A Héli-Boréal Aérospatiale AS 350-B2 (C-GFBW) from Sept-Îles/Héli-Boréal, QC (Heli) (CHB4) crashed on the ice in the Sept-Îles bay after an emergency landing. No impact.
UPDATE from the Joint Rescue Coordination Centre: The Joint Rescue Coordination Centre (JRCC) Halifax indicated that, shortly after departure, the helicopter had problems with its windshield and had to clean it off. The pilot made the decision to land on the ice south of CHB4 instead of returning to CHB4. During the landing, the pilot lost visibility in blowing snow. The helicopter crashed on its side (rolled over). The incident happened around 1150Z and the emergency locator transmitter (ELT) alerted JRCC. There were two individuals aboard the helicopter. Unknown damages.

Aviation Incident Report #17565: The mandatory instruction sign announced that Runway 08/26 was damaged while a Government of Canada, Department of National Defence Bell 412 helicopter taking part in the "Arctic Warrior" wintertime operation was hovering over Taxiway Alpha. The top of the sign, as well as the plastic panel for Runway 08/26 (terminal side), flew away. The sign for Runway 08/26 could not be immediately found. It is most likely that it was buried under the snow. No damage to the aircraft. No impact on flight operations. An investigation is currently taking place. It is assumed that the clasp that secured the top of the sign may have been improperly attached, and that it was therefore not working properly to secure the top cover with a certain degree of tension.

Aviation Incident Report #17514: When the employee arrived on site at 1200Z (normal opening time at the airport), the access gate leading to the runway side was open and would not close again. After a check, it was found that 3 of the 4 plastic casters supporting the sliding gate were broken. The employ unsuccessfully attempted to force the gate to close. The only person on site when the employee arrived was the driver of a snow removal vehicle that was plowing snow.

Incident at BAIE-COMEAU QC (CYBC)

Aviation Incident Report #17528: Around 1830Z, at Baie-Comeau, QC (CYBC), a Sky Jet M.G. Beech A100 (C-GJBV) from Baie-Comeau, QC (CYBC) to Quebec/Jean Lesage, QC (CYQB) established initial contact with Mont-Joli radio with a request to depart from Runway 28 to fly to CYQB. Mont-Joli radio reported that a Cessna on skis was doing circuits on a parallel track with snow and that the aircraft was on skis. Mont-Joli radio reported C-GJBVs position to the aircraft on skis and queried its intentions. After several queries, the aircraft on skis mentioned that it was going to climb and then exit from the centreline. Mont-Joli radio and C-GJBV were of the understanding that the Cessna would exit the centerline, so C-GJBV took the time to taxi up the runway and then took off. During takeoff, immediately after rotation, the Cessna began its turn in the crosswind. This aircraft was directly in C-GJBVs takeoff path, and there was no possibility for passing above. C-GJBV had to turn left and maintain a low altitude to fly under the aircraft.

An Okanagan Aviation Academy Cessna 172M (C-GJES) from Salmon Arm, BC (CZAM) to Kelowna, BC (CYLW) touched down and made a left turn on the runway, into the snow on the east side of the runway. Crash alarm was pulled, and emergency vehicles attended the scene right away. The runway was closed via NOTAM.
UPDATE: TSB Report #A22P0006: C-GJES, an Aurora Aviation Academy Inc. Cessna 172M, was conducting flight training exercises near Kelowna Intl, BC (CYLW) with a student pilot on board. During the landing roll at CYLW, directional control was lost. The aircraft departed the left side of the paved surface of Runway 34, in the vicinity of Taxiway B. The aircraft did not collide with any obstacles. The aircraft became stuck in the snow and the student pilot shut down the engine to prevent a prop strike. Aircraft rescue fire fighting personnel responded and assisted with towing the aircraft back onto the runway. There were no injuries and no apparent damage to the aircraft. The operator will complete an aircraft maintenance inspection prior to returning the airplane to service.

Pilatus PC-12/47 (C-FWAL/AB776) from Thunder Bay, ON (CYQT) to Pickle Lake, ON (CYPL) was inbound and estimating arrival in 6 minutes. The vehicle exited the runway at 1619Z and the aircraft was delayed by 24 minutes. No other operational impact.

A Thunder Airlines Beech A100 (THU103) from Timmins/Victor M. Power, ON (CYTS) to Moosonee, ON (CYMO) was taxiing for departure from Runway 03 when they got stuck in the snow 50 feet from the centreline. A voice NOTAM was put out for Runway 03/21 being closed at 1237Z. At 1238Z, THU103 was unstuck so a vehicle (ST27) cancelled the voice NOTAM. THU103 cancelled their flight plan and taxied back to their hangar. No other operational impact.

Incident at 3.50NM NW KINGFISHER LAKE ON (CNM5)

TSB Report #A22C0006: C-FDOD, a Bell Helicopter Textron 412 operated by Wisk Air Helicopters, was returning from a work site on a power line to the work camp at Kingfisher Lake, ON, with the pilot and 6 passengers on board. While enroute, the No. 1 "FUEL LOW" (quantity) caution annunciator illuminated. The pilot commenced a precautionary landing and planned for a possible single engine landing. During the descent, at approximately 200 feet above ground level, the No. 1 engine (Pratt and Whitney Canada PT6T-3B) lost power, accompanied by illumination of the associated caution/warning annunciators. While on short final, the No. 2 engine began to fluctuate and then lost power. A successful auto-rotative landing was conducted onto the frozen, snow covered surface of Kingfisher Lake with both engines inoperative. There were no injuries and the helicopter was undamaged. The helicopter was refueled the next day and flown to the Kingfisher Lake camp. After further inspection, the helicopter was ferried to the operators maintenance base under a flight permit. The operators maintenance personnel subsequently discovered several discrepancies. Loose B nuts in the No.1 fuel tank prevented the ejector pump from transferring fuel aft, over the baffle, to the boost pump side of the fuel tank. The examination also determined that the No. 2 engine was starved for fuel due to leaking flapper valve plates that, in a normal flight attitude, allowed fuel to move forward, through the baffle, at a rate that exceeded the recapture capacity of the ejector pump. The effect of these discrepancies was to trap unusable fuel forward of the baffle in each fuel tank. Further discrepancies were noted such as a damaged No. 2 low level float switch and inaccurate fuel quantity indication systems. The operator is conducting checks on the remaining aircraft in the fleet.

The vehicle was instructed to exit onto Taxiway F, but they did not make the turn and were still on the runway with FSY193 short final. FSY193 conducted a missed approach and landed at 1904Z.

Upon landing on Runway 23, a Brantford Flying Club Cessna 172N (C-GBGZ) on a flight to Brantford, ON (CYFD) slid off the runway into a snow bank. The pilot suspects minor aircraft damage to the propeller. 1 soul on board (SOB), uninjured. Runway 05/23 was closed via NOTAM until aircraft is moved. No other impact to operations.
UPDATE: TSB Report #A22O0016: C-GBGZ, a Cessna 172 registered to the Brantford Flying Club, was rented by a member who planned to conduct several touch-and-go night circuits on Runway 23 at Brantford, ON (CYFD), and was the sole occupant of the aircraft. The runway was mostly bare, with some scattered patches of compact snow and ice. Following the first landing, when the pilot applied power to begin the takeoff roll, the aircraft began to skid to the right. The pilot corrected to the left, and the aircraft veered to the left, across the runway, towards the snow at the runway edge. The pilot reduced power before the aircraft entered the snow. The pilot was uninjured. The aircraft sustained damage to the nose wheel and propeller. The ELT activated as a result of the runway excursion.

During landing, a privately registered Piper PA-46-600TP, on a flight to Stratford, ON (CYSA), went off the end of Runway 17 at CYSA, causing damage to the aircraft.
UPDATE: Aviation Incident Report #17507: A privately registered Piper PA-46-600TP on a flight to Stratford, ON (CYSA) was landing on Runway 17. It appeared that the directional control was lost and the aircraft exited the runway to the right-side. The aircraft struck an abbreviated precision approach path indicator (APAPI) light box and continued travelling across the graded area, and came to rest perpendicular to the runway edge. Weather conditions were VFR, with winds from the south and less than 8 knots. The runway was 95% bare and dry. Runways were closed for several hours.
UPDATE: TSB Report #A22O0013: A privately registered Piper PA-46-600TP was on a visual flight rules flight from Kitchener/Waterloo, ON (CYKF) to the Stratford, ON (CYSA) with the pilot as sole occupant. During the landing roll on Runway 17, the pilot felt the aircraft swerving and attempted a correction before deciding to reject the landing. After they added power for the go-around, the right main wheel entered the snowbank beside the runway, causing the aircraft to exit the runway surface into an area of undisturbed snow. The aircraft collided with some airport signage before the nose wheel sheared off. The aircraft came to a stop upright in a snow covered area beside the runway. The pilot exited the aircraft uninjured, the aircraft had been damaged significantly. The propeller was destroyed, and there was damage to the wing, cowling, and engine mounts. The ELT was activated during the runway excursion.
