The flight service station (FSS) requested that a Richcopter Inc. Cessna 150M (C-GUDB) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), on final approach, conduct a go-around. After returning to the ramp for an aircraft (A/C) inspection, C-GYGH proceeded onto Taxiway Alpha without contacting the FSS at 1932Z. No impact.

A privately registered, amateur-built Robinson Special from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) started backtracking on Runway 09 at the same time that an Acer Cold War Museum Aero L-29 Delfin (C-GGRI) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) was also already backtracking. The Robinson Special did not wait for coordination to be performed with C-GGRI before going onto the runway. No impact on operations.

An Air Inuit de Havilland DHC-8-102 (AIE870) on a flight from Kuujjuarapik, QC (CYGW) reported that it had to return onto the apron; no reason was given. Clearance cancelled at 1850Z. The aircraft departed at 1900Z.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome, runway or taxiway shutdown)

An Air Canada Airbus BD-500-1A11 (C-GUPG/ACA665) from Halifax/Stanfield, NS (CYHZ) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) blew a tire on landing. ACA665 ran off the end of the runway, without incident, onto 06R. An inspection was performed and nothing was found. Impact: 3 aircraft (TSC21, SYB203 and ACA401) were delayed for about 10 to 15 minutes due to the inspection. No other impact on operations.

An Air Canada service vehicle (PKM462F) drove onto Taxiway Delta at Montréal/Pierre-Elliott-Trudeau, QC (CYUL) without clearance. It got stuck in a snowbank and was abandoned with lights off and without any notice. No impact.

QXE2517 read back the instructions but turned onto "A" before contacting the tower. No operational impact.

A Coastal Pacific Aviation Ltd. Cessna 172M (C-GTCN) from Abbotsford, BC (CYXX) to Abbotsford, BC (CYXX) called ready for departure. They were instructed to hold short due to traffic on final. Read back was given. C-GTCN started to taxi onto the runway. C-GTCN was instructed to stop. Another Cessna 172 that was 1NM final was instructed to pull up. C-GTCN was instructed to continue and take-off clearance was given when it was safe.

Incident at TERRACE BC (CYXT)

At the gate, while passengers were exiting, a Jazz de Havilland DHC-8-402 (C-GKUK/JZA8445) from Vancouver, BC (CYVR) to Terrace, BC (CYXT) reported a passenger having a medical emergency. JZA8445 said that an ambulance was already called but requested assistance from the flight service station (FSS) to ensure the ambulance could get onto the apron to park beside the aircraft.

The aircraft was instructed to contact ground control on frequency 121.9 MHz when off of the runway. C-GEIT taxied eastbound on Taxiway Golf until ground control reached out to get a "hold short".

Incident at LANGLEY REGIONAL BC (CYNJ)

An AAA Aviation Ltd. Cessna 172I (C-GUNE) from Langley, BC (CYNJ) to Langley, BC (CYNJ) landed on Runway 25 and was instructed to exit via Taxiway G and contact the ground controller. The aircraft started taxiing onto Taxiway G without contacting the ground controller. The controller reached out and issued an instruction to "taxi on Taxiway G and hold short of Runway 19". C-GUNE did not read back the hold short. The controller required multiple transmissions to get a read back.

Incident at NANAIMO AIRPORT (Incursion - runway - vehicle)

A truck (ST22) proceeded onto Runway 16/34 after being authorized to proceed via Taxiway C and hold short of Runway 16/34. ST22 read back the hold short instruction. An Alkan Air Beech 200 (AKN216T) from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD) was 2NM final for Runway 34 and ST22 had to quickly exit the runway onto the grass to vacate the runway.

Incident at CAMPBELL RIVER BC (CYBL)

A 49 North Helicopters Bell 206B (C-FJOR) from Campbell River, BC (CYBL) to Campbell River, BC (CYBL) was departing from Runway 12 and the specialist requested, using "can you" phraseology, that C-FJOR sequence behind a Sealand Flight Cessna 172K (C-FXYJ) from Campbell River, BC (CYBL) to Campbell River, BC (CYBL) when joining the circuit for Runway 12. C-FJOR queried traffic again and elected to turn in front of traffic entering the downwind (C-FXYJ). C-FJOR then executed an autorotation for the runway that had them cut in front of another helicopter, a 49 North Helicopters Robinson R22 Beta (C-FZWQ) from Campbell River, BC (CYBL) to Campbell River, BC (CYBL), that was final for the grass area south of Runway 12, and turned onto final for Runway 12, which was still occupied by a departing Pacific Coastal Airlines Beech 1900 (PCO707) from Campbell River, BC (CYBL) to Campbell River, BC (CYBL). C-FJOR initially stated they were going to land behind the PCO707. C-FJOR ended up landing behind the PCO707 that was departing Runway 12, cutting in front of C-FZWQ, who was on final for the grass area. The "can you" phraseology was to allow time for the PCO707 to backtrack and depart.

C-FNJF was observed taxiing onto Runway 26R, at the intersection of Runway 18, without permission. After a takeoff clearance for Runway 18, C-FNJF was observed taking off from Runway 26R.

Beech 1900D (C-FMCN/PCO1610) from Tofino/Long Beach, BC (CYAZ) to Vancouver, BC (CYVR) reported a traffic alert and collision avoidance system (TCAS) resolution advisory (RA) and followed the TCAS instructions to climb. PCO1610 reported to Vancouver Terminal and was returned to a descent and onto the approach once the RA conflict was resolved. No operational impact.

A privately registered Piper PA-28-151 on a flight to Williams Lake, BC (CYWL) exited Runway 12/30 onto Taxiway B, which does not have winter maintenance. A vehicle (ST21) assisted the aircraft to get back onto Runway 12/30. The aircraft continued on their flight plan after a delay of approximately 15 minutes. No other impact to operations.

A privately registered, amateur-built Mustang Bushby Mustang II from Langley, BC (CYNJ) to Langley, BC (CYNJ) at Hangar 39 was issued taxi clearance F for departure Runway 25. The aircraft was observed taxiing past F onto E without clearance. The aircraft was re-issued taxi clearance E, cross Runway 01, K, and B. The aircraft turned left onto Runway 01. Ground stopped the aircraft and told them to taxi K and B. The aircraft was cleared the TRINITY departure with an altitude restriction of 1000 feet. The tower later observed the aircraft climbing through 1700 feet heading towards THUNDERBIRD.

Incident at CHILLIWACK BC (CYCW)

Aviation Incident Report #17519: An Okanagan Aviation Academy Cessna 172M (C-GWJZ) was conducting circuits on Runway 07 at Chilliwack, BC (CYCW). Shortly after C-GWJZ made a downwind radio call preparing to start a simulated power-off landing (P-180), a privately registered Cessna 182P from Chilliwack, BC (CYCW) to Chilliwack, BC (CYCW) made a radio call while crossing the middle field to join the downwind, stating they were number 2. C-GWJZ continued the circuit, advising they were turning final for a full stop landing. They landed just past Taxiway Charlie, followed by a short backtrack to exit the runway onto Charlie (the quickest way out of the runway). The Cessna 182P landed behind C-GWJZ while they were turning around and still on the runway surface. The distance between the threshold of Runway 07 and Taxiway Charlie is approximately 1,300 feet. C-GWJZ continued to taxi out onto Charlie, and after the hold short line, they called to advise they were clear of the runway.

The aircraft was then subsequently towed back to the apron by airport management (APM) staff.

Incident at FORT ST. JOHN BC (CYXJ) (Communication error, Incursion - runway - vehicle)

At Fort St. John, BC (CYXJ), a vehicle was noticed approaching Runway 12/30. Over the frequency, other vehicles was asked if there was a vehicle near the runway protected area (RPA) for Runway 12/30. A car (ST63) called and said they were close to, but outside of, the RPA for Runway 12/30. Then, a car (ST61) called for permission onto the RPA for Runway 12/30 and was granted it. The vehicle was then removed due to inbound aircraft, and when ST61 reported off, so did ST63. North Peace Airport Services (NPAS) was called, and the Flight Service Station (FSS) was then informed that ST63 believed they were getting permission onto the runway when ST61 was granted permission on the runway. All vehicles were off the runway with no impact to operations.

Incident at Kelowna (Aerodrome, runway or taxiway shutdown)

An Okanagan Aviation Academy Cessna 172M (C-GJES) from Salmon Arm, BC (CZAM) to Kelowna, BC (CYLW) touched down and made a left turn on the runway, into the snow on the east side of the runway. Crash alarm was pulled, and emergency vehicles attended the scene right away. The runway was closed via NOTAM.
UPDATE: TSB Report #A22P0006: C-GJES, an Aurora Aviation Academy Inc. Cessna 172M, was conducting flight training exercises near Kelowna Intl, BC (CYLW) with a student pilot on board. During the landing roll at CYLW, directional control was lost. The aircraft departed the left side of the paved surface of Runway 34, in the vicinity of Taxiway B. The aircraft did not collide with any obstacles. The aircraft became stuck in the snow and the student pilot shut down the engine to prevent a prop strike. Aircraft rescue fire fighting personnel responded and assisted with towing the aircraft back onto the runway. There were no injuries and no apparent damage to the aircraft. The operator will complete an aircraft maintenance inspection prior to returning the airplane to service.
