Piper PA-23-250 (C-GNZQ/SCANER4) from Trois-Rivières, QC (CYRQ) to Montréal/Mirabel, QC (CYMX) at the same altitude. Traffic was passed.

Incident at SEPT-ÎLES QC (CYZV) (Incursion - runway - vehicle)

A Hélicoptères Canadiens Aerospatiale AS-355-N (C-FXIH) from Sept-Îles, QC (CYZV) to Sept-Îles, QC (CYZV) reported that it had been cut off by a vehicle (a TSAS tractor) on the final approach and takeoff area (FATO) while the vehicle in question was leaving the fixed-base operator (FBO) to go to the main apron. The aircraft had to abort its final approach to let it pass. No other impact.

While departing the Montréal/St-Hubert, QC (CYHU) zone near the bridge-tunnel, an A.L.M. Par Avion Inc. Cessna 172M (C-GITN) was seen in the CYHU zone flying toward Saint-Mathieu-de-Beloeil, QC (CSB3). At the Institut de recherche d’Hydro-Québec (IREQ), the aircraft veered north without any communication with the tower. Traffic information was passed to a Pilatus PC12 on approach.

Aviation Incident Report #17136: At 1448Z, an Air Inuit de Havilland DHC-6-300 (C-GKCJ/AIE681) on a cargo flight from Kuujjuaq, QC (CYVP) to Kangiqsualujjuaq (Georges River), QC (CYLU) called the community aerodrome radio station (CARS) on mandatory frequency (MF) 122.1. Initial contact was at 7NM to the west, low level estimated time of arrival (ETA) 3 minutes for Runway 16. All necessary numbers were passed on advisory. During its final approach, no final call was passed to the CARS office. AIE681 did not follow the 15NM radius call or make a final approach 16 call.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

A Pascan British Aerospace Jetstream Model 3212 (PSC587) of Quebec/Jean Lesage, QC (CYQB), en route to Bonaventure, QC (CYVB), reported an engine failure and requested to be rerouted to CYQB. PSC587 declared an emergency. PSC587 landed on Runway 06 at 1420Z without incident. No impact. The National Operations Centre (NOC), the Transportation Safety Board of Canada (TSB), the Aviation Operations Centre (AOC) and the Joint Rescue Coordination Centre (JRCC) Trenton were notified.
UPDATE: TSB Report #A21Q0055: A Pascan Aviation British Arerospace Jetstream 3212 (C-GPSK/PSC587) was on an instrument flight rules flight from Québec/Jean-Lesage, QC (CYQB) to Bonaventure, QC (CYVB). While the aircraft was in cruise flight, while passing over the VODIX fix, the “Duct Light On” right engine warning light illuminated. The Quick Reference Handbook (QRH) checklist was completed and the crew shut the engine down. An emergency was declared and the crew then requested to divert to CYQB. The aircraft landed without incident on Runway 06 at CYQB.

An Air Creebec de Havilland DHC-8-106 (C-FDWO/CRQ5936) from Val-dOr, QC (CYVO) to Rouyn-Noranda, QC (CYUY) advised of receiving a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) while on left base for Runway 26 at CYUY. Two Bombardier CL-215-6B11 (CL-415 series) aircraft crossed the runway centre line of Runway 26 at an estimated distance of 6NM to 8NM, without having established any radio contact. The first Bombardier CL-215-6B11, having been sighted visually by the flight service station (FSS), had been passed as traffic to CRQ5936 with approximate position and altitude, but the second was not seen.

C-FLTA conducted a emergency practice maneuver 4.2NM to the north and, on its recovery, performed an avoidance maneuver in relation to C-FKNQ, which was passing behind. No impact.

A Lanaudiair Inc. Cessna 172N (C-GTIR) on a flight to Lachute, QC (CSE4) entered the Montreal/Mirabel, QC (CYMX) zone to the west, towards CSE4, at 1800 feet. The aircraft never called the tower and the tower was unable to establish communications. C-GTIR transited the zone for 6NM, between 1800 feet and 1600 feet. Impact: a privately registered Cessna 172M heading towards CSE4 was not passed traffic. Converging routes and similar altitudes.

Incident at 8NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FWEZ/WEN3543) from Montreal/Pierre Elliott Trudeau, ON (CYUL) to Toronto/Lester B. Pearson, ON (CYYZ) was cleared to 5,000 feet. After briefly levelling off, the aircraft was observed at 5500 feet, 2.5NM behind a Jazz Bombardier CL-600-2D24 (C-GOJZ/JZA8970) from Ottawa/Macdonald-Cartier, ON (CYOW) to Montreal/Pierre Elliott Trudeau, QC (CYUL).
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Shortly after departure, the crew was made aware of potential conflicting traffic 1000ft above their assigned altitude. Visual confirmation of the traffic was made by the crew. At the time of acknowledging and passing the conflicting traffic, the crew was assigned a heading by air traffic control (ATC). The pilot that was flying, upon hearing the instruction given by ATC, began to both initiate the turn (as directed), but also self-initiated a climb. The climb was initiated in error due to the flying pilot anticipating a higher altitude because the potential traffic no longer posed a threat. The flying pilot quickly realized their error, and elected to gradually level off for guest comfort. The 400ft deviation in altitude can be attributed to the gradual altitude capture rather than an abrupt one (again, because the conflicting traffic posed no threat). The Flight Safety Standards pilot debriefed with the flight crew following the event, and reviewed the following contributing factors leading to the altitude deviation, which included: expectation of being assigned a clearance (i.e. confirmation bias); role transfer and the subtle disorientation operating from a seat not commonly occupied by the Pilot Flying (PF); and a lightly loaded aircraft, namely in increased climb performance.

Aviation Incident Report #16977: A Héli-Tremblant Robinson R44 (C-GCHI) was observed passing about 75 feet from the port side of a Coast Guard boat, near the wheelhouse, at the Trois-Rivières port. Incident at DELTA / DELTA HERITAGE AIR PARK BC (CAK3)

Aviation Incident Report #17150: An individual reported that a 544211 BC Ltd. Navion (C-FEZI) on a flight to Delta/Delta Heritage Air Park, BC (CAK3) flew at a low altitude over people walking along the dyke south of CAK3. The aircraft came from over the water to the south, passed northbound at about 200 feet over the dyke and aerodrome, then climbed into the circuit and landed at CAK3.

Incident at PITT MEADOWS BC (CYPK)

A privately registered Navion from Langley, BC (CYNJ) to Langley, BC (CYNJ) entered the Class C control zone of Pitt Meadows, BC (CYPK), at 1500ft above sea level (ASL), without contacting air traffic control (ATC). Traffic had to be passed an Island Coastal Aviation Inc. Cessna 172M (C-FEFY) in the circuit at Pitt Meadows, BC (CYPK), but ATC could not reach the Navion aircraft on the tower frequency.

Incident at South Pender Island, BC

Aviation Incident Report #17074: An individual reported that an Ocean Air Floatplanes Cessna 180J (C-FERX) on a flight passing over South Pender Island, BC, flew over homes, raptors and migratory wildlife at an altitude of 152 meters. The aircraft also crossed the South Bluffs, over houses, at a low altitude ranging from approximately 50 to 100 meters.

Incident at PENTICTON BC (CYYF) (Other operational incident)

The flight service station (FSS) observed a kite surfer passing back and forth off the approach (APCH) for Runway 34 at Penticton, BC (CYYF). Incident reported to the Royal Canadian Mounted Police (RCMP). No impact to operations.
