Incident at MONT-JOLI QC (CYYY) (Aerodrome, runway or taxiway shutdown)

At Mont-Joli, QC (CYYY), a vehicle (ST22) advised that Runway 06/24 and Runway 15/33 would be closed for less than an hour, starting at 1050Z. A voice NOTAM was issued. The Montreal area control centre (CZUL) was notified. At 1135Z, the airport manager (APM) transmitted a runway surface condition (RSC) /Canadian Runway Friction Index (CRFI), and Runway 06/24 was reopened. A NOTAM was issued by the APM for the closure of Runway 15/33. A Provincial Airlines de Havilland DHC-8-314 (C-GYCV/PVL3055) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Mont-Joli, QC (CYYY) had to enter into a holding pattern and their arrival was delayed by about 20 minutes.

A privately registered, amateur-built Glasair II RG, from St. Jean, QC (CYJN) to St. Jean, QC (CYJN) was cleared to taxi and to hold short of Runway 20. The aircraft was taxiing without re-reading the Air Traffic Services (ATS) authorization. ATS made several attempts to get the aircrafts read-back to hold short. Two aircraft in the circuit were instructed to ascend Runway 20 as a safety measure, and to allow for time to re-establish communications with the Glasair II RG aircraft.

Incident at SEPT-ÎLES QC (CYZV)

A privately registered Piper PA-31 from Baie-Comeau, QC (CYBC) to Sept-Îles, QCF (CYZV) indicated on initial contact their intention to use Runway 27. When the aircraft reported on base (less than 1NM south of the airport), it was observed turning on short final for Runway 31. Once on the ground, the aircraft backtracked on the runway and the flight service station (FSS) asked them twice to hold short of Runway 27 as an aircraft was on final. The Piper PA-31 did not respond and taxied rapidly to the threshold of Runway 27, where they called in "clear of the runway". The FSS requested that they clear quickly. The Piper PA-31 left the runway before the aircraft on approach passed the threshold. No impact.

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (Incursion - manoeuvring area, Communication error)

At Montreal/Mirabel, QC (CYMX), a vehicle (SV352+A+1) was towing an Airbus A343 (HFM564P) from the industrial terminal to the main terminal. The driver of SV352 communicated with the tower by telephone and was instructed to hold short of Taxiway H. This was due to a F18 that was taxiing on H. Once the F18 passed in front of them, SV652+A+1 taxied on Taxiways H and I without authorization or communication with the ground controller. Once on Taxiway I, the vehicle and controller were in contact. No impact.

Incident at BAGOTVILLE QC (CYBG)

Aviation Incident Report #16540: At Bagotville, QC (CYBG), a runway incursion was committed by a foreman at the intersection of Runway 18/36 and Runway 11/29 while a Jazz de Havilland DHC-8-301 (C-GETA/JZA8684) from Montreal/Trudeau, QC (CYUL) to Bagotville, QC (CYBG) was landing on Runway 36. Foreman was instructed to proceed on the airfield, holding short of Runway 36, and had read back the restriction. A few minutes later, the foreman was on Runway 29, driving towards Runway 36 faster than expected, so the ground controller reiterated to the foreman to hold short of Runway 36. This was followed by the instruction to stop immediately when the vehicle was seen crossing the hold short line. The vehicle was then immobilized at the center of the main intersection when the aircraft touched down. Ground control issued instructions to the vehicle to vacate the runway without delay, while the tower controller told the pilot about the unauthorized vehicle at the main intersection. The pilot had a visual on the vehicle and, after touching down, decided to pull up. The aircraft ended up conducting an overshoot less than a 100 above the vehicle when it was exiting the runway. The aircraft stayed on the tower frequency and came back for a landing on the active runway (Runway 29) without further incident.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Aviation Incident Report #16539: Upon pushback from the gate, the pilot of a Royal Jordanian Airlines Boeing 787-8 (JYBAH/RJA90G) from Montreal/Trudeau, QC (CYUL) to Detroit/Wayne County, MI (KDTW) specified that they did not need to proceed to the de-icing facilities, and the aircraft was cleared to taxi, and instructed to hold short of Runway 06R. As the aircraft passed Taxiway AM, the pilots of an Air Canada Airbus A320 (ACA321) queried as to whether RJA90G had been de-iced. The controller mentionned that RJA90G did not request de-icing, to which ACA321 replied that this decision should be challenged. RJA90G had already been handed off to the tower at this point, so the information was relayed to the tower via the hotline. The information was then relayed to the pilot of RJA90G. Regardless, RJA90G departed without de-icing. Approximately 1 minute after the call from ACA321, the pilots of an Air Canada Boeing 787-8 (ACA417) that was waiting on the north ramp as RJA90G taxied past mentionned that one of their passengers reported that RJA90Gs wings were covered with snow. In the hour preceding RJA90Gs departure, all flights departing from the apron required de-icing services.

Aviation Incident Report #16560: An Air Inuit de Havilland DHC-6 series 300 (C-FAIY) (cargo flight) did not call when on final for Runway 16 at 1847Z. An Air Inuit de Havilland DHC-8-102 (C-FDAO) (scheduled passenger flight) radioed while taxiing for departure from Runway 16, and had to hold short on the taxiway. C-FAIY then communicated to C-FDAO in French, which was not understood. C-FAIY landed on Runway 16, then C-FDAO taxied to the threshold of Runway 34 to wait for C-FAIY to exit Runway 16/34. Once C-FAIY was clear of the runway, C-FDAO backtracked for departure.

Incident at DOLBEAU-ST-FÉLICIEN QC (CYDO) (Incursion - runway - vehicle, Communication error)

Aviation incident report #16575: The crew of an Exact Air Cessna 310R (C-FTNF), with a flight instructor and a pilot on board, was conducting a VFR training flight from Chicoutimi/St-Honoré, QC (CYRC) to Chicoutimi/St-Honoré, QC (CYRC). The aircraft had planned to make a complete stop at Dolbeau-St-Félicien, QC (CYDO) after it had conducted a simulated IFR approach on Runway 29 at CYDO. Two communications were made on the CYDO aerodrome traffic frequency (ATF) to transmit their position/intention for a complete stop (1st at 9 NM and 2nd at 3 NM from the runway threshold). The first aircraft left the circuit at that time, after it had taken off from Runway 29, and a second aircraft coordinated with C-FTNF, on simulated IFR approach, lined up on Runway 29 for takeoff. Approximately 1 NM from the runway threshold, the pilots heard a vehicle report that it was taxiing on the taxiway, short of Runway 29. The pilots then reported that they were on short final for Runway 29 for a complete stop. The vehicle continued to taxi without any indication of stopping, crossed the hold-short line, and drove onto Runway 29 up to the threshold. The pilots then conducted a go-around approximately 200 ft from the ground. Weather conditions were SKCLR +12SM. Strobe lights and ground traffic lights went on. The pilots indicated via radio that they were pulling up. The driver of the vehicle then reported having poor radio reception. Traffic had to take off on Runway 29 (a CQFA Beechcraft with an instructor on board), and it confirmed that communication transmitted by the crew of C-FTNF were perfectly comprehensible on the ATF. It also seemed that a tractor driven by an airport staff member was moving around at the airport without a radio system with it, so ground operators initiated a relay via an unknown other method to inform it of the movement of the aircraft in the CYDO zone. Impact: A go-around and a cancellation of the originally planned complete stop at CYDO.

Incident at 30NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

An Air Canada Airbus A320-211 (C-FDSN/ACA485) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Toronto/Lester B. Pearson, ON (CYYZ) requested a return to the airport due to a vibration in engine #2; no emergency was declared while the aircraft was inbound for Runway 06R at CYUL. The aircraft then requested the presence of Aircraft Rescue and Fire Fighting (ARFF) services on the holding bay for Runway 24L to check the engine. No impact on operations.

Incident at SEPT-ÎLES QC (CYZV) (Incursion - runway - vehicle)

At Sept-Îles, QC (CYZV), vehicles (TK92+2) were cleared on Taxiway B-A and holding short of Runway 09/27. The flight service station (FSS) saw the vehicles drive onto the runway, and the FSS asked them to leave, without any impact.

Incident at MONTRÉAL / ST-HUBERT QC (CYHU) (Incursion - runway - vehicle)

A snowplow (CA80) notified the tower that it was stuck on Runway 24R at 0105Z. At 0110Z, the vehicle advised the tower that it was holding short of Runway 24R. Impact on operations: a Pascan British Aerospace Jetstream Model 3212 (PSC478) from Quebec/Jean Lesage, QC (CYQB) to Montreal/St-Hubert, QC (CYHU) conducted a go-around on Runway 24R and a Pascan British Aerospace Jetstream Model 3212 (PSC882) from Bagotville, QC (CYBG) to Montreal/St-Hubert, QC (CYHU) had to reject its approach and fly a holding pattern. The two aircraft landed between 0116Z and 0120Z.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

Although it had received and read back clearance to turn right and hold short of Taxiway Romeo from Runway 24L, a Propair Inc. Beech 100 (PRO4651) from Montreal/Pierre Elliott Trudeau, QC (CYUL) to Chibougamau/Chapais, QC (CYMT) turned left and taxied on Taxiways Romeo and Lima far from Runway 24L. No impact.

Incident at MONTRÉAL / ST-HUBERT QC (CYHU)

Although it had received and read back the instruction to hold short of Runway 06R, a Cargair Ltd. Cessna 152 (C-FOPC) from Montreal/St-Hubert, QC (CYHU) to Montreal/St-Hubert, QC (CYHU) crossed the stoplines, and the aircraft came to a stop after the air traffic services (ATS) unit intervened. Impact: an aircraft on final was instructed to pull up.
