Incident at NANAIMO AIRPORT

At 1946Z, an Island Express Air Inc. Piper PA-31 (C-GXPE/IAX100-T) from Abbotsford, BC (CYXX) to Nanaimo, BC (CYCD) made its initial call while 4NM final for Runway 16, for a straight-in simulated instrument landing system (ILS) 16 approach. No prior call had been made before entering the CYCD control zone. Impact to Operations: active Runway 34 with 2 opposite direction aircraft just airborne, a Vital Aviation Ltd. Cessna 172I (C-FXFN) from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) that departed at 1944Z and a privately registered Colonial C-2 from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) that departed at 1945Z. At the time of IAX100-Ts initial call, C-FXFN was at altitude 012, 2NM out slightly left of the runway heading and just starting a left turn on course. The Colonial C-2 aircraft was just airborne at an altitude of 008, 1NM out and still climbing the runway heading. Both departing aircraft turned clear of the approach path prior to IAX100-T landing.

At 0035Z, a call was received from the Victoria (YYJ) terminal (TML) advising of a Montair Aviation Inc. Cessna 152 (C-GRCD) from Pitt Meadows, BC (CYPK) to Nanaimo, BC (CYCD) that would be landing at CYCD with an electrical issue. Pilot advised there was no emergency but requested assistance.

A foreign registered Airbus EC 130 T2 helicopter (ZKHKV) from Fairchild/Port Angeles, WA (KCLM) to Nanaimo, BC (CYCD) flew transborder without an active flight plan. The pilot-in-command (PIC) indicated they did file and activate their flight plan with the United States (US) flight service station (FSS), but no flight plan was received at the Kamloops (YKA) flight information centre (FIC).

Incident at NANAIMO AIRPORT

The Victoria, BC (CYYJ) Terminal advised that a privately registered, amateur built Vans RV-7A from Chilliwack, BC (CYCW) to Nanaimo, BC (CYCD) had declared an emergency while inbound to CYCD. Emergency vehicles called. Aircraft landed safely at 1849Z. Operational impact: one IFR aircraft had to wait on the ground, and one VFR aircraft had to orbit outside the zone prior to landing.

Incident at VANCOUVER CENTRE / ACC DE VANCOUVER (Flight plan – activation, Communication navigation surveillance/air traffic system)

Internet filed a flight plan (FP) received for an Alkan Air Beech 300 (AKN183) from Nanaimo, BC (CYCD) to Vancouver, BC (CYVR). Received phone call from the pilot of an AKN235, requesting to close VFR FP. The Pilot was advised (adz) no VFR FP on file for AKN235, only ones for AKN183. The Pilot advised they made the change of identification on FP with Vancouver, BC (ZVR) IFR flight planning. ZVR IFR flight planning confirmed they received the call and changed the VFR codes, but change of FP was never received by Kamloops, BC (YKA) flight information centre (FIC).

A Morningstar Air Express Inc. Cessna 208B (MAL7072) from Nanaimo, BC (CYCD) to Vancouver, BC (CYVR) reported a bird strike on departure from Runway 16 at 2313Z. Pilot advised they had no problems and were continuing on their IFR flight.

Incident at 3NM VICTORIA INTL BC (CYYJ)

Both a Jazz de Havilland DHC-8-402 (C-GUJZ/JZA8070) from Victoria, BC (CYYJ) to Vancouver, BC (CYVR) and a Jazz de Havilland DHC-8-402 (C-GGFJ/JZA8270) from Nanaimo, BC (CYCD) to Vancouver, BC (CYVR) read back the clearance issued to JZA8270. Only one aircraft was heard with a fraction of the second aircrafts transmission at the end. Within seconds, the controller took immediate action once it was noticed that JZA8070 was not on the expected heading, and conflicting with an American, corporately registered Cessna 208B (N740MD), origin and destination unknown.

A Jazz de Havilland DHC-8-402 (JZA8262), from Nanaimo, BC (CYCD) to Vancouver, BC (CYVR), had departed IFR off of Runway 34, climbing to the Nanaimo (YCD) non-directional beacon (NDB) and was passed Husky traffic 2NM East of the NDB at FL017. JZA8262 reported traffic in sight. No impact to operatons.

With a privately registered, amateur built Zenair Zodiac CH640 from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) on short final at mid point of Runway 34, a privately registered, amateur built Laird Smith RV-6A from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) began taxiing to position for departure from Runway 34. Flight service station (FSS) advised of traffic and the pilot of the Laird Smith RV-6A reported that they would hold. The Laird Smith RV-6A aircraft crossed the hold line before the Zenair Zodiac CH640 touched down. No operational impact. The threshold of Runway 34 was displaced by 1002 feet.

Incident at NANAIMO AIRPORT

A privately registered Cessna 177RG from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) departed at 1729Z for right hand circuits R34. Target dropped to a primary target at 1731Z. At 1733Z, the pilot called on a cell telephone to advise they had experienced a full electrical failure. The pilot elected to proceed direct to Thetis Island to hand pump the gear down. An Aircraft Rescue and Fire Fighting (ARFF) vehicle (Red 1) was contacted and placed on standby on the apron. The aircraft landed at 1746Z. Coordination occurred throughout over the cell telephone. No impact to operations.

Emergency communications (ECOMM) and the local Royal Canadian Mounted Police (RCMP) detachment were advised. No impact to operations.

Accident at CAMPBELL RIVER BC (CYBL)

A privately registered Cessna 185B from Nanaimo, BC (CYCD) to Campbell River, BC (CYBL) landed Runway 30 at 1817Z. While attempting to exit the Runway at Taxiway A the aircraft ground looped and stuck its left wing on the runway surface. The aircraft also broke its axle. Assistance was required to remove the aircraft from the runway. The aircraft was removed at 1848Z.

Strong emergency locator transmitter (ELT) bursts were heard on 121.5 starting at 1935Z and ending at 1937Z. The Joint Rescue Coordination Centre (JRCC) Victoria advised that ELT was encoded to a privately registered Nanchang CJ6A from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) which was parked at the North Hangars. The area control centre (ACC) shift manager was advised. No operational impact.

An American privately registered Cessna 185E from Everett, WA (KPAE) to Campbell River, BC (CAE3) entered Ladysmith Harbour and the Nanaimo, BC (CYCD) mandatory frequency (MF) at an altitude of 800ft. The aircraft transited the zone at 800ft, northwest bound and exited to the NW at 500ft. There was no contact with the aircraft at any time. CYCD flight service station (FSS) attempted contact. Impact to operations: A Vital Aviation Cessna 172K (C-GTXA) in the circuit for Runway 34 was requested to turn left after a touch and go.

A Victoria Flying Club Cessna 172R (C-GKMY) was observed operating over Porlier Pass at 3500. Impact: a Jazz de Havilland DHC-8-402 (JZA8262) from Nanaimo, BC (CYCD) to Vancouver, BC (CYVR) and a Montair Aviation Inc. Cessna 172R (C-FMIC) had to be vectored around C-GKMY.
