An Envoy Air Embraer ERJ 170-200 LR (N230NN/ENY3981) on a flight from Montreal/Pierre Elliott Trudeau Intl (CYUL) to Dallas/Fort Worth Intl, TX (KDFW) had a technical issue and requested to return to CYUL. An emergency was declared for an overweight landing. The flight landed Runway 24L at 1146Z with no impact on operations.
Update: TSB Report A19Q0183: N230NN, an Envoy Air Inc. Embraer ERJ-175 (ENY3981) was on an IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Dallas-Fort Worth Intl., TX (KDFW). During the initial climb, the flight attendant accidentally deployed the cabin passenger oxygen masks. The crew declared an emergency in preparation for a rough landing, returned to CYUL, and landed without incident on Runway 24L. There were no injuries. Maintenance services put the deployed oxygen masks back into service.

C-FMAO was cleared to take off with a right turn on a heading of 320. An American Airlines Boeing 737-823 (N861NN/AAL2702) from Miami Intl, FL (KMIA) to Montreal/Pierre Elliott Trudeau, QC (CYUL) was cleared to land. C-FMAO taxied slowly, and the controller decided to pull AAL2702 up on a heading of 205.

TSB Update #A19Q0058: A Black Diamond Aviation Group LLC Learjet 60 (N112BD) was on an instrument flight rules (IFR) flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to White Plains/Westchester County, NY (KHPN). After the aircraft had been cleared to take off from CYUL and during its initial climb, a loss of separation occurred with a Sky Regional Airlines Embraer ERJ 170-200 (CFRQP/SKV7727) from CYUL to Dallas-Fort Worth, TX (KDFW), which had taken off just before N112BD. The longitudinal separation had been reduced to 2.6 NM, while 3 NM was required. The crew of N112BD was given a radar vector to restore the required separation. The two aircraft continued their flights to their respective destinations.

An American Airlines Boeing 737-800 (N980AN/AAL1424) from Dallas/Fort Worth, TX (KDFW) to Vancouver, BC (CYVR), on the GRIZZ6 arrival into CYVR for Runway 26R, was told to expect vectors to final well in advance. Once established on an intercept heading of 290, AAL1424 was cleared for the area navigation (RNAV) approach. AAL1424 was then observed in a hard turn of approximately 60 degrees to the right and proceeded through the centreline for Runway 26R. As a result, a Flair Airlines Ltd. Boeing 737-86N (C-FFLA/FLE415) from Edmonton, AB (CYEG) to Vancouver, BC (CYVR) had to be vectored off the approach to ensure IFR separation and was re-sequenced into the downwind.

Update TSB Report #A19P0024: C-FUJZ, a Bombardier CL-600-2D24 aircraft operated by Jazz Aviation LP, was conducting flight JZA8626 from Vancouver Intl (CYVR), BC to Dallas-Fort Worth Intl (KDFW), TX with 4 crew members and 65 passengers on board. During the climb to cruising altitude after the departure from CYVR, the flight crew noted a petroleum smell in the cabin and the flight deck. Symptoms of light-headedness, headache, tingling, nausea, sore throat, and irritated eyes were also reported by some crew members. The pilots donned their oxygen masks, while the flight attendants used portable oxygen. No emergency was declared, however the aircraft returned to CYVR where it landed without further incident. After the landing, the flight crew taxied the aircraft to the gate, and passengers and crew deplaned normally. Emergency Medical Services met the aircraft, and paramedics assessed all passengers and crew. Affected crew members were transported to hospital for further evaluation. The operator’s maintenance determined that the air contamination was the result of turbine oil entering the number 1 engine compressor section, from the A-sump pump aft seal. The number 1 engine (General Electric CF34-8C5) was replaced.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence.On departure from Vancouver Intl, BC (CYVR) the flight crew of a Jazz Bombardier CL-600-2D24 (C-FUJZ/JZA8626) on a flight from Vancouver Intl, BC (CYVR) to Dallas/Fort Worth Intl, TX (KDFW) noted a strong, pungent odour. Communication with the Incharge Flight Attendant confirmed that the Passenger cabin was also affected. The air quality was noted as being worse in the Flight Deck. One of the flight crew went through the passenger cabin to better understand the scope of the problem. The back of the cabin had a mild odour with it becoming progressively worse towards the front. The poor air quality affected the crew more than the passengers. The crew decided to return to CYVR as they were suffering from varying symptoms of light headedness, headache, tingling, nausea and irritated eyes. The flight and cabin crew members donned oxygen, and passengers were given therapeutic oxygen if requested. An emergency was not declared by the flight crew. The aircraft returned and landed overweight without further incident at CYVR, and taxied to the gate for deplaning. Announcements were made in the air and on the ground advising passengers of the situation; medical services were on the bridge offering assistance to anyone who accepted it. On two previous occasions there were reports of oil/fuel smell in the cockpit; the No. 1 Air-Conditioning Pack was deferred initially; when it recurred the Auxiliary Power Unit (APU) was suspected and deferred (the No. 1 pack was cleared). Subsequently, troubleshooting concluded the air contamination was from the left engine, borescope inspections revealed traces of oil from the first stage of variable guide vanes. The minimum equipment list (MEL) was applied and the aircraft repositioned to Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) under a Special Flight Permit. The engine will be replaced and sent for further investigation / repair. A Service Difficulty Report (SDR) has been submitted.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

TSB #A19O0064: C-GTAQ, a Jazz Aviation LP DHC-8-300 aircraft (AC8613), was taxiing for departure from runway 05 at Toronto/LBPIA (CYYZ) when smoke and flames were reported emanating from the left main brakes. The aircraft stopped on the taxiway and the passengers were evacuated. ARFF responded to the site but their services were not required as the fire had self extinguished. Maintenance found the left main landing gear brakes were seized and heat damaged; the wheel overpressure plugs melted and the tires went flat. Damage was limited to the brakes and wheels, however, the left main landing gear was replaced as a precaution.
UPDATE Aviation Incident Report: Following a long taxi, while holding short of Runway 05 at Toronto/Lester B. Pearson Intl, ON (CYYZ), smoke and flames were visible in the left-hand (L/H) main landing gear brake area. Passengers had indicated to the flight attendant that sparks were coming off the L/H brakes during taxi. The flight attendant advised the first officer of the issue. At this time the brakes caught fire. The decision to evacuate the aircraft was made. All crew and passengers (total 51) evacuated safely from the 1L and 1R doors, there were no injuries. Aircraft Rescue and Fire Fighting (ARFF) responded. Passengers and crew were bussed back to the Terminal. Company Maintenance was dispatched as the brakes were seized. The aircraft was towed to the hangar. TSB notified. Currently the aircraft is not in service and troubleshooting is on-going. Service difficulty report (SDR) filed.
UPDATE AOR 248693-V1: A SkyWest Airlines Bombardier CL-600-2C10 (Series 700) (SKW3033) from Toronto/Pearson, ON (CYYZ) to Chicago/OHare, IL (KORD) and the North Tower controller reported flames observed from the left side of a Jazz de Havilland DHC-8-300 (C-GTAQ/JZA8613) from Toronto/Pearson, ON (CYYZ) to Sudbury, ON (CYSB) in Holding Bay 05. One Delta was declared and the Aircraft Rescue and Fire Fighting (ARFF) unit was dispatched immediately. JZA8613 was evacuated, with 51 souls onboard. Runway 05 was closed until 0230Z, and a ground stop was implemented with average delays of 45 minutes. A Republic Airlines Embraer EMB-170 (RPA3458) from Chicago/OHare, IL (KORD) to Toronto/Pearson, ON (CYYZ), a WestJet Boeing 737-800 (WJA714) from Vancouver, BC (CYVR) to Toronto/Pearson, ON (CYYZ), and a Sky Regional Airlines Embraer ERJ 170-200 SU (SKV7664) from Dallas/Fort Worth, TX (KDFW) to Toronto/Pearson, ON (CYYZ) were instructed to initiate missed approaches to Runway 05 and subsequently landed on Runway 06R.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Following a long taxi, while holding short of Runway 05 (CYYZ), smoke and flames were visible in the left-hand (L/H) Main Landing Gear Brake area of C-GTAQ. Passengers had indicated to the flight attendant (FA) that sparks were coming off the L/H brakes during taxi. The FA went to the cabin around row 8 or 9 and noted orange/red glow coming from the L/H gear. The FA, using the aircraft interphone, advised the First Officer of the issue. At this time, the brakes caught fire, the FA made this apparent to the First Officer. The decision to evacuate the aircraft was made. All crew and passengers (total 51) were evacuated safely from the 1L and 1R doors, there were no injuries. Aircraft Rescue and Fire Fighting (ARFF) responded. Passengers and crew were bussed back to the terminal. Company maintenance was dispatched as the brakes were seized. The aircraft was towed to the hangar. Transportation Safety Board (TSB) has been notified and is investigating. Currently the aircraft is not in service and troubleshooting is on-going. A service difficult report (SDR) has been submitted and will be updated with the results of the investigation/troubleshooting.

A Sky Regional Airlines Embraer ERJ 170-200 SU (C-FRQK/SKV7662) from Dallas, TX (KDFW) to Toronto/Lester B. Pearson, ON (CYYZ) reported a bird strike on landing Runway 06L. Strike confirmed and 1 falcon retrieved intact. No impact on operations.
