Accident at In the vicinity of: HALL BEACH NU (CYUX)

TSB #A19Q0179: C-FBHF, a Canadian Helicopters Ltd. Bell 212, was on a visual flight rules (VFR) flight from the Hall Beach, NU (CYUX) airport to a site located 51 nautical miles east-northeast of CYUX. The helicopter was transporting fuel to the site. Due to a problem related to the reservoirs on site, the pilot could not land on the helipad for this purpose. It therefore chose another landing spot. During the second takeoff from this landing zone, the pilot heard a "bang" and felt significant vibrations. The pilot landed immediately and turned off both engines. The main rotor blades had hit the upper part of the stairway safety cage on a reservoir located on the take-off track. The pilot, who was alone on board, was not injured.

Incident at IQALUIT NU (CYFB) (ATM - inaccurate aeronautical information, ATM - NAVAIDS/radar, ATM - weather observation systems)

Failure of the NAV CANADA Auxiliary Radar Display System (NARDS), the Extended Computer Display System (EXCDS) and the weather data entry (WDE) occurred between 1602Z and 1603Z at Iqaluit, NU (CYFB). Two aircraft were on final, a Canadian North Inc. de Havilland DHC-8-106 (C-GRGO/MPE611) from Hall Beach, NU (CYUX) to Iqaluit, NU (CYFB), and a First Air Aerospatiale/Aeritalia ATR 42-500 (C-FTIQ/FAB870) from Igloolik, NU (CYGT) to Iqaluit, NU (CYFB), and it was not possible to "land" the first aircraft, MPE611, in EXCDS until 1 minute after the actual landing time.

Accident at 1.5NM E Teslin Lake

TSB Report #A18P0149: C-FZEL, a private float-equipped Cessna A185E aircraft operated by Jennings River Wild Adventures, was en route from Hall Lake, BC to Coconino Lake, BC with 2 persons on board. Approximately 1.5 nm out on final at 700 feet AGL, the engine (Teledyne Continental IO-520-D) lost power abruptly. During the forced landing approach to a small lake, the aircraft struck trees and came to rest in approximately 7 feet of water. The pilot and the passenger received minor injuries, and swam to shore. However, the aircraft was substantially damaged. The ELT activated, and there was no fire. The two occupants were evacuated several hours later by Yukon EHS helicopter.

Update TSB #A18P0126: A privately registered Found Brothers FBA-2C3 aircraft, was attempting to land on a private airstrip at Hudson Island, BC. During the final landing phase the aircrafts airspeed was higher than normal and the aircraft landed hard. After the initial touchdown the left main gear and nose gear collapsed causing the aircraft to end over, coming to rest inverted. The ELT activated and search and rescue responded. The pilot sustained serious injuries and was medevac'd to hospital. The three passengers sustained minor injuries.

In addition, the aircraft reported a possible "unruly situation" in the cabin. The aircraft contacted Operational Services (OPS), paramedics and Royal Canadian Mounted Police (RCMP) services on standby. No further details provided. Priority given. The aircraft landed Runway 30 at Hamilton, ON (CYHM) at 2106Z.
UPDATE: Aviation Incident Report#15207: A WestJet Boeing 737-700 (WJA168) on a flight from Edmonton, AB (CYEG) to Toronto/Lester B. Pearson, ON (CYYZ) reported a passenger-related incident. A passenger was acting like he/she was under the influence of an intoxicant, was delusional and would not return to their assigned seat. Emergency Medical Services (EMS) and the Royal Canadian Mounted Police (RCMP) were requested by the crew upon arrival to CYHM. After arrival the passenger was escorted by RCMP to the arrivals hall.

Incident at 120NM S POND INLET NU (CYIO)

TSB Report #A18Q0201: C-FTIQ, a first air AĆ©rospatiale ATR-42-500, was on instrument flight rules (IFR) flight FAB882 from Iqaluit, NU (CYFB) to Arctic Bay, NU (CYAB) with 3 crew members and 12 passengers on board. Approximately 120 NM south of Pond Inlet, NU (CYIO), while the aircraft was in cruising flight in uncontrolled airspace at FL220, the crew noticed a plane on a converging track and at the same altitude. At that moment, a Calm Air ATR-72 (CAV484), which was on an IFR flight from CYIO to Hall Beach, NU (CYUX), was also in cruising flight at FL220. The crew of FAB882 turned on the landing lights and, at the same time, the crew of CAV484 reported as level at FL220 on frequency 126.7 MHz. The crew of FAB882 noticed the traffic on the traffic alert and collision avoidance system (TCAS) at the same altitude, turned off the autopilot, and initiated a climb to FL229. The aircraft passed each other at approximately 2 NM laterally and 900 ft vertically. There was no damage and no one was injured.

Aviation Incident Report #15393: A WestJet Boeing 737 800 (WJA1447) from McCarran International, NV (KLAS) to Edmonton, AB (CYEG) arrived into CYEG and the ground crew found a white powder substance in the forward cargo hold. They advised the flight crew, who then got the airport to call the Royal Canadian Mounted Police (RCMP) and Canada Border Services Agency (CBSA) to come and inspect the substance. CBSA was first on site and searched the forward cargo hold for any other substance and also inspected the white powder. RCMP looked into the cargo hold and allowed the ramp to offload the bags. After the bags were offloaded, CBSA searched each bag before being offloaded into the Customs hall.
