Incident at KUUJJUARAPIK QC (CYGW)

Aviation Incident Report #17287: A Montmagny Air Service Inc. Partenavia P68 Observer (C-GJMZ) on a flight from Kuujjuarapik, QC (CYGW) was on Apron 2 getting ready to depart. The aircraft called La Grande Rivière, QC (CYGL) Flight Service Station (FSS) to advise them they would be taxiing on Taxiway Bravo and taking Runway 0-3. Before CYGL FSS could advise Vehicle (Staff-4-0 plus 1), the aircraft taxied from Taxiway Bravo onto Taxiway Alpha before all vehicles were reported off Taxiway Alpha.

Incident at 75NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Dangerous goods/hazardous materials, Aerodrome, runway or taxiway shutdown)

En route to Vancouver BC, (CYVR) an Air Canada Airbus A330-300 (ACA301) on a flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) reported losing its hydraulic system and that it was possible that hydraulic fluid had spilled onto the manoeuvring areas at CYUL. ACA301 did not declare an emergency but requested the presence of Aircraft Rescue and Fire Fighting (ARFF) upon arrival at CYVR. Coordination was done. Impact: Runway 06R/24L was not available from 1228Z to 1238Z causing delays in take-off for several aircraft including an Air Canada Airbus A220-300 (ACA1073), a Jazz Embraer 170-200SU (JZA7552), a Jazz Bombardier CL-600-2B19 (JZA8696), an Air Canada Airbus A321 (ACA926), an Air Canada Boeing 737 MAX 8 (ACA1296) and a Jazz de Havilland DHC-8-402 (JZA7955), and, on arrival, an Air Nunavut (BFF20) on a MEDEVAC flight.

Incident at SEPT-ÎLES QC (CYZV)

An American, privately registered Maule M-7-235 from Rivière-du-Loup, QC (CYRI) to Sept-Îles, QC (CYZV) landed on Runway 27, and when it tried to make a U-turn, the aircraft ended up completely outside the runway lights. The aircraft performed a few manoeuvres in the grass before rolling back onto the runway and exiting at Delta. Impact: delay to an inbound Exact Air inc. Beech A100 (ET828) from Chicoutimi/Saint-Honoré, QC (CYRC) to Sept-Îles, QC (CYZV), which had to extend downwind and conduct a 360° on final.

A True North Airways Piper PA-31-350 (C-GCTH) from Val-dOr, QC (CYVO) to Sudbury, ON (CYSB) taxied onto the apron and then took off from Runway 18 at 1225Z with no communication and without IFR clearance. A Sky Jet M.G. Inc. Beech 200 (LSJ447) from Quebec/Jean-Lesage, QC (CYQB) to Val-dOr, QC (CYVO), on approach for Runway 36, reported having the traffic on its traffic alert and collision avoidance system (TCAS) and passing within 2 NM of the traffic.

Incident at KUUJJUAQ QC (CYVP) (Incursion - manoeuvring area, Incursion - runway - pedestrian, Aerodrome, runway or taxiway shutdown)

While boarding an Air Inuit Ltd. Beech B300 (C-FKMG/AIE542) from Kuujjuaq, QC (CYVP) to Puvirnituq, QC (CYPX), a passenger ran out of the aircraft, across part of the apron, and onto Taxiway A and Runway 13/31. Airport advised. Impact: Runway 13/31 and Taxiway A were closed from 1530Z to 1540Z.
UPDATE: During the boarding process, a passenger being escorted by two agents escaped and ran on the runways at Kuujjuaq, QC (CYVP). The police responded and removed the passenger from the airport. An Air Inuit aircraft (AIE683) on a flight to Kuujjuaq, QC (CYVP) was diverted due to the occurrence.

A privately registered American Aviation AA-1 from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) turned right onto Runway 28 after receiving an instruction to taxi to the left on Runway 10. The aircraft was instructed to stop and turn around as it passed the hold lines for Runway 06L. Impact: a Cargair Cessna 152 (C-GBJU) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU), on short final for Runway 24R, received an instruction to conduct a pull-up. No other impact on operations.

At Montreal/Mirabel, QC (CYMX), a vehicle (PE63) was observed on Runway 06/24 without authorization. The vehicle had requested to proceed onto Taxiway B - CNX from the holding bay. The vehicle operator made a mistake with their request, as they were on the approach route to Runway 24. No operational impact.

Accident at RIMOUSKI QC (CYXK)

A privately registered Jodel D-9 from Rimouski, QC (CYXK) to Rimouski, QC (CYXK) reported that it was on final for Runway 07 at CYXK, and then made no further contact. An aircraft that was following the Jodel D-9 mentioned that the Jodel D-9 had overran the runway. The Jodel D-9 exited the runway on the south side, traveled about 200 feet south of the runway, and then rolled over onto its roof. The passengers exited on their own. A vehicle (ST25) notified by people present at the airport provided assistance to recover the aircraft. Damage unknown. No other impact. The Transportation Safety Board of Canada (TSB), the Aviation Operations Centre (AVOPS) and the National Operations Centre (NOC) have been notified.

Incident at MONTRÉAL / ST-HUBERT QC (CYHU) (Aerodrome, runway or taxiway shutdown)

After landing, a privately registered Cirrus SR20 from Montreal/Mirabel, QC (CYMX) to Montreal/St-Hubert, QC (CYHU) exited Runway 06L onto Taxiway Kilo. Its engine stopped while it was still in the protected area of the runway. The aircraft was towed a few minutes later. Impact on operations: two aircraft landed on Runway 06R and departures were delayed.

After landing on Runway 24L and exiting onto Taxiway G, a Cargair Cessna 152 (C-FRHT) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) struck a taxiway light. The runway was closed for several minutes during the cleanup. Other aircraft were redirected to Runway 24R. No other impact on operations.

Airport personnel advised of the presence of 2 dogs on the runway while an Air Inuit de Havilland DHC-8-314 (C-GRAI/AIE854) from Quebec/Jean Lesage, QC (CYQB) to Kuujjuarapik, QC (CYGW) was on the approach. A vehicle (ST40) was authorized to enter onto the runway, and it advised that the dogs had left. AIE854 landed without incident. No impact.

The aircraft was manually moved by airport firefighters onto Taxiway B3. Impact: a Pascan Pilatus PC-12 (PSC366) from Rouyn-Noranda, QC (CYUY) to Montreal/Pierre Elliott Trudeau, QC (CYUL) on final for Runway 24R, was reassigned to Runway 24L.

Incident at VERNON BC (CYVK)

Aviation Incident Report #17163: A privately registered Mooney M20C from Vernon, BC (CYVK) to Vernon, BC (CYVK) landed Runway 05 and veered off the north side of the runway, striking the Taxiway Bravo exit sign and shearing the sign off the frangible posts at 1513Z. The aircraft then proceeded back onto the runway and exited at Taxiway Bravo. Staff Vehicle 33 responded and removed the broken sign off of the active area in the grass. A runway inspection was completed. No other aircraft in the pattern at the time. CYVK resumed normal operations at 1520Z.

Incident at FORT ST. JOHN BC (CYXJ) (Other operational incident, ATM - ILS irregularity)

An alarm sounded and the instrument landing system (ILS) glide path for Runway 30 went onto "standby" at 2245Z. Mower (TT150) was working in the glide path critical area with permission. Technology Operations Coordinator (TOC) advised that this is expected behaviour when a large vehicle passes close to the glide path. No traffic was using the glide path or inbound. The glide path was reset when work in the critical area was complete at 2257Z. No impact on operations.

Incident at Kelowna

A Hanson Aviation Corp. Cessna 172M (C-GDZQ) from Kelowna, BC (CYLW) to Kelowna, BC (CYLW) was instructed to hold short Taxiway C. The aircraft pulled onto Taxiway C. Short delay for a WestJet Encore de Havilland DHC-8-402 (WEN3311) to exit the runway. No operational impact.

Accident at FORT ST. JOHN BC (CYXJ) (Aerodrome, runway or taxiway shutdown)

During departure, a privately registered Cessna 170B from Fort St. John, BC (CYXJ) to Whitehorse/Erik Nielsen, YT (CYXY) veered to the side of the runway and its wheel came off. The aircraft crashed and tipped onto its side. The Aircraft Rescue and Fire Fighting (ARFF) unit responded and advised that there were no injuries, but there was a fuel leak on the runway. No emergency services needed. A NOTAM was issued to close Runway 12/30 and Taxiway Alpha. All pertinent parties were informed. The runway was returned to service at 2055Z.

The engine of a Victoria Flying Club Cessna 172S (C-GSDZ) from Victoria, BC (CYYJ) to Victoria, BC (CYYJ) stopped working while the aircraft was on Runway 09, after landing. C-GSDZ was disabled on Runway 09 until the Aircraft Rescue and Fire Fighting (ARFF) unit and a tug vehicle helped to push the aircraft off Runway 09 and onto Taxiway Golf. The tug pulled C-GSDZ back to the Victoria Flying Club without further incident. No impact on operations.
