A Jazz Bombardier CL-600-2B19 (C-GKGC/JZA8767) from Val-dOr, QC (CYVO) to Rouyn-Noranda, QC (CYUY) was cleared for an area navigation approach (RNAV) on Runway 26 at CYUY, even though a dynamiting operation was in progress 12 NM east of CYUY. A NOTAM was in effect, prohibiting clearance to conduct an approach on Runway 26 during the dynamiting activity.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

An Orizon Aviation Québec Cessna 152 (C-GJBF) from Quebec/Jean Lesage, QC (CYQB) to Quebec/Jean Lesage, QC (CYQB) on final for Runway 24 stopped responding to communications. The aircraft conducted a pull-up despite a green light sent by the tower. C-GJBF then squawked 7600, a green light was sent to it, and it landed. A Cargair Ltd. Piper PA-23-250 (C-GARI) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) on area navigation (RNAV) approach 24 had to conduct a pull-up to avoid conflict with C-GJBF.

On the area navigation (RNAV) Z into Kelowna, BC (CYLW), a WestJet Encore Ltd. de Havilland DHC-8-402 (C-FQWE/WEN3419) from Edmonton, AB (CYEG) to Kelowna, BC (CYLW) received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) alert with a Carson Air Ltd. Swearingen SA227-AT (C-GTTM/CA8057) from Kelowna, BC (CYLW) to Kelowna, BC (CYLW) when descending through 7300ft. WEN3255 climbed 300ft and then proceeded on course for their arrival once the conflict was resolved. WEN3255 was passed traffic.

Incident at QUALICUM BEACH BC (CAT4) (Parachute-related event)

A privately registered Cessna 182T from Victoria, BC (CYYJ) to Qualicum Beach, BC (CAT4) was on final for Runway 29 on the IFR area navigation (RNAV) 29 approach at CAT4 when the pilot-in-command (PIC) requested deviation from COMOX terminal due to parajumpers being dropped on the approach path at 5000 feet. The Cessna 182T landed safely at CAT4. No other impact on operations.

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: A Central Mountain Air Ltd. aircraft (flight GLR705) on a flight to Terrace, BC (CYXT) conducted a missed approach for Runway 33 due to ground wind direction. GLR705 flew the area navigation (RNAV) Z for the Runway 15 approach and landed safely. No impact on operations.

A 2049174 Alberta Ltd. Cessna P210N (C-GZLI) from Vancouver/Boundary Bay, BC (CZBB) to Tofino/Long Beach, BC (CYAZ) conducted a missed approach during area navigation (RNAV) 29 due to an unstable approach. C-GZLI cancelled its IFR, joined the circuit as VFR, and landed safely on Runway 16. No operational impact.

Incident at 11NM E VANCOUVER INTL BC (CYVR)

A Horizon Air Industries Embraer ERJ 170-200 LR (N644QX/QXE2898) from Seattle-Tacoma, WA (KSEA) to Vancouver, BC (CYVR), vectored to a 290° for the area navigation (RNAV) 26R approach, descended to 2,000 feet 11NM East of the threshold and 1NM South of the final approach course. Last assigned altitude was 3,000 feet. No operational impact.

A United Parcel Service Co. Airbus A300-600F (N152UP/UPS476) from Louisville, KY (KSDF) to Hamilton, ON (CYHM) was observed to descend below the minimum sector altitude (MSA) (3100 feet) after being cleared direct to LOGLI intersection for the area navigation (RNAV) Z Runway 12 approach. The pilot was advised via the CYHM tower and was observed to stop their descent at 2500 feet until in position for LOGLI intersection. No impact on operations.

Incident at OTTAWA / CARP ON (CYRP)

Aviation Incident Report #16624: At approximately 1900Z, a privately registered Mooney M20J from Carp, ON (CYRP) to Carp, ON (CYRP) was braking high and away from a privately registered Glider PiK-20 EX from Carp, ON (CYPR) to Carp, ON (CYRP) that was taking off from Runway 28. Airport personnel called the Mooney M20J on the radio to make sure they were alright, and they responded promptly. Airport personnel tried calling the Glider Pik-20 EX three times, with no response. They waited for the Mooney M20J to land before conducting an interview with the pilot. The aircraft was conducting an overshoot of area navigation (RNAV) 10 and conducting a windsock inspection to determine the runway for their final approach, making all the required radio calls as they proceeded. It was at the point when they pulled up to conduct an overshoot and climb that they noticed the Glider PiK-20 EX coming towards them, but they were able to evade. At 1914Z, after the incident and interview, the pilot of the Glider PiK-20 EX called, advising that their microphone became disconnected and they had not noticed. Absolutely no radio calls were heard from the PiK-20 EX prior to this.

No communication received prior to this call. An Ornge Global Air Pilatus PC-12/47E (C-GRXN/PUL107) from Sioux Lookout, ON (CYXL) to Thunder Bay, ON (CYQT) was turning 4NM final for Runway 25. No other operational impact.

Incident at OTTAWA / CARP ON (CYRP)

Aviation Incident Report #16545: At approximately 1315Z, a radio call was received from a Rockcliffe Flying Club Cessna 172N (C-GQUO) stating that they were on a 7 mile final for the simulated area navigation (RNAV) Runway 10, for a landing at Ottawa/Carp, ON (CYRP). Immediately following this call, an aircraft was heard departing Runway 28, which had them taking off directly into the path of C-GQUO. Hearing no radio calls of any kind prior to this, it was discovered it was an MNB Aviation Cessna 172M (C-FEUH). A call was made to C-GQUO to give them a heads up about the departing traffic when, all of a sudden, C-FEUH started talking on the radio. Both planes were able to communicate and avoid each other. C-GQUO departed the zone and C-FEUH remained in the circuit where it became apparent that there was a malfunction with their radio. C-FEUH landed, taxied in, and shut down. The radio is being examined by maintenance.

Incident at LONDON ON (CYXU)

A privately registered Cessna 182T on an IFR flight from Toronto/Buttonville, ON (CYKZ) to London, ON (CYXU) was on the area navigation (RNAV) Z 33 approach. An Aero L-39C (ST41) on a VFR flight from London, ON (CYXU) to London, ON (CYXU) was doing local circuits (CCTS) on Runway 33. The Tower turned ST41 left base ahead of the Cessna 182T (for final Runway 33). ST41 did not turn as quick as expected. As a result, the tower initiated an instruction to the Cessna 182T to break off the approach with a right turn heading 060. No other impact on operations.

Incident at CORNWALL REGIONAL ON (CYCC)

The pilot of a Cornwall Aviation (1979) Ltd. Cessna 172N (C-GUWV) in the circuit at Cornwall, ON (CYCC) reported observing a "near miss" between 2 other aircraft. The pilot reported that an approaching aircraft stated they were over an inbound fix on the area navigation (RNAV) Runway 28 Approach at CYCC, when in fact they were not. Another aircraft in the circuit came within "one wingspan" of the approaching aircraft. Both aircraft performed a go-around, which the pilot stated caused yet another unsafe situation. The approaching aircraft looked to be a Tobago. The pilot is looking into the aircraft identification (ACID) for that aircraft. This incident was reported by phone to the London Flight Information Centre (FIC). Operational impact unknown.

An Air Canada Embraer ERJ 190-100 IGW (ACA458) from Toronto/Lester B. Pearson, ON (CYYZ) to Ottawa/Macdonald-Cartier, ON (CYOW) was cleared for the area navigation (RNAV) Z Runway 25 approach via ADVOS transition. Once established, ACA458 advised tower that due to traffic they had been kept too high and could not continue on the approach. No impact on operations.

Incident at LONDON ON (CYXU) (Other operational incident)

A Swoop Inc. Boeing 737-800 (WSW208) from Edmonton, AB (CYEG) to London, ON (CYXU) was instructed to initiate a missed approach on Runway 15 at 1637Z due to a corporately registered Dassault Aviation Falcon 2000 (N925JG) from origin and destination unknown, landing on Runway 27 at the same time. WSW208 was transferred back to XU sector, completed the area navigation (RNAV) 15 and landed.

Incident at THOMPSON MB (CYTH)

While on the area navigation (RNAV) 06, the pilot of a Missinippi Air-Care Beech 200 (C-FICU/MA04) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Thompson, MB (CYTH) informed the flight service station (FSS) that they were missing their front gear indicator. C-FICU cancelled its IFR and conducted 3 circuits (CCT). The FSS advised that the gear appeared to be down. C-FICU landed without incident at 0344Z.

Incident at YORKTON MUNI SK (CYQV) (ATM - NAVAIDS/radar)

The Yorkton (YQV) very high frequency omnidirectional range (VOR) 115.8 was already NOTAMd as unmonitored. A NAV CANADA Bombardier CL-600-2B19 (C-GFIO/NVC101) from Saskatoon/John G. Diefenbaker, SK (CYXE) to Prince Albert (Glass Field), SK (CYPA) arrived at Yorkton at 1750Z for a flight check of the area navigation (RNAV) and the Yorkton VOR approaches. NVC101 could not detect any signal from the VOR. NVC101 completed the check of the RNAV approaches at 1825Z and still there was no signal from the VOR. Winnipeg Technical Operations Coordinator (TOC) advised. Winnipeg TOC issued a NOTAM for the Yorkton VOR, which noted that the VOR was unserviceable until May 29th, 2020 at 21359Z.

A Nolinor Aviation Boeing 737-229C (C-GNRD/NRL946) from Val-dOr, QC (CYVO) to Rankin Inlet, NU (CYRT), on approach for Runway 31, executed an overshoot due to an unstable approach. Aircraft returned to OVESA for another attempt at the Area Navigation (RNAV) approach Runway 31. Aircraft missed again due to an unstable approach at 1530Z and returned to OVESA to hold. At 1600Z, pilot advised they were proceeding to their alternate destination of Meadowbank, NU (CMB2). No impact to operations.

Pilot advised returning to the initial approach waypoint for RNAV 31 to attempt another approach. Aircraft landed on Runway 31 at 1525Z without incident. No impact to operations.

A WestJet Boeing 737-700 (WJA441) on a flight from Toronto/Lester B. Pearson Intl, ON (CYYZ) to Edmonton Intl, AB (CYEG) was taken off the area navigation (RNAV) Y GOPEV transition for Runway 20. Air traffic control (ATC) was unable to contact the aircraft. No other impact to the operation.

Incident at GOVIB intersection

A WestJet Boeing 737-800 (C-FZRM/WJA1591) from Atlanta, GA (KATL) to Calgary, AB (CYYC), on the area navigation (RNAV) Y Runway 17R approach, reported a landing gear issue on final approach. WJA1591 was vectored off the approach until the issue was resolved. WJA1591 landed on Runway 17L at 0255Z.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On log page AK46374, the flight crew of WJA1591 reported that on approach into CYYC, when gear lowered, three red gear disagree lights illuminated. They went through the check list and the lights went out. Maintenance cleaned landing gear control lever between outer lever shaft and the outer lever sleeve. Ground tested serviceable in accordance with (IAW) Fault Isolation Manual (FIM) 32-30 TASK 816. Aircraft returned to service on log page AK46374. 5) Aircraft has flown 27 cycles since the occurrence no further related issues.

Incident at PENDL intersection

A KLM Royal Dutch Airlines Boeing 787-9 (KLM677) from Amsterdam, Netherlands (EHAM) to Calgary, AB (CYYC), flying the BIRKO arrival for Runaway 29, closed the area navigation (RNAV) Z approach, linking PENDL to HENRI rather than turning downwind. The controller subsequently vectored the aircraft downwind. No impact to operations.

Incident at WHITEHORSE/ ERIK NIELSEN INTL (CYXY)

An Air Canada Airbus A320-211 (C-FPDN/ACA277) from Vancouver, BC (CYVR) to Whitehorse, YT (CYXY) conducting an area navigation (RNAV) approach for Runway 14R received a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and was obliged to level the aircraft at the OVUMA fix instead of continuing descent. The aircraft that caused the RA was an arriving Alkan Air Beech 200 (C-GMOC/AKN513) MEDEVAC from Dawson City, YT (CYDA) to Whitehorse, YT (CYXY) and was observed west of the position of ACA277.

Incident at IQALUIT NU (CYFB) (ATM - ILS irregularity)

A First Air Boeing 737-436 (C-FFNM/FAB101) from Ottawa/Macdonald-Cartier, ON (CYOW) to Iqaluit, NU (CYFB) reported that the vertical guidance portion of the localizer performance with vertical guidance (LPV) (area navigation (RNAV)) approach for Runway 34 was not working. Technical Operations Coordinator (TOC) advised at 1525Z. No other impact on operations. An Air Inuit Ltd. Boeing 737-33A(C-GFFN/AIE790) from Ottawa/Macdonald-Cartier, ON (CYOW) to Iqaluit, NU (CYFB) on the LPV approach (RNAV) for 34 reported on 18/02/2020 at 1243Z that everything was normal. Weather at the time VFR.

A WestJet Boeing 737-6CT (WJA166) from Vancouver, BC (CYVR) to Edmonton, AB (CYEG) was cleared for an area navigation (RNAV) Y Runway 02 approach with a crossing restriction at APKIT of 8000. A 10770582 Canada Inc. Cessna 172N (C-GYPB) from Calgary/Springbank, AB (CYBW) to Edmonton/Villeneuve, AB (CZVL) was operating at 8500 VFR and passing through the approach path 4NM East of APKIT. WJA166 was passed traffic. At APKIT, WJA166 reported that they had a traffic advisory and wanted to be vectored around the traffic. WJA166 was vectored off of the RNAV Y approach and cleared for a visual approach. No impact on operations.

An Air Canada Airbus A330-300 (ACA588) from Kahului, Hawaii (PHOG) to Calgary, AB (CYYC) was cleared for the area navigation (RNAV) Z approach for Runway 17L. One nautical mile north of PERNA, the final approach waypoint, ACA588 was observed to be climbing and reported executing a missed approach due to an unstable approach. ACA588 was subsequently vectored for another approach for Runway 17R, no operational impact.

Incident at CALGARY INTL AB (CYYC)

A WestJet Encore de Havilland DHC-8-402 (WEN3213) from Kamloops, BC (CYKA) to Calgary, AB (CYYC) commenced a missed approach while joining final for Runway 17R due to an aircraft issue. No operational impact.
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. While WEN3213 was on the area navigation (RNAV) Y 17R, turbulence caused the autopilot to disengage. As the autopilot is required for this approach, the crew elected to conduct a go-around.

A Pacific Coastal Airlines Limited Saab 340 (PCO3003) from Lethbridge, AB (CYQL) to Calgary, AB (CYYC) was pulled off a four mile final and a WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3348) from Edmonton, AB (CYEG) to Calgary, AB (CYYC) had the area navigation (RNAV) Y approach clearance cancelled and was vectored to final.

A Summit Air aircraft (SU8143) was cleared for the area navigation (RNAV) approach (APP) for Runway 02 via VIGVI transition. Aircraft lost RNAV between VIGVI and HAMPO, re-gained on short final. Navigational error. No impact to operations.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ) (ATM - NAVAIDS/radar)

Two aircraft on final for Runway 23 at Halifax/Stanfield, NS (CYHZ) appeared to be left of the centreline with the Surveillance Fusion System (FUSION) source selected on the intermediate approach waypoint (IWP) display. A WestJet Boeing 737-8CT (C-FCSX/WJA254) from Toronto/Lester B. Pearson, ON (CYYZ) to Halifax/Stanfield, NS (CYHZ) was on the area navigation (RNAV) Y and confirmed that they were on the centreline. An Air Canada Airbus A320-214 (C-FXCD/ACA626) from Toronto/Lester B. Pearson, ON (CYYZ) to Halifax/Stanfield, NS (CYHZ) was on the instrument landing system (ILS) 23 and also advised that they were showing on the centreline. The Technical Operations Coordinator (TOC) was advised. No impact on operations.

A Jazz Bombardier CL-600-2D24 (C-FKJZ/JZA8794) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Halifax/Stanfield, NS (CYHZ) deviated from the area navigation (RNAV) Runway 32 approach by descending to 3000 feet before EMBAX. The approach reads that EMBAX is to be crossed at 4000 feet or above.

A Provincial Aerospace Cessna S550 (C-FABF/SPR920) from St. Johns, NL (CYYT) to Halifax/Stanfield, NS (CYHZ) deviated from the area navigation (RNAV) Runway 32 approach by descending below 4000 feet before GABAP. The approach reads that GABAP is to be crossed at 4000 feet or above.

Incident at SAINT JOHN NB (CYSJ) (ATM - weather observation systems)

The ceilometer was indicating 1000 feet. An Exploits Valley Air Services Beech 1900D (C-GLHO/EV767) from Halifax/Stanfield, NS (CYHZ) to Saint John, NB (CYSJ) missed approach for area navigation (RNAV) Runway 32. Ceilometer found to be frozen. Once refreshed, ceiling indicated 400 feet.
