A Panorama Fixed Wings Ltd. Pilatus PC-12 (PNO729) from Quebec/Jean Lesage Intl, QC (CYB) to Montreal/St-Hubert, QC (CYHU), established at 4NM final for Runway 24R, reported being struck by a green laser at 2-3NM west of their position. No other impact. Royal Canadian Mounted Police in Quebec (GDQ) advised.

Incident at KUUJJUARAPIK QC (CYGW) (ATM - inaccurate aeronautical information, Aerodrome - runway or taxiway surface condition)

An erroneous runway report was issued at Kuujjuarapik, QC (CYGW). An Air Inuit de Havilland DHC-8-314 (C-FYAI/AIE803) from Umiujaq, QC (CYMU) to Kuujjuarapik, QC (CYGW) reported a windrow, 1 to 2 feet high, on the runway surface, 15 feet from the edge of the runway, on the west side. The aircraft also reported a windrow on Taxiway A. The 2 windrows were not reported in the runway surface condition (RSC) report. AIE803 also advised that they had taxied on Taxiway A because a snow bank was blocking access to the ramp.

Incident at 20NM S CHISASIBI QC (CSU2) (Aerodrome, runway or taxiway shutdown)

A Propair Inc. Beech A100 (C-GJJF/PRO4210) from Rouyn-Noranda, QC (CYUY) to Chisasibi, QC (CSU2) declared an emergency 30NM West of La Grande Rivière, QC (CYGL) due to a left engine failure. Emergency procedures were carried out. The aircraft touched down at 2316Z. Runway re-opened at 2330Z. Impact: delay for an Air Inuit de Havilland DHC-8-300 (AIE861) from Puvirnituq, QC (CYPX) to La Grande Rivière, QC (CYGL), which landed at 2338Z.

UPDATE: TSB Report #A20Q0101: C-FDOU, a Beech Aircraft Corporation A100 operated by Propair Inc. (flight PRO4210), was on an instrument flight rules flight from Rouyn-Noranda, QC (CYUY) to Chissasibi, QC (CSU2) with 2 crew members and 2 passengers on board. At 2245Z, with the aircraft on initial approach approximately 20 nautical miles south of CSU2, a left engine (Pratt & Whitney Canada PT6A-28) oil pressure fluctuation occurred. The crew shut down the engine, declared a state of emergency, and diverted to La Grande Rivière, QC (CYGL). The aircraft landed safely, and there was no injury or damage.

An American, corporately registered Cessna 510 (N862GS) from Sault Ste. Marie, ON (CYAM) to La Grande Rivière, QC (CYGL) was observed flying 5,000 feet above sea level (ASL) in a northeast (NE) direction without making contact with the air traffic services (ATS) unit. N862GS made its initial call while in the zone, 11NM west of the station, at 3000 feet ASL. Landed at 1430Z. No loss of separation, but impact on operations: delay for a Hydro-Québec de Havilland DHC-8-301(HYD153) from La Grande Rivière, QC (CYGL) to Bagotville, QC (CYBG) which was about to take-off from Runway 31.

Incident at 30NM WNW MATAGAMI QC (CYNM)

In flight, a Propair Inc. Beech A100 (C-FEYP/PRO4230) on a MEDEVAC flight from Rouyn-Noranda, QC (CYUY) to La Grande Rivière, QC (CYGL) advised that they were initiating an emergency descent due to a pressurization problem. The aircraft did not declare an emergency or request assistance. No impact on operations.
Update: TSB Report #A20Q0098: C-FEYP, a Beech A100 operated by Propair, was on flight PRO4230 (MEDEVAC) from Rouyn-Noranda, QC (CYUY) to La Grande Rivière, QC (CYGL) with 4 crew members on board. In cruise flight, at flight level 210 about 30 nautical miles (NM) west-northwest of Matagami, QC (CYNM), the aircraft underwent a complete depressurization. The crew notified ATC that they were initiating an emergency descent due to a pressurization problem. Once level at 8,000 feet ASL, the crew cancelled the emergency and the flight to CYGL was completed. There were no injuries.

An R44 helicopter flew through the Chicoutimi/St-Honoré, QC (CYRC) control zone, 2NM West of CYRC, without communication. The aircraft was found to be a Héli-Tremblant Robinson R44 (C-GVUB) on a flight to Manic 5/Lac Louise, QC (CSH8) that was reached on 126.7 MHz. Impact: traffic information was passed to a Cessna in the circuit.

Incident at TROIS-RIVIÈRES QC (CYRQ) (Aerodrome, runway or taxiway shutdown)

Aviation Incident Report # 16757: An École De Pilotage Évolution Inc. Diamond DA 20-C1 (C-GEWM) on a flight to Trois-Rivieres, QC (CYRQ) landed at 1355Z on a portion of the runway that was closed by NOTAM. The runway was closed between the threshold of Runway 05 and Taxiway Charlie for patching operations. The pilot was warned twice by the universal communications (UNICOM) attendant that they had to operate east of Taxiway Charlie, but still landed west of Charlie. The offset threshold was identified by orange cones. No worker was on the runway at the time of the incident.

Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16706: A Government of Canada, Department of National Defence Dassault Ajet (CHETA28) from Bagotville, QC (CYBG) to Bagotville, QC (CYBG) was inbound to the airfield via the West and was cleared to join initial (3NM final at 2000 feet) via the South, while a privately registered Cub L4BX from Lac des cèdres, QC to Lac Otis, QC (without a transponder) was flying towards the airfield at 2500 feet. Both aircraft came to 1NM from each other at co-altitude without traffic passing or situation awareness of the second aircraft. In reaction to seeing a civilian aircraft to its left, CHETA28 dived to get below the altitude of the Cub L4BX to avoid any risk of collision. The Cub L4BX had no transponder and a misunderstanding let the Tower Controller think the pilot was heading towards the water (La Baie); therefore, no conflict with CHETA28 was expected. No further incident, both aircraft landed safely at their destinations.

Incident at LA SARRE QC (CSR8)

The pilot of a 4431804 Canada Inc. Cessna 182P (C-GJAD) from St-Hyacinthe, QC (CSU3) to La Sarre, QC (CSR8) declared a MAYDAY. Their intentions were to land on Route 11, West of Lac Abitibi. They finally managed to get to their destination of CSR8. No known impact, pilot safe and sound.

Incident at TROIS-RIVIÈRES QC (CYRQ)

Aviation Incident Report # 16680: At 1556Z, 5 parachutists deviated from their course and found themselves near the runway at Trois-Rivières, QC (CYRQ). An airport employee went to retrieve them with a truck. 2 tandem parachutists landed near the threshold of Runway 23. 2 other tandem parachutists and 1 solo parachutist landed 100 feet West of the weather station. The parachutists jumped from a Parachutisme Adrénaline Inc. Cessna P206A (C-FMHQ) from Trois-Rivières, QC (CYRQ) to Trois-Rivières, QC (CYRQ). 1 aircraft was on initial climb and C-FMHQ was on final approach at the time of the incident.

Accident at 1NM WSW ROUGEMONT QC (CTY5)

TSB Report #A20Q0061: A Kangook XLITE paramotor, in private operation, carried out a local flight according to visual flight rules (VFR) from a field located 1 nautical mile west-southwest of Rougemont (CTY5), QC. After a flight of about 30 minutes, then in short final without an engine, the pilot made a left turn in order to benefit from the little wind. The left side of the wing then lost lift and the pilot lost control of the aircraft and crashed. The pilot sustained minor injuries and the aircraft sustained significant damage.

Incident at 5NM NE MONTRÉAL / ST-HUBERT QC (CYHU)

A privately registered Robinson R44 II on a flight from St-Mathieu-de-Beloeil, QC (CSB3) was observed by radar to be inside the Montreal/Saint-Hubert, QC (CYHU) control zone, about 5NM northeast (NE) of CYHU. The aircraft crossed the CYHU control zone along Highway 20, Eastbound from the LH Lafontaine tunnel bridge to 2NM West of CSB3, at an altitude of 1200ft, without having obtained authorization. The aircraft reported attempting to contact the tower several times but said there was no answer. No impact. (Duplicate AOR-264915-V1 deleted after information merged)

Incident at 15NM W QUÉBEC / JEAN LESAGE INTL QC (CYQB)

An Exact Air Beech A100 (C-FLTS/ET410) from Quebec/Jean Lesage Intl, QC (CYQB) to Val-dOr, QC (CYVO) requested to return to CYQB due to a fuel leak. During the descent toward Runway 06, ET410 declared PAN PAN and requested emergency services on the side of the runway. ET410 landed on Runway 06 at 1719Z without issue. No impact to operations.
Update TSB Report #A20Q0062: C-FLTS, a Beech A100 King Air type aircraft operated by Exact Air under the designator ET410, was flying under instrument flight rules from Quebec/Jean Lesage, QC (CYQB) to Val-dOr, QC (CYVO). While the aircraft was at an altitude of 8,000 feet uphill after departure, the crew observed a slight fuel leak from the left engine nacelle. The crew requested a return to CYQB due to a fuel leak. 15 nautical miles west of CYQB, during the descent for Runway 06, the crew declared a PAN PAN. The aircraft landed without problems on Runway 06 at 1719 UTC with the presence of the aircraft fire fighting and rescue unit (ARFF). The maintenance department discovered a leak via the nacelle probe.

During the aircrafts approach to Montreal/Aeroparc Ile-Perrot, QC (CSP6), the controller contact the aircraft, which was identified as a privately registered Cessna 177. No impact.

The air traffic controller (ATC) cleared an American corporately registered Raytheon Hawker 900XP (N900XP) from West Palm Beach Intl, FL (KPBI) to Quebec/Jean Lesage Intl, QC (CYQB) to fly at 5 000 ft. The pilot read back 5 000 ft, then descended to 4 500 ft, above a CYA at 4 000 ft. No impact on operations. N900XP was in a state of emergency due to lack of fuel. The aircraft conducted a missed approach on Runway 06 due to unstable approach. The aircraft landed without incident at 1946Z. No impact on operations.

Ground staff indicated that a Héli-Boréal Inc. Aerospatiale AS 350 B-2 (C-GLQM) from Havre-St-Pierre, QC (CYGV) had taken off from the airport at CYGV towards the west. Radio (RDO) at Madeleine 122.0 was never contacted. Traffic in the mandatory frequency (MF) zone was informed of its presence. No impact on operations.

Accident at 8NM W ALMA QC (CYTF)

Several aircraft at high altitude reported hearing an emergency locator transmitter (ELT) signal in the Roberval, QC (CYRJ) area. The Joint Rescue Coordination Centre (JRCC) Trenton was notified. The signal was coming from the accident site of a Government of Quebec Bell 206L-4 (C-GSQA), which crashed West of Alma, QC (CYTF). 1 person on board (POB) was injured. RSCU450 en route to the crash site. No service provided. (Information from duplicate record AOR 260983-V1 / CADORS 2020Q0188 merged before deleting duplicate occurrence)
TSB Update #A20Q0015: C-GSQA, a Bell 206-L4 helicopter privately operated by the Government of Quebec (QUE49), was on a visual flight rules (VFR) flight from Alma, QC (CYTF) to La Tuque, QC (CYLQ). Earlier, the aircraft had been participating in a search and rescue operation for snowmobilers reported missing. Eight minutes after its takeoff, the aircraft crashed into the frozen surface of Lake Saint-Jean. The pilot was seriously injured and was rescued by another helicopter participating in a search. The emergency locator transmitter (ELT) went off. The aircraft was substantially damaged.

Incident at LANGLEY REGIONAL BC (CYNJ) (Aerodrome, runway or taxiway shutdown)

A privately registered, amateur-built Murphy Rebel from Langley, BC (CYNJ) to Langley, BC (CYNJ) landed on Runway 01 and exited the runway at Taxiway E to the west side. The aircraft had a problem with control due to wet brakes and exited the taxiway onto the grass. The aircraft was not across the hold line and was therefore obstructing the runway. A privately registered Cessna 172K on a flight to Langley, BC (CYNJ) was told to pull up and go around, and operations were changed to Runway 07.

At 1200Z, a target appeared on radar approximately 10NM East of Tofino/Long Beach, BC (CYAZ), and continued west through the North side of the CYAZ control zone. The target reappeared 3NM North of CYAZ and continued descending towards Tofino harbour. Multiple attempts at communication were made. No impact to operations.

Incident at PENTICTON BC (CYYF) (Weather balloon, meteor, rocket, CIRVIS/UFO)

A firework was shot in front of a BP Aviation Ltd. Cessna 172M (C-GHBM) from Penticton, BC (CYYF) to Penticton, BC (CYYF) while the aircraft was in the right downwind for Runway 34. The firework may have originated from the school field west of Penticton hospital. There was no damage to the aircraft. The incident was reported to the Royal Canadian Mounted Police (RCMP).

A Skyquest Aviation Ltd. Cessna 152 (C-GTYQ) from Langley, BC (CYNJ) to Langley, BC (CYNJ) was cleared for the left base for Runway 19, but joined the right base for Runway 01 instead. After being corrected by the tower, the pilot made several attempts to join the circuit, but had difficulty following procedures and conducted an overshoot on final to spin on the west side of the control zone.

The pilot called the Kamloops flight service station (FSS) via cell phone, just west of Savona, BC, with an estimated time of arrival (ETA) of 0042Z. The aircraft landed safely at 0047Z. No impact to operations.

A Pacific Coastal Airlines Beech 1900 (PCO9191) from Vancouver, BC (CYVR) to Port Hardy, BC (CYZT) reported a bear on the west side of Runway 29 at CYZT. The Vancouver (YVR) area control centre (ACC) controller was advised.

A traffic alert and collision avoidance system (TCAS) resolution advisory (RA) was reported by a WestJet Encore Ltd. Boeing 737-8CT (C-FWSE/WEN3281) from Calgary, AB (CYYC) to Penticton, BC (CYYF) at approximately 4 miles final for Runway 16. A Valhalla Helicopters Inc. Bell 206B (C-GFWZ) from Penticton, BC (CYYF) to Penticton, BC (CYYF) was 2 to 3 miles west of WEN3281, travelling southbound. Traffic not passed until queried.
