A WestJet Encore Ltd. de Havilland DHC-8-402 (WEN3202) from Calgary, AB (CYYC) to Fort St. John, BC (CYXJ), on final for Runway 03 on the correct slope, reported that the precision approach path indicators (PAPIs) were "white-white-red-white". A Central Mountain Air Ltd. Beech 1900D (GLR743) from Prince George, BC (CYXS) to Fort St. John, BC (CYXJ), on final 7 minutes later, reported the same thing. The North Peace Airport Services (NPAS) unit was advised and confirmed that there was an issue with the PAPIs after an inspection. A NOTAM was then issued by NPAS.

Incident at DAWSON CREEK BC (CYDQ)

An unknown aircraft later identified as an American, corporately registered Cub Crafters CC11-160 (N189PD) on a flight to Fort St. John, BC (CYXJ) was squawking 1200 as it was approaching the Dawson Creek, BC (CYDQ) control zone (CZ) from the southeast (SE), travelling northwest bound. Attempts to contact the aircraft were made three times, with no response heard. Other aircraft in the area was a Shock Trauma Air Rescue Service aircraft (STARS5), which landed without any traffic conflict. No impact to operations.

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review: After receiving clearance, a North Cariboo Flying Service Ltd Beech 200 (C-FNNC/NCB3508) from Fort Nelson, BC (CYYE) to Fort St. John, BC (CYXJ) reported a stuck brake and shut down at the entrance to Taxiway A. NCB3508 was given a new clearance and departed at 1811Z, without incident. No impact to operations.

Incident at FORT ST. JOHN BC (CYXJ) (Communication error, Incursion - runway - vehicle)

At Fort St. John, BC (CYXJ), a vehicle was noticed approaching Runway 12/30. Over the frequency, other vehicles was asked if there was a vehicle near the runway protected area (RPA) for Runway 12/30. A car (ST63) called and said they were close to, but outside of, the RPA for Runway 12/30. Then, a car (ST61) called for permission onto the RPA for Runway 12/30 and was granted it. The vehicle was then removed due to inbound aircraft, and when ST61 reported off, so did ST63. North Peace Airport Services (NPAS) was called, and the Flight Service Station (FSS) was then informed that ST63 believed they were getting permission onto the runway when ST61 was granted permission on the runway. All vehicles were off the runway with no impact to operations.

Incident at FORT ST. JOHN BC (CYXJ)

An unidentified aircraft, squawking 1200, was observed entering the control zone from the southeast (SE). 5 vehicles were removed from Runway 12/30 and an attempt was made to reach the aircraft on 118.5 MHz. The aircraft, observed to be an R44, passed over the SE corner of the airport between 3000 and 3500 ASL, at 1758Z, before turning back southeast and climbing. The R44 exited the zone and established contact as a privately registered Robinson R44 II on a flight to Fort St. John, BC (CYXJ). The aircraft proceeded back inbound and landed at 1811Z.

An American, privately registered Piper PA-18 from Fort St. John, BC (CYXJ) to Grande Prairie, AB (CYQU) was observed entering Taxiway B without establishing communication on the mandatory frequency. Communication was established by the flight service station (FSS). A vehicle (ST61) was working on Runway 03/21 with access to Taxiway B but was reporting off on Runway 12/30 (closed) when the aircraft was observed. No impact on operations.

Incident at VANCOUVER INTL BC (CYVR) (Aerodrome, runway or taxiway shutdown)

A WestJet Encore Ltd. de Havilland DHC-8-402 (C-FENO/WEN3202) from Vancouver, BC (CYVR) to Fort St. John, BC (CYXJ) had a mechanical emergency. WEN3202 returned to CYVR with a landing gear issue. The Aircraft Rescue and Fire Fighting (ARFF) unit was on site to assist. The aircraft landed without incident. Runway 26R was closed as result, but reopened once vacated.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After takeoff, the left-hand (LH) main landing gear (MLG) failed to retract. Appropriate Quick Reference Handbook (QRH) actions were carried out and the aircraft (A/C) returned to the point of departure. Maintenance replaced the LH MLG retraction actuator. Service Difficulty Report (SDR) 20220125009 was filed in connection with this event.
UPDATE: TSB Report #A22P0005: C-FENO, a WestJet Encore de Havilland DHC-8-402 (flight WEN3202), was conducting an instrument flight rules flight from Vancouver Intl, BC (CYVR) to Fort Saint John, BC (CYXJ) with 4 crew members and 17 passengers on board. When the flight crew selected gear up after take-off from CYVR, the annunciators displayed an amber “main left gear door” indication, and a red “left main gear” indication. The crew actioned the landing gear door malfunction checklist, completed the alternate gear extension procedure, and decided to return to CYVR. They did not declare an emergency but indicated a need for emergency response vehicles to be on stand-by. The airplane landed without incident and was met by aircraft rescue fire fighting vehicles. Maintenance found the left retract actuator was bypassing internally and replaced the retract actuator. The inspection also found the right main landing gear alternate extension cam mechanism assembly and the alternate extension up-lock release cable were broken. Those were replaced and, after a rigging check, the airplane was returned to service.

Shortly after departure from Runway 30, the Edmonton (YEG) Centre informed the Flight Service Station (FSS) that a North Cariboo Flying Service Beech 200 (NCB860) on a MEDEVAC flight from Fort St. John, BC (CYXJ) to Fort Ware, BC (CAJ9) would be returning to CYXJ. The pilot said it was a gear issue, and that no assistance would be required. North Peace Airport Services (NPAS) was advised of the situation. The aircraft returned to CYXJ and landed on Runway 30 without incident. No impact to operations.

Diefenbaker, AB (CYXE) was delayed by approximately 30 minutes while company maintenance checked the status of C-GADIs gear.
