The Halifax (HZ) Terminal Control Unit (TCU) advised that a Jazz Embraer ERJ 170-200 SU (C-FEKI/JZA7540) on a flight from Toronto/Lester B. Pearson, ON (CYYZ) was inbound to Halifax/Stanfield, NS (CYHZ) with a cracked windscreen. No assistance required. The Halifax International Airport Authority (HIAA), the Airport Duty Manager (ADM) and the Aircraft Rescue and Fire Fighting (ARFF) unit were advised. The aircraft landed without incident or impact at 1513Z.

Incident at GANDER INTL NL (CYQX)

A Newfoundland and Labrador Air Ambulance Service Beech B300C (C-GNLO) from Deer Lake, NL (CYDF) to Gander, NL (CYQX) executed a missed approach due to a landing gear issue. The aircraft advised that the issue was resolved and requested a diversion to St. Johns, NL (CYYT). Subsequently, the aircraft declared an emergency and landed without incident in CYYT. No impact on operations.
UPDATE: TSB Report #A21A0003: C-GNLO, a Government of Newfoundland and Labrador Beech B300C, was conducting a flight from Deer Lake, NL (CYDF) to Gander, NL (CYQX). On approach to CYQX, the landing gear was selected "down" but the crew did not receive a "left-hand main landing gear down" green indication. The crew cancelled their IFR clearance, carried out the checklist items, and consulted with their maintenance department. It was decided to divert to St. Johns, NL (CYYT), where an emergency was declared in order to have emergency services standing by as a precaution. The aircraft landed at CYYT without further incident. A faulty "left-hand main landing gear down" indication lamp was replaced, and the aircraft was returned to service following operational checks.

The National Voice Communication System (NVCS) position of Deer Lake (YDF) was unserviceable (U/S) at 0740Z. Frequency 122.3 MHz was serviceable on the portable radio backup and frequency 122.6 MHz was serviceable on the NVCS. Frequencies 239.6 MHz, 243.0 MHz, 121.5 MHz, and 122.3 MHz were NOTAMed as U/S at 1034Z. The NVCS YDF and St. Anthony (YAY) positions were serviceable at 1426Z. Frequencies 239.6 MHz, 243.0 MHz, 121.5 MHz, 122.3 MHz, and 122.6 MHz were serviceable on the NVCS at 1426Z.

Incident at 75NM S STEPHENVILLE NL (CYJT) (Aerodrome, runway or taxiway shutdown)

A Provincial Aerospace Beech 200 (C-GTJZ/SPR04) from St. Johns, NL (CYYT) to St. Johns, NL (CYYT) encountered an electrical smell in the cockpit shortly after take-off, declared an emergency, and requested clearance back to its point of departure. C-GTJZ was cleared as requested, and landed safely at 1339Z. No traffic. Impact to operations: The runway was closed for 5-10 minutes to vector the aircraft.

UPDATE: TSB Report #A20A0034: C-GTJZ, a Beech B200 aircraft operated by PAL Aerospace Ltd., was conducting patrol flight SPR04 from St. Johns, NL (CYYT) to Deer Lake, NL (CYDF). The aircraft descended to the mission altitude of 500 feet above sea level, and at about 75NM South of Stephenville (CYJT), NL, the crew detected an electrical odour in the aircraft. The crew ceased the patrol and climbed to a safe altitude for troubleshooting. The crew consulted the Quick Reference Handbook (QRH), enlisted a cabin crewmember to investigate for signs of heat or smoke, and requested to return to CYYT. Onboard mission equipment was unplugged and no source of the smell could be detected. The crew declared "PAN PAN" and shut off all non-essential electrical equipment. There was no change in the odour, and no sign of smoke or heat. The crew ventilated the cabin using ram-air and the flight landed at CYYT without further incident. Emergency services inspected the aircraft and no heat source could be detected. Maintenance personnel subsequently determined that a transistor on a fuel transfer system circuit board failed and overheated.

Incident at DEER LAKE NL (CYDF)

A Westjet Boeing 737-8CT (C-GWSA/WJA518) from Toronto/Lester B. Pearson, ON (CYYZ) to Deer Lake, NL (CYDF) conducted a missed approach to Runway 25 at 1523Z due to a flap issue (could not extend flaps). The aircraft was issued a hold and then conducted an approach without flaps.
Update: TSB Report #A20A0030: C-GWSA, a Boeing 737-8CT aircraft operated by WestJet, was conducting flight WJA518 from Toronto/LBPIA, ON (CYYZ) to Deer Lake, NL (CYDF). The flight was cleared for the approach to Runway 25 in CYDF, and the crew contacted the CYDF flight service station (FSS) to report that they were unable to extend the flaps for the landing. The crew conducted a missed approach and were issued a hold. The crew completed checklist actions and contacted maintenance control regarding the flap condition. The crew determined they could not resolve the flaps issue, declared a PAN PAN, briefed the lead flight attendant, and conducted a flapless landing without further incident. Maintenance personnel replaced the faulty flap SKEW sensor #4 connector.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. TRAX records were reviewed back to June 1, 2020, and only one related issue has been recorded; the day before, in Toronto, a defect was recorded on log page (LP) AK89272 regarding the flaps not extending on pre-flight checks. A Flap Slat Electronic Unit (FSEU) BITE check was accomplished, with 3 fault codes present (27-51417-Flap Lever Sensor, 27-53279-L-28VAC EXC INV and 27-53280-R 28VAC EXC INV.). Toronto (YYZ) maintenance troubleshot the system by accomplishing Fault Isolation Manual (FIM) 27-51 Task 811, Task 822 and Task 823, and all codes on the Flap Slat Electronic Unit (FSEU) were cleared. The system was tested multiple times and the fault codes did not return and the trailing edge flap system operated normally, indicating that the fault was intermittent in nature. The aircraft was returned to service. On the next flight into Deer Lake, the Flight Crew recorded a flap asymmetry defect on LP 98274. On July 22, 2020 the flight crew of flight WJA518 requested emergency crews to be standing by on WJA518s arrival into Deer Lake, NL (CYDF) due to a flap extension issue. The flaps would not extend when selected for landing. The aircraft landed without incident and taxied to the gate. Aircraft 607 was dispatched from Toronto on a rescue mission with 2 mechanics and associated troubleshooting parts on board. Mechanics arrived in Deer Lake and troubleshot the flap system, with the following results: Flap Slat Electronic Unit (FSEU) Built-in Test Equipment (BITE) check accomplished; FIM 27-59 Task 809 accomplished; Found flap skew connector D11820 with 9.6 Ohms between pins 1 and 2, and also found CB C213 (FC TE Flap Pos/Skew SNSR and IND Left) on P6-2 panel "popped". Pins 1 and 2 were shorting out intermittently due to corrosion/moisture in the connector. Connector D11820, including pins 1 and 2, was replaced in accordance with (IAW) Boeing WDM 27-53-21 and SWPS 20-61-00, as well as the #4 Flap Skew Sensor (IAW Aircraft Maintenance Manual (AMM) 27-59-01). The Sensor was "normalized" (zeroed) and the system was tested serviceable; RII was accomplished; the aircraft has been returned to service and the flaps were operated on the subsequent take-off and landing, with no issues.

Incident at 15NM NNW ST. JOHN'S INTL NL (CYYT)

A Provincial Airlines de Havilland DHC-8-314 (C-GYCV/PVL411) on a flight from Deer Lake, NL (CYDF) to St. Johns Intl, NL (CYYT) at about 15NM final Runway 16 declared an emergency through Gander (YQX) Area Control Centre (ACC) due to a hydraulic failure. The aircraft landed at 1854Z without incident, taxied in and cancelled emergency.
UPDATE TSB Report #A20A0002: C-GYCV, a De Havilland DHC8-300 aircraft operated by PAL Airlines, was conducting flight PVL411 from Deer Lake (CYDF) , NL to St. John’s Intl. (CYYT), NL. When the aircraft was approximately 15NM final from RWY 16 the crew contacted Gander Area Control Center and declared an emergency due to a hydraulic failure. The aircraft landed uneventfully with airport rescue and firefighting standing by. Company maintenance determined that a flared fitting on the number one (#1) hydraulic system manifold had cracked which depleted the fluid level below the quantity required for the system to work. The aircraft was repaired and returned to service. The company has submitted a Service Difficulty Report to Transport Canada.

Air traffic controller (ATC) instructed the crossing aircraft to hold short of crossing the runway. No impact on operations.

A Jazz de Havilland DHC-8-402 (C-GGOI/JZA8882) from Halifax/Stanfield, NS (CYHZ) to Deer Lake, NL (CYDF) returned to land due to a pressurization problem. No assistance required, landed safely at 0259Z. No operational impact.
UPDATE AOR 259737-V1: A Jazz de Havilland DHC-8-402 (C-GGOI/JZA8882) on a flight from Halifax/Stanfield Intl, NS (CYHZ) to Deer Lake, NL (CYDF) reported a pressurization problem. The aircraft lost pressure in the cabin due to blown seals in door. No assistance required, the flight returned to Halifax. The aircraft landed at CYHZ at 0259Z. Halifax Joint Rescue Coordination Centre (JRCC) was advised. No impact on operations.

An emergency locator transmitter (ELT) signal was heard on 121.5 for 2 seconds, source was a Newfoundland Helicopters Limited aircraft which inadvertently did an ELT test at 1411Z.
