Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

An Air Algérie Airbus A330-202 (7TVJC/DAH2700) from Algiers, Algeria (DAAG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was instructed to conduct a go-around on Runway 06R because a Sky Regional Airlines Embraer ERJ 170-200 SU (C-FEIQ/SKV7742) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Newark, NJ (KEWR) had lined up on the runway while a Chartright Air Bombardier BD-100-1A10 (C-FXGO/HRT242) from Toronto/Lester B. Pearson, ON (CYYZ) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was unable to exit via Taxiway L, which was blocked by a Government of Quebec Canadair CL600-2B16 (C-GQBQ/QUE20) on a MEDEVAC flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Quebec/Jean Lesage, QC (CYQB). No other impact on operations.

Incident at 20NM QUÉBEC / JEAN LESAGE INTL QC (CYQB)

A Commutair/United Express Embraer EMB-145XR (UCA4986) from Quebec/Jean Lesage, QC (CYQB) to Newark, NJ (KEWR) declared an emergency due to the presence of smoke in the aircraft and requested a return to CYQB. The aircraft landed on Runway 06 without incident at 1145Z. The runway inspection was completed at 1150Z. No impact on operations.
UPDATE from TSB report #A20Q0031: N16183, an Embraer EMB-145XR operated by CommutAir under flight number UCA4986, was flying under instrument flight rules from the Quebec/Jean Lesage, QC (CYQB) airport, bound for Newark (KEWR) in the United States, with 41 passengers and 3 crew members. During the climb to approximately 10,000 feet, the crew noticed the presence of fumes in the cockpit. The crew subsequently put on their oxygen mask, declared a state of emergency and requested a return to CYQB. The aircraft touched down on Runway 06 without incident with the aircraft rescue and firefighting (ARFF) services were on standby. There is had no injuries or damage. Inspection of the aircraft determined that a left engine oil leak (#1) was the source of the problem. A convoy flight to Albany (KALB), NY, will be made in order to perform a more thorough inspection.

A United Airlines Embraer EMB-145XR (N14143/UCA4966) from Quebec/Jean Lesage, QC (CYQB) to Newark, NJ (KEWR) declared an emergency due to a landing gear problem. Emergency vehicles were deployed. The aircraft landed without incident at 2138Z. No impact on operations.
UPDATE from TSB report A20Q0030: N14143, an Embraer EMB-145XR operated by CommutAir under flight number UCA4966, was flying under instrument flight rules from the Quebec City/Jean Lesage, QC (CYQB) airport to the destination of Newark (KEWR) in the United States, with 42 passengers and 3 crew members. During the initial climb after takeoff, the Engine-Indicating and Crew-Alerting System (EICAS) generated a message of disagreement between the position of the landing gear control lever in the retracted position while the landing gear remained in the extended position. The crew declared a state of emergency, requested a return to CYQB and completed the Quick Reference Handbook (QRH) checklist. The aircraft touched down on Runway 06 without incident, with the aircraft rescue and fire fighting (ARFF) services on standby. There were no injuries or damage.

A Jazz Bombardier CL-600-2B19 (C-FDJA/JZA7944) from Montreal/Trudeau, QC (CYUL) to Newark, NJ (KEWR) was in position and waiting for takeoff and an Air Canada Airbus A320-211 (C-FPWD/ACA782) from Los Angeles, CA (KLAX) to Montreal/Trudeau, QC (CYUL) was on approach. ACA782 was instructed to conduct a pull-up.

Incident at 5609N / 06557W

According to an automatic dependent surveillance – contract (ADS-C) report, an El Al Israel Airlines Boeing 787-9 (ELY027/4XEDK) from Tel Aviv, Israel (LLBG) to Newark, NJ (KEWR) was not following the route in its flight plan, that is, MOFAT ALB. The report indicated instead that the aircraft followed MOFAT HANAA ALB. When questioned, the aircraft confirmed that the flight plan had been entered incorrectly. No impact on operations.

The only traffic in the area was a United Airlines Boeing 777-200 (UAL89) from Newark/Liberty, NJ (KEWR) to Beijing, China (ZBAA) at FL 310. No impact on operations.

No emergency declared, no fuel to dump. North American Aerospace Defence Command (NORAD) advised. No impact on operations.

Incident at NOSIK intersection (Communication navigation surveillance/air traffic system)

The controller handed an Endeavor Air Inc. Bombardier CL-600-2D24 (EDV5059) from Newark, NJ (KEWR) to Minneapolis/St. Paul, MN (KMSP) off to the Minneapolis Air Route Traffic Control Centre (ZMP) at approximately the same time that the ZMP handed a Cargojet Airways Ltd. Boeing 767-300 (CJT516) from Vancouver, BC (CYVR) to Hamilton, ON (CYHM) off to the Toronto Centre (CZY) controller. The aircraft were flying in opposite directions, at FL360 and FL370 respectively, and approximately 80NM apart. The ZMP controller changed the code on EDV5059 and the data tag was no longer visible to the CZY controller. Moments later, the CZY controller started to descend CJT516. The ZMP controller turned EDV5059 to avoid a conflict and advised the CZY controller.

Impact on operations: a Jazz Bombardier CL-600-2B19 (C-GTJA/JZA8857) from Newark, NJ (KEWR) to Ottawa/Macdonald-Cartier, ON (CYOW) was placed into a holding pattern for a period of 15 minutes, and it landed at 0303Z. Tower closed from 0242Z to 0303Z.

Incident at NOAHA intersection

A Chartright Air Bombardier CL-600-2B16 (HRT085) on a flight from Newark Intl, NJ (KEWR) to Toronto/Lester B. Pearson Intl, ON (CYYZ) executed a go around due to a landing gear indication problem. HRT085 performed a checklist to troubleshoot and then requested fire trucks to stand by for landing. HRT085 landed Runway 23 at 2317Z without incident and was escorted to the gate by Aircraft Rescue and Fire Fighting (ARFF). No emergency was declared. No operational impact.
UPDATE TSB Report #A20O0008: C-FNBM a Bombardier CL600-2B16 aircraft was conducting Chartright Air flight HRT085 from Newark Liberty International Airport (KEWR), NJ to Toronto/Leaster B. Pearson Intl. (CYYZ), ON. On approach, the Flight crew executed a go-around due to a landing gear indication problem. After the go-around, HRT085 performed the checklist items to troubleshoot the problem and then requested fire trucks to stand by for landing. The aircraft landed without further incident in CYYZ and taxied to the gate.

Came back 10 minutes after and landed no impact on operations.

Incident at Newark International, NJ (KEWR)

TSB Report #A20F0137: C-GOJA, a CRJ 100 was operated by Jazz flight JZA8022 from Toronto Lester B. Pearson International Airport, (CYYZ), ON, to Newark Liberty International Airport (KEWR), NJ, USA, when the crew reported a flap fail at 14 degrees on approach to Newark, advised air traffic control (ATC) they had a maintenance issue to work through and broke off the approach. No emergency or PAN PAN declared, ATC elected to send the trucks out. Flight crew advised flight attendant and passengers that it would be an abnormal landing. Flight landed without incident.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. This is the second of three recent events for this aircraft. The first event was covered by CADORS No. 2020O1017, and the third did not result in a CADORS. This event (second) was reported as a Flap Extension fail at 14 degrees on approach to KEWR on August 10, 2020. Following extensive troubleshooting, an electrical connector and the Flaps Electronic Control Unit were replaced. The aircraft was returned to service following rigging and operation tests of the Flap System. Subsequently, the Flap System failed at 42 degrees after landing on August 23, 2020 (Flight No. 8028). The right-hand (R/H) Brake Position Sensor Unit (BPSU) was moved to the left-hand (L/H) position and a replacement BPSU was installed in the R/H position. The aircraft was again returned to service following rigging and operational tests. There has been no recurrence. The operator has plans to submit a Service Difficulty Report.

Update – TSB Report #A19Q0204: N14148, a Commutair Embraer-145XR (UCA5005), was on an IFR flight from Newark Liberty Intl/Newark, NJ (KEWR) to Montreal/Pierre Elliott Trudeau, QC (CYUL). During the landing on Runway 24R at CYUL the aircraft could not stop before the runway edge, which was temporarily moved due to repair work on the runway. The length of the available runway was 7 068 ft. After the aircraft passed the end of the runway, it was cleared to taxi onto the closed portion of the runway so that it could get to the passenger bridge.There was freezing rain at the time of the landing. The aircraft was not damaged and no one was injured.

At departure from Runway 24L at 1510Z, an American corporately owned Embraer EMB-145XR (N14198/UCA4961) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Newark, NJ (KEWR) informed air traffic services (ATS) of a bird strike, and then, of a problem with engine 1. UCA4961 declared an emergency at 1514Z and came back to land at CYUL. UCA4960 landed without incident on Runway 24L at 1520Z. Impact: a few departures were delayed by approximately 10 min.
Update TSB #A19Q0089: N14198, an Embraer EMB-145XR aircraft operated by CommutAir, was conducting flight UCA4961 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Newark/Liberty Intl (KEWR), NJ with 3 crew members and 48 passengers on board. During the initial climb out of CYUL, a bird was ingested into the left engine. The engine rolled back, then spooled back up to normal operation. The flight crew briefly smelled an odour, and proceeded to turn off the number 1 bleed; the smell dissipated. Both ignition switches were turned on, a MAYDAY was declared, and an immediate return to CYUL was initiated where the aircraft landed without further incident. The operator’s maintenance completed a compressor wash, along with a borescope inspection of the engine. The damages were within limits, and the aircraft was returned to service.

Incident at 30NM SEPT-ILES QC VOR/DME (YZV) (ATS operating irregularity)

A United Airlines Boeing 777-200 (UAL56) from Paris, France (LFPG) to Newark, NJ (KEWR) and an Air Canada Boeing 787-9 (ACA879) from Zurich, Switzerland (LSZH) to Toronto/Lester B. Pearson, ON (CYYZ) had less than required separation after air traffic control (ATC) issued a climb clearance to UAL56.
TSB #A19Q0054: N788UA, a United Airlines Boeing 777-200, was on instrument flight rules (IFR) flight UAL56 from Paris/Charles de Gaulle, France (LFPG) to Newark/Liberty Intl, NJ (KEWR). C-FRTU, an Air Canada Boeing 787-9, was on instrument flight rules (IFR) flight ACA879 from Zürich-Kloten, Switzerland (LSZH) to Toronto/Lester B. Pearson Intl, ON (CYYZ). UAL56 was level at FL360, while ACA879 was also level at FL380. The two aircraft followed headings that were similar, but divergent. Approximately 30 NM southwest of Sept-Îles, QC (CYZV), UAL56 was cleared to climb to FL380. There was a loss of separation when UAL56 established itself at FL380 and the horizontal distance between the 2 aircraft was reduced to 8.8 NM. The required separation was 10 NM horizontally, or 1 000 ft vertically.

A United Airlines Boeing 777-300 (N2138U/UAL79) from Newark, NJ (KEWR) to Tokyo/Narita, Japan (RJAA) had problems with the aircraft systems. It was impossible to chill or heat up food. UAL79 decided to return to KEWR. North of Trois-Rivières, QC (CYRQ), UAL79 dumped fuel between 2015Z and 2100Z. No impact on operations.
