Accident at KASKATTAMA, MB (A108)

TSB Report #A21C0020: C-FFVZ, a de Havilland DHC3T operated by Kississing Lake Lodge (dba Wings Over Kississing), was conducting a flight from Kaskattama, MB, to Gillam, MB (CYGX) with 1 pilot onboard. During the takeoff from Runway 12 at Kaskattama, C-FFVZ struck wind-hardened snowdrifts and the aircraft became airbone soon after. The flight continued uneventfully to CYGX. Upon landing in CYGX, it was noted that the tail wheel and ski were missing. The aircraft sustained substantial damage to its rear fuselage and rudder.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The companys Person Responsible for Maintenance (PRM) reported that, upon inspection, it was determined that all the damage was primarily sustained by the rudder and fuselage skin and the tail wheel assembly. The tail wheel assembly was replaced with a serviceable assembly and temporary repairs to the skin were made before the ferry flight. The aircraft was subsequently in maintenance at the Flin Flon facility, undergoing repairs. All the repaired rudder and empennage skins were being replaced with new skins. No other damages were found.

A Missinippi Air-Care Beech B200 (MA02) from Winnipeg/James Armstrong Richardson, MB (CYWG) to Flin Flon, MB (CYFO) rejected its take-off due to an indication. No emergency declared. No assistance required.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. The aircraft, a Missinippi Air-Care Beech B200 (MA02), rejected its take-off due to a right-hand (R/H) torque indication that became erratic upon application of take-off power. The pilot contacted maintenance, and Missinippi aircraft maintenance engineers (AMEs) went to Winnipeg/James Armstrong Richardson, MB (CYWG) to inspected the aircraft. AMEs found damaged wiring on a wire bundle that passes through a bulkhead on the engine mount. The wiring was repaired, engine runs were completed, and the torque indication checked as serviceable. The aircraft was released.

An inspection of the aircraft revealed the right propeller had struck a ramp light; severing a propeller blade tip.

CYFO airport staff confirmed that the Cessna 180 was still on the water, and the pilot of the aircraft informed FSS that the aircrafts radio had failed.

Later, a Perimeter Aviation Swearingen SA226-TC (C-GYRD/PAG07) from Flin Flon, MB (CYFO) to Winnipeg/Richardson, MB (CYWG) called to depart from Runway 01 and was given the landing Beech B200 as traffic. Aircrafts departure was delayed while attempting to verify the location of the landing aircraft.
