Pearson, ON (CYYZ) heard an emergency locator transmitter (ELT) at 1730Z at FL200 over Orillia/Matchedash Lake, ON (CPU5). A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3409) from Quebec/Jean Lesage, QC (CYQB) to Toronto/Lester B. Pearson, ON (CYYZ) heard the ELT at the same time at FL220 over Arnprior, ON (CNP3). The Joint Rescue Coordination Centre (JRCC) Trenton confirmed that an accidental ELT activation occurred at Ottawa/Gatineau, QC (CYND). On final to Kuujjuarapik, QC (CYGW), an Air Tunilik Inc. de Havilland DHC-3 (C-FSVP) notified the flight service station (FSS) that it needed to do a check. A few minutes later, the pilot indicated that there were problems with the skis, which were lowered. C-FSVP decided to divert to La Grande Rivière, QC (CYGL). It landed on Desaulniers Lake at 2136Z without incident.

An American, corporately registered Cirrus SR22T (N561DJ) from Saranac Lake/Adirondack, NY (KSLK) to Montreal/Pierre-Elliott-Trudeau, QC (CYUL) reported brake and hydraulic issues. Only an Air Canada Airbus A330 returning from a run-up needed to be towed, because access to Taxiway AC proved impossible. No further impact on operations.

The Montreal/Mirabel, QC (CYMX) tower heard an intermittent emergency locator transmitter (ELT) signal. The signal was also heard by a Royal Leasing Company (2007) Ltd. Eurocopter EC120B (C-GRML). The Eurocopter heard it intermittently when it was 9NM northwest of Achigan Lake. At 2047Z, when it was approximately 2NM north of CYMX at an altitude of 4,000 ft, C-GRML heard an intermittent signal. The Joint Rescue Coordination Centre (JRCC) Trenton was notified. No operational impact.

A Chrono Aviation Inc. Pilatus PC-12 (NDL543) from Pickle Lake, ON (CYPL) to Quebec/Jean-Lesage, QC (CYQB) was instructed to conduct a pull-up for Runway 24 because a Dorval Aviation Inc. Cirrus SR20 (C-GVEO) was still on the runway and did not have time to exit before NDL543 crossed the threshold.

A Delta Air Lines Airbus A319-114 (N333NB/DAL1156) from Salt Lake City, UT (KSLC) to Vancouver, BC (CYVR) conducted a missed approach for Runway 26L. Audio reviewed.

An American, privately registered Cessna 182M (N71076) on a flight plan from Whitehorse/Erik Nielsen, YT (CYXY) to Watson Lake, YT (CYQH) to Fort Nelson, BC (CYYE). The aircraft became overdue as of 0156Z. Checklist was followed, and the aircraft updated its estimated time of arrival (ETA) to 0230Z. No impact on operations.

A formation flight of 2 Vans aircraft, a privately registered, amateur-built Vans RV-10 from Kelowna, BC (CYLW) to Williams Lake, BC (CYWL) and a privately registered, amateur-built Vans RV-7A from Kelowna, BC (CYLW) to Williams Lake, BC (CYWL), turned tight base and in between two other aircraft, a SGD Consulting Inc. Piper PA-28-140 (C-FVFM) from Kelowna, BC (CYLW) to Williams Lake, BC (CYWL), and a Pacific Coastal Airlines Ltd. Beech 1900D (C-FMCN) from Vancouver, BC (CYVR) to Williams Lake, BC (CYWL), that were on final for Runway 30. As a result, the Vans RV-10 aircraft landed while another aircraft was still on the rollout. The Vans RV-7A then broke off the approach and conducted a go-around due to the traffic on the runway.

A 2309543 Alberta Ltd. Beech 58P (C-FOBA) from Kelowna, BC (CYLW) to Pemberton, BC (CYPS) departed for Pemberton and cleared the zone westbound, over the bridge, at around 2600 feet. The aircraft descended below the zone and went north over Okanagan Lake. C-FOBA then continued north over the lake, climbed to 4000ft into the zone and never called. The tower reached out on 119.6 MHz and tried the en route frequency, but could not get a response from either. Near Okanagan centre, the flight turned westbound and left the zone. No operational impact.

A 2309543 Alberta Ltd. Beech 58P (C-FOBA) on a flight to Kelowna, BC (CYLW) was instructed to fly the Westbank Arrival and indicated to the tower that they were not familiar with the published procedures. C-FOBA was then instructed to fly northbound over Okanagan Lake. Abeam midfield, the aircraft turned southbound without authorization.

An American, corporately registered Lake LA-4 200 (N53JN) from Dawson Creek, BC (CYDQ) to Fort Nelson, BC (CYYE) and an American, corporately registered Aerofab Lake LA 250 (N193TM) from Dawson Creek, BC (CYDQ) to Fort Nelson, BC (CYYE) landed on Runway 22 using the wrong frequencies. Both aircraft used 126.7 MHz instead of 122.5 MHz. No additional impact to operations.

An American, corporately registered Airbus Helicopters AS 350 B3 (N353JR) from Abbotsford, BC (CYXX) to Williams Lake, BC (CYWL) established two-way communication on one mile final to landing at the airport.

A privately registered Lake LA-4-200 from Victoria, BC (CYYJ) to Victoria, BC (CYYJ) reported a rough-running engine on approach to Runway 09. No emergency was declared. The aircraft rescue and fire fighting (ARFF) unit was called out on stand-by. The aircraft landed safely on Runway 09. No impact on operations.

A Southern Interior Flight Centre Cessna 172S (C-GICG) from Kelowna, BC (CYLW) to Kelowna, BC (CYLW), departing the zone, reported a blue laser strike from a group of homes in the southwest (SW) corner of Wood Lake, caused temporary blindness. Shift Manager (SM) was advised and a laser strike reporting form was completed.

An Airsprint Inc. Cessna 525B (ASP567) from Port Hardy, BC (CYZT) to Kelowna, BC (CYLW) reported nearly hitting a large balloon inside the control zone while on a visual approach and crossing the west shore of Okanagan lake. No operational impact.

At 1453Z, a Decosse Customs Ltd. Mooney M20B (C-FGUS) on a flight from Cranbrook/Canadian Rockies, BC (CYXC) reported engine trouble and advised they were returning. Aircraft was unable to make it to the airport. Aircraft headed to an old drag strip at Wasa Lake. The aircraft landed safely and reported with the flight service station (FSS). No assistance required. The Vancouver (VR) Shift Manager advised the Transportation Safety Board of Canada (TSB).

A Nanaimo Flying Club Cessna 172I (C-GVBA) from Nanaimo, BC (CYCD) to Victoria, BC (CYYJ) transited Class C extension airspace from Cowichan Bay to Sooke Lake at 2400 ft with no air traffic control (ATC) contact.

A Swoop Inc. Boeing 737-8CT (C-FYBK/WSW213) from Edmonton, AB (CYEG) to Kelowna, BC (CYLW), inbound (IB), cancelled its IFR clearance and was cleared straight-in as number 1. A Skylink Express Inc. Beech 1900C (C-FJXO/SLQ750) from Vancouver, BC (CYVR) to Kelowna, BC (CYLW), inbound, cancelled its IFR clearance and entered the zone over Okanagan Lake while not on the frequency. SLQ750 headed directly for a right base tied to final with WSW213 and SLQ750 was not talking to the Kelowna tower. When they switched over, the tower turned SLQ750 downwind to follow, and WSW213 had SLQ750 in sight.

An American, privately registered Piper PA-28R-180 from Watson Lake, YT (CYQH) to Smithers, ON (CYYD) experienced radio failure while en-route to CYYD. The aircraft arrived "no radio" (NORDO) at CYYD.

The pilot of Central Mountain Air Ltd. de Havilland DHC-8-102 (C-GYSJ/GLR8216) from Williams Lake, BC (CYWL) to Prince George, BC (CYXS) reported a bird strike on departure. The staff vehicle retrieved the bird. No impact to operations.

Aviation Incident Report #17527: A group of skiers were waiting for a Blackcomb Helicopters Aerospatiale AS350 B3 (C-GZBA) to pick them up when they were approached by snowmobilers that were disputing their ability to use the area. The snowmobilers did not behave appropriately around C-GZBA, which was trying to land. Operational impact: C-GZBA had to land over 150 feet away.

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-GJWE/WEN3287) from Vancouver, BC (CYVR) to Prince George, BC (CYXS) was descending out of FL200 and encountered a laser strike originating from the south end of Quesnel. Police advised.

A VFR target later identified as a Government of Canada, Department of National Defence McDonnell Douglas CF-18 Hornet (ODIN21) from Cold Lake, AB (CYOD) to Comox, BC (CYQQ) passed through the control zone south of Tofino/Long Beach, BC (CYAZ) at 005 and 360 Knots. Aircraft did not call on the mandatory frequency (MF) and did not respond to the flight service station (FSS) attempts to contact.

A Central Mountain Air Ltd. de Havilland DHC-8-102 (C-GYSJ/GLR8016) from Smithers, BC (CYYD) to Dease Lake, BC (CYDL) declared an emergency and requested to return to CYYD. GLR8016 reported smoke in the cabin and requested emergency crews on site. Landed safely at 1753Z, without incident.

The Vancouver (VR) Area Control Centre (ACC) Shift Manager received a call from Maple Ridge Emergency Services regarding an aircraft accident in Alouette Lake, BC. No emergency locator transmitter (ELT) was heard and radar playback did not have adequate coverage to capture the event. Emergency services arrived on site and supported with a helicopter (AIR5). The aircraft was later identified as a privately registered, amateur-built Vans RV-4 from Pitt Meadows, BC (CYPK) to Pitt Meadows, BC (CYPK), and the 2 souls on board were retrieved alive. No operational impact. Joint Rescue Coordination Centre (JRCC) Victoria, National Operations Centre (NOC), Transportation Safety Board of Canada (TSB), and Transport Canada (TC) advised.

An American, corporately registered Cub Crafters CC11-160 (N189PD) from Fort Nelson, BC (CYYE) to Watson Lake, YT (CYQH) was observed in a high speed taxi on Apron 1 towards Runway 22. The tail dragger was moving fast enough to lift the tail. The flight service station (FSS) made multiple attempts to contact the aircraft. The aircraft entered Taxiway Alpha and stopped at the runway hold short line before responding to the FSS. The aircraft departed without incident.

A Penticton Flying Club-COPA Flight 50 Cessna 172L (C-GBFB) from Penticton, BC (CYYF) to Kelowna, BC (CYLW) was inbound from the southwest (SW) and was told to fly northbound over the eastern shore of Okanagan Lake. The tower cleared the aircraft for the left downwind for Runway 34, however, C-GBFB read back "right downwind". Tower said "negative", cleared the aircraft for the left downwind, and the readback was correct. C-GBFB crossed the midfield and joined the right downwind.

A privately registered Piper PA-28-151 on a flight to Williams Lake, BC (CYWL) exited Runway 12/30 onto Taxiway B, which does not have winter maintenance. A vehicle (ST21) assisted the aircraft to get back onto Runway 12/30. The aircraft continued on their flight plan after a delay of approximately 15 minutes. No other impact to operations.

An American, privately registered Cessna 180C from Golden, BC (CYGE) to Watson Lake, YT (CYQH) reported "MAYDAY" on 121.5 MHz. The call was heard by a passing high flyer, who passed the information to the Edmonton Centre. The Cessna 180C reported fuel exhaustion over Birches Lake (approximately 70NM southeast of CYQH) while en route to CYQH, and made a successful emergency landing on Birches Lake. There were 2 persons on board who had camping supplies, but were in need of assistance. No injuries. No damage. This information was passed to the Edmontn Centre and, subsequently, to the Joint Rescue Coordination Centre (JRCC) Victoria. The JRCC advised it will coordinate assistance. Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: The Prince George, BC (CYXS) tower issued a clearance to a Northern Thunderbird Air Inc. aircraft (NTA201) on a MEDEVAC flight to Prince George, BC (CYXS) 19 minutes prior to the tower closing. The tower closed 8 minutes later and the Williams Lake, BC (YWL) Flight Service Station (FSS) assumed the remote aerodrome advisory service (RAAS). At 0650Z, the Area Control Centre (ACC) issued an approach clearance to a WestJet Encore aircraft (WEN), unaware that NTA201 had an outstanding departure clearance. After the tower closed, there was no guarantee that NTA201 would not depart on their own.

The pilot of a privately registered Lake LA-4 from Abbotsford, BC (CYXX) to Abbotsford, BC (CYXX) reported losing their fuel cap on the runway. The aircraft taxied back to the apron. No operational impact.

An Arduini Helicopters Ltd. Bell 206B (C-GIWB) from Williams Lake, BC (CYWL) to Williams Lake, BC (CYWL) entered the Quesnel, BC (CYQZ) control zone (CZ) prior to making contact. The aircraft first made contact and reported they were 3.7NM from the airport.

A specialist observed the door panel fall off a Government Of Canada, Department Of National Defence Boeing CH147 (VALKY42) from CFB Cold Lake, AB (CYOD) to Fort Nelson, BC (CYYE) just before touch down. After landing and exiting the runway, the aircraft sent crew members to retrieve it. No further impact to operations.

Aviation Incident Report #17495: An individual reported observing an aircraft flying south at an altitude of approximately 20 feet above the frozen Tuc-el-nuit Lake in Oliver, BC. The lake is surrounded by residences, and the aircraft was approximately 300 to 400 feet from one of them.

An NDK Air Ltd. de Havilland DHC-3 (C-FYLZ) on a flight to Pickle Lake, ON (Water) (CKG4) reported final for CKG4 at 1534Z and never reported on the water. At 1656Z, C-FYLZ was inbound from the North to land at CKG4 with no radio contact on the mandatory frequency (MF) due to radio issues. A de Havilland DHC-8-100 arrived on Runway 27 at 1659Z, but did not report seeing C-FYLZ.

An Ornge Global Air Inc. Pilatus PC-12/47E (PUL102) from Sioux Lookout, ON (CYXL) to Poplar Hill, ON (CPV7), en route to CPV7, reported an issue with their deicing equipment and requested to return to CYXL and asked to descend from 16000 feet to 7000 feet. The aircraft landed without incident or impact to operations at 2010Z. The Aircraft Rescue and Fire Fighting (ARFF) unit was not requested or required.

A Superior Airways Limited Cessna 208 (C-GTBY), on a flight from Red Lake (Howey Bay) (water), ON (CKS4), departed from CKS4 without an advisory from Kenora (YQK) radio.

A Wasaya Airways de Havilland DHC-8-102 (C-GOKN/WSG749) from Angling Lake/Wapekeka, ON (CKB6) to Pickle Lake, ON (CYPL) reported a traffic alert and collision avoidance system (TCAS) traffic advisory in reference to an Air Tindi Ltd. de Havilland DHC-7-103 (C-GUAT/TIN805) from Pickle Lake, ON (CYPL) to Big Trout Lake, ON (CYTL).

An American, privately registered Stinson 108-2 on a transborder flight from Grand Rapids/Itasca County, MN (KGPZ) to Fort Frances, ON (Water) (CJM8), with an estimated time of arrival (ETA) of 1430Z, went overdue at 1530Z. Information was forwarded to the Joint Rescue Coordination Centre (JRCC) Trenton. The pilot called and reported down and safe on Perch Lake at 1740Z. The flight plan (FP) was closed.

A Waterloo-Wellington Flying Club Cessna 172S (C-GOIQ) from Kitchener/Waterloo, ON (CYKF) to Kitchener/Waterloo, ON (CYKF) was cleared for the right base for Runway 26 but did not turn final, which put the aircraft into conflict with the downwind traffic, a Waterloo-Wellington Flying Club Cessna 172S (C-GLXP) from Kitchener/Waterloo, ON (CYKF) to Kitchener/Waterloo, ON (CYKF). The downwind traffic climbed to avoid the aircraft. Then C-GOIQ turned back towards the traffic on final, a Wellmark Electric Inc Lake LA-4 from Kitchener/Waterloo, ON (CYKF) to Kitchener/Waterloo, ON (CYKF), which required air traffic control (ATC) to move the aircraft on final away from the offending aircraft.

Osnaburgh Airways called the Flight Service Station (FSS) to report the arrival of an Osnaburgh Airways Limited de Havilland DHC-3 (C-FCZO) on a flight to Pickle Lake, ON (Water) (CKG4). The aircraft was unable to communicate while inbound due to radio issues. No known impact to operations.

An Osnaburgh Airways de Havilland DHC-2 MK. I (C-GMAU) from Pickle Lake, ON (water) (CKG4) to Pickle Lake, ON (water) (CKG4) called when final at 1447Z and was asked to report down. The aircraft did not report down. Arrival was verified with the company via landline.

A Wasaya Airways Pilatus PC-12/45 (C-FWAQ/WSG724) from Sioux Lookout, ON (CYXL) to Wunnumin Lake, ON (CKL3) reported a minor electrical issue and advised returning to CYXL. No assistance required. No impact to operations.

An Excellent Adventures Outposts & Air Service Ltd. Cessna 208B (C-FHWA) from Thunder Bay, ON (CYQT) to Red Lake, ON (CYRL) was disabled on Taxiway A. No access to the main apron/terminal area.
