Aviation incident report: An Air Borealis de Havilland DHC-6 series 300 (C-GNFZ) arrived at 1610Z on March 13, 2019. Once it was parked, a Canada Post truck backed up towards the plane with the help of apron workers and the pilot, and the unloading began. Without notice, another truck came and backed up beside the Canada Post truck. Since it was smaller, it was not visible to the ground employees. Once the pilot noticed the presence of this truck, it unsuccessfully tried to warn the driver. The truck his the aircrafts left elevator. No other impact on airport operations.

Incident at 60NM SEPT-ÎLES QC (CYZV)

A Sky Jet M.G. Inc. Beech A100 (C-GJBV/VSL512), which was on a MEDEVAC flight from Lourdes-de-Blanc-Sablon, QC (CYBX) to Sept-Îles, QC (CYZV), declared an emergency due to electrical problems. The aircraft landed without incident at 1840Z. No impact on operations.
Update TSB Report #A19Q0022: C-GJBV, a SkyJet M.G. Beech A100 King Air, was on IFR medical evacuation (MEDEVAC) flight VSL512 from Lourdes-de-Blanc-Sablon, QC (CYBX) to Sept-Îles, QC (CYZV). Approximately 60 nautical miles east of CYZV, the crew experienced right generator failure, followed by a loss of electrical power in the left generator. Although the checklist in the Quick Reference Handbook (QRH) was completed, the crew did not manage to isolate the left generator and had to continue the flight with the battery as the sole source of electrical power. Electrical systems that were not essential for flight were turned off to reduce the consumption of electricity, and the crew declared an emergency. The landing at CYZV was conducted without incident. There were no injuries.
UPDATE: Aviation incident report #: A Sky Jet M.G. Beech A100 (C-GJBV) made an emergency landing due to an electrical problem. There was a break in communications during the emergency.

A pilot saw and heard a radial engine aircraft land on water near the hospital in Roberval around 0130Z on June 15 and heard the same aircraft take off between 1000Z and 1100Z on June 16. The aircraft had not made any contact with Rouyn radio. No known impact on operations. A flight plan existed on a privately registered de Havilland DHC-2 MK. I from Roberval, QC (CYRJ) to Blanc-Sablon, QC (CYBX), with which we had no contact. The Quebec flight information centre (FIC) stated that the aircraft was in fact en route.

After an airport vehicle (ST21+1BX) reported to be off of the runway, a Provincial Airlines de Havilland DHC-8-311 (C-GNCF/PVL8322) from Lourdes-de-Blanc-Sablon, QC (CYBX) to La Romaine, QC (CTT5) positioned itself on Runway 23 for take-off. The aircraft reported that there was equipment that was within 200 feet of the runway protected area (RPA). When the vehicle was questioned about the equipment, the driver mentioned being aware of the equipment but noted that they were unable to move it. PVL8322 was notified and took off. No impact on operations.

The flight originated as a VFR round robin (RR) but the aircraft encountered weather issues NE of CYBX and the pilot stated that they air-filed IFR with Quebec radio on 119.575. The pilot stated that they did not receive an air traffic control (ATC) clearance (class G airspace) and no record of an IFR flight plan has been found.

A Department of National Defence Government of Canada Agusta-Westland EH 101 (RSCU906) on a MEDEVAC flight from Lourdes-de-Blanc-Sablon, QC (CYBX) to St. Anthony, NL (CYAY) taxied for a VFR departure following the IFR route. No contact at takeoff and no response afterwards. The weather observer confirmed the takeoff, but there was no visual contact. The Montreal area control centre (CZUL) was notified to verify whether Gander, NL (CYQX) was in contact with the aircraft. No contact. When contacted, the Halifax (HZ) Joint Rescue Coordination Centre (JRCC) mentioned that the helicopter had landed at Forteau (9 NM east of CYBX). HZ JRCC called again to report that the aircraft had left Forteau bound for CYAY. No contact. CZUL was notified.

At 1210Z and position 35NM west of Lourdes-de-Blanc-Sablon (CYBX), QC, a Keewatin Air Beech B200 (C-FRMV/KEW202) on a flight from Lourdes-de-Blanc-Sablon (CYBX), QC, to Sept-Îles (CYZV), QC, reported smoke in the cockpit and requested to return to CYBX. The flight was re-cleared as requested and landed at 1221Z.
UPDATE: TSB Report#A17Q0139: C-FRMV, a Beech King Air BE20 aircraft operated by Keewatin Air under KEW202, was conducting a medevac flight from Lourdes-de-Blanc-Sablon, QC (CYBX) to Sept-Iles, QC (CYZV) with 4 passengers and 2 crew members on board. Shortly after departure from CYBX, the crew noticed a burning smell in the cockpit, followed by the flight nurse reporting that traces of smoke could be seen in the cabin. The source of smoke was identified as coming from under the middle seat area in the cabin. The crew initiated a return to CYBX, and switched the medical inverter to OFF after which the smoke appeared to dissipate. As the aircraft was preparing to land at CYBX, as the crew selected flaps to approach, the flap motor circuit breaker popped. The circuit breaker was reset successfully and the aircraft landed without further event. While taxiing back to the terminal, the flap motor circuit breaker popped a second time when the flaps were selected to the up position. The crew did not reset the circuit breaker. After investigation, the operator?s maintenance found that the flap travel limit switch cam assembly had cracked, allowing the flap to bottom out within the flap track, just before the travel limit switch being met. This allowed the flap motor to run continuously to hold the flaps in position versus the gearbox holding the flaps in position. Flap travel limit switch cam assembly was changed and, as a precautionary measure, flap motor and gearbox were also changed. The aircraft tested serviceable and was returned to service.

Incident at LOURDES-DE-BLANC-SABLON QC (CYBX)

UPDATE TSB Report #A17Q0120: C-FSKN, a Beech B200 King Air aircraft operated by Keewatin Air, was conducting medevac flight KEW203 under IFR from Chevery, QC (CYHR) to Lourdes-de-Blanc-Sablon, QC (CYBX). During the approach to CYBX, the flight crew did not receive the down and locked indication upon selection of the landing gear down. The landing gear was cycled and the left landing gear down and lock light did not illuminate. The red light on the gear handle, indicating that the landing gear is in transit, would extinguish at the end of the cycle when the landing gear was expected to be in the down and lock position. No emergency was declared, however, the flight crew requested that the airport activate emergency services as a precautionary measure. The aircraft landed at 0417Z without further incident. The operator?s maintenance discovered that the LAMP ASSY-LANDING GEAR DOWLOCK was not properly secured and that the left LANDING GEAR DOWNLOCK light was burned. The LAMP ASSY-LANDING GEAR DOWLOCK was reinstalled properly and the left LANDING GEAR DOWNLOCK light was replaced.
While it was inbound, a Keewatin Air Beech B200 (C-FSKN/KEW203), on a local flight from Lourdes-de-Blanc-Sablon (CYBX), QC, reported that it had not received confirmation of an extended and locked gear. The Montreal (CZUL) area control centre (ACC) and the airport manager (APM) at CYBX were notified. Emergency services were notified and arrived at about 0410Z. The aircraft landed at 0417Z without further incident.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. LAMP ASSY-LANDING GEAR DOWNLOCK was found not properly secured and Left LANDING GEAR DOWNLOCK light found Burned. LAMP ASSY-LANDING GEAR DOWNLOCK installed properly, and Left LANDING GEAR DOWNLOCK light replaced. Landing gear cycled several times up and down with no faults found. Aircraft was returned to service.

Incident at LOURDES-DE-BLANC-SABLON QC (CYBX)

UPDATE: JRCC Halifax SARSUM Report#[H2017-00125]: (512140N 0573291W - Lourdes-de-Blanc-Sablon, QC (CYBX). Montreal area control centre (ACC) reported that flight LAL911 had reported an inflight emergency due to mechanical issues. The aircraft successfully returned to CYBX and landed safely.
An Air Labrador Beech B100 MEDEVAC (LAL911), from Lourdes-de-Blanc-Sablon (CYBX), QC, to Natashquan (CYNA), QC, declared an emergency when it was inbound for CYBX because of an engine problem. The aircraft had decided beforehand not to land at CYNA because of an engine problem and to turn around near CYBX without declaring an emergency. It landed without incident at 0256Z. The aircraft was, however, unable to leave the runway.
UPDATE: TSB Report#A17Q0019: C-FJEA, a Beech A100 King Air aircraft operated by Air Labrador, was conducting flight LAL911M on an IFR flight from Lourdes-de-Blanc-Sablon, QC (CYBX) to Natashquan, QC (CYNA) with 2 crew members and a medic on board. At top of descent towards CYNA, the flight crew set the power levers to 1100 pounds of torque and noticed an abnormal left engine torque indication of 400 pounds. After troubleshooting, it was determined that the left engine (PWC PT6A-28) was still operating, however the torque was not responding to power lever movement. The flight crew elected to return to CYBX. While descending through 10 500 feet, the Captain noticed that the airspeed had decreased to 140 knots and decided to feather the propeller in order to decrease induced drag, after which the airspeed increased to 160 knots. As a precaution, the flight crew requested ARFF services be put on standby. The aircraft landed without further event and a precautionary engine shutdown was carried out on the runway. The operator?s maintenance performed an inspection and found that the linkage from the number 1 power lever to the number 1 fuel control unit (FCU) had become loose and dislodged. The linkage was re-secured and checks were carried out. The aircraft was returned to service.

Incident at LOURDES-DE-BLANC-SABLON QC (CYBX)

A Provincial Airlines de Havilland DHC-8-311 (C-GPAR/PVL907), from Lourdes-de-Blanc-Sablon (CYBX), QC, to Goose Bay (CYYR), NL, did not report that it had taken off after receiving Runway 05 departure advisory service. At 1620Z, the aircraft confirmed to the controller on duty at Montréal control centre that it had taken off. No impact.
