crew brought the engine to idle, consulted the checklist for an uncommanded propeller feather and informed Goose Bay ATC that they were returning. Approximately one minute later, the propeller moved out of the feather position and then back in again. On the return flight to Goose Bay, the left propeller beta light illuminated at which point the crew shut down the engine (Pratt & Whitney PT6A-27). After completing the checklist for the beta light and consulting with maintenance, the circuit breaker was pulled from the beta system and the engine was restarted; the propeller functioned normally. The aircraft landed at Goose Bay with no further incident. More details to follow from the operator.
At 14:12Z, SPR955, DHC 6, enroute from Goose Bay (CYYR) to Nain (CYDP), declared an emergency due to an engine failure. The aircraft returned to Goose Bay and landed without incident. TSB Case Closed.

Incident at KUUJJUAQ QC (CYVP) (Communication navigation surveillance/air traffic system)

An Aerospatiale AS-350 helicopter operated by Canadian Helicopters was conducting a visual flight rules (VFR) flight from Nain, NL (CYDP) to Kuujjuaq (CYVP). At 1559Z, the pilot made radio contact with the Kuujjuaq (CYVP) flight service specialist (FSS), but then the FSS was unable to establish communications with the pilot. The FSS saw the helicopter land approximately 5 NM to the south. The helicopter pilot contacted the FSS at 1609Z and confirmed that he had experienced communication problems. **** ** **** Un hélicoptère Aerospatiale AS-350 exploité par Hélicoptères Canadiens, effectuait un vol selon les règles de vol à vue (VFR) depuis Nain, NL (CYDP) à destination de Kuujjuaq (CYVP). The northeast bound aircraft penetrated the airspace to be protected for westbound aircraft not yet radar identified.

Incident at ADIZ entre Nain (CYDP) et Kuujjuaq (CYVP)

Air Inuit Twin Otter AIE652 was on a VFR from Nain (CYDP) to Kuujjuaq (CYVP). After receiving arrival advisory service, the pilot was asked by flight service specialist (FSS) if a flight plan had been submitted to pass through the air defence identification zone (ADIZ) for the trip to and return from Nain (CYDP). The pilot said that a flight plan had been submitted to the Quebec City Flight Information Centre (FIC) to pass through the ADIZ for the outbound leg to Nain but not for the return. The Quebec Flight Information Centre was contacted and no flight plan or information pertaining to the aircraft had been received. *** ** *** AIE652, un Twin Otter exploité par Air Inuit, effectuait un vol selon les règles de vol à vue (VFR) depuis Nain (CYDP) à destination de Kuujjuaq (CYVP). Après avoir reçu le service consultatif d'arrivée, le pilote a été questionné par le spécialiste d'information de vol (FSS) pour savoir si un plan de vol avait été déposé pour traverser la zone d'identification de défense aérienne (ADIZ) à l'aller et au retour de Nain (CYDP). Incident at KUUJJUAQ QC (CYVP) (Communication navigation surveillance/air traffic system, ATM - other)

C-GNLK, a Bell 206 operated by Canadian Helicopters Ltd., was conducting a visual flight rules (VFR) flight between Nain, N.L. (CYDP), and Kuujjuaq (CYVP). Upon arrival, the pilot notified the flight service specialist that he had a VFR flight plan CYDP-CYVP open via Goose Bay. The specialist never received the flight plan, and when informed, neither had the Québec (CYQB) flight information centre (FIC). According to NAV CANADA, there seems to have been a lack of communication between the specialist and the pilot.

Due to a fire in the Allendale, NL, telephone company (Aliant) facility the Gander (CYQX), St. John?s (CYYT), Stephenville (CYJT), St. Anthony (CYAY), CYAY 5680, Hopedale (CYHO), and Nain (CYDP) frequencies, CYYT TWR landlines, Holyrood and Marble Mountain radars were unserviceable from 00:46Z until all were restored at 07:20Z. Halifax (CYHZ) FIC also carried contingency routing for Deer Lake (CYDF) FSS from 01:10Z to 06:53Z. Nil TSB.
