Incident at GANDER INTL NL (CYQX)

At 2000Z, an Exploits Valley Air Services Ltd. Beech 1900D (C-GLHO/EV123T) from Gander, NL (CYQX) to Gander, NL (CYQX) declared an emergency due to an issue with the aircrafts equipment. The Emergency Response Services (ERS) unit was notified. The aircraft landed safely on Runway 31 at 2013Z. No impact on operations.

The tower initiated an overshoot for an Exploits Valley Air Services Beech 1900D (C-GLHO/EV768) on a flight from Saint John, NB (CYSJ) to Halifax/Stanfield Intl, NS (CYHZ) at approximately 1.5NM final Runway 05 due to a Jazz de Havilland DHC-8-402 (C-GYJZ/JZA8809) on a flight from Sydney/J.A. Douglas McCurdy, NS (CYQY) to Halifax/Stanfield Intl, NS (CYHZ) still on short final Runway 32. EV768 subsequently landed Runway 32. No operational impact.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

After landing Runway 14 and vacating in Taxiway M, an Exploits Valley Air Services Beech 1900D (C-GLHO/EV121) on a flight from Gander Intl, NL (CYQX) to Halifax/Stanfield Intl, NS (CYHZ) was told to taxi via D and hold short of Runway 23. A Jazz de Havilland DHC-8-402 (C-GGNW/JZA8635) on a flight from Halifax/Stanfield Intl, NS (CYHZ) to Ottawa/MacDonald-Cartier Intl, ON (CYOW) was rolling Runway 23 as EV121 was observed crossing the hold short line and entering the runway protected area. Tower controller instructed JZA8635 to abort takeoff and JZA8635 was able to stop prior to Taxiway D.
UPDATE TSB Report #A20A0005: C-GLHO a Beechcraft 1900D aircraft operated by Exploits Valley Air Services (EVAS) was conducting flight EV121 from Gander Intl (CYQX), NL to Halifax Stanfield Intl (CYHZ), NS, which was a maintenance repositioning flight with crew only. After landing on runway 14 the EVAS crew was cleared to taxi "Delta" to hold short of runway 23 and read it back. C-GGNW, a de Havilland Dash 8-400 aircraft operated by Jazz, was conducting flight JZA8635 from CYHZ to Ottawa Intl (CYOW), ON, with 78 passengers. As JZA8635 accelerated on runway 23 for departure the crew noted that EV121 did not stop as directed and continued as if to cross in front of the departing aircraft. JZA8635 applied braking just as the tower controller told them to reject the takeoff. The ground controller also called EV121 directing them to stop. JZA8635 stopped about 30 meters from the intersection, EV121 stopped just short of entering the active runway. The Jazz flight taxied back to runway 23 and departed for CYOW. EVAS has conducted an internal SMS review.

An Exploits Valley Air Services Ltd Beech 1900D (C-GLHO/EV751) from Charlottetown Airport, PEI (CYYG) to Halifax/Stanfield Intl, NS (CYHZ) was pulled up by the tower due to a target appearing on airport surface detection equipment (ASDE) indicating midfield on Runway 23. Halifax International Airport Authority (HIAA) was advised and the runway was inspected. Nothing was found. The aircraft landed with no further impact.

UPDATE to event: An Aviation Safety Program Analyst looked into this occurrence and "Incursion - manoeuvring area" was removed as an event.
An Exploits Valley Air Services Ltd. (C-GLHO/EV803) from Gander International, NL (CYQX) to Goose Bay, NL (CYYR) was instructed to taxi via Alfa and hold short Runway 03. The aircraft turned onto Taxiway Echo. The tower instructed the aircraft to stop but no response was heard or observed. A second call by the tower succeeded in stopping the aircraft.
UPDATE to narrative: An Aviation Safety Program Analyst looked into this occurrence and "An Exploits Valley Air Services Ltd. (C-GLHO/EV803)..." was replaced with "An Exploits Valley Air Services Ltd. Beech 1900D (C-GLHO/EV803)...".

Incident at ST. JOHN'S INTL NL (CYYT) (Aerodrome - runway or taxiway surface condition)

An Exploits Valley Air Services Beech 1900D (C-GLHO/EV7794) on a flight from Deer Lake, NL (CYDF) to St. John's, NL (CYYT) on final for Runway 29 was pulled up approximately 3 miles final due to a windrow on the centerline of the runway. Sweepers were advised to vacate Runway 29 when the aircraft was turning at 5 miles final. Sweepers indicated that there was a windrow on the center line as they were vacating the runway and asked to remove it prior to the arrival of the aircraft. Vehicles were instructed to continue vacating the runway as there was not enough time to clear the windrow prior to landing. Tower pulled the landing aircraft at 3 miles final.The windrow was subsequently cleared and the aircraft landed safely on their second approach.

An Exploits Valley Air Services Beech 1900D (C-GLHO/EV7756) on a flight from Halifax/Stanfield, NS (CYHZ) to Charlottetown, PE (CYYG) was cleared for an area navigation (RNAV) approach Runway 21. At 2315Z, CYYG flight service station (FSS) reported that the aircraft was conducting a missed approach. The pilot reported that the onboard RNAV system had failed and requested a VOR approach Runway 28. The flight was re-sequenced and cleared as requested and landed safely at 2337Z. No impact on operations.

Incident at ST. JOHN'S INTL NL (CYYT) (Aerodrome, runway or taxiway shutdown)

A United States Air Force McDonnell Douglas C-17 Globemaster II (RCH605) on a flight from (KTCM) to St. John's, NL (CYYT) landed Runway 29 at 1149Z and taxied to parking via Runway 16, Runway 20 and Foxtrot. Runways 29, 16, 20 and Foxtrot were all closed due to foreign object debris (FOD) inspection by the airport authority. An Exploits Valley Beech 1900D (C-GLHO/EV7783) on a flight form St. John's, NL (CYYT) to Gander, NL (CYQX) requested taxi for departure off Runway 29 from the terminal ramp at 1151Z. EV7783 was instructed to taxi into Golf and hold short Runway 34 until the FOD inspection was completed. EV7783 was delayed approximately 13 minutes before departing Runway 29 at 1204Z.

The controller observed EV7753 descending through 8000 feet and queried the pilot. Pilot confirmed that 8000 feet was the cleared altitude and corrected. The flights descended were as low as 7400 feet; no operational impact.

Incident at ST. JOHN'S INTL NL (CYYT) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

An Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7784) from Gander, NL (CYQX) to St John's, NL (CYYT) was 4 miles final on approach to Runway 16, STAFF 25 informed ground controller that there was possible foreign object debris (FOD) on Runway 16 at intersection of Runway 29 which required inspection. Runway 16 and Runway 29 were closed for inspection. EV7784 was told to level off on the approach and switch back to Gander centre due to runway closure. Runway 29 and Runway 16 were inspected and reopened. A German registered aircraft (DBHGN) was delayed on departure Runway 29 for approximately 10 minutes. EV7784, Air Saint-Pierre (SPM1128), Air Labrador (LAL8313), and Jazz (JZA8994) were delayed landing for approximately 10-15 minutes. All aircraft departed and landed Runway 29 after reopening with no further impact to operations.

A Canlink Aviation Diamond DA 20-C1 (C-FVBM) on a local flight from Fredericton, NB (CYFC) was pulled up to accommodate the backtrack for EV7778. No other operational impact.

On initial contact with CYQX centre flight was cleared to maintain FL120. At 0215Z, the Low Domestic controller noticed the flight climbing through FL143 on radar and requested the flight to confirm their altitude. Flight was then observed descending to maintain FL120. No traffic.

Incident at GANDER INTL NL (CYQX) (Aerodrome, runway or taxiway shutdown)

Following a second departure at 1443Z, an Exploits Valley Air Services Beech 1900D (C-GHLO / EV7803) from Gander, NL (CYQX) to St. John's, NL (CYYT) returned to CYQX due to another gear indication problem. No assistance required. The aircraft landed Runway 03 and advised that they were unable to taxi the aircraft. EV7803 exited Runway 03 at 1527Z via tow mule and Runway 03/21 was reopened following inspection. While on a 10 NM final for Runway 03, a Pal Airlines de Havilland DHC-8-300 (PVL921) had to be switched to Runway 13. (See CADORS 2016A0926 for first occurrence)

Incident at GANDER INTL NL (CYQX)

An Exploits Valley Air Services Beech 1900D (C-GLHO/EV7803) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) departed from Runway 13 at 1141Z, and shortly thereafter, advised that they were requesting to return CYQX due to a gear indication problem. The flight landed without incident at 1202Z. No emergency was declared and no other traffic was affected.
AOR 207641-V2: Shortly after departure, an Exploits Valley Air Services Beech 1900D (C-GLHO/EV7803) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) decided to return to CYQX reporting gear indications. No assistance was required. The aircraft landed on Runway 13 at 1201Z and remained on the runway indicating an issue with their nose gear. Maintenance support went to EV7803 on Runway 13 at approximately 1215Z and EV7803 taxied to apron at 1225Z. As a result, an inbound Exploits Valley Air Services (EV7783) was required to land on Runway 03.

An Exploits Valley Air Services Beech 1900D (C-GLHO/EV7811) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) departed Runway 31 at 1949Z and shortly thereafter, advised of suspected smoke in the cockpit and that they were returning to CYQX. The flight landed Runway 31 without incident at 2009Z. Aircraft Rescue and Fire Fighting (ARFF) was standing by but the pilot advised that no further assistance was required upon arrival. No other traffic was affected. The pilot later advised that the issue was vapour from the air conditioning unit.

At 2032Z, aircraft reported that landing gear had been locked down since departure. Aircraft requested to land at Gander, NL (CYQX) to have company mechanic address the issue. No emergency declared. No traffic, no assistance required. Flight landed safely at 2057Z.

Incident at ST. JOHN'S INTL NL (CYYT) (Aerodrome, runway or taxiway shutdown, Aerodrome - runway or taxiway surface condition)

An Air Saint-Pierre Cessna F406 (FOSPJ / SPM7448) from Saint-Pierre, Saint-Pierre and Miquelon (LFVP) to St. John's, NL (CYYT) landed Runway 34 at 1454Z and was given authorization for Taxiway Charlie. Controller issued a caution due 100% ice. Aircraft entered Charlie, and immediately slipped one hundred and eighty degrees. The pilot had to shut down the engines to await assistance from ground crews. The aircraft never left the manoeuvring surface and was however too near the intersection of Runway 34. Unable to use the Runway until the aircraft was removed. An Exploits Valley Air Services Beech 1900D (C-GLHO / EV7791) from St. John's, NL (CYYT) to Deer Lake, NL (CYDF) was ready for taxi, and was delayed approximately 25 minutes. No other impact to operations.

Incident at FRENN INTERSECTION

An Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7607) from Halifax, NS (CYHZ) to Fredericton, NB (CYFC) was inbound to CYFC cleared to 4000 feet. A Department of National Defence Bell 412 (WOLF11) from Gagetown, NB (CYCX) to Fredericton, NB (CYFC) was at 3000 feet IFR doing local activities around CYFC then enroute to Saint John, NB (CYSJ). Both aircraft were passed traffic and EV7607 advised they had the traffic in sight. The controller observed EV7607 descending through 4000 feet when passing traffic information to WOLF11. EV7607 had descended to 3500 feet and after being advised to maintain 4000 feet again started to climb.

Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

Update from Maintenance & Manufacturing: The flight crew reported that the landing gear lights did not illuminate during the approach. The aircraft was removed from service and inspected by maintenance. The annunciator control was found at fault and replaced. Gear swings and function checks were carried out. The system was tested serviceable and the aircraft was returned to service.
While on approach Runway 05, an Exploits Valley Air Services Beech 1900D (C-GLHO / EV7589) from Moncton, NB (CYQM) to Halifax, NS (CYHZ) advised the tower of an indicator problem and asked to circle to troubleshoot. 9 minutes later, aircraft advised they were inbound for landing but requested Emergency Response Services (ERS) on standby as a precaution. Aircraft landed safely at 1126Z. As a result, several arrivals were delayed up to 10 minutes.
UPDATE: TSB Report#A15A0007: The Exploits Valley Air Service Ltd. Beechcraft 1900D, registration C-GLHO, was operating as EV7589 on a scheduled flight from Moncton, NB to Halifax. NS (CYHZ). On approach to CYHZ the crew selected the landing gear down and noted that they had not received a landing gear down position indication. The crew elected to overshoot and coordinated with the CYHZ tower to hold so that they could troubleshoot the problem. The crew could see that the main landing gear was down, tested the down position lamps as serviceable and noted that the in-transit indication was extinguished. The crew then selected a manual extension to assure landing gear hydraulic system pressure was present and returned to CYHZ for a landing with ARRF requested to stand by. The flight landed without further incident. Company maintenance replaced an annunciator system printed circuit board and confirmed correct indication during functional testing. A service difficulty report was filed by the operator with Transport Canada.

A privately registered Cessna 172F on a local flight from Charlottetown, PE (CYYG) received airport advisory that included active Runway 03 and traffic. The aircraft advised they would hold short of Runway 03 on taxiway A. The aircraft was observed to have passed the hold line on taxiway A for Runway 03. The position of the aircraft was passed to a landing Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7582) from Halifax, NS (CYHZ) to Charlottetown, PE (CYYG). EV7582 landed Runway 03 without incident. Moncton, NB (CYQM) shift manager advised.

Incident at ST. JOHN'S INTL NL (CYYT) (FOD (foreign object debris), Aerodrome, runway or taxiway shutdown)

An Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7620) from Gander, NL (CYQX) to St John's, NL (CYYT) reported possible bird strike on landing. Airport authority personnel found an intact seagull scattered on Runway 29 near intersection Runway 16/34. Runway 29 closed until 1351Z. Air Canada (ACA688) delayed 7 minutes on arrival due to closure

Incident at In the vicinity of: FREDERICTON INTL NB (CYFC)

A Canlink Aviation Diamond DA 20-C1 (C-FSXA) on a local flight from Fredericton, NB (CYFC) was given an altitude restriction of 2000ft reference an Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7607) from Halifax, NS (CYHZ) to Fredericton, NB (CYFC) passing above him at 3000ft. C-FSXA was observed climbing through 2300ft and when queried by the tower controller, pilot confirmed they were at 2300ft.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT (Incursion - runway - vehicle)

An Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7590) from Halifax, NS (CYHZ) to Moncton, NB (CYQM). The aircraft was cleared to land at approximately 15nm final for runway 06. The landing clearance was cancelled shortly after due to a vehicle on the runway. The vehicle was instructed to vacate the runway. EV7590 was reissued a landing clearance at 12nm final. No impact on operations.

Incident at GANDER INTL NL (CYQX)

At 0830Z, EVAS dispatch called to say that they had an inadvertent ELT activation of an Exploits Valley Air Services Beech 1900D (C-GLHO) going off for approximately 20 minutes in their hanger. Nothing was heard on 121.5. At 0845Z, Halifax Rescue Coordination Centre called enquiring about an ELT hit on SARSAT at the airport. Tower advised them of C-GLHO's inadvertent activation and gave Halifax, NS (CYHZ) RCC the dispatch number for EVAS so they could follow up on the issue.

Incident at GOOSE BAY NL (CYYR)

An Exploits Valley Air Services Beech 1900D (C-GLHO/ EV7647) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) reported to Goose Bay RATCON that the aircraft had an unsafe gear indication and the pilot had to manually lower the landing gear for arrival. Aircraft landed Goose Bay at 1321Z without incident.

Incident at DEER LAKE NL (CYDF)

Gander ATC advised an Exploits Valley Air Services Ltd Beech 1900D (C-GLHO / EV7632) inbound to Deer Lake, NL (CYDF) from Goose Bay, NL (CYYR) accidentally set off their ELT 50 NM from St. Augustin, QC (CYIF) and could not shut if off until they landed. Aircraft arrived CYDF at 1915Z without incident. the pilot advised CYDF Flight Service Station (FSS) that they were unable to turn off ELT and requested permission to continue on to St. John's, NL (CYYT). Air Traffic Control and the Joint Rescue Coordination Centre (ATC/JRCC) advised OK and the aircraft departed CYDF at 1944Z.

An Exploits Valley Air Services Beech 1900D (C-GLHO / EV7643) from Gander, NL (CYQX) to Goose Bay, NL (CYYR) departed Runway 21 and reported a possible bird strike. The remains of one seagull were recovered. Information was relayed to the aircraft. No operational impact.
