Aviation Incident Report #17514: When the employee arrived on site at 1200Z (normal opening time at the airport), the access gate leading to the runway side was open and would not close again. After a check, it was found that 3 of the 4 plastic casters supporting the sliding gate were broken. The employ unsuccessfully attempted to force the gate to close. The only person on site when the employee arrived was the driver of a snow removal vehicle that was plowing snow.

Aviation Incident Report: Seattle Air Route Traffic Control Center (ARTCC) advised that an Air Canada Boeing 737 MAX 8 (ACA529) from Calgary, AB (CYYC) to Honolulu, HI (PHNL) reported a Level 1 disturbance on board due to four passengers failure to comply with the Transportation Security Administration (TSA) security directive on face masks. The aircraft diverted and landed at Vancouver, BC (CYVR) without further incident. The cockpit remained secure. A Law Enforcement Officer (LEO) met the aircraft at the gate. The four passengers were interviewed and allowed to exit.

At the gate, while passengers were exiting, a Jazz de Havilland DHC-8-402 (C-GKUK/JZA8445) from Vancouver, BC (CYVR) to Terrace, BC (CYXT) reported a passenger having a medical emergency. JZA8445 said that an ambulance was already called but requested assistance from the flight service station (FSS) to ensure the ambulance could get onto the apron to park beside the aircraft.

Incident at Kelowna

TSB Report #A22P0003: C-FYPB, a Swoop Inc. Boeing 737-8CT, was preparing for flight WSW419 from Kelowna, BC (CYLW) to Toronto/Lester B. Pearson Intl, ON (CYYZ) with 6 flight crew and 126 passengers on board. The aircraft was parked at the gate in CYLW and configured to be de-iced by Executive Aviation. The doors were closed, engines were off, and the APU was running. The de-ice truck was parked outside the aircraft clearance zone near the left wing with 1 operator in the bucket. The driver of the de-ice truck exited the vehicle to push another aircraft. Approximately 5 minutes later, the truck began rolling towards the aircraft and stopped when it collided with the left hand engine nacelle. The aircraft was deplaned and there were no reported injuries. The aircraft sustained damage to the outboard leading edge of the left-hand engine inlet cowl. Maintenance replaced the left engine inlet cowl and the aircraft was returned to service. The de-ice trucks braking system was inspected and function tested, with no faults found. Executive Aviation is conducting an internal investigation.

A Central Mountain Air Ltd. Beech 1900D (C-FCMN/GLR727) from Kelowna, BC (CYLW) to Prince George, BC (CYXS) went "no radio" (NORDO) after positioning on Runway 16. The aircraft squawked "7600". The aircraft exited the runway and returned to the gate. It was determined that the aircraft was "receiver only" (RONLY).
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. Maintenance staff were flown to the aircraft for investigation. "Comm 1" and "Comm 2" ground tested serviceable. Audio control panel replaced in accordance with (IAW) 31-10-00 for troubleshooting (T/S). Ground tested multiple times. Checked "okay". No other defect noted.

UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. During the take-off roll, the flight crew experienced a fuselage door indication. The flight crew rejected its takeoff and returned to the gate. The baggage door was cycled successfully. The aircraft departed, with no further issues.

Pearson, ON (CYYZ) declared that they could not confirm if the landing gear was down and initiated a missed approach. After actioning a checklist, WEN3651 announced they had the gear down, but would stop on the runway due to a loss of nose wheel steering and declared a "PAN PAN". Emergency equipment was put on stand-by and WEN3651 was vectored for Runway 23. WEN3651 landed uneventfully at 2231Z and the aircraft was subsequently towed to the gate. Several departures were delayed for approximately 15 minutes during this process.

UPDATE: TSB Report #A22C0002: C-GKQG, a de Havilland DHC-8-402 operated by Porter Airlines Inc., was on the ramp in Thunder Bay, ON (CYQT) after conducting flight POE639. A small fuel leak out of the right engine was reported while the aircraft was parked at the gate. The operators maintenance personnel attempted to stop the fuel leak by pulling the PULL Fuel Hydraulic Cut Off handle; however, fuel continued to leak. ARFF was dispatched and the aircraft was towed back to the hangar without further incident. The operators maintenance replaced the Fuel Metering Unit on the right hand engine and the aircraft was returned to service.
