At Port Hardy, BC (CYZT), a vehicle (ST46), while operating the full length of Runway 11/29, exited onto Runway 16/34 without authorization from the flight service station (FSS). A Pacific Coastal Airlines Limited Saab 340B (C-FPCZ/PCO841) from Vancouver, BC (CYVR) to Port Hardy, BC (CYZT) was conducting the instrument landing system (ILS) Runway 11 with intentions to land on Runway 11. No impact to safety.

After a Lufthansa Boeing 747-400 (DLH493) from Vancouver, BC (CYVR) to Frankfurt, Germany (EDDF) was taxied to position on Runway 08R, they advised they needed 90 seconds in position to clear aircraft engines after de-icing. A Pacific Coastal Airlines Saab 340B (C-FPCZ/PCO846) from Port Hardy, BC (CYZT) to Vancouver, BC (CYVR) was issued a missed approach due to this. PCO846 vectored for second approach and landed Runway 08L at 0053Z.

Incident at MEDICINE HAT AB (CYXH) (ATM - inaccurate aeronautical information)

A Pacific Coastal Airlines Ltd. Saab 340 (PCO3014) from Calgary, AB (CYYC) to Medicine Hat, AB (CYXH) phoned the Edmonton (YEG) flight information centre (FIC) and advised they were unable to make contact on the CYXH remote communications outlet (RCO). The YEG FIC confirmed with airborne aircraft the RCO was unserviceable. The technical operations coordinator (TOC) was advised.

Incident at CALGARY INTL AB (CYYC) (Aerodrome, runway or taxiway shutdown)

A Pacific Coastal Airlines Saab 340B (C-GPCF/PCO3014) from Calgary, AB (CYYC) to Medicine Hat, AB (CYXH) had a hydraulic leak from its number 2 engine. PCO3014 became disabled on Taxiway K and was eventually towed back to the gate. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Description of occurrence is not correct. While C-GPCF aircraft was holding short, it was noticed that the hydraulic pumps were running continuously and accumulator pressure indicators for both main and outboard brakes indicated zero. Tower was advised and aircraft requested assistance as it no longer had brakes. On inspection of aircraft, fluid was visible on ground around #4 main wheel. Maintenance determined that a weight retaining screw had failed, allowing one of the wheel weights to dislodge and damage the associated brake assembly, which in turn starting leaking hydraulic fluid. Brake and wheel assembly replaced and aircraft returned to service

A Pacific Coastal Airlines Saab 340 (PCO3016) from Calgary, AB (CYYC) to Medicine Hat, AB (CYXH) was cleared to taxi across Runway 29 at the same time a WestJet Boeing 737-6CT (WJA190) from Abbotsford, BC (CYXX) to Calgary, AB (CYYC) was cleared across Runway 29 in the opposite direction. WJA190 crossed Runway 29 first, then PCO3016 continued to taxi. No operational impact.

A privately registered Cessna T240 from Calgary/Springbank, AB (CYBW) to Medicine Hat, AB (CYXH) squawked communication failure at 0256Z. Aircraft landed at 0303Z. Pilot advised afterwards that they were "receiver only" (RONLY). Operational impact: A Jazz de Havilland DHC-8-102 (JZA7940) from Medicine Hat, AB (CYXH) to Calgary, AB (CYYC) was held on the ramp and a Pacific Coastal Airlines Ltd. Saab 340 (PCO3016) from Calgary, AB (CYYC) to Medicine Hat, AB (CYXH) was manoeuvred East of CYXH until the Cessna T240 arrived.

Instead, the aircraft turned left onto Runway 35L (a non-active runway). Ground corrected the error by assigning Charlie 6, then Charlie, Kilo.

A Pacific Coastal Airlines Ltd. Saab SF340A (PCO3041) from Calgary, AB (CYYC) to Cranbrook/Canadian Rockies, BC (CYXC) declared "Mayday" due to severe turbulence and requested to return to CYYC. Aircraft was given priority and returned to CYYC. Aircraft landed without incident at 1829Z. No impact to operations.
UPDATE TSB Report #A20W0009: C-GOIA, a Pacific Coastal Airlines Saab-Fairchild 340B, was operating as flight PCO3041 from Calgary International (CYYC), AB to Cranbrook (CYXC), BC. The crew encountered severe turbulence 52 nm SSW of CYYC, declared an emergency and returned to CYYC. Following the flight, maintenance inspected the aircraft for any anomalies that would have resulted from the flight through the turbulence. No defects were found. The aircraft was returned to service.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. A "Severe Turbulence Inspection" was carried out in accordance with (IAW) SF340 Aircraft Maintenance Manual (AMM) 05-50-00-06, with no defects found. Aircraft released back into service. Safety Management System (SMS) Report #8732 opened for further investigation.

Bird Strike on Front windscreen entered in journey logbook and maintenance carried out inspection on aircraft. Additional impact points identified on randome and delamination tap test carried out. No further damage identified on aircraft.

Update TSB #A19P0097: C-FPCU, a Saab 340B aircraft operated by Pacific Coastal Airlines, was conducting flight PCO871 from Vancouver Intl (CYVR), BC, to Port Hardy (CYZT), BC, with 3 crew members and 14 passengers on board. During cruise, the AVIONICS SMOKE annunciator illuminated. The crew conducted vital actions, declared an emergency and completed the Quick Reference Checklist. The crew detected no signs of smoke. The crew returned to land at CYVR and the aircraft landed uneventfully. On landing, the CYVR Fire Services inspected the aircraft and no heat signal was detected. The operators maintenance replaced the smoke detector.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Lightening strike was confirmed. Lightening entered aircraft nose radome with main exit point at rudder tip fairing. Radar became inoperative as did timer for de-ice boots. Aircraft returned to Port Hardy where company Maintenance assessed the lightening damage. Aircraft was ferried to CYVR under company ferry authority where full lightening inspection was carried out

A Pacific Coastal Airlines Saab 340B (C-FPCU/PCO100) from Victoria International, BC (CYYJ) to Vancouver International, BC (CYVR) reported a bird strike while landing on Runway 08R. The runway was closed for inspection, and one gull was found. The runway re-opened at 1552Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. There was no bird strike. The pilots reported a dying seagull on the runway after landing and taxiing clear. They did not hit nor report a bird strike. It appears that the Controller misunderstood the message from the flight crew.

Three aircraft were being sequenced for Runway 27 at CYXE. A loss of separation occurred when less than required wake turbulence minima existed between the first aircraft, a Westjet Encore Ltd. de Havilland DHC-8-402 (C-GEEN/WEN3266) from Edmonton, AB (CYEG) to Saskatoon/Diefenbaker, SK (CYXE), and the second aircraft, a 1867829 Alberta Ltd. Pilatus PC-12/47 (C-FIPN) from Regina, SK (CYQR) to Saskatoon/Diefenbaker, SK (CYXE). The second aircraft was then vectored to follow the third aircraft, a Transwest Air Ltd. Saab 340B (C-GTJX) from Prince Albert (Glass Field), SK (CYPA) to Saskatoon/Diefenbaker, SK (CYXE), and a subsequent loss of surveillance separation occurred between the second and third aircraft.
UPDATE TSB Report #A19C0150: C-GEEN, a de Havilland DHC-8-402 operated by WestJet Encore as flight WEN3226 was conducting a flight from Edmonton Intl (CYEG), AB to Saskatoon/John G. Diefenbaker Intl (CYXE), SK. WEN3226 was cleared for the RNAV Y Runway 27. C-FIPN, a Pilatus PC 12/47 operated by 1867829 Alberta Ltd., was conducting a flight from Regina Intl (CYQR) SK to CYXE. C-FIPN was sequenced behind C-GEEN and was cleared for RNAV Z Runway 27 by Winnipeg Centre. Both aircraft were cleared for the approach and switched to CYXE tower frequency. C-GTJX, a Saab 340B operated by Transwest Air Ltd, was conducting a flight from Prince Albert/Glass Field (CYPA), SK to CYXE. C-GTJX was sequenced behind C-GEEN and C-FIPN for the RNAV Z Runway 27. During the approach, a loss of separation for wake turbulence occurred between C-GEEN and C-FIPN. The required separation was reduced to 2.9 nautical miles when the required separation is 4 nautical miles. C-FIPNs approach clearance was cancelled and C-FIPN was asked to contact Winnipeg Center. The controller cleared C-FIPN to turn right to a heading of 360 degrees, bringing the aircraft north of the approach path for Runway 27. The controller subsequently cleared C-FIPN to turn further right to a heading of 270 degrees. During the vectoring of C-FIPN, lateral separation between C-FIPN and C-GTJX was reduced to 2 nautical miles when 3 nautical miles in required. The controller cleared C-FIPN to turn to a heading of 360 degrees again, followed by another clearance to turn right to a heading of 270 degrees. The crew of C-FIPN asked for confirmation of the last clearance and the controller corrected the clearance to a right turn to a heading of 090 degrees. C-FIPN subsequently conducted the RNAV Z Runway 27 uneventfully. All aircraft landed safely in CYXE.
