Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

Aviation Incident Report #17878: A Sasair Inc. (Centre de Formation Aéronautique de Québec) Cessna 172M (C-GLFQ) was taxiing on Apron 3 eastward in an attempt to reach the end of the apron to perform a run-up on it. While the Cessna 172M was nearing the Apron 3 intersection and the access route leading to Ailes Québécoise/Orizon/iFly, etc., a Collège iFly Inc. Cirrus SR20 (C-GVEO)-type aircraft was also approaching the intersection. The Cirrus SR20 turned right onto Apron 3 but did not notice the Cessna 172M coming from the left. The Cessna 172M had to brake to allow the Cirrus SR20 to pass on the right; the Cirrus SR20s left wing came one foot from catching onto the Cessna 172Ms propeller.

Incident at UMIUJAQ QC (CYMU)

Aviation Incident Report #17569: At Umiujaq, QC (CYMU), an Airport Maintenance vehicle with a snow blower contacted the air-to-ground radio with the Observer/Communicator (O/C) and advised they were going onto the runway for maintenance. The O/C advised that there was no reported traffic. While on the taxiway, the vehicle observed on their right an Air Inuit de Havilland DHC-6-300 (C-GMDC/AIE610) from Umiujaq, QC (CYMU) to Kuujjuarapik, QC (CYGW) on the runway. O/C advised the vehicle not to proceed on the runway. AIE610 landed without any communication and did not attempt to conduct an overshoot despite the vehicle on the taxiway. The pilots later advised being on the wrong frequency and there was no O/C for one week.

Incident at OTTAWA / GATINEAU QC (CYND) (Incursion - runway - vehicle)

While a NOTAM closing the runway was about to expire (1700Z), a vehicle (ST22) drove onto the closed runway to remove the cones and stayed after the NOTAM expired. Impact: takeoff for a Richcopter Inc. Cessna 150M (C-GDXD) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) was delayed.

Incident at OTTAWA / GATINEAU QC (CYND) (Incursion - manoeuvring area)

While a section of Taxiway C was closed due to construction, a truck was seen driving on an open section of Taxiway A (for a short time). The truck then drove onto the correct taxiway (C). No impact on operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

A Piedmont Airlines Embraer EMB-145LR (N647AE/PDT6100) from Montréal/Pierre-Elliott-Trudeau, QC (CYUL) to Philadelphia, PA (KPHL) received and read back instructions to taxi on Taxiway Golf and Runway 10. PDT6100 taxied on Runway 06L, which was closed by NOTAM. The controller gave instructions to turn around and go back onto Runway 10. No impact.

Incident at KUUJJUARAPIK QC (CYGW)

In flight, an Air Inuit Ltd. de Havilland DHC-8-102 (AIE853) from Kuujjuarapik, QC (CYGW) to Kuujjuaq, QC (CYVP), reported that it needed to return to land at the airport due to a problem with the landing gear door. Clearance was cancelled. The issue was resolved during the flight and AIE853 continued onto CYVP. Air traffic control (ATC) was advised and a reclearance was issued.

Incident at KUUJJUAQ QC (CYVP) (Aerodrome, runway or taxiway shutdown)

After a privately registered, amateur-built Chandler PC14, on a flight from Kuujjuaq, QC (CYVP) departed from Runway 07, the flight service station (FSS) notified the aircraft that an object had dropped onto the runway. The aircraft landed to pick up the object. A ground inspection did not find any other objects. Impact on operations: a Canadian North aircraft (AKT166) was delayed on the ground for about eight minutes.

A vehicle convoy (ASI631) was authorized onto Taxiway J from the de-icing centre, but proceeded onto Taxiway K. A Skylink Express Inc. Beech 1900C (C-GSKU/SLQ738) from Montreal/Mirabel, QC (CYMX) to Quebec/Jean Lesage, QC (CYQB) was on Taxiway I, turning onto Taxiway K, and was instructed to hold short of the de-ice entrance. The vehicles were removed from Taxiway K and SLQ738 completed its taxi without incident.

The flight service station (FSS) requested that a Richcopter Inc. Cessna 150M (C-GUDB) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), on final approach, conduct a go-around. After returning to the ramp for an aircraft (A/C) inspection, C-GYGH proceeded onto Taxiway Alpha without contacting the FSS at 1932Z. No impact.

A privately registered, amateur-built Robinson Special from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) started backtracking on Runway 09 at the same time that an Acer Cold War Museum Aero L-29 Delfin (C-GGRI) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND) was also already backtracking. The Robinson Special did not wait for coordination to be performed with C-GGRI before going onto the runway. No impact on operations.

Incident at KUUJJUARAPIK QC (CYGW)

An Air Inuit de Havilland DHC-8-102 (AIE870) on a flight from Kuujjuarapik, QC (CYGW) reported that it had to return onto the apron; no reason was given. Clearance cancelled at 1850Z. The aircraft departed at 1900Z.

An Air Canada Airbus BD-500-1A11 (C-GUPG/ACA665) from Halifax/Stanfield, NS (CYHZ) to Montréal/Pierre-Elliott-Trudeau, QC (CYUL) blew a tire on landing. ACA665 ran off the end of the runway, without incident, onto 06R. An inspection was performed and nothing was found. Impact: 3 aircraft (TSC21, SYB203 and ACA401) were delayed for about 10 to 15 minutes due to the inspection. No other impact on operations.

The aircraft proceeded to taxi onto Taxiway Foxtrot and Taxiway Delta, towards a fuel truck on opposite direction. The aircraft was told to turn around and taxi via Taxiway Delta and Taxiway Alpha. The aircraft turned around but passed Taxiway Alpha without turning. No operational impact.

A privately registered, amateur-built Raab Thunderbird from Penticton, BC (CYYF) to Penticton, BC (CYYF) advised receiving a landing gear indication. The flight service station (FSS) advised the gear appeared to be down. The aircraft exited the runway on landing roll due to the aircraft issue. Pilot attempted to taxi back onto the runway to turn around and got stuck.

A truck (ST61) had authorization to proceed onto Runway 12/30 and Taxiway A, crossing Runway 03/21 until further advised, for an inspection, but was observed proceeding well onto Runway 03/21. The flight service station (FSS) contacted ST61, confirmed its intentions, and authorized the vehicle onto Runway 03/21 and Taxiway B, crossing Runway 12/30 until further advised. No aircraft were operating in the area. No impact on operations.

While requesting to proceed from Taxiway D onto Taxiway C, a vehicle (ESCORT1+1) was seen passed the hold short line at the threshold of Runway 22. No operational impact. Runway 14/22 was not in operation at the time.

Incident at CASTLEGAR / WEST KOOTENAY REGIONAL BC (CYCG) (Incursion - manoeuvring area)

At Castlegar/West Kootenay, BC (CYCG), a forestry maintenance vehicle went on a short portion of the Taxiway B between the forestry apron and the maintenance garage en route to a helicopter parked in the field, west of the service road. The operator of the truck phoned the flight service station (FSS). The truck has been advised and understands that they cannot proceed onto Taxiway B without an escort and were told which route to take.

A truck (Shell 487) pulled onto Taxiway D without calling ground control. The truck was then given permission for Taxiway D. No impact on operations.

A Coastal Pacific Aviation Cessna 172M (C-GIJB) from Abbotsford, BC (CYXX) to Abbotsford, BC (CYXX) failed to make a turn onto Taxiway Bravo and taxied through the grass to the apron. An aircraft on final was instructed to pull-up and a foreign object debris (FOD) inspection was requested.

An American, corporately registered Cessna 182P (N1456S) on a flight to Kelowna, BC (CYLW) had exited Runway 16 at Taxiway A and was instructed to taxi via Taxiway F to Shell. N1456S subsequently taxied onto A3, which had been "voice-closed" by the airport authority. When queried, the pilot indicated that they had been told by Shell to go to A3, as their ramp was full. No impact on operations.

Upon landing, a Canadian Aviation College Cessna 172L (C-FTWE) from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK) exited the runway surface, off to the side, and hit a taxi light before regaining control back onto the runway. The pilot exited onto the taxiway without further incident.
