Incident at PICKLE LAKE ON (CYPL) (Communication error)

A vehicle (TK58) was working on the runway and taxiways since 2101Z. No response was received at 2210Z when they were instructed to exit. The three following aircraft were inbound at the time: a Wasaya Airways Beech 1900D (C-GWOX/WSG748) from Fort Severn, ON (CYER) to Pickle Lake, ON (CYPL); a Wasaya Airways de Havilland DHC-8-102 (C-FABA/WSG815) from Summer Beaver, ON (CJV7) to Pickle Lake, ON (CYPL); and a North Star Air Ltd. Douglas DC3C (C-FVNS/BF105) from Lansdowne House, ON (CYLH) to Pickle Lake, ON (CYPL). The first aircraft was estimating an arrival at 2225Z. The airport manager (APM) was called on their cell and they confirmed at 2222Z that no vehicles were on the runway or taxiways, and that the vehicle operator must have forgotten to call once they were off. No aircraft were delayed, and no other operational impact.

Immediately after departing from Runway 34, a North Star Air Ltd. Pilatus PC-12/45 (C-GEOW/BF712) on a flight from Sioux Lookout, ON (CYXL) advised they would be returning to CYXL due to an engine issue. The pilot advised that their cowling was open and wouldnt require assistance. The aircraft landed on Runway 16 without issue. No impact to operations and the Winnipeg centre was advised.

Pilatus PC-12 (BF712) from Sioux Lookout, ON (CYXL) to Poplar Hill, ON (CPV7). Both aircraft were level at 15,000 feet and 14,000 feet, respectively. Traffic information was passed to both aircraft prior to the encounter and there was no operational impact.

The aircraft conducted a fuel burn approximately 20NM northeast of CYTH. Declined assistance, landed without incident at 1606Z. No impact to operations.

Accident at 23NM ESE CAMROSE AB (CEQ3)

A call was received from Strathcona Fire regarding a Delta Helicopters Bell 206B (C-FCQJ) that crashed approximately 3NM southeast of Woodglen, Alberta. Police/Fire/Ambulance and Shock Trauma Air Rescue Service (STARS) were en route to scene. Joint Rescue Coordination Centre (JRCC) Trenton advised that the emergency locator transmitter (ELT) was activated.
UPDATE: TSB Report #A22W0005: C-FCQJ, a Bell 206B operated by Delta Helicopters was conducting an aerial wildlife survey approximately 23NM East Southeast of Camrose, AB (CEQ3) with the pilot and three passengers on board. Approximately an hour and 25 minutes into the flight, the helicopter impacted terrain. The helicopter remained upright, there was no post-impact fire, and the 406 MHz ELT activated. The pilot and three passengers received serious injuries.

Incident at THOMPSON MB (CYTH) (Aerodrome - power failure, ATM - operations)

A commercial power failure occurred at approximately 1150Z. The backup generator failed to start, and the uninterruptible power supply (UPS) was used until failure at 1320Z. The Thompson, MB (CYTH) flight service station (FSS) closed from approximately 1320Z to 1450Z. Two aircraft, a North Star Air ATR-GIE ATR 72-212A (BF501) from Thompson, MB (CYTH) to Rankin Inlet, NU (CYRT) and a Missinippi Air-Care Beech B200 (C-FTYO/MA01) from Thompson, MB (CYTH) to Winnipeg/James Armstrong Richardson, MB (CYWG) were known to have departed while the FSS was closed.

Incident at 11NM THOMPSON MB (CYTH)

A North Star Air ATR-GIE ATR 72-212A (C-FNIJ/BF503) on a flight from Thompson, MB (CYTH) requested to return to CYTH and declared an emergency at 2107Z. The aircraft subsequently landed at 2116Z, with no further issues. No operational impact.
UPDATE: TSB Report #A22C0003: C-FNIJ, an ATR 72-212A operated by North Star Air, was conducting flight BF503 from Thompson, MB (CYTH) to Brochet, MB (CYBT). Shortly after takeoff from CYTH, the crew observed an intermittent master caution light related to low engine oil pressure on the right engine (Pratt & Whitney Canada PW127F). The crew opted to return to CYTH. During their return, the oil pressure gauge indicated decreasing oil pressure and, after consulting the QRH, the crew shut down the right engine as a precautionary measure. An emergency was declared and an uneventful landing in CYTH was conducted. A subsequent inspection by the operator revealed that the right engines oil cap was not secured, which allowed oil to vent out. It was also noted that the check valve designed to prevent oil from venting out if the oil cap is not secure was not functioning. A fleet wide inspection of all North Star Air ATR aircraft is being conducted by the operator.

A Flair Airlines Boeing 737 MAX 8 (FLE604) from Edmonton, AB (CYEG) to Abbotsford, BC (CYXX) deviated from the HOPE standard terminal arrival (STAR) procedure and commenced a descent, which placed the aircraft below the minimum vectoring altitude. The controller immediately issued instructions to ensure safety. The pilot was visual with terrain at all times. No operational impact.

Incident at ABBOTSFORD BC (CYXX) (Communication navigation surveillance/air traffic system)

TSB Report #A21P0041: C-GEWR, a de Havilland DHC-8-402, was operating as WestJet Encore flight WEN3233 from Calgary, AB (CYYC) to Abbotsford, BC (CYXX). While on the Hope 3 arrival, and past the ITGIL waypoint, ATS YYJ terminal cleared the flight to OMINO DIRECT MISAR for the RNAV Z 07 approach. This clearance was not in compliance with NAV CANADAs "Manual of Air Traffic Services" and the Aeronautical Information Circular (AIC 12/21) "Procedural Changes To Air Traffic Control Clearances From An Open STAR To Final Approach Course". The clearance was refused by the flight crew and vectors to intercept the approach were requested and provided. There was no further incident and no loss of separation. The operator advised that there was a similar occurrence on the 14th of June, where the flight crew were offered the same non-standard clearance, which was also refused. There was no conflicting traffic and/or loss of separation. The TSB had completed report A21W0023 into an occurrence on the 11th of May, 2021, that involved non-standard ATC phraseology leading to a "Loss of Separation" event.

While inbound on the GRIZZ6 arrival, a foreign, corporately registered Cessna 650 (XBJST) from Cabo San Lucas, Mexico (MMSL) to Vancouver, BC (CYVR) deviated from the standard arrival route. When queried, the pilot confirmed they were on the GRIZZ standard terminal arrival (STAR). The controller confirmed that X8JST was off-course and provided instructions to regain the GRIZZ6 arrival. The aircraft landed safely at CYVR. No operational impact.

Children observed on the runway at Fort Hope, ON (CYFH). The pilot of a North Star Air Pilatus PC-12/45 (C-GVKC/BF708) from Fort Hope, ON (CYFH) to Thunder Bay, ON (CYQT) delayed departure for a few minutes until the children left the runway. During the takeoff roll, the children were observed by the pilot throwing things at the aircraft. The aircraft departed safely. No other impact to operations. Ontario Provincial Police (OPP) advised and responding.

Unable to verify the arrival of a Wasaya Airways de Havilland DHC-8-102 (C-GANF/WSG814) from Fort Severn, ON (CYER) to Pickle Lake, ON (CYPL). A second aircraft, a North Star Air Pilatus PC-12/45 (C-GVKC/BF708) from Round Lake (Weagamow Lake), ON (CZRJ) to Pickle Lake, ON (CYPL), was on final when the service was restored.

Incident at DRYDEN REGIONAL ON (CYHD) (ATM - weather observation systems)

A Hydro One Networks Inc. Aerospatiale AS350 B3 (C-GHOY) departed from Dryden, ON (CYHD) in 1/4 mile visibility (VIS), reportedly. The pilot had submitted a pilot weather report (PIREP) indicating that VIS and ceiling were unlimited. The Sioux Lookout flight service specialist (FSS) checked the cameras, and they showed the same thing, but only two were operational, so the specialist was unable to override the Automated Weather Observation System (AWOS) for authorization. Also, only 5 minutes after, a second aircraft, a North Star Air Ltd. Pilatus PC-12/45 (C-GKAY/BF707) from Thunder Bay, ON (CYQT) to Dryden, ON (CYHD), achieved a visual on the airport 15 miles back and through 070, which confirms C-GHOYs report.

The aircraft was observed descending through 12,700 feet and was immediately instructed to level at 12,000 feet due to opposite direction aircraft climbing to 11,000 feet. BF712 levelled at 12,000 feet. No operational impact.

Runway 25 was the active runway and no coordination was made with the control tower prior to the aircraft being switched over to the tower frequency. The tower controller subsequently approved Runway 12 and there was no operational impact.

A Skycare Air Ambulance Piper PA-31-350 (C-GJHX) from Sandy Lake, ON (CZSJ) to Sioux Lookout, ON (CYXL) declared an emergency and advised that services were required. The airport operator (APO), the Winnipeg area control centre (ZWG) Centre Shift Manager (CSM) and Skycare were advised. Aircraft conducted a low and over. Staff vehicle advised that all 3 landing gear appeared "down". C-GJHX rejoined a downwind and landed safely at 0027Z. All stations were advised. A North Star Air Ltd. Pilatus PC-12/45 (C-FKPA/BF709) from Fort Hope, ON (CYFH) to Sioux Lookout, ON (CYXL), inbound, was delayed by approximately 10 minutes.
