Incident at 60NM NW SAULT STE. MARIE ON (CYAM)

A Cargojet Airways Aerospatiale ATR 42-300 (CJT587) from Thunder Bay, ON (CYQT) to Winnipeg/James Armstrong Richardson, MB (CYWG) relayed a MAYDAY call from a privately registered Cessna 152, from Marathon, ON (CYSP) to Sault Ste. Marie, ON (CYAM), to the Winnipeg (WPG) area control centre (ACC). The Cessna 152 reported a rough running engine while following Highway 17 in the vicinity of Agawa Bay, ON. The CYAM tower confirmed that it was in contact with the Cessna 152, which was 20 minutes from landing. The Cessna 152 declared an emergency with the tower and landed safely at 0315Z. Incident at 11NM LA GRANDE RIVIÈRE QC (CYGL)

An American, corporately registered Cessna 510 (N862GS) from Sault Ste. Marie, ON (CYAM) to La Grande Rivière, QC (CYGL) was observed flying 5,000 feet above sea level (ASL) in a northeast (NE) direction without making contact with the air traffic services (ATS) unit. N862GS made its initial call while in the zone, 11NM west of the station, at 3000 feet ASL. Landed at 1430Z. No loss of separation, but impact on operations: delay for a Hydro-Québec de Havilland DHC-8-301(HYD153) from La Grande Rivière, QC (CYGL) to Bagotville, QC (CYBG) which was about to take-off from Runway 31.

Incident at NORTH BAY ON (CYYB)

Aviation Incident Report #16835: A passenger submitted a complaint about a Bearskin Airlines Swearingen SW4 aircraft (BLS371) from North Bay, ON (CYYB) to Fort Francis, ON (CYAG), with en route stops at: Sudbury, ON (CYSB); Sault Ste. Marie, ON (CYAM); Thunder Bay, ON (CYQT); and Kenora, ON (CYQK). The individual reported that the aircraft was scheduled to depart CYYB at 1115Z. The aircraft taxied out to the runway, but rejected its takeoff, because it did not gain any thrust. The aircraft returned to the gate, where passengers exited the aircraft. Passengers departed CYYB on a different aircraft at 1218Z.

A Bearskin Airlines Fairchild SA227-CC (C-GYTL/BLS384) on a flight from Thunder Bay, ON (CYQT) diverted to Sault Ste. Marie, ON (CYAM) due to smoke in the cockpit. The aircraft landed without further incident.
UPDATE: TSB Report #A20O0098: A Bearskin Airlines Fairchild SA227-CC (C-GYTL), flight BLS384, was on route from Thunder Bay, ON (CYQT) to Sudbury, ON (CYSB) and approximately 40NM North of Sault Ste. Marie, ON (CYAM) when the crew noticed smoke in the flight deck. The flight crew carried out the checklist, declared a "PAN PAN" and diverted to CYAM. The aircraft landed without further incident. Maintenance inspected the aircraft and found that a center pedestal back lighting circuit panel had failed.

The pilot of an Airborne Sensing Corporation Piper PA-31-325 (C-FPXL) filed a controlled VFR flight plan (FP) with the London flight information centre (FIC) with an estimated time of departure (ETD) of 1630Z from Sault Ste. Marie, ON (CYAM) to Muskoka, ON (CYQA). The flight plan went overdue at 1730Z. The pilot called to file a FP out of North Bay, ON (CYYB) and advised that they changed their destination from CYQA to CYYB and had "cancelled" their flight plan with the Toronto Centre. The pilot was advised of the FIC flight information service en route (FISE) frequencies in the area and that FICs provide VFR flight plan alerting.

Multiple reports of an emergency locator transmitter (ELT) signal near Sault Ste. Marie, ON (CYAM). The Joint Rescue Coordination Centre (JRCC) Trenton was advised that a privately registered Cessna 182F on floats at the Bush Float Museum was confirmed as the source, and now under water. The National Operations Centre (NOC), Aviation Operations Centre (AVOPS), and the Transportation Safety Board of Canada (TSB) were advised. No known NAV CANADA involvement. No operational impact.

A Bearskin Airlines Swearingen SW4 (BLS377) from Sudbury, ON (CYSB) to Sault Ste. Marie, ON (CYAM) struck a bird on landing.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After C-FXUS/BLS377 landed in Sault Ste. Marie, the tower requested they carry out a runway inspection for birds while taxiing to the ramp. One dead bird was seen on the runway. After shutdown, the flight crew carried out a walk around inspection of the aircraft, with no signs of a bird strike. It was determined the dead bird was from a previous flight.

Accident at 15NM NE SAULT STE. MARIE ON (CYAM)

Joint Rescue Coordination Centre (JRCC) Trenton called the Toronto Area Control Centre (ACC) Shift Manager to advise they received a call from North Bay Ontario Provincial Police (OPP) to report an aircraft accident. A privately registered Piper J3L-65 floatplane crashed on a lake at 46 40.40N 84 15.20W (North of Sault Ste. Marie, ON). Two souls on board, both out of the aircraft with minor injuries. No known NAV CANADA involvement. (Duplicate CADORS 2020O1064 was deleted after information was merged)
Update: Aviation Incident Report #16729: A privately registered Piper J3L-65 from Spruce Lake, ON to Upper Island Lake, ON made a glassy water landing, bounced once, and then the right float dug into the water causing the aircraft to flip. The aircraft remained floating inverted until it was towed to shore, removed from the water and disassembled for salvage. (Duplicate CADORS 2020O1064 was deleted after information was merged)
Update: TSB Report #A20O0074: A privately registered Piper J3L-65 on floats was landing on Upper Island Lake, ON, on glassy water. The aircraft bounced after touchdown, and on the subsequent contact with the water the right float dug into the water and the aircraft overturned. The two occupants were helped from the water by nearby boaters before being brought to the hospital. The aircraft remained inverted but floating, and was towed to shore and removed from the water the next day. The aircraft was substantially damaged and will not be returned to service.

Incident at TORONTO / OSHAWA EXECUTIVE AIRPORT ON (CYOO)

An American, corporately registered Cessna 525A (N21RA) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) reported a fuel pressure issue shortly after departure. The pilot switched off the right engine (1 of 2) and requested an emergency landing. Fire trucks were called, and arrived approximately 1 minute before the aircrafts safe landing. Aircraft taxied in under its own power without issue. Fire trucks stood down. All appropriate agencies were notified. Minimal impact on operations.
Update TSB Report #A20O0051: The pilot of a corporately registered Cessna 525A (registration N21RA) on route from Oshawa, ON (CYOO) to Sault Ste Marie, ON (CYAM) declared an emergency shortly after departure due to a fuel pressure issue. The pilot shutdown the right engine (Williams, FJ-44), returned to Oshawa and landed without incident.

Incident at SAULT STE. MARIE ON (CYAM)

TSB Report #A20O0033: The Sunwing Airlines Boeing 737-800 aircraft (SWG374, C-FJVE) was on climb out from Sault Ste. Marie (CYAM), when the flight crew received a Fuel Imbalance Alert for the right tank. The flight crew declared a PAN PAN and the aircraft levelled off at FL270. The crew initiated the fuel imbalance checklist and determined that the Crossfeed Valve was open. The checklist was completed and the fuel was allowed to balance the loads in the tanks. The flight continued to destination without further faults.

Incident at 120NM SE THUNDER BAY ON (CYQT)

A Bearskin Airlines Swearingen SA227-DC (C-GJVW/BLS377) from Sault Ste. Marie, ON (CYAM) to Thunder Bay, ON (CYQT) declared an emergency due to an engine fire light. BLS377 continued to CYQT with a single engine, and landed without incident.
UPDATE TSB Report #A20O0039: Bearskin Airlines flight BLS377, a Fairchild SA-227DC aircraft (C-GJVW), was on route from Sault St. Marie (CYAM), ON to Thunder Bay (CYQT), ON when the right engine (GARRETT, TPE331-12UHR) fire lights illuminated. The flight crew performed the emergency procedures, shut down the engine and declared an emergency. The aircraft continued to Thunder Bay and landed without incident. No evidence of fire was detected during inspection, maintenance are investigating the cause of the fire indication.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Metro 23 aircraft C-GJVW had an inflight engine fire indication. Engine was shut down, fire bottle was discharged and the aircraft landed safely in CYQT. Maintenance found that the fire control module was unserviceable (U/S) and was replaced. Aircraft returned to service after a successful test flight.

Incident at SELAP intersection

A Jazz de Havilland DHC-8-300 (JZA8330) from Sault Ste. Marie, ON (CYAM) to Toronto/Lester B. Pearson, ON (CYYZ) was instructed to fly direct DULPA after SELAP intersection for the Instrument Landing System (ILS) 05, which was correctly read back. Just prior to SELAP, JZA8330 made an unexpected left turn towards ERBUS. The controller cancelled the approach clearance and vectored JZA8330 for an intercept. There was no conflict and JZA8330 landed at 2211Z.

A Bearskin Airlines Fairchild SA227-DC (C-GJVO/BLS373) from Sault Ste. Marie, ON (CYAM) to Thunder Bay, ON (CYQT) initiated a missed approach due to nose wheel indication. Aircraft landed safely, no operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. C-GJVO carried out a missed approach due to a nose landing gear (NLG) having no down and locked green light indication. Eventually the green light illuminated and the aircraft landed safely. Maintenance found the NLG Right-Hand (R/H) down-lock switch intermittent. Switch replaced and aircraft returned to service.

TSB Report #A20O0013: C-GROV, an Airbus A220-300 aircraft operated by Air Canada flight AC318, was on route from Calgary (CYYC), AB to Montreal/Pierre Elliott Trudeau (CYUL), QC in an area forecasting light to moderate turbulence. The flight encountered severe turbulence that was not forecasted lasting 5-10 seconds. The seat belt lights were on and the cabin crew seated; however, one passenger struck the ceiling above their seat and suffered cuts to their head. A medical emergency was declared to get priority with ATC, the flight landed without further incident.

SYB109 was instructed to fly heading 220 degrees, and climb to 3,000 feet. The aircraft was observed in a right hand turn, conflicting with departing traffic, an Air Canada Rouge Airbus A321-211 from Toronto/Lester B. Pearson, ON (CYYZ) to Puerto Plata, Dominican Republic (MDPP), on Runway 23.

SYB109 was instructed to fly heading 220 degrees, and climb to 3,000 feet. The aircraft was observed in a right hand turn, conflicting with departing traffic (JZA8330 and ROU1820) on Runway 23.
Update TSB Report #A20O0022: C-FSQY, a Bombardier CL-600-2B16 flight SYB109 operated by Skyservice Business Aviation, was on final approach to Toronto/Lester B Pearson Intl. (CYYZ), ON. Due to high rate of closure with the preceding arrival, C-FACT, a De Havilland DHC-8-300 aircraft operated by Jazz Aviation LP flight JZA330, SBY109 was instructed to pull up and go around. Spacing was reduced to 2.4 NM horizontal where 3 NM was required. SYB109 was instructed to fly heading 220 degrees and climb to 3,000 feet. The aircraft was observed in a right hand turn conflicting with departing traffic on RWY 23, C-GHPJ an Air Canada Rouge flight ROU1820 Airbus A321-211 aircraft. SYB109 was subsequently instructed to turn left to heading 150 degrees to correct the conflict.
