Incident at 44NM GRANDE PRAIRIE AB (CYQU)

A Fltlink Inc. Cessna 340 (C-GRFL) that was enroute from Grande Prairie, AB (CYQU) to Olds-Didsbury, AB (CEA3) reported a rough-running engine. The aircraft returned to CYQU without incident. No assistance needed, no emergency declared. No impact to operations.
UPDATE: Duplicate AOR 274420-V1: A Fltlink Inc. Cessna 340 (C-GRFL) on an IFR flight from Grande Prairie, AB (CYQU) to Olds-Didsbury, AB (CEA3) departed CYQU at 2129Z but had to return to CYQU due to a mechanical issue. Accident at OLDS-DIDSBURY AB (CEA3) (Aerodrome, runway or taxiway shutdown)

A Springbank Air Training College Ltd. Cessna 172P (C-GSKF) from Calgary/Springbank, AB (CYBW) to Olds-Didsbury, AB (CEA3) landed short of the runway, causing the nose wheel to separate. The aircraft came to rest on the runway. A NOTAM was issued, closing the runway until the aircraft was removed. No impact to NAV CANADA operations. One VFR aircraft was diverted to a nearby aerodrome.
UPDATE: TSB Report #A20W0079: C-GSKF, a Cessna 172P aircraft operated by Springbank Air Training College, was being used for a private pilot flight test originating from and returning to Calgary/Springbank, AB (CYBW), with one pilot examiner and one student pilot on board. As part the flight test profile, an inspection pass exercise was being conducted on Runway 28 at Olds-Didsbury, AB (CEA3). After the inspection pass, the student pilot performed a go-around and, at that time, the engine (Avco Lycoming O-320-DJ2) lost power. Control was passed from the student to the pilot examiner (PIC). The PIC elected to conduct an immediate left turn to join the downwind for landing on Runway 28. While maneuvering to land on Runway 28, the left wing tip contacted the snow covered grass area north of and adjacent to the threshold of the runway. Shortly after, the nose contacted the ground, resulting in the collapse of the nose landing gear, and damage to the propeller. The aircraft continued to slide on a track of approximately 310°M, until it contacted a ditch that runs parallel to and north of the runway. The aircraft came to rest facing in a near north direction. The aircraft was substantially damaged, but no injuries were reported. Post-accident inspection by maintenance found that there was little fuel remaining in the aircrafts fuel system.

Incident at OLDS-DIDSBURY AB (CEA3) (Aerodrome, runway or taxiway shutdown)

The pilot of a privately registered, amateur-built, Lancair-ES from Calgary/Springbank, AB (CYBW) to Calgary/Springbank, AB (CYBW) advised that the front landing gear collapsed upon landing at Olds-Didsbury, AB (CEA3). The propeller struck the ground. No injuries. NOTAM issued for runway closure. No further impact to operations. The Transportation Safety Board of Canada (TSB) and Aviation Operations Centre (AVOPS) were advised.
UPDATE: Aviation Incident Report #16805: The pilot-in-command (PIC) and operator reported that this flight, originating at CYBW, was a currency and fuelling flight. Upon departure, the temperature was 18 degrees Celsius, barometric pressure was 29.75, and winds were 280 at 10 knots. The aircraft flew at 5500 in controlled airspace, as well as outside of the control area, to CEA3, then joined the left downwind for Runway 28. The pilot weather report (PIREP) was received, and the aircraft turned base and final uneventfully. Visibility was extremely hazy but VFR. While on short final, the aircraft encountered a down draft approximately 50 feet above ground level (AGL) and 75 feet from the threshold. The landing gear touched the ground, the aircraft experienced a bounce and touched down on the runway, with the front landing gear causing a front gear collapse and then a subsequent propeller strike. The aircraft was unable to leave the runway and assistance was provided by local pilots. In addition, the NAV CANADA National Operations Centre (NOC) reported that the nose gear collapse occurred during roll out at CEA3, that there was 1 pilot on board who exited the aircraft uninjured, and that the Transportation Safety Board (TSB) was notified and released the scene to permit the aircraft to be towed off the runway.

Incident at OLDS-DIDSBURY AB (CEA3) (Aerodrome, runway or taxiway shutdown)

At 1743Z, an Olds-Didsbury, AB (CEA3) employee called to report that a privately registered, amateur-built Glasair SH-2R from Rocky Mountain House, AB (CYRM) to Olds-Didsbury, AB (CEA3) had performed a gear-up landing at CEA3. Spoke to the pilot, there was damage to the propeller and the belly. The Shift Manager and Joint Rescue Coordination Centre (JRCC) were advised. Both runways at CEA3 were closed via NOTAM due to the disabled aircraft. The runways opened at 1825Z. Impact to operations unknown.

Accident at 1NM NW OLDS-DIDSBURY AB (CEA3)

Accident report received regarding a privately registered Grumman GA-7. No impact on operations.
Update TSB Report #A19W0142: C-GVTB, a privately operated Grummond GA-7, was conducting a flight from Olds-Didsbury (CEA3), AB with one pilot on board. After take-off on Runway 28, and approximately 1.0 NM northwest of CEA3, there was a complete loss of engine power on the left engine (AVCO LYCOMING, O-320-D1D). The pilot elected to force land the aircraft straight ahead. On ground contact C-GVTB contacted a copse of trees and substantial damage was incurred to the nose gear, left engine and propeller (HARTZELL, HC-F2YL-2UF), tail section and both wing surfaces. The pilot sustained minor injuries and an ELT signal was received by the Joint Rescue Coordination Centre (JRCC) Trenton. The owner will report to TSB findings for the loss of power.

Update TSB Report #A19W0126: C-FHRE, a privately owned Van’s RV-9A aircraft, was conducting a training flight from Olds-Didsbury, (CEA3), AB with one instructor pilot and one student pilot onboard. The student pilot was attempting to take off from Runway 04 and experienced difficulty with a nose high attitude and a left cross-wind. The instructor pilot took control as the aircraft started to settle and the main wheels contacted a berm at the end of the runway resulting in damage to the propeller and nose wheel. There were no injuries reported.

The Royal Canadian Mounted Police (RCMP) called the flight information centre (FIC) to report that a Olds-Didsbury Flying Association Piper PA-28-180 (C-FCJK) hit a power line and then landed at Olds-Didsbury, AB (CEA3).
UPDATE: TSB Report #A18W0006: C-FCJK, a Piper PA-28-180 aircraft operated by the Olds Didsbury Flying Association, was on a local flight from Olds/Didsbury, AB (CEA3). C-FCJK was conducting touch and go landings during the hours of darkness. Three circuits were completed and during final approach for the last landing, approximately 800 feet from the Rwy 28 threshold, the aircraft contacted power lines. Airport power was lost and the runway lighting extinguished. C-FCJK landed uneventfully. The pilot, sole occupant, was uninjured; C-FCJK received damage to the propeller, windscreen and vertical stabilizer.

The Pilot of a privately registered Cessna 172M filed a VFR flight plan from Olds-Didsbury, AB (CEA3) to Drumheller, AB (CEG4) with an estimated-time-of-arrival (ETA) of 1814Z. No modification or arrival messages received by 1844Z, and a communication search was initiated. Information request (INREQ) commenced at 1914Z. Trenton Joint Rescue Coordination Centre (CYTR JRCC) and Royal Canadian Mounted Police (RCMP) advised. The pilot contacted Edmonton Flight Information Centre (CYEG FIC) via cell phone at 1946Z and said the Flight Plan (FPL) estimated time enroute (ETE) should have been 04+00 and updated FPL. CYTR RCC and RCMP advised aircraft located.

Aviation Incident Report#14944: The Olds Didsbury flying association Cessna 150 was on final approach for runway 10 at Olds Didsbury, ON (CEA3) and planning to do a touch and go. There was a cross wind from the right about 15 kts. The touchdown was uneventful. The ground roll became unstable due to excessive rudder input. The Check pilot took control of the aircraft on the ground roll but it veered off to the right of the runway and came to a stop in a ditch. There was damage to the nose wheel, nose cone, propeller. There were no injuries.

Accident at OLDS-DIDSBURY AB (CEA3)

UPDATE: TSB Report#A17W0091: a privately operated Amateur Built Supermarine Spitfire MK 26B aircraft, was conducting a local flight from Olds-Didsbury, AB (CEA3). The aircraft was lined up for takeoff on Runway 28 and the pilot began the takeoff roll with winds from 230° M at 4 knots. As the aircraft accelerated, the pilot lifted the tail off the ground, however the nose veered to the left. The aircraft became airborne in ground effect and the pilot was unable to maintain directional control. The aircraft continued to drift to the left while beginning to settle and contacted the ground to the left of Runway 28. The main landing gear broke and the aircraft came to rest approximately 60 feet off the south side of the runway surface. The pilot, who was the sole occupant, received minor injuries and the ELT activated.
A privately registered, amateur-built, Supermarine Spitfire MK 26B, on a flight from Olds-Didsbury, AB (CEA3) crashed on take off at CEA3. Log and records at Edmonton, AB (CYEG) flight information centre (FIC) has no records on file. No services provided by CYEG FIC.
UPDATE: JRCC Trenton SARSUM Report#[T2017-00994]: (514272N 1140640W - CEA3 : OLDS DIDSBURY). Received a call from Transportation Safety Board (TSB) Edmonton advising of an aircraft crash on take-off at the Olds Didsbury Airport (CEA3). The 1 person on board (POB) had suffered minor injuries and was taken by paramedics to the hospital. Ontario Provincial Police (OPP) had secured the scene and handed it over to Transportation Safety Board (TSB) after taking aircraft photos. Emergency locator transmitter (ELT) was confirmed shut off.

A privately registered Cessna 310R from Olds-Didsbury, AB (CEA3) to Abbotsford, BC (CYXX) was cleared by the Princeton, BC (DC) Controller to 10,000 on the Hope 2 arrival referencing a WestJet Boeing 737 700 (WJA151) was inbound to CYXX. On hand-off to the Abbotsford, BC (XX) Controller, the Cessna descended to 9400 and was cleared to 9000 feet. When queried, the pilot responded that their last cleared altitude was 11,000 or 10,000. WJA151 was radar clear of the Cessna when they descended through 10,000. No operational impact.

Incident at OLDS-DIDSBURY AB (CEA3)

MAYDAY from a privately registered Cessna 182L on a local flight from Olds-Didsbury, AB (CEA3) was picked up by an Opsmobil Inc. Cessna 172R (C-FIGL) and relayed to Edmonton, AB (YEG) flight information centre (FIC). A passenger from Cessna 182L called to report that due to an engine failure, the aircraft had completed a forced landing in a field 10 east of CEA3. There were no injuries and no damage to the aircraft reported by the pilot.
UPDATE: TSB Report#A16W0172: a privately operated Cessna 182L, was on a local round robin flight from Olds-Didsbury, AB (CEA3). Approximately 10 nautical miles East of CEA3, the engine (Teledyne Continental O-470-R) lost all power. The pilot broadcast a MAYDAY call which was relayed by another aircraft to air traffic services. A successful forced approach was conducted into a field with no damage to the aircraft and no injuries to the pilot or passenger; the ELT did not activate. Maintenance personnel found a considerable amount of ice in the carburetor air box. The ice was removed, engine runs completed and the aircraft departed the field

A Calgary Flying Club Champion 7ECA (C-GIBB) on a local flight out of Calgary, AB (CYBW) slid off the end of the runway at Olds-Didsbury, AB (CEA3). The aircraft's propeller was damaged. No injuries were sustained. The pilot advised Edmonton flight information centre (FIC) and stated they did not require any assistance.
UPDATE: TSB#A16W0159: C-GIBB, a Champion (Citabria) 7ECA aircraft operated by the Calgary Flying Club, was operating on a VFR flight from Calgary/ Springbank, AB (CYBW) to Olds-Didsbury, AB (CEA3). Upon touchdown at CEA3, directional control was lost during the rollout after the pilot conducted a threepoint landing. The aircraft veered to the right and departed the runway surface. Power was added in an effort to regain control, however the aircraft went over a drainage ditch and came to rest on the other side. The pilot and the passenger were not injured, and the ELT was not activated.

ATC issued them a lower clearance limit. C-GMDM descended a second time without clearance and no contact with ATC. No operational impact.

UPDATE: TSB#A16W0093: The privately operated Cessna A185F, C-GGRQ, was on a VFR flight from Olds-Didsbury, AB (CEA3) to Ft. Chipewyan, AB (CYPY). During landing on Runway 04, the pilot experienced a gusting rear quartering tailwind during roll out. Directional control was lost (ground loop) and the aircraft exited the right side of the runway. The left main landing gear collapsed resulting in damage to the propeller and left wing tip. The pilot, who was the sole occupant, was uninjured
A Special Projects Inc. Cessna A185F (C-GGRQ) from Olds-Didsbury, AB (CAE3) to Fort Chipewyan, AB (CYPY), was landing when crosswinds blew the aircraft to the right and into the grass. The left strut broke on impact with ground.

An American corporately registered Beech C90 (N904DK / HAILST1) from Old-Didsbury, AB (CEA3) to Red Deer, AB (CYQF) struck a magpie on landing roll out on Runway 34. No impact on operations.

Incident at OLDS-DIDSBURY AB (CEA3)

UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Upon receiving the report, the Royal Canadian Mounted Police (RCMP) sent a Constable to the location where the fireworks was reported. The RCMP found no evidence of fireworks activity.
A Fly Right Ltd. Cessna 172M (C-GWMB) from Calgary, AB (CYBW) to Olds-Didsbury, AB (CEA3) was in the circuit when the pilot reported fireworks being discharged in the vicinity of the aircraft (exploding approximately 50 feet from the aircraft), while on two separate final approaches. There was no impact on the flight. The pilot asserts it seemed intentional. The area control center (ACC) shift manager and the Royal Canadian Mounted Police (RCMP) were advised. Incident at SWAN RIVER MB (CZJN)

The pilot of a J & J Baldwin Limited Cessna 172M (C-GHAN), from Olds-Didsbury, AB (CEA3) to Swan River, MB (CZJN), failed to close the flight plan. A Communications Search was initiated and the Joint Rescue Coordination Centre (JRCC) Trenton was advised. Pilot was located at home by JRCC. Communications Search was terminated and flight plan was closed.

At 1810Z, a Unity Group Ltd. de Havilland DHC-6 Series 300 (C-GKPV), from Olds-Didsbury (CEA3), AB, diverted to Airdrie (CEF4), AB, due to mechanical issues. No impact to the operation.

Incident at OLDS-DIDSBURY AB (CEA3)

Multiple reports of a strong Emergency Locator Transmitter (ELT) signal from 1443Z until 1612Z northeast of Calgary, AB. Joint Rescue Coordination Centre (JRCC) Trenton advised ELT was from a G. Gatez Farms Ltd. Air Tractor AT-401 (C-GBTK) found at Olds-Didsbury, AB (CEA3).
Follow-up information received from Enforcement: A Civil Aviation Safety Inspector (CASI) has reviewed this occurrence and the appropriate action has been taken.

Incident at In the vicinity of: OLDS-DIDSBURY AB (CEA3)

UPDATE: JRCC SARSUM Trenton Report#[T2015-00854]: (514272N 1140640W - CEA3: OLDS DIDSBURY). Edmonton area control centre (ACC) reported an aircraft declaring a PAN PAN. Aircraft stated they were landing in Chestermere AB. Investigation determined that the aircraft had actually continued on and landed at Olds Didsbury. Had a rough running engine. No further distress or need for assistance.
At 1735Z, a privately-registered Cessna A185F called "PAN PAN PAN" due to engine issues. Joint rescue coordination centre (JRCC) Trenton was advised. The aircraft landed safely in Olds Didsbury, AB (CEA3).

Incident at LETHBRIDGE AB (CYQL) (Flight plan ? activation)

A privately-registered Van's RV-7A, from Hot Springs, SD (KHSR) to Olds-Didsbury, AB (CEA3), conducted a transborder flight arriving in Canada without an active flight plan. Pilot filed a VFR flight plan with Fore Flight; departing KHSR with a stop over at Lethbridge, AB (CYQL) and a final destination of CEA3. Pilot stated he activated flight plan with Denver Centre. Flight plan was not processed.

A privately registered Mooney M20K from Olds/Didsbury, AB (CEA3) to Vernon, BC (CYVK) climbed to 14,500 feet into controlled airspace without clearance. No impact to operations.
