Incident at Kelowna

TSB Report #A22P0003: C-FYPB, a Swoop Inc. Boeing 737-8CT, was preparing for flight WSW419 from Kelowna, BC (CYLW) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 6 flight crew and 126 passengers on board. The aircraft was parked at the gate in CYLW and configured to be de-iced by Executive Aviation. The doors were closed, engines were off, and the APU was running. The de-ice truck was parked outside the aircraft clearance zone near the left wing with 1 operator in the bucket. The driver of the de-ice truck exited the vehicle to push another aircraft. Approximately 5 minutes later, the truck began rolling towards the aircraft and stopped when it collided with the left hand engine nacelle. The aircraft was deplaned and there were no reported injuries. The aircraft sustained damage to the outboard leading edge of the left-hand engine inlet cowl. Maintenance replaced the left engine inlet cowl and the aircraft was returned to service. The de-ice trucks braking system was inspected and function tested, with no faults found. Executive Aviation is conducting an internal investigation.

Incident at Kelowna (Aerodrome, runway or taxiway shutdown)

An Okanagan Aviation Academy Cessna 172M (C-GJES) from Salmon Arm, BC (CZAM) to Kelowna, BC (CYLW) touched down and made a left turn on the runway, into the snow on the east side of the runway. Crash alarm was pulled, and emergency vehicles attended the scene right away. The runway was closed via NOTAM.
UPDATE: TSB Report #A22P0006: C-GJES, an Aurora Aviation Academy Inc. Cessna 172M, was conducting flight training exercises near Kelowna Intl, BC (CYLW) with a student pilot on board. During the landing roll at CYLW, directional control was lost. The aircraft departed the left side of the paved surface of Runway 34, in the vicinity of Taxiway B. The aircraft did not collide with any obstacles. The aircraft became stuck in the snow and the student pilot shut down the engine to prevent a prop strike. Aircraft rescue fire fighting personnel responded and assisted with towing the aircraft back onto the runway. There were no injuries and no apparent damage to the aircraft. The operator will complete an aircraft maintenance inspection prior to returning the airplane to service.

Incident at CRANBROOK / CANADIAN ROCKIES INTL BC (CYXC)

A privately registered, amateur-built Murphy Elite from Cranbrook/Canadian Rockies, BC (CYXC) to Cranbrook/Canadian Rockies, BC (CYXC) landed on Taxiway A after encountering a strong crosswind on Runway 16.
UPDATE: TSB Report #A22P0007: A privately registered Murphy Elite airplane was conducting a local flight from Cranbrook/Canadian Rockies Int’l, BC (CYXC), with 1 pilot and 1 passenger on board. The airplane departed with the winds reported at 270° at 12 knots, gusting up to 18 knots; a direct crosswind to the runway. After a 20-minute flight, the aircraft returned and the crosswind had increased to 16 knots, gusting up to 20 knots. To reduce the amount of crosswind, the pilot elected to land across Runway 16, lining up to roll out on Taxiway A; this reduced the crosswind angle by 45°. A landing was accomplished with the aircraft stopping on the taxiway. There were no injuries and the airplane was not damaged.

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3297) from Vancouver, BC (CYVR) to Prince George, BC (CYXS) landed on Runway 33 and reported hitting a coyote. Maintenance recovered the animal from the runway. No operational impact.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft hit a coyote on roll out with its nose landing gear. A visual inspection of the nose landing gear and doors was carried out, with no damage found. Reference: aircraft maintenance manual (AMM) 32-21-00-201-801.

A North Cariboo Flying Service Ltd. British Aerospace Avro 146 (C-GSUI/NCB1512) from Calgary, AB (CYYC) to Terrace, BC (CYXT) experienced an engine failure on short final for Runway 33. The engine was shut down during the roll out. No impact to operations.
UPDATE: TSB Report #A22P0002: C-GSUI, a North Cariboo Flying Service Ltd. British Aerospace Avro 146 Series RJ100, was operating as flight NCB1512 from Calgary Int’l, AB (CYYC) to Terrace, BC (CYXT) with 4 crew and 77 passengers on board. On short final for an ILS approach, at approximately 400 feet above ground level, the aircraft encountered wind shear, causing an aerodynamic performance degradation and deviation from the glide slope. The aircraft auto-throttle system compensated for the performance change and enabled the crew to regain the glide slope. The crew then observed a GEN 4 caution light for the No. 4 engine (Avco Lycoming, LF-507-1F) and noted the engine continued to produce power based on the associated gas producer speed (N1) indication. The crew proceeded with the landing and touched down without further incident. Once on the ground, the crew found the No.4 engine was now indicating a decrease in N1 with an exceedance in the exhaust gas temperature (EGT) limitation. The No. 4 engine was shut down on the taxiway, with no additional indications or warnings. There were no reported injuries or damage to the aircraft. Following arrival, the crew inspected the engine and no damage or leakage was found. When weather permits, the aircraft will be ferried on the 3 remaining engines to CYYC for completion of the required maintenance actions.

At 1448Z, a Wasaya Airways Hawker Siddeley HS 748 Series 2A (C-GLTC/WSG801) from Kasabonika, ON (CYAQ) to Pickle Lake, ON (CYPL) started squawking an emergency code and it was seen on the NAV CANADA Auxiliary Radar Display System (NARDS). WSG801 declared an emergency with the flight service station (FSS), on the Pickle Lake, ON (CYPL) frequency 122.2 MHz, at around 1515Z, as they were operating on only one of two engines. The aircraft landed safely at 1540Z and needed a tow off the runway. WSG801 was off the runway at 1552Z. The landing of a Sigfusson Northern Ltd. Cessna 208B (C-GOSN) was delayed by 10 minutes. No other impact to operations.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Incursion - runway - vehicle)

A vehicle (Tractor 244) was given instructions to taxi via Taxiway Sierra and hold short of Runway 33R, which the vehicle read back correctly. Tractor 244 was observed crossing the hold short line and was instructed to stop. An Air Transat Airbus A321-271NX (C-GOIS/TSC516) from Toronto/Lester B. Pearson, ON (CYYZ) to Montego Bay/Sangster, Jamaica (MKJS) was rolling for take-off from Runway 33R at the time. No impact to operations. The Aviation Operations Centre (AVOPS) and the Transportation Safety Board of Canada (TSB) were notified.

Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

A Cargojet Airways Boeing 757-236 (C-FKCJ/CJT470) from Iqaluit, NU (CYFB) to Ottawa/Macdonald–Cartier, ON (CYOW) executed a missed approach for Runway 32 due to wind shear on final. The aircraft landed safely at 2159Z. No impact on operations.
