Incident at CHURCHILL MB (CYYQ)

A 3097448 Manitoba Ltd. Cessna 208B (C-GAAX) on a flight from Churchill, MB (CYYQ) was taxiing for departure Runway 33 and passed as traffic to a Prairie Helicopters Bell 206B (C-FWGS) on a flight from Churchill, MB (Heli) (CHB2) and landing at Churchill, MB (Heli) (CHB2) north of the airport westbound along the shoreline for CHB2. The flight service station (FSS) attempted contact several times with C-FWGS to pass departing traffic with no response. C-GAAX had C-FWGS visual and departed with no incident. C-FWGS reported final and advised they could not hear FSS transmissions.

Incident at EDMONTON FIC / FIC D' EDMONTON (ATM - weather observation systems)

At approximately 2215Z, the Edmonton flight information centre (FIC) noticed the lightning layer on the Collaborative Flight Planning System (CFPS) was not working. Lightning was available on the Flight Service Station Weather Graphics System (FWGS) radar composite and Canadian Lightning Detection Network (CLDN) maps. Winnipeg/James Armstrong Richardson, MB (CYWG) FIC was experiencing the same problem. The Technical Operations Coordinator (TOC) was notified and also reported the outage to Operational Systems Requirements (OSR) in Ottawa. At approximately 0020Z, the data on CFPS returned to service.

Incident at NATIONAL (Canada) (ATM - inaccurate aeronautical information)

At 1305Z, a major weather information outage occurred and lasted until 25/0102Z. All weather information available via Flight Information Management System (FIMS), Flight Service Station Weather Graphics System (FWGS), Multipurpose Information Display System (MIDS), Automatic Data Acquisition and Processing System (ADAPS) and Aviation Weather Web Site (AWWS) unavailable during this period. Operational impact: preflight briefings and inflight services were provided using external websites; increased workload due to unavailability of AWWS.

Incident at KAMLOOPS BC (CYKA) (ATM - other)

At 0029Z, the FIC received a failed server message via the FIMS. TOC was advised. Servers returned to service at 0210Z. Failure affected all weather information on FIMS, PATWAS, FWGS and MIDS. Operational impact to the FIC during this outage was high due to the inability to provide current/updated data during weather briefings. (AOR 148901 is a duplicate of this event)

Incident at KAMLOOPS BC (CYKA) (ATM - inaccurate aeronautical information)

It was discovered that certain significant meteorological information (SIGMETs) were not available through the primary alphanumeric weather briefing source (FIMS) resulting in a failure to broadcast the SIGMETs and a potential that pilots were not provided with those SIGMETs during weather briefings. The discovery was made by a Specialist who referenced another weather source (FWGS). The SIGMETs that were missing were WSCN32 products and at the time of discovery A13 and C2 were valid. The FIMS log showed that the last WSCN32 SIGMET was received on Dec 19 at 2356z. The reason for the loss of data was due to a Supervisor removing Bulletin Filing Criteria entries from FIMS and then inadvertently entering the wrong product name upon restoring the Bulletin Filing Criteria entry. The reason for removal of the products in the first place was an attempt to restore the auto printing function of FIMS that was lost after the last version upgrade of FIMS. The removal and restoration of the Bulletin Filing Criteria entries has appeared to fix the auto print problem. There were no known hazardous occurences as a result.

NAV CANADA staff at London F.I.C. advised of a Metdec failure at London F.I.C., resulting in no weather information being available. They reported that the radar, satellite, METAR, TAF and all other alphanumeric systems were not updating on FWGS, FIMS or MIDS. The only weather available was on the NAV CANADA internet website. Ops. impact -- unknown.

Brandon FSS reported that C-FWGS, an AB-206 Longranger, operated by Prairie Helicopters did not report final for landing at the Country fest site 3 miles south Dauphin. Brandon FSS contacted CYDN RCMP to confirm that C-FWGS was on the ground safe at the Country fest site. C-FWGS advised on their next flight that they had reported final but must have been too low for FSS to receive it. No impact to operations.

Incident at QUÉBEC / JEAN LESAGE INTL QC (CYQB) (Aerodrome - power failure, ATM - other)

Following a power failure that occurred when the generator was being inspected from 0235Z to 0312Z, the Quebec City (CYQB) FIC lost use of the national voice communications system (NVCS), FSS Weather Graphics System (FWGS), the multi-purpose information display system (MIDS), the ACD and facsimile system as well as all frequencies. The impact on ATC operations is unknown. The Quebec City control tower also lost the use of the national voice communications system (NVCS), the Nav Canada auxiliary radar display system (NARDS), the extended computer display system (EXCDS), the World Wide TAAM User Group (TUG), the total airport and airspace modeller (TAAM) and the operational information display system (OIDS). With regard to the control tower, there was no impact on ATC operations due to the time of the incident and because there was very little traffic.
Update #2: According to 101502-V2, this event occurred when the uninterruptible power system (UPS) failed as a result of a scheduled maintenance.
Update #1: The OPI was changed to ANS and Airspace.

Pilots requiring Aviation Weather Briefing Service (AWBS) were referred to Edmonton FIC. FWGS became serviceable at 0125Z. The Kamloops FIC was unable to provide AWBS full service from 2250Z until 0125Z.

Edmonton FIC reported that some time between 2300z and 0000z the weather circuit failed. All current weather for NAV CANADA facilities in Northern Alberta, Saskatchewan and Manitoba was not available on MIDS, FWGS or AWWS website. Briefers were unable to provide up to date weather and altimeters to pilots during briefings or FISE contacts. The system was restored at 0200z.

Incident at KAMLOOPS BC (CYKA) (ATM - other)

The Kamloops FIC Weather Graphics System (FWGS) failed at 0445Z until 1630Z. All products (charts, RADAR, satellite) stopped being received by the FWGS system. Operators on the midnight shift noticed late charts at 0600Z but decided to wait (assuming that they were simply input late) as briefing traffic was virtually non-existent late at night. At 1145Z an operator called the NOC to confirm charts were being sent as normal. She then called the TOC to advise that no products were being received by the FWGS system. TOC logged the event but did not call out a technician. At 1430Z a technician reported for duty and began to address the problem. All AWBS briefing requests were referred to Edmonton FIC. At 1630Z the FWGS system was back on line at all positions. It took a couple of hours for most major products tp be updated in the data base. The technician advised that the problem originated with the CMOS clock on the system server. When it became incorrect, it went out of sync with the national server. No mitigation with reference to future occurrences could be suggested.

Loss of commercial power occurred at 1810z at Penticton. All systems in FSS failed (MIDS, FWGS,NVCS) for approx two minutes and required resetting. NICAN transceiver used for communications. Second power failure occurred at 1818z again for approx 2 minutes with same results requiring same FSS action.

Power failed at Penticton Airport at 1940z. FSS Voice switch, MIDS, MONCO, Wind, Altimeter, FWGS, NCAMS failed. Power restored after 10 minutes. Power failed again at 2005z for another 10 minutes, all systems again restored within 10 minutes. Further power failure occurred at 2045z, with systems restored after 10 minutes.

The St. Catharines (Niagara District) Airport (CYSN) experienced a power failure due thunderstorms. The power was out from 2103Z until 2255Z. Systems affected were: airport lighting panel, VHF DF, ceilometer, ATIS, RTI, clocks, NCAMS and FWGS computers. After one (1) hour and 22 minutes (1:22), the MIDS and AFTN circuit were shut down with contingency routing in effect. There was very little operational impact.
