During excavation work at the edge of Runway 06-24, a transmission wire was severed. The Very High Frequency Omnidirectional Range (VOR), Distance Measuring Equipment (DME), Runway 06 Localizer (LOC), Runway 06 Visual Range (RVR), Precision Approach Path Indicator Exterior Light (PAPI) for Runway 24, and Runway 24 threshold identification lights (RTIL) were unserviceable. NOTAM issued.

Incident at FORT ST. JOHN BC (CYXJ) (Aerodrome - visual aids)

A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3202) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ), on final for Runway 03 on the correct slope, reported that the precision approach path indicators (PAPIs) were "white-white-red-white". A Central Mountain Air Ltd. Beech 1900D (GLR743) from Prince George, BC (CYXS) to Fort St. John, BC (CYXJ), on final 7 minutes later, reported the same thing. The North Peace Airport Services (NPAS) unit was advised and confirmed that there was an issue with the PAPIs after an inspection. A NOTAM was then issued by NPAS.

At Thunder Bay, ON (CYQT), the precision approach path indicators (PAPIs) for Runway 07 and Runway 25 failed at 0427Z and were unable to be reset. A WestJet Encore Ltd. de Havilland DHC-8-402 (C-GWRE/WEN3219) from Toronto/Lester B. Pearson, ON (CYYZ) to Thunder Bay, ON (CYQT) was 5NM final at the time. The Emergency Response Operations Centre (EROC) was notified of the issue. No other operational impact.

At Thunder Bay, ON (CYQT), the lighting panel was showing incorrect intensity settings for various precision approach path indicators (PAPIs) (i.e. the panel was showing PAPIs are on when they are actually off) when the winter heating mode was used. No impact to operations prior to this being discovered.

Incident at FORT MCMURRAY AB (CYMM) (FOD (foreign object debris))

At Fort McMurray, AB (CYMM), a staff vehicle reported that bird remains were found on the runway, near the Runway 26 precision approach path indicators (PAPIs). Although there had been multiple departures and arrivals in the preceding half hour, the vehicle operator advised the remains were not fresh. McMurray Aviation was advised anyways, as most of the arrivals and departures were theirs. A Jazz flight (JZA8422) was also advised, and said they found nothing on their walkaround.

The Aircraft Rescue and Fire Fighting (ARFF) unit was advised and scared the animal off the runway at approximately 1920Z. ARFF will advise the trapping team of the situation.

Incident at FORT ST. JOHN BC (CYXJ) (Aerodrome - visual aids)

At Fort St. John, BC (CYXJ), a privately registered Piper PA-18-150 had been conducting circuits on the grass beside Runway 03. After arriving on its last circuit, the aircraft was observed taxiing over the precision approach path indicator (PAPI) for Runway 21 on its way to the ramp. The pilot confirmed that it had struck the PAPI. Airport authority advised.
Aviation Incident Report #17114: Fort St. John, BC (CYXJ) personnel responded immediately after the occurrence. The privately registered Piper PA-18-150 from Fort St. John, BC (CYXJ) to Fort St. John, BC (CYXJ) was inspected by CYXJ personnel and it sustained damage to its propeller. The precision approach path indicator (PAPI) unit was removed from the field and the PAPI was NOTAMd unserviceable. The airport electrician checked the unit and the damage was minor. The unit was repaired, reinstalled, and returned to service on May 29, 2021 at 0030Z. NOTAM was cancelled. There was no operational impact.

Truck (ST22) asked and received permission on to Runway 13/31 to service the Runway 13 precision approach path indicator (PAPI). Shortly after, a Can-West Corporate Air Charters Cessna U206F (C-GICJ) from High Level, AB (CYOJ) to Fox Lake, AB (CEC3) called to request departure off 31. They were informed of vehicle traffic during the advisory, and indicated they would hold short. The Flight Service Station (FSS) was unable to reach ST22 on radio, flashed lights, called the cell of operator, and had Truck (ST21) head on to the runway to remove ST22. C-GICJ was delayed for 5 minutes. No further impact.

Incident at PUKATAWAGAN MB (CZFG) (Aerodrome, runway or taxiway shutdown)

The Pukatawagan, MB (CZFG) airport operator (APO) called to close the runway due to a disabled aircraft stuck in the turning area. NOTAM issued until 20h00 local time.

Narrative updated to read as follows: "The Pukatawagan, MB (CZFG) airport operator (APO) called to close the runway due to a disabled aircraft, a Missinippi Air-Care Piper PA-31-350 (C-GMKO/MA433) from Pukatawagan, MB (CZFG) to The Pas/Grace Lake, MB (CJR3), stuck in the turning area. NOTAM issued until 20h00 local time."
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. On June 16, 2021, the pilot of C-GMKO aborted their take-off, which resulted in both main wheel tires being worn through. Missinippi Aircraft Maintenance Engineers (AMEs) were flown to Pukatawagan, MB (CZFG) the next morning, where both main wheels were replaced with spare built-up wheel assemblies ("assys"). No other damage found. The aircraft was released.
UPDATE: Aviation Incident Report #17159: At 2315Z, a Missinippi Air-Care Piper PA-31-350 (C-GMKO) with an en route stop at Pukatawagan, MB (CZFG) was taking off from Runway 15. A Missinippi Agent saw C-GMKO lift off, but it still had its landing gear down. C-GMKO lost power or cross wind, toke aircraft down, and landed 100 feet before the precision approach path indicator (PAPI) area. They continued down Runway 33 at full power, and when they used the brakes, both tires were blown on landing gear left and right side. The aircraft passed the threshold light area, turning towards the left-side turn where it stopped. The Missinippi Agent called airport staff and advised of a disabled aircraft. Airport staff arrived and observed the aircraft at the end of the threshold area of Runway 33, 20 feet from the drop-off edge (40-foot drop). They assisted the pilot in removing the aircraft. The aircraft was attached to a tow bar and a loader was used to tow it to the apron. The Airport Manager (APM) called for a runway closure from 0000Z to 0100Z, and advised when the aircraft was away from the area to cancel the runway closure.

While conducting a touch-and-go training flight, an Orizon Aviation Québec Inc. Cessna 152 (C-GJBF) from Quebec/Jean Lesage, QC (CYQB) to Quebec/Jean Lesage, QC (CYQB) exited Runway 24 and stopped on the grass on the south side of Runway 24, between the precision approach path indicators (PAPIs) and Taxiway A. No damage reported to the aircraft. No injuries. Services were deployed. The aircraft exited the runway under its own power without incident. Impact: redirection to Runway 29 of an Exact Air Beech A100 (C-FXDE/ET828) from Port-Menier, QC (CYPN) to Quebec/Jean Lesage, QC (CYQB) and a Government of Canada, Department of National Defence Beech 350 (CFC1605) from Goose Bay, NL (CYYR) to Quebec/Jean Lesage, QC (CYQB). No other impact on operations.

At 0005Z the pilot of a privately registered, amateur-built Just Aircraft Highlander on a flight to to Campbell River, BC (CYBL) was given a wind check of 050 degrees at 07kts when they reported final for Runway 12. Upon landing the aircraft ground looped. The pilot regained enough control to continue on to Taxiway B. Initially, the pilot reported no damage to the aircraft, and did not require assistance. An evening runway inspection identified the Runway 12 precision approach path indicator (PAPI) was damaged. The PAPI was NOTAMed as unserviceable (U/S).

At 07022115Z, the Kamloops (YKA) flight information centre (FIC) received a fax to issue a NOTAM that the Penticton, BC (CYYF) precision approach path indicators (PAPIs) for Runway 16/34 were unserviceable (U/S) until (TIL) approximately (APRX) 2007031700Z. At 07031709Z, the YKA FIC received a call from the CYYF airport manager (APM) saying that the NOTAM could be cancelled; however, the NOTAM had been issued with a TIL time of 07031700Z and had already been removed from the aeronautical fixed telecommunications network (AFTN).

Incident at KINGSTON ON (CYGK) (Incursion - runway - pedestrian, Communication error)

At Kingston, ON (CYGK), workers were observed by the flight service station (FSS) to be working on the Runway 19 precision approach path indicator (PAPI) within the runway protected area (RPA) of Runway 01/19. There were no local procedures conducted to permit this operation without coordination from the FSS. A vehicle (ST24) was authorized earlier to proceed to the glide path road and was observed nearby the workers. The pilot of a Thunder Airlines Ltd. Mitsubishi MU2B (THU860) from Kingston, ON (CYGK) to North Bay, ON (CYBB) advised taxiing for Runway 19, and was advised of the presence of the workers. There was no response from the vehicle when it was called. The vehicle established communication over the phone after it had exited.

The Prince Albert (Glass Field), SK (CYPA) airport contacted the Winnipeg Flight Information Centre (FIC) to file a NOTAM for the precision approach path indicator (PAPI) of Runway 26. The FIC specialist filed the NOTAM under an incorrect airport, Saskatoon/John G. Diefenbaker, SK (CYXE), instead of CYPA. The NOTAM was corrected at 2252Z.

Aviation Incident Report #16814: At Mayo, YT (CYMA), the Runway 07/25 precision approach path indicator (PAPI) lights were unserviceable (U/S), due to a cable break as a result of airport maintenance activities. There is currently no scheduled service at CYMA that would require PAPI; however, charter operations, MEDEVAC and Royal Canadian Mounted Police (RCMP) operations may be impacted. Stakeholders have been notified. A NOTAM was issued to indicate that the PAPIs were unserviceable. Repair of cable break scheduled for September 30, 2020.
