Incident at KUUJJUARAPIK QC (CYGW) (Aerodrome - runway or taxiway surface condition)

While clearing the runway for the arrival of 2 aircraft at 1430Z, a vehicle (GR153) reported that there was an 8-inch gravel swath in the centre of the runway. The 2 aircraft decided to make standby circuits to allow the operators to remove the windrow. An Air Inuit de Havilland DHC-8-300 (AIE802), from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Kuujjuarapik, QC (CYGW) decided to fly directly to their alternate airport after 30 minutes in the holding pattern. An Air Inuit de Havilland DHC-8-300 (AIE880), from La Grande-Rivière, QC (CYGL) to Kuujjuarapik, QC (CYGW) conducted their approach after a 40 minute hold.

Incident at KANGIQSUALUJJUAQ (GEORGES RIVER) QC (CYLU)

Aviation Incident Report #17136: At 1448Z, an Air Inuit de Havilland DHC-6-300 (C-GKCJ/AIE681) on a cargo flight from Kuujjuaq, QC (CYVP) to Kangiqsualujjuaq (Georges River), QC (CYLU) called the community aerodrome radio station (CARS) on mandatory frequency (MF) 122.1. Initial contact was at 7NM to the west, low level estimated time of arrival (ETA) 3 minutes for Runway 16. All necessary numbers were passed on advisory. During its final approach, no final call was passed to the CARS office. AIE681 did not follow the 15NM radius call or make a final approach 16 call.

A Jazz Embraer ERJ 170-200 SU (JZA7623) from Boston, MA (KBOS) to Montreal/Pierre Elliott Trudeau, QC (CYUL) landed on Runway 24R without making radio contact with the tower. JZA7623 was on the wrong frequency. A 7903812 Canada Inc. Lancair LC41-550FG (C-GCHE) on a flight from Montreal/Pierre Elliott Trudeau, QC (CYUL), departing Runway 24L westbound, was delayed slightly.

Incident at SEPT-ÎLES QC (CYZV)

An American, privately registered Maule M-7-235 from Rivière-du-Loup, QC (CYRI) to Sept-Îles, QC (CYZV) landed on Runway 27, and when it tried to make a U-turn, the aircraft ended up completely outside the runway lights. The aircraft performed a few manoeuvres in the grass before rolling back onto the runway and exiting at Delta. Impact: delay to an inbound Exact Air inc. Beech A100 (ET828) from Chicoutimi/Saint-Honoré, QC (CYRC) to Sept-Îles, QC (CYZV), which had to extend downwind and conduct a 360° on final.

Incident at PUVIRNITUQ QC (CYPX) (Security acts, Incursion - runway - vehicle)

Aviation Incident Report #17049: At 0408Z at Puvirnituq, QC (CYPX), two people on an all-terrain vehicle (ATV) drove the full length of the runway at full speed. The runway edge lights were turned on at full intensity in an attempt to make the ATV realize CYPX was aware of their presence. At 0412Z, they were gone. There was no reported traffic at the time.

A Dorval Aviation Cessna 172L (C-FHNO) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Montreal/Pierre Elliott Trudeau, QC (CYUL) entered the Ottawa/Gatineau, QC (CYND) zone, performed a touch-and-go, and left the zone without any call on the mandatory frequency (MF). Several aircraft in the circuit had to make adjustments. No other impact on operations.

Incident at CHIBOUGAMAU / CHAPAIS QC (CYMT) (ATM - inaccurate aeronautical information, ATM - NAVAIDS/radar)

There was a loss of communication with an Air Creebec de Havilland DHC-8-314 (CRQ5641) from Val-dOr, QC (CYVO) to Chibougamau, QC (CYMT) at CYMT, and a loss of the radar source from Chibougamau (MT). Technical Operations Coordinator (TOC) advised. Air Creebec dispatch was called to make sure the aircraft was on the ground.

Incident at OTTAWA / GATINEAU QC (CYND)

After making an initial call on the ramp, a 2854-9699 Quebec Inc. Beech C-23 (C-FDXR) from Ottawa/Gatineau, QC (CYND) to Lachute, QC (CSE4) was told to standby due to other traffic. As the aircraft started taxiing towards Taxiway Alpha, several unsuccessful attempts were made by the flight service station (FSS) to contact the aircraft. C-FDXR then proceeded to take position and depart from Runway 27 from the intersection, while a Les Levés Aeroscan Inc. Piper PA-31 (SCANER4) from Ottawa/Gatineau, QC (CYND) to Ottawa/Gatineau, QC (CYND), which had just landed, was still on the runway.

Accident at CHICOUTIMI / ST-HONORÉ QC (CYRC)

At 1755Z, a College denseignement general et professionnel de Chicoutimi Beech C23 (C-GLYC) encountered issues on its takeoff run at Chicoutimi/St-Honore, QC (CYRC). While taking off from Runway 30, the aircraft was observed losing control, making a sharp right turn and exiting the runway. The aircraft then crashed into the infield. The three persons aboard the aircraft were uninjured, however, the aircraft suffered damage. The extent of the damage is unknown. Emergency services were dispatched to the scene. The National Operations Centre (NOC), the Aviation Operations Centre (AVOPS) and the Transportation Safety Board of Canada (TSB) were notified. An American, corporately registered Cessna 560 Citation Ultra (N5686/DCM6247) from Montreal/St-Hubert, QC (CYHU) to Chibougamau/Chapais, QC (CYMT) did not make initial contact or broadcast on the mandatory frequency (MF) during its approach to CYMT. The aircraft responded after two attempts by the Airport Advisory Service (AAS), and the request for the aircraft to report when off of the runway was given. There was no further communication with the aircraft. The arrival was confirmed by the ground station.

The dial-up remote communications outlet (DRCO) of Matagami, QC (CYNM) did not activate, even after several attempts. The Montreal Area Control Centre (CZUL) and the Technology Operations Coordinator (TOC) were notified and requested that a NOTAM be issued until March 19th in order to have time to make a diagnosis. C-GWZB was instructed to make a 360 degree turn to avoid traffic departing from Runway 19. Traffic was passed to the pilot of a Skyquest Aviation Ltd. Cessna 152 (C-GTYI) from Langley, BC (CYNJ) to Langley, BC (CYNJ), who reported traffic in sight. The pilot was issued vectors to depart the control zone to the north.

An American privately registered, amateur-built Vans RV-6A on a transborder flight from Dietz Airpark, OR (KOR40) to Nanaimo, BC (CYCD) entered the CYCD control zone without establishing contact until on the downwind for Runway 16. Upon establishing contact, the pilot advised the point of departure was KOR40 and no flight plan was filed. The pilot advised making numerous calls believed to be on the wrong frequency.

Incident at NANAIMO AIRPORT

The Nanaimo, BC (CYCD) designated runway was Runway 34. At 2055Z, 2 VFR aircraft, a British Columbia Helicopters Robinson R66 (C-GEAK) from Nanaimo, BC (CYCD) to Campbell River, BC (CYBL) and a Victoria Flying Club Cessna 172S (C-GXTZ) from Nanaimo, BC (CYCD) to Nanaimo, BC (CYCD), were given departure advisories for Runway 34. At 2059Z, C-GEAK reported ready to depart from Runway 34 at the same time that a Jazz Bombardier DHC-8-402 (JZA8257) from Vancouver, BC (CYVR) to Nanaimo, BC (CYCD) was observed on radar 5NM East of the CYCD non-directional beacon (NDB) and tracking towards the NDB with no radio contact or coordination from the Victoria terminal. The flight service station (FSS) asked C-GEAK and C-GXTZ to delay departure due to JZA8257 being inbound for an unknown runway without establishing contact. JZA8257 established contact at 2100Z, advising it was on a visual approach for Runway 16. JZA8257 would have been on a direct head-to-head course with the departing VFR traffic had the FSS not requested the VFR traffic to wait. JZA8257 landed at 2103Z, only 3 minutes after making initial contact. C-GEAK and C-GXTZ were delayed due to the uncoordinated opposite runway traffic (IFR) and the resulting wake turbulence.

A privately registered Piper PA-28-151 from Langley, BC (CYNJ) to Langley, BC (CYNJ) crossed the hold short line on Taxiway B with a privately registered, amateur-built Rand KR2S backtracking on Runway 19 for departure, and a privately registered, amateur-built Vans RV9A on 2 miles final for Runway 25. The Rand KR2S aircrafts take-off clearance was cancelled and the Vans RV9A aircraft was instructed to make a 360 degree turn for spacing. Minor impact to operations.

Incident at CAMPBELL RIVER BC (CYBL)

A Pacific Professional Flight Centre Piper PA-28-151 (C-GDVP) from Campbell River, BC (CYBL) to Vancouver/Boundary Bay, BC (CZBB) departed from Runway 30 and was asked by the flight service station (FSS) to continue its climb out prior to making a right turn eastbound, in order to avoid the jump zone. The aircraft was observed drifting north of the extended centreline, but west of the jump zone. It was later reported by the jump zone ground control that the aircraft flew between the runway and parachute landing area on take off.

Accident at KAMLOOPS BC (CYKA)

An American, privately registered Piper PA-12 crashed upon landing at Kamloops, BC (CYKA). The aircraft was disabled on Runway 09/27. There was 1 soul on board (SOB) and there were no injuries. NAV CANADA, the Federal Aviation Authority (FAA), the Transportation Safety Board of Canada (TSB) and the Royal Canadian Mounted Police (RCMP) were notified. UPDATE: TSB Report #A21P0036: A privately registered Piper PA-12 equipped with amphibious floats, with 1 pilot on board, was conducting a VFR flight from Goldendale, WA (S20) to Merrill Field, AK (PAMR). After completing the leg from Abbotsford, BC (CYXX) to Kamloops, BC (CYKA), the aircraft bounced upon initial contact with the runway surface during the landing. During the second runway contact, as the aircraft was settling, the aircraft began an airframe shimmy, which resulted in a loss of control. The aircraft veered off the centreline and towards the runway edge. As the aircraft approached the grass, the RH main landing gear folded under the fuselage and the aircraft rotated 180 degrees, with the RH wing making contact with the ground. The aircraft came to rest in the grass alongside the runway on its belly. The pilot egressed the aircraft with minor injuries. The aircraft was substantially damaged.

The make and model of the navigation system was a Garmin GNS 400. The issue resolved itself before the aircraft landed.

A Tasman Helicopters Ltd. Bell 212 (C-FVTS) from Vancouver/Boundary Bay, BC (CZBB) to Manning, AB (CFX4) entered the Williams Lake, BC (CYWL) control zone (CZ) without making contact with the CYWL flight service station (FSS) and landed at the fuel pumps. The FSS was able to establish contact with the pilot on 126.7 MHz once the aircraft was visually confirmed at the pumps.

The make and model of the global navigation satellite system (GNSS) was a Garmin GNS 400. Issue was resolved before landing.

An American, privately registered Cessna 172D from Grande Prairie, AB (CYQU) to Fort Nelson, BC (CYYE) landed on Runway 26 without prior radio contact. Impact to operations: departing aircraft from the Runway 08 infield (north of Runway 08) had to make a very early turn out to avoid the arriving aircraft.

An American, privately registered Champion 7EC from Fort Nelson, BC (CYYE) to Whitehorse/Erik Nielsen, YT (CYXY) taxied and entered the taxiway without making radio contact. The flight service station (FSS) had to attempt multiple radio calls in order to hail the pilot. Eventually, contact with the pilot was established. No impact to operations.

C-GTYB was instructed to make an early turn westbound as a Government Of Canada, Department Of National Defence Sikorsky S-92A (STGR24) from Victoria, BC (CYYJ) to Victoria, BC (CYYJ) was on approach for Runway 07 and had to execute an early missed approach.

Incident at QUESNEL BC (CYQZ)

A privately registered Malcolm Merlin GT from Quesnel, BC (CYQZ) to Quesnel, BC (CYQZ) made initial contact with the flight service station (FSS) to advise of its intentions of conducting 2 circuits. The FSS provided the remote aerodrome advisory service (RAAS) and instructed the aircraft to report once it was on the take-off roll. Approximately 10 minutes later, the aircraft reported taxiing clear of the runway. The aircraft then informed the FSS that it had completed both circuits without making any reports to the FSS.

An American, corporately registered Robinson R44 II (N442PW), travelling from Washington State to Ketchikan, landed at Nanaimo, BC (CYCD) without a transborder flight plan (FP). After making a second stop at Campbell River, BC (CYBL), the pilot contacted Pacific Radio to inquire about the FP. The Ketchikan flight service station (FSS) had no plan. The pilot had reportedly air-filed a FP. Leidos had no FP either.

A Langley Flying School Inc. Cessna 152 (C-GPUK) from Langley, BC (CYNJ) to Langley, BC (CYNJ), in the left downwind for Runway 19, was instructed to make a right-hand 360 turn. The aircraft acknowledged the clearance but did not comply, and conflicted with a privately registered Cessna 170B from Langley, BC (CYNJ) to Langley, BC (CYNJ) on the left base over Highway 01. C-GPUK was then instructed to make a 90 degree right-hand turn in order to avoid the Cessna 170B.

The aircraft was then observed taxiing in on Taxiway Delta without making contact with ground control. No impact on traffic.

Incident at VANCOUVER / BOUNDARY BAY BC (CZBB)

An Insignia College Ltd. Cessna 172K (C-GDON) from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) departed from Runway 07 and declared "MAYDAY" due to "control issues" through 400 feet. The aircraft was given the option to land on any runway. C-GDON landed on Runway 25 without issue and exited via Taxiway G. The aircraft advised the ground controller that it had experienced a "flaps issue".
UPDATE: TSB Report #A21P0017: C-GDON, a Cessna 172K aircraft operated by Insignia College, was on the first take-off for circuits at Vancouver/Boundary Bay, BC (CZBB) with one student pilot on board. The student took off from Runway 07 and, during the climb, realized the aircraft airspeed was decreasing. As the aircraft was nearing a stall at 300 feet AGL, the pilot regained control by lowering the nose to keep the speed up. The pilot declared a "MAYDAY" and ATC gave the pilot the option of "any runway". Other traffic was cleared to allow C-GDON runway access. The pilot managed to make a 180 degree turn to land on Runway 25 without further issues. After landing, the pilot realized they had set "full flaps" and did not set them for take-off prior to departure, as required in the pre-flight checklist. Ground taxi clearances were obtained and the aircraft taxied back to the company facility.

The aircraft continued to fly towards Poppy, setting up for the right base. The aircraft was instructed to fly direct west, and then was later instructed to start its downwind turn. The aircraft continued flying west to join base, cutting off the traffic it was supposed to follow on final for Runway 01. The aircraft was instructed to make an immediate southbound turn.

Incident at LANGLEY REGIONAL BC (CYNJ)

A privately registered Piper PA-28-235 from Langley, BC (CYNJ) to Vancouver/Boundary Bay, BC (CZBB), cleared outbound via the Cloverdale departure, flew 2.5NM northwest (NW) of the airport and climbed to 1500ft before making a left turn towards Cloverdale. No operational impact.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

Aviation Incident Report #17237: A passenger on a WestJet Boeing 737-8CT (C-FCSX) on a flight to Toronto/Lester B. Pearson, ON (CYYZ) was acting strange during the flight. They were complaining about wearing a mask and trying to take it off, and they were making the passengers around them uncomfortable. The passenger attempted to purchase items for another passenger that was not sitting with them. The Captain requested police meet the aircraft at CYYZ. Police escorted the passenger out of the airport.
