Incident at Adolfo Suarez Madrid / Barajas int'l (LEMD) (FOD (foreign object debris))

TSB Report #A20F0021: C-GHOZ, a Boeing 767-300 operated by Air Canada, was conducting flight ACA837 from Madrid / Adolfo Suárez Madrid-Barajas Intl (LEMD), Spain to Toronto / Lester B. Pearson Intl (CYYZ), ON with 8 crew members and 130 passengers onboard. During takeoff, rubber from the number 5 main landing gear tire detached. Some debris was found on the runway and others were ingested by the left engine. As a precaution, the flight crew performed an in-flight shut down of the left engine. After burning off fuel to avoid an overweight landing, the aircraft returned to LEMD for an uneventful landing. There were no injuries to the occupants. Preliminary inspection of the aircraft shows damage to the engine and left main landing gear. The Spanish CIAIAC is investigating [Ref: A-006/2020]

Incident at Enroute Madrid (LEMD)

TSB #A19F0202: C-FNOI, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight ACA836 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Madrid/Adolfo Suárez Madrid-Barajas Intl (LEMD), Spain with 10 crew members and 251 passengers on board. After departure from CYYZ, the flight crew received an EICAS message indicating low tire pressure on wheel no 6. After consultation with maintenance, the flight continued to LEMD, where a Pan Pan was declared for the deflated tire. The flight landed uneventfully. There were no injuries or damage to the aircraft. The no 6 tire was found to be punctured and replaced.

Pearson Int'l, ON (CYYZ) landed Runway 33L and was instructed to taxi on Runway 06L, hold short E. The readback was correct. GGN7265 continued to taxi beyond E and exited the runway protected area south of Runway 33R as an Air Canada Boeing 787-9 (C-FVNB/ACA837) from Adolfo Suarez Madrid-Barajas Airport (LEMD) to Toronto/ Lester B. Pearson, ON (CYYZ) was approximately 1 nm final for Runway 33R. No operational impact.

A KLM Royal Dutch Airlines Boeing 787-9 (KLM31) from Amsterdam Airport Schiphol, Netherlands (EHAM) to Toronto/Pearson International, ON (CYYZ) was descending quickly through 12,900 feet for 11,000 feet when the aircraft reported a traffic alert and collision avoidance system resolution advisory (TCAS-RA) when approximately 0.5 nautical miles beside and 3,600 feet above an Air Canada Boeing 787-9 (C-FSRI/ACA837) from Adolfo Suárez Madrid-Barajas, Spain (LEMD) to Toronto/Pearson International, ON (CYYZ) that was on descent through 9,300 feet to 8,000 feet. ACA837 also reported a traffic alert and collision avoidance system resolution advisory (TCAS-RA). No operational impact.

TSB Report#A18F0062: C-GHKR, an Airbus 330-343 aircraft operated by Air Canada was conducting flight ACA836 from Toronto/Lester B. Pearson Intl., ON (CYYZ) to Madrid/Adolfo Suárez Madrid-Barajas Intl., Spain (LEMD) with 10 crew members and 214 passengers on board. During cruise, approximately half way through the flight, the yellow hydraulic pressure indication went to zero. The flight crew contacted the operator's maintenance, and continued to destination. The hydraulic system fault resulted in a reduced flap flow rate and inoperative RH thrust reverse. The flight landed without incident in LEMD. The operator's maintenance found the yellow hydraulic pressure manifold supply check valve leaking due to a worn seal. The check valve was reinstalled with new seals.

Incident at JOOPY INTERSECTION

UPDATE: TSB Report#A17A0071: EC-JYT, a Bombardier CL-600-2B16 Challenger 604 aircraft operated by Global Jet, was conducting a flight from Teterboro, NJ (KTEB) to Madrid/Adolfo Suárez Madrid-Barajas Intl, Spain (LEMD) with 3 crew members and 2 passengers on board. Approaching waypoint JOOPY (4830N/52W) during cruise flight, the flight crew received a hydraulic number one high temperature caution indication. The QRH was completed and the number 1 engine was shut down. The flight crew declared an emergency and requested a diversion to Gander Intl, NL (CYQX) where the aircraft landed without further incident. Maintenance personnel located a broken hydraulic line, which drained the system and created a system overheat. The line was replaced and the aircraft was returned to service.
A TAG Aviation Espana Bombardier CL-600-2B16 (604) (ECJYT) from Teterboro, NJ (KTEB) to Madrid?Barajas, Spain (LEMD) reported hydraulic problems. The aircraft diverted to Gander, NL (CYQX). The aircraft landed without incident at 1522Z.

At 0411Z, a 45N040W position report was received for an American Airlines Boeing 777 200 (N776AN/AAL68) from Miami, FL (KMIA) to Madrid, Spain (LEMD). The flight had progressed 45N040W at 0352 proceeding to 45N030W. The flight`s previous position was 42N050W. Gander did not receive an estimate from New York for this flight. There was no conflicting traffic for this flight so a clearance was issued based on flight plan route and a verbal estimate was passed to LPAZ when ICAO controller had all the necessary details.

At 1525Z, a Lineas Aereas de Espana Airbus A340-600 (ECJCZ/IBE6275) on a flight from Madrid, Spain (LEMD) to Chicago/O'Hare, IL (KORD) entered Gander's airspace over BOBTU without an estimate from New York (NY). At the same time, a United Airlines Boeing 767-400 (N77066/UAL43) on a flight from Rome, Italy (LIRF) to Washington/Dulles, DC (KIAD) arrived over BOBTU at a flight level different than passed by NY. There was no loss of separation and no other impact to operations.

Clearance was re-issued and the flight proceeded as cleared.

An American Airlines Airbus A330-200 (AAL741) from Madrid, Spain (LEMD) to Philadelphia, PA (KPHL) was given DCT ACK upon check-in and replied that ACK was not on their flight plan. When queried, AAL741 advised that the filed routing was SUPRY TUSKY LFV J174 HTO J121 SARDI V139 BRIGS JIIMS2 KPHL. This differed from the routing that was in the system from Gander (QX) which was SUPRY N110B WHALE ACK J62 SHLEP J121 SARDI V139 MANTA V139 BRIGS JIIMS2 KPHL. The aircraft was only slightly north of profile at any time. No impact on the operation.

UPDATE TSB #A16A0105: N662UA, a Boeing B767-322 aircraft operated by United Airlines, was conducting flight UAL51 from Newark/Liberty Intl, NJ (KEWR) to Adolfo Suárez Madrid-Barajas Intl, Spain (LEMD). While in cruise flight at FL340, the first officer's windshield cracked. The crew declared an emergency and requested a return back to KEWR where the aircraft landed without further incident. The operator?s maintenance replaced the windshield and returned the aircraft to service.
A United Airlines Boeing 767 300 (UAL51) from Newark, NJ (KEWR) to Madrid, Spain (LEMD) declared an emergency due to a cracked windshield. The aircraft requested clearance back to KEWR and descent to FL260 initially. UAL51 was cleared direct KEWR and subsequently requested descent to FL100 but decided to level off at FL240. No assistance was required. No impact to operations. UAL51 departed the flight information region (FIR) at 0216Z.

Flight was queried but still confirmed the wrong coordinate at 50W. Shanwick advised the flight had received the new electronic routing clearance at 1211Z and read back the clearance at 1212Z. AAL37 said they did not receive the clearance. Flight was re-cleared to correct routing at 1507Z.
