Incident at 20NM VAL-D'OR QC (CYVO)

A Nolinor Aviation Boeing 737-200 (NRL951) from Val-d’Or, QC (CYVO) to Montreal/Mirabel, QC (CYMX) decided to depart VFR from CYVO and obtain IFR clearance on takeoff. NRL951 flew to 12,500 ft without clearance and showed up on radar at 13,000 ft before getting IFR clearance. No impact on operations.

A Go Hélico Inc. Eurocopter EC120B (C-GLCN) took off from Quebec/Capitale Hélicoptère, QC (Heli) (CCH7) to conduct a tour of the city at an altitude of 1,400 ft. During the departure from the airport, the controller was no longer able to establish communication with the pilot. C-GLCN flew in the control zone for 10 minutes before reestablishing communication. Several VFR aircraft were flying over the city. A Sasair Cessna 152 (C-FCYU) had to be instructed to change its altitude to avoid the aircraft.

Despite being cleared for 1,600 ft and correctly reading back the clearance, a Propair Inc. Beech 1900D (PRO4601) from Geraldton, ON (Greenstone Regional) (CYGQ) to Montreal/Pierre-Elliott-Trudeau, QC (CYUL) continued its descent to 1,200 ft. Impact: PRO4601 flew below the minimum vectoring altitude (MVA) of the airspace.

Incident at HAVRE ST-PIERRE QC (CYGV) (Other operational incident)

Aviation Incident Report #17565: The mandatory instruction sign announced that Runway 08/26 was damaged while a Government of Canada, Department of National Defence Bell 412 helicopter taking part in the "Arctic Warrior" wintertime operation was hovering over Taxiway Alpha. The top of the sign, as well as the plastic panel for Runway 08/26 (terminal side), flew away. The sign for Runway 08/26 could not be immediately found. It is most likely that it was buried under the snow. No damage to the aircraft. No impact on flight operations. An investigation is currently taking place. It is assumed that the clasp that secured the top of the sign may have been improperly attached, and that it was therefore not working properly to secure the top cover with a certain degree of tension.

Numerous contact attempts were made on the mandatory frequency (MF), as well as various other frequencies on portable radio. Comox, BC (CYQQ) was advised. The pilot contacted the CYQQ tower when 6NM east of CYBL.

Incident at COURTNEY AIRPARK BC (CAH3) (FOD (foreign object debris))

Aviation Incident Report #17498: A flock of ducks flew into the path of a privately registered Cessna 150H from Courtenay Airpark, BC (CAH3) to Courtenay Airpark, BC (CAH3) shortly after it left the runway. The aircraft made contact with a number of them. After impact, the pilot checked their instruments to ensure everything was operating correctly, climbed to approximately 400 feet, and made a left crosswind turn to join the circuit and land on Runway 13. After landing, the pilot witnessed airpark members cleaning the debris off the runway, which included 4 deceased ducks and the shattered lens cover from the aircrafts landing light. The aircraft sustained damage to their landing light lens cover, a dent in the leading edge of the port side wing, duck debris on the nose wheel, and some staining on the propeller.

Accident at ABBOTSFORD BC (CYXX) (Aerodrome, runway or taxiway shutdown)

A privately registered Mooney M20C from Salmon Arm, BC (CZAM) to Chilliwack, BC (CYCW) flew "no radio" (NORDO) into the Abbotsford, BC (CYXX) control zone (CZ). The aircraft landed on Runway 01 with its gear up. Undercarriage and propeller were damaged. Emergency vehicles approached the aircraft after it stopped on the runway. Operational Impact: multiple aircraft were moved out of the way. Runway closed.

A Toronto Airways Cessna 172S (C-GFFQ) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) was cleared for take-off. The pilot then informed the tower that their engine had shut down. A privately registered Cirrus SR22 from Peterborough, ON (CYPQ) to Toronto/Oshawa, ON (CYOO), on final, was instructed to conduct an overshoot, and a Durham Flight Centre Inc. Cessna 1721 (C-GBMK) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO) flew a full field circuit. The pilot was able to restart the engine and taxied back to their apron. No substantial impact on operations.

A Memento Mori Investments Limited Robinson R44 (C-GVBO) from Toronto/Buttonville, ON (CYKZ) to Toronto/Billy Bishop, ON (CYTZ) flew too close to a 2141409 Ontario Inc. Cessna 172M (C-FGRI) from Burlington, ON (CZBA) to Burlington, ON (CZBA). C-FGRI was towing a banner and C-GVBO was instructed to pass behind, and did have C-FGRI in sight. It appeared on radar as if they would not pass behind, so the pilot was questioned. No confirmation was given, so C-FGRI was instructed to do an orbit, which they replied to but did not action. C-FGRI also had C-GVBO in sight and said they had to descend to avoid a potential collision. C-GVBO claimed to have passed above the banner and behind the aircraft, with traffic in sight at all times. C-FGRI said it was unacceptably close and had to descend to avoid C-GVBO.

Incident at KERPU intersection

A Brampton Flying Club Cessna 172R (C-GNUE) from Brampton-Caledon, ON (CNC3) to Toronto/Oshawa, ON (CYOO) departed from CNC3 and requested flight following to CYOO via the city centre. C-GNUE was not visible to the controller and was assigned a code, which was read back. C-GNUE appeared as a "chicken foot" as it entered Class C airspace. The transponder for C-GNUE finally appeared as the aircraft was crossing through the localizer for Runway 05, with an Icelandair Boeing 737 MAX 8 (ICE37B) from Reykjavik/Keflavik, Iceland (BIKF) to Toronto/Lester B. Pearson, ON (CYYZ) on final. The controller passed traffic to C-GNUE and instructed them to descend. The tower passed VFR traffic to ICE37B. The tower passed traffic to an Air Canada Rouge Airbus A321-211 (ROU1657) from Tampa, FL (KTPA) to Toronto/Lester B. Pearson, ON (CYYZ) and C-GNUE continued southbound and crossed the localizer for 06L, with ROU1657 on final. No other operational impact.
UPDATE: TSB Report #A22O0004: C-GNUE, a Cessna 172R aircraft owned by Brampton Flying Club, was on a rental round-trip visual flight rules (VFR) flight from Brampton-Caledon, ON (CNC3), with a stop at the Oshawa, ON (CYOO) airport via the Toronto City Center, with one pilot and one passenger onboard. At 2301, the Toronto Terminal provided the occurrence pilot with a transponder code, and the pilot readback the correct code. The aircraft then entered Toronto Pearson International, ON (CYYZ) Class C controlled airspace without Air Traffic Control (ATC) clearance. At that point, the transponder code was not appearing on the ATC radar. The aircraft continued, crossing eastbound through the localizer of Runway 05 at CYYZ, where it appeared on ATC radar. The aircraft flew in front of flight ICE37B, a Boeing 737 MAX 8 aircraft on final approach for Runway 05, resulting in a minimum separation of 300 feet vertical and 1 nm lateral. Subsequently, the aircraft crossed the localizer for Runway 06L at CYYZ in front of flight ROU1657, an Airbus A321 aircraft on final approach, resulting in a minimum separation of 900 feet vertical and 0.3 nm lateral. After crossing the localizer for Runway 06L, the occurrence pilot exited the CYYZ control zone under the direction of the Toronto Terminal, and the flight continued without further incident.

TSB Report #A22F0016: C-GMIU, an Air Canada Boeing 737 MAX 8, was conducting flight ACA1054 from Vancouver Intl, BC (CYVR), to Phoenix/Sky Harbour Intl, AZ (KPHX). During cruise, in vicinity of Portland Intl, OR (KPDX), the flight crew experienced a failure of the Left Inertia Reference System (IRS), the autopilot, and the autothrottle. Flight crew did not declare an emergency, descended to FL270 and hand flew the aircraft for the remainder of the flight to KPHX, where it landed without further incident. Maintenance reset the Inertial Reference Unit (IRU) and performed a Built-In Test Equipment (BITE) test on the IRUs, with no fault found.

A Buffalo Airways Ltd. Beech 100 (BFL101) from Red Deer, AB (CYQF) to Yellowknife, NT (CYZF) entered the Edmonton terminal Class C airspace at 10,500 ft without authorization. Air traffic control (ATC) attempted to contact the aircraft, with no answer. Aircraft flew through entire terminal area without contact or authorization.
