Incident at INUKJUAK QC (CYPH) (Aerodrome, runway or taxiway shutdown)

Aviation Incident Report #17255: An Air Inuit de Havilland DHC-8-314 (C-FAID) on a flight to Inukjuak, QC (CYPH) landed and took off on a runway that had been closed by NOTAM for construction.
UPDATE: Aviation Incident Report #17282: An Air Inuit de Havilland DHC-8-314 (C-FAID) on a cargo flight to Inukjuak, QC (CYPH) landed on a closed runway. A NOTAM was issued for closure due to construction projects. The aircraft landed safely. No workers were in the airfield and no personnel were injured. No operational impact.

A Hydro-Quebec de Havilland DHC-8-311 (C-GJNL/HYD350) from Baie-Comeau, QC (CYBC) to Havre-Saint-Pierre, QC (CYGV) had to abort its takeoff because of too many birds. HYD350 returned to the terminal. A Government of Quebec de Havilland DHC-8-202 (QUE30-M) on a MEDEVAC flight to Baie-Comeau, QC (CYBC) landed afterwards. A vehicle (ST31) conducted a bird check and HYD350 took off at 1303Z. No other impact.

A Cargair Piper PA-23-250 (C-GPAV) on a flight from Montreal/St-Hubert, QC (CYHU) took off from Runway 24L. A Chrono Aviation Pilatus PC-12 (NDL321) from Montreal/Pierre-Elliott-Trudeau, QC (CYUL) to Montreal/St-Hubert, QC (CYHU); a Pascan Saab 340A (PSC312) from Quebec/Jean-Lesage, QC (CYQB) to Montreal/St-Hubert, QC (CYHU); a Panorama Fixed Wings Pilatus PC-12 (PNO782) from Quebec/Jean-Lesage, QC (CYQB) to Montreal/St-Hubert, QC (CYHU); and an Air Cascades Piaggio P-180 Avanti (C-GLEM) on a flight from Montreal/St-Hubert, QC (CYHU), landed on 24L. The runway was inspected and reopened at 2228Z.

Incident at GASPÉ / MICHEL-POULIOT QC (CYGP)

A privately registered, amateur-built Denney Kitfox IV took off and was heading for Gaspé/Michel-Pouliot, QC (CYGP). There was a valid NOTAM in place (i.e. Remotely Piloted Aircraft System (RPAS) operation at the CYGP airport), and a drone was inbound for CYGP. After several coordinations with the pilot and the centre, the pilot decided to return to land at Fromenteau lake. The drone resumed its approach towards the airport after several minutes in the holding circuit.

TSB Report #A21Q0033: A privately registered, amateur-built Superstol was on a local visual flight rules flight from Saint-Mathieu-de-Beloeil, QC (CSB3) airport with 2 passengers on board. On departure from Runway 33, the aircraft veered right and exited the runway. The pilot selected full flaps and took off, but the aircraft collided with a parked vehicle and a hangar. 2 minor injuries. The aircraft was heavily damaged.

Incident at HAVRE ST-PIERRE QC (CYGV)

TSB Report # A21Q0021: C-GJBV, a Beech A100 operated by Sky Jet MG Inc., was on an instrument flight rules flight departing from Port-Menier, QC (CYPN) airport at destination of Havre-Saint-Pierre, QC (CYGV). When the first officer landed, the aircraft was slowed down, without using the brakes, to about 55 knots. The captain then took control and applied the brakes. The aircraft turned suddenly to the left, and the captain attempted to regain control of the steering by applying the right brake and reversing the thrust of the engines. The aircraft exited the runway, hitting a runway light with the nose wheel, and when the landing gear was on gravel, directional control returned to normal. The captain then taxied the aircraft to the apron without incident. There were no injuries. After checking with maintenance, the main landing gear assemblies were replaced, as well as the nose wheel tire. The quality of the brakes was checked during the dismantling of the wheels and the brake tests did not reveal any faults. The aircraft was returned to service.

A privately registered, amateur-built Vans RV-9A, on a flight to Val-dOr, QC (CYVO), turned on final for Runway 36 while a Propair Beech A100 (PRO100T) from Rouyn-Noranda, QC (CYUY) to Rouyn-Noranda, QC (CYUY) was in position on Runway 18 at CYVO. The Vans RV-9A was advised to climb due to the traffic on the runway. The aircraft conducted a pull-up and joined the right downwind for Runway 36. PRO100T took off from Runway 18 two minutes later. No other impact.

After several unsuccessful attempts by the flight service station (FSS) to contact C-GQVQ, the aircraft took off from Runway 24 without establishing contact and without expressing its intentions. C-FLTA conducted a emergency practice maneuver 4.2NM to the north and, on its recovery, performed an avoidance maneuver in relation to C-FKNQ, which was passing behind. No impact.

A responsible person called to report that a Willow Bay Helicopters Ltd. Robinson R66 (C-GZBB) did not arrive on time. There were 3 people on board the aircraft and 2 hours and 30 minutes of fuel available upon departure. The aircraft took off from Ste-Anne-du-Lac (time unknown and approximate position 4653N / 07520W), north of Mont-Laurier, QC (CSD4), to fly along the Mithcinamecus river and back to Ste-Anne-Du-Lac for 2245Z. At 2335Z, the responsible person called back to say that C-GBZZ had made contact to advise that it would arrive at its destination in 30 minutes. The Montreal (ZUL) Shift Manager was advised.

A Pascan flight (PSC346) from Gaspe/Michel-Pouliot, QC (CYGP) to Quebec/Jean Lesage, QC (CYQB) took off from Runway 28 at 2020Z and no traffic was passed to PSC346 because the presence of TIGR337 was unknown. On April 28th at 1454Z, the Quebec (YQB) Flight Information Centre (FIC) received a NOTAM cancellation request in order to open Runway 13/31 at Sept-Îles, QC (CYZV). The request was for 1600Z, but was transmitted immediately and was effective on the NOTAM Entry System (NES) at 1458Z. A Provincial Airlines de Havilland DHC-8-311 (C-GVPA/PVL1902) from Sept-Îles, QC (CYZV) to Wabush, NL (CYWK) taxied and then took off from Runway 13/31 at 1502Z.

A JetFlite Bombardier CL-600-2B16 (OHWIW/JEF22) from Montreal/Mirabel, QC (CYMX) to Montreal/Pierre Elliott Trudeau, QC (CYUL) took off at 1855Z, after a 25 minute delay, because it did not have PPR for its take-off. No other impact on operations.

A Go Helico Inc. Robinson R44 II (C-GKUN) from Gaspé/Michel-Pouliot, QC (CYGP) to Rimouski, QC (CYXK) took off at 1425Z, westbound. At 1633Z, the aircraft contacted the flight service specialist (FSS) for the Departure Advisory Service, while on the ground at CYXK. There had been no previous contact between the two. The aircraft suspected a problem with its radio, as it had attempted to contact the air traffic service (ATS) unit a few times without success. Unknown impact.

PVL1902 reported being on a VFR departure. Another aircraft was descending in the opposite direction, so the controller took action to maintain spacing between the aircraft.

An explosive engine was found in the city of Ancienne-Lorette. It was decided to transport the explosive engine to the perimeter of the Quebec/Jean Lesage, QC (CYQB) airport with the aim of detonating it. The CYQB airport authority requested that no arrivals or departures be allowed for a period of 05 minutes before and after the explosion. The explosion took place at 0055Z. No impact. The National Operations Centre (NOC) and the Aviation Operations Centre (AVOPS) were advised.

While taxiing, a Air Inuit de Havilland DHC-8-314 (C-FYAI/AIE802) from Kuujjuarapik, QC (CYGW) to Umiujaq, QC (CYMU) advised that there were 2 dogs on the runway. The attendant was notified. A few minutes later, the 2 dogs moved away to the east side, more than 200 feet from the runway. AIE802 took off a few minutes later, without incident. No other impact on operations.

Aviation Incident Report #17196: An individual reported an aircraft flew within 50 feet of their house, which is in a highly populated area in Quathiaski Cove, BC. The aircraft was travelling from the southeast to the northwest (NW). The aircraft landed on Gowlland Harbour at the extreme south end, adjacent to a 150-foot anchored vessel (the only vessel in the harbour). The aircraft took off again heading NW out of Gowlland Harbour. The individual was unable to read the registration number on the aircraft.

Incident at FORT ST. JOHN BC (CYXJ) (Incursion - runway - animal)

A WestJet Encore Ltd. Bombardier DHC-8-402 (C-GDEW/WEN3100) from Fort St. John, BC (CYXJ) to Calgary, AB (CYYC) took position on Runway 12 for departure and informed the flight service station (FSS) that there was a coyote on the runway. The aircraft then delayed its departure until the coyote was clear. A Central Mountain Air Ltd. Beech 1900D (C-GCML/GLR783) from Edmonton, AB (CYEG) to Fort St. John, BC (CYXJ) was in the downwind for Runway 12 and extended to allow time for the animal to clear and WEN3100 to depart. The North Peace Airport Services (NPAS) unit was called to check on the coyote. It was then scared off of the airport grounds.

Incident at 50NM ENE Kelowna (CYLW)

At 2153Z and 50NM ENE of Kelowna, BC (CYLW), a VFR target was observed climbing through FL125 and proceeding NE bound. The target continued its climb and levelled at FL175 at 2200Z. The Edmonton (EG) sector BA was informed and took control of the controller jurisdiction symbol (CJS). Subsequent investigation revealed the aircraft was a Venture Alliance International Inc. Lancair IVP (C-FZID) on a VFR plan from Oliver, BC (CAU3) to Edmonton/Cooking Lake, AB (CEZ3).
