Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16540: At Bagotville, QC (CYBG), a runway incursion was committed by a foreman at the intersection of Runway 18/36 and Runway 11/29 while a Jazz de Havilland DHC-8-301 (C-GETA/JZA8684) from Montreal/Trudeau, QC (CYUL) to Bagotville, QC (CYBG) was landing on Runway 36. Foreman was instructed to proceed on the airfield, holding short of Runway 36, and had read back the restriction. A few minutes later, the foreman was on Runway 29, driving towards Runway 36 faster than expected, so the ground controller reiterated to the foreman to hold short of Runway 36. This was followed by the instruction to stop immediately when the vehicle was seen crossing the hold short line. The vehicle was then immobilized at the center of the main intersection when the aircraft touched down. Ground control issued instructions to the vehicle to vacate the runway without delay, while the tower controller told the pilot about the unauthorized vehicle at the main intersection. The pilot had a visual on the vehicle and, after touching down, decided to pull up. The aircraft ended up conducting an overshoot less than a 100 above the vehicle when it was exiting the runway. The aircraft stayed on the tower frequency and came back for a landing on the active runway (Runway 29) without further incident.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB)

TSB Report #A20Q0043: C-GGOF, a Jazz Aviation LP Dash-8 Q400 (JZA8732), was on an instrument flight rules (IFR) flight from Montreal Pierre Elliott Trudeau, QC (CYUL) to Quebec Jean Lesage, QC (CYQB) with 65 passengers and 4 crew members. During the flight, a member of the cabin crew became physically incapacitated. The second member of the cabin crew assigned a passenger to watch the emergency exits at the back of the aircraft for the landing. The aircraft landed at its destination without incident. No one was injured, and there was no damage to the aircraft.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

TSB Report #A20Q0024: C-GABO, a Jazz Aviation LP Dash-8-300 (DH8C) (JZA8970), was on an IFR flight from Ottawa/Macdonald-Cartier, ON (CYOW) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 48 passengers and 3 crew members on board. After the landing and while the aircraft was taxiing to the terminal, a passenger alerted the crew to flames coming out from the right main gear. The crew brought the aircraft to a stop and requested the intervention of Aircraft Rescue and Fire Fighting (ARFF) services. The fire quickly went out and the aircraft was towed away to the passenger bridge. When the aircrafts right main gear was checked, a breakage was found in two bearings in the inboard wheel.

Incident at N VILRO intersection

A weak, constant emergency locator transmitter (ELT) signal was heard for over 10 minutes and reported by 2 aircraft in the same location, one at FL220 (a Jazz de Havilland (JZA8781) at 0940Z) and the other at FL240 (a Jazz de Havilland (JZA8945) at 1002Z). The Joint Rescue Coordination Centre (JRCC) in Trenton was notified. The source was identified on the ground as the signal from a Syncrude Canada Ltd. Cessna 560 (C-FSNC) heading to St-Hyacinthe, QC (CSU3). No impact.
UPDATE AOR 263176-V3: Aircraft registration information for ELT source changed to a privately registered Piper PA-32R-300. Narrative updated to read: "The source was identified on the ground as the signal from a privately registered Piper PA-32R-300 heading to St-Hyacinthe, QC (CSU3)."

A Jazz de Havilland DHC-8-402 (C-GGFP/JZA7977) on a flight from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) to Toronto/Billy Bishop, ON (CYTZ) declared an emergency due to a gear indication and elected to return to CYUL. JZA7977 cancelled the emergency at 0006Z and landed safely at 0020Z. No impact on operations.
TSB Report Update #A20Q0041: An Air Canada Jazz Dash 8-400 (C-GGFP/JZA7977) was on an IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Toronto City/Billy Bishop, ON (CYTZ). During the initial climb during departure from CYUL and after the retract gear function was selected, the gear first went into the retracted position but then went out again shortly afterwards, even though the extension function had not been selected. The crew carried out the manual gear extension procedure. An emergency was declared; the crew asked to return to CYUL, where the aircraft landed without incident. The landing gear selector panel was changed and the aircraft was returned to service.

UPDATE TSB Report #A20Q0035: C-GGOF, a Jazz Aviation LP Dash-8 402 (JZA7960), was on an IFR flight from Toronto/Billy Bishop Toronto City Airport, ON (CYTZ) to Montreal Pierre Elliott Trudeau, QC (CYUL). During the approach, the crew noticed light smoke near the right side of the windshield, as well as the smell of burnt electrical wires. The windshield heat system was turned off, which caused the smoke to dissipate. The crew declared an emergency and the aircraft landed without incident.

When the de-icing operation was complete, the technician approached the horizontal stabilizer to conduct a tactile check of the surface. The crane of the bucket truck came into contact with the bottom of the stabilizer, causing minor damage. There were no injuries.

TSB Report #A20Q0003: C-FSOU, a Jazz Aviation LP Dash-8-300 (DH8C) (JZA8684), was on an IFR flight from Montreal/Pierre Elliott Trudeau, QC (CYUL) to CFB Bagotville, QC (CYBG) with 49 passengers and 3 crew members. Immediately after takeoff from Runway 24L, the interior wheel in the left landing gear broke off of the aircraft and fell onto the runway. The crew declared an emergency and remained in flight for approximately 1 hour to burn off fuel and reduce the aircrafts weight before returning to CYUL and landing. Aircraft Rescue and Fire Fighting (ARFF) services were deployed. The aircraft landed on Runway 24R without incident with 1 wheel remaining on the left landing gear. The aircraft cleared the runway via Taxiway B2 and came to a stop. Passengers were brought to the terminal by bus. There were no injuries, and there was no other damage to the aircraft.

Incident at MIVAX intersection (Communication navigation surveillance/air traffic system)

The air traffic services (ATS) unit cleared a Jazz de Havilland DHC-8-301 (C-GETA/JZA8765) from Gaspé/Michel-Pouliot, QC (CYGP) to Quebec/Jean Lesage Intl, QC (CYQB) to descend to A060 towards CYQB. The aircraft read back A060. ATS indicated a cleared altitude of A080 in the Canadian Automated Air Traffic Service (CAATS) and transferred the aircraft to the OR sector. No impact on operations.

Incident at 30NM ST-JEAN QC VOR/DME (YJN) (Communication navigation surveillance/air traffic system)

An American registered Learjet 35A (N244LJ) from Page Field, FL (KFMY) to Quebec/Jean Lesage, QC (CYQB) was cleared to descend to FL320 but read back FL230. This readback error was not corrected by air traffic services (ATS). Impact: separation with a Jazz Bombardier CL-600-2D24 (C-FJZD/JZA8630) from Ottawa/Macdonald-Cartier, ON (CYOW) to Halifax/Stanfield, NS (CYHZ) at FL310 was not maintained. No other impact on operations.

A Jazz Bombardier CL-600-2B19 (C-GKGC/JZA8767) from Val-dOr, QC (CYVO) to Rouyn-Noranda, QC (CYUY) was cleared for an area navigation approach (RNAV) on Runway 26 at CYUY, even though a dynamiting operation was in progress 12 NM east of CYUY. A NOTAM was in effect, prohibiting clearance to conduct an approach on Runway 26 during the dynamiting activity.
