An Air Inuit de Havilland DHC-8-314 (AIE833) from Québec/Jean-Lesage, QC (CYQB) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) hit a fox during its takeoff roll and continued its takeoff and flight. Firefighters removed the fox remains.

Accident at CHARLEVOIX QC (CYML)

TSB Report #A22Q0112: A privately operated Cessna 120X was on a visual flight rules flight from Charlevoix, QC (CYML) to a private field in St-Ambroise, QC with a pilot and one passenger on board. During the takeoff roll, the aircraft tilted to the left at the same time the wheels left the ground. The left wing touched the ground and the aircraft exited on the left side of Runway 15. The aircraft was greatly damaged and the pilot suffered minor cuts.

The pilot did not acknowledge the clearance and did not initiate the takeoff roll. The tower controller issued instructions to pull up to an Air Canada Airbus A330-343 (C-GFUR/ACA885) from Geneva, Switzerland (LSGG) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) while it was 1.5 NM on final for Runway 06R because USC422 was still on the runway. The pilot had changed the frequency and was using the departure one. ACA885 returned to land at the airport at 1924Z.

When the passengers were deplaning at CYUL gate C84, the luggage conveyor hit the fuselage near the rear cargo door, causing structural damage. No injuries.

Pilatus PC-12 (AM360) from Amos/Magny, QC (CYEY) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) pulled up from Runway 24L because an Air Canada Airbus BD-500-1A11 (ACA1206) from Montréal/Pierre-Elliott-Trudeau, QC (CYUL) to Miami, FL (KMIA) was still in position after being instructed to abort takeoff due to the previous landing, a Porter Airlines de Havilland DHC-8-402(POE459) from Toronto/Billy Bishop, ON (CYTZ) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) that had a nose gear bleed line issue when leaving the runway via exit A4. After being disabled on A4 for ten seconds, POE459 was able to exit the runway. ACA1206, which had not begun its takeoff roll when it was instructed to abort takeoff, was able to take off at 1352Z and AM360 landed at 1354Z.

After a local flight at Lac Ouareau near the municipality of Saint-Donat, QC, around 2100Z, while taxiing on the water after landing, a privately registered, amateur-built Maranda AMF S-14D flipped over on the water due to a gust of wind. The pilot was the only occupant on board and was not injured. The Transportation Safety Board of Canada (TSB) was notified and left the scene.
UPDATE: Aviation Incident Report: Following a local flight on lake Ouareau, near the town of St-Donat, QC, at approximately 17h00 local, when taxiing over the water, a privately registered Maranda AMF S-14D encountered a gust of wind that flipped the airplane over. The sole occupant did not suffer any injuries. The Transportation Safety Board was notified and has released the scene.

Accident at 50NM NW FORESTVILLE

The Joint Rescue Coordination Centre (JRCC) Trenton reported the crash of a privately registered, amateur-built, Air Command Commander 285 in the Valin Mountains.
UPDATE: TSB Report #A22Q0074: a privately registered, amateur-built Commander 285 aircraft was on a visual flight rules flight from Lac Klemka, QC, to Lac Sébastien, QC (Water) (CTD3), with 1 pilot and 2 passengers on board. The aircraft taxied to the northwest of the lake in order to take off in a southeasterly direction. During the takeoff roll, the aircraft did not take off in the area where the floats normally lift off from the water. The pilot attempted to manoeuvre to lift the right-side float to facilitate takeoff; however, the left float lifted up suddenly. The pilot then reacted by bringing the aircraft back to wings-level. The aircraft subsequently took off without the pilot controlling the manoeuvre. Seeing that the distance with terrain ahead and to the right was too short to fly above, the pilot turned left. Afterwards, the pilot felt the aircraft descending. He made sure that the engine power controls were set at full throttle and lowered the aircrafts nose to pick up speed. The left wing inclined suddenly and the aircraft was no longer maintaining its altitude. The pilot cut off the engine and the aircraft collided with the water. The 3 occupants on board were able to evacuate and swim to the closest shore. They were rescued by witnesses who assisted them by boat. The aircraft sank. Two helicopters were sent to the location to evacuate and transport the 3 injured people to a Chicoutimi hospital.

The aircraft exited the runway via Taxiway C. Emergency services were not requested. The aircraft returned to the terminal; no impact.
UPDATE: TSB Report #A22Q0049: C-GDPI, A Propair Inc. Beechcraft A100 operating as PRO4600 was on an instrument flight rules (IFR) flight from Val-d’Or, QC (CYVO) to Rouyn-Noranda, QC (CYUY). During the takeoff roll, smoke appeared in the aircraft and takeoff was aborted. The aircraft then returned to the apron. There were no injuries and no damage occurred. An inspection after the incident revealed that a small amount of engine oil had spilled into the engine compartment, and, when heating, emitted smoke that was sucked in through the air intake.

A Héli-Boréal Aérospatiale AS 350-B2 (C-GFBW) from Sept-Îles/Héli-Boréal, QC (Heli) (CHB4) crashed on the ice in the Sept-Îles bay after an emergency landing. No impact.
UPDATE from the Joint Rescue Coordination Centre: The Joint Rescue Coordination Centre (JRCC) Halifax indicated that, shortly after departure, the helicopter had problems with its windshield and had to clean it off. The pilot made the decision to land on the ice south of CHB4 instead of returning to CHB4. During the landing, the pilot lost visibility in blowing snow. The helicopter crashed on its side (rolled over). The incident happened around 1150Z and the emergency locator transmitter (ELT) alerted JRCC. There were two individuals aboard the helicopter. Unknown damages.
UPDATE: TSB Report #A22Q0029: C-GFBW, an Airbus AS350B2 helicopter, was on a visual flight rules (VFR) flight from the Héli-Boréal company’s facilities, located 7 nautical miles (NM) Northwest of Sept-Îles, QC (CYZV), to Baie-Comeau, QC (CYBC). Upon departure, the aircraft’s windshield suddenly iced over. The pilot initiated a descent toward the frozen surface of Sept-Îles Bay in order to land. The aircraft came into contact with the frozen surface of the bay while the aircraft was still in forward speed. It rocked around and then landed on its side. The pilot and the passenger were able to evacuate the aircraft without difficulty and were driven to the Sept-Îles hospital. The aircraft was destroyed.

Emergency vehicles were dispatched. The aircraft landed without incident at 2322Z and left via B2. A runway inspection was performed and no anomalies were found. No impact on operations.
UPDATE: TSB Report #A22Q0027: C-GTSR, an Air Transat Inc. Airbus 330-243 (TS399) was on an instrument flight rules flight from Punta Cana Intl, Dominican Republic (MDPC) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL). Shortly after takeoff, the crew saw a low tire pressure message on the ECAM (electronic centralized aircraft monitor), along with a similar message on the panel for wheel number 4. While in flight, the crew contacted the companys maintenance service; a decision was made to continue the flight to its destination. Upon approach to CYUL, an emergency (PAN PAN) was declared and the aircraft landed without incident. The Aircraft Rescue and Fire Fighting (ARFF) unit inspected the landing gear and the aircraft was towed to the designated parking area.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

On departure from Runway 24L, a Skyservice Business Aviation Inc. Bombardier BD-700-1A11 (C-GSLU) from Montreal/Pierre-Elliott-Trudeau, QC (CYUL), to Teterboro, NJ (KTEB), initiated a right turn while the standard instrument departure (SID) indicated climbing the runway heading. Clearance for takeoff of a Hydro-Quebec de Havilland DHC-8-300 (HYD160) from Montreal/Pierre-Elliott-Trudeau, QC (CYUL) to Quebec/Jean-Lesage, QC (CYQB) was cancelled to ensure separation.

Incident at Montreal/Passport Hélico Heliport, QC (CPP8)

A 9206-7685 Quebec Inc. Robinson R66 (C-GZJD) from Montreal/Passport Helico, QC (Heli) (CPP8) to Montreal/Passport Helico, QC (Heli) (CPP8) was instructed to climb to 4,000ft and fly on a north heading. Correct readback from C-GZJD. C-GZJD turned suddenly on a south heading, without notifying the air traffic controller (ATC).

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX)

A Cargojet Airways Boeing 757-200 (CJT626) from Hamilton, ON (CYHM) to Montreal/Mirabel, QC (CYMX) was instructed to conduct a go-around for Runway 06 due to the presence of a Cargojet Airways Boeing 757-200 (CJT952) from Montreal/Mirabel, QC (CYMX) to Covington/Cincinnati, KY (KCVG), which was in position on the runway and took additional time to get airborne. Impact on operations: loss of separation between CJT626 and CJT952. CJT626 landed at 0502Z.
UPDATE: TSB Report #A22Q0010: GTCJ, a Cargojet Airways Ltd. Boeing 757-223 (CJT626), was on an instrument flight rules flight from Hamilton/John C. Munro, ON (CYHM) to Montréal/Mirabel, QC (CYMX). While the aircraft was on final, the crew was instructed to pull up because C-FLAJ, a Cargojet Airways Ltd. Boeing 757-223 (CJT952), was trying to take off in order to fly from Cincinnati/Northern Kentucky International, KY (KCVG). The required separation standard of 1,000 ft or 3 NM was not met.
