At Vancouver/Boundary Bay, BC (CZBB), a vehicle without a beacon was reported maneuvering on Taxiway Echo by an aircraft in the run-up bay for Runway 25. The vehicle was observed by the tower controller crossing Runway 25 via Echo and Foxtrot for Apron 3. The vehicle was then observed leaving the airport property. No impact on operations.

Incident at TORONTO / BILLY BISHOP TORONTO CITY AIRPORT ON (CYTZ) (Security acts, Incursion - manoeuvring area, Incursion - runway - pedestrian)

At Toronto/Billy Bishop, ON (CYTZ), a pedestrian entered the airfield from the North seawall, walked across Taxiway Alpha, along Taxiway Echo, and entered the protected area of active Runway 08, before being intercepted by the Aircraft Rescue and Fire Fighting (ARFF) unit and airport security. Airport security detained the pedestrian along Taxiway Alpha and the North seawall until the marine police unit arrived to take them into custody. No impact to operations.

Immediately after takeoff, a 7506406 Canada Inc. Agusta AW139 (C-GYNH/LF4) on a Medevac flight from Ottawa/MacDonald-Cartier Intl, ON (CYOW) landed on Taxiway Echo, where they had just taken off from, due to a hydraulic problem. Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW)

After receiving approach clearance and being switched to the tower, a Mesa Airlines Bombardier CL-600-2C10 (ASH6042) from Washington-Dulles, VA (KIAD) to Ottawa/MacDonald-Cartier, ON (CYOW) never called the Tower nor responded to numerous calls. ASH6042 landed Runway 25, exited on Echo and taxied to its gate without talking to the Tower or Ground at all. No impact.

Incident at YELLOWKNIFE NT (CYZF) (ATM - other)

Ground was working, cross-coupled, with the tower frequency. An Air Tindi de Havilland DHC-6-300 (TIN230) from Yellowknife, NT (CYZF) to Whati, NT (CEM3) was given taxi clearance to taxi via Taxiway Bravo and Charlie for Runway 28 at 2207Z. At 2208Z, there were 2 to 3 calls that stepped on each other and a Buffalo Airways Curtiss-Wright C46 (BFL310) from Yellowknife, NT (CYZF) to Hay River/Merlyn Carter, NT (CYHY) taxied from Delta onto Bravo without clearance for Runway 28. A privately registered Cessna A185E was also calling the tower at 2208Z, ready for departure off of Long Lake. BFL310 was instructed to hold short of Bravo and requested details for their flight. Impact: A truck (RED 1) proceeding from Bravo to Delta, and an Air Tindi Cessna 208 (TIN212) from Wekweeti, NT (CYWE) to Yellowknife, NT (CYZF) that was taxiing on Bravo to Echo, needed to give way.

The Aircraft Rescue and Fire Fighting (ARFF) unit found a small hawk dead on the runway. The aircraft was advised.

A 8348944 Canada Inc. Robinson R44 II (C-FJUC) on a VFR flight from Quebec/Jean Lesage Intl, QC (CYQB) to Quebec/Jean Lesage Intl, QC (CYQB) 4 NM east of the airport declared a PAN PAN due to an erratic engine. Aircraft Rescue and Fire Fighting (ARFF) services were notified. The aircraft landed on Taxiway Echo without a problem at 1355Z. Echo was closed until 1430Z.

A vehicle (P44) was instructed to taxi via Taxiway ECHO and DELTA, cross Runway 29, and Taxiway GOLF, and hold short of Runway 06. P44 instead taxied via Taxiway ECHO and Runway 06/24. Impact: a 9385-2911 Québec Inc. Cessna 150J (C-GOMQ) flying to Quebec/Jean Lesage Intl, QC (CYQB) was instructed to overshoot Runway 06. No other impact.

Accident at 5NM NE VALCOURT QC (CSQ3)

Missing aircraft: A Cargair Ltd. Cessna 172M (C-GSEN) took off from Montreal/Mirabel, QC (CYMX) to conduct approaches at Sherbrooke, QC (CYSC). The aircraft never arrived at CYSC. The last radar echo was at 0146Z. The Joint Rescue Coordination Centre (JRCC) in Trenton was notified. Search and rescue operations were in progress. On September 7, 2019, the Joint Rescue Coordination Centre (JRCC) in Trenton confirmed it had found aircraft C-GSEN at 1718Z northwest of Lake Brompton, Estrie (QC). The pilot had died. Sûreté du Québec (SQ) was on the scene. The Transportation Safety Board of Canada (BST) was notified and went to the accident site. NAV CANADA and the National Operations Centre (NOC) were notified.
Update: TSB Report #A19Q0153: A Cessna 172 (C-GSEN) operated by Cargair Ltd. was conducting a night VFR flight from Montreal/Mirabel (CYMX), QC, to Sherbrooke (CYSC), QC, with one pilot on board. While the aircraft was cruising at 3,500 feet approximately 20 NM to the west of CYSC, it quickly lost altitude, collided with trees and crashed into the ground. The pilot was fatally injured and the aircraft was destroyed.

A privately registered, amateur-built Wheeler Express from Chicoutimi/Saint-Honoré, QC (CYRC) to Chicoutimi/Saint-Honoré, QC (CYRC) received and read back the instruction to taxi on ECHO up to Runway 12 and to contact the tower when it was short of Runway 36, which was in service. Air traffic services (ATS) saw the aircraft crossing the Runway 36 stopline. Impact on operations: ATS asked a Beech 23, on short final for Runway 36, to overshoot.

A privately registered, amateur-built Wagaero Cuby Sport Trainer from Chicoutimi/Saint-Honoré, QC (CYRC) to Chicoutimi/Saint-Honoré, QC (CYRC) received and read back the instruction to taxi on Echo up to Runway 12 and hold short of Runway 36. Air traffic services (ATS) saw the aircraft cross the Runway 36 stopline. No impact on operations.

Incident at MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL) (Aerodrome - runway or taxiway surface condition, Aerodrome, runway or taxiway shutdown)

Although it received and read back the instruction to clear Runway 06L via Taxiway B1 after landing, a Jazz Bombardier CL-600-2B19 (series 100) (C-GUJA/JZA8512) from Charlottetown, PE (CYYG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) did so by exiting via Taxiway E; it took a high-speed exit. Impact on operations: JZA8501 was instructed to reject its approach on Runway 06L and was reassigned to Runway 06R for its landing. Runway 06L was closed from 1113Z to 1242Z. Due to poor weather conditions and to the state of the runways surface, Runway 06R was closed from 1236Z to 1243Z for snow removal. The following departures were delayed: TSC954, ROU1986, and ROU1842. The National Operations Centre (NOC) and the Transportation Safety Board of Canada (TSB) were notified.
Update - TSB Report #A19Q0001: C-GUJA, a Jazz Aviationi LP Bombardier CL-600-2B19 CRJ200, was on instrument flight rules (IFR) flight JZA8512 from Charlottetown, PE (CYYG) to Montreal/Pierre Elliott Trudeau, QC (CYUL) with 3 crew members and 49 passengers on board. At landing on Runway 06L at CYUL, air traffic control (ATC) instructed the flight crew to exit on Taxiway B1. Although the instruction was read back correctly, the pilot-in-command steered the aircraft towards Taxiway Echo, which was closed by NOTAM. When he noticed this error, the pilot-in-command applied the brakes with the intention of bringing the aircraft onto Runway 06L. However, a layer of ice covered the ground within the protected area on Taxiway Echo, which prevented the aircraft from braking effectively and turning. The aircraft ended its run on the lawn. Runway 06L was closed temporarily to allow for the towing. The aircraft did not sustain any damage on the landing gear, and no one was injured.

Incident at LANGLEY REGIONAL BC (CYNJ)

A Skyquest Aviation Cessna 152 (C-GTYQ) from Langley, BC (CYNJ) to Langley, BC (CYNJ) was issued taxi instructions but refused the instruction and held their position due to traffic that was short final for Runway 25.
UPDATE from Flight Operations: A Civil Aviation Safety Inspector (CASI) looked into this occurrence. After landing on Runway 25 at CYNJ, C-GTYQ was advised by tower to "taxi Golf, hold short Runway 19", which was read back by the pilot. The controller then issued a landing clearance to an AAA Aviation Ltd. Cessna 172I (C-GUNE) on Runway 25. C-GTYQ was then cleared by tower to “cross Runway 19, taxi Echo east apron”. The pilot of C-GTYQ refused the clearance due to traffic on short final for Runway 25. The controller then advised that C-GTYQ would not be entering Runway 25 and again cleared C-GTYQ to “cross Runway 19, taxi Echo east apron”. The pilot of C-GTYQ again refused the clearance due to traffic on short final for Runway 25. The controller then instructed C-GTYQ to hold short of Runway 19. After C-GUNE landed, C-GTYQ was cleared to cross Runway 19.

Cessna 172R (C-GZPK) from Victoria, BC (CYYJ) to Vancouver/Boundary Bay, BC (CZBB) was instructed to taxi from Apron 3 via Sierra, Echo, and to hold short of Runway 03 (owned by ground). Aircraft was observed by the ground controller crossing the eastern Runway 03 hold line on Echo. Controller instructed aircraft to continue across Runway 03 and stop on Echo short of Runway 14. No impact to operations.

Accident at VANCOUVER / BOUNDARY BAY BC (CZBB) (Aerodrome, runway or taxiway shutdown)

A privately registered Harvard 4 from Vancouver/Boundary Bay, BC (CZBB) to Vancouver/Boundary Bay, BC (CZBB) was landing on Runway 07. After landing, the aircraft became airborne again and crashed just East of the airport, near the threshold of Runway 25 and Taxiway Echo. Emergency vehicles responded to the scene. No injuries reported. Runway 07/25 was unavailable, and there was significant impact to operations.
Update TSB #A19P0104: a privately operated Harvard 4 aircraft, was conducting a local flight from Vancouver/Boundary Bay (CZBB), BC with 1 pilot and 1 passenger on board. During landing, the aircraft touched down long, overran the runway, and impacted the airport perimeter fence. There were no injuries. The aircraft sustained substantial damage.

A SkyQuest Aviation Cessna 172M (C-GCKA) from Langley Regional, BC (CYNJ) to Langley Regional, BC (CYNJ) veered off into the grass on the left side of Runway 01 while attempting to do a touch and go. The aircraft drove on the grass for approximately 200ft before regaining control of the aircraft and informing tower they would exit the runway at echo and taxi around for another attempt at the circuit.

Incident at VICTORIA INTL BC (CYYJ)

An American, privately registered Cessna 150L from Victoria, BC (CYYJ) to Duncan, BC (CAM3) obtained VFR departure instructions from Victoria clearance delivery. Several minutes later, ground control observed the aircraft taxiing westbound along Taxiway Echo without a clearance, . Ground control called the aircraft on 121.9 and instructed the pilot to hold short of Runway 14, which was active at the time. The pilot read back the hold short instruction and held short of Runway 14. The aircraft departed Runway 09 without further incident. No impact on other operations.

TSC588 did not turn onto Taxiway Echo and continued taxiing on Taxiway Golf, onto Runway 09. No traffic was on the approach or departing. TSC588 was backtracked from Taxiway Golf for departure from Runway 09.

South Ground cleared a Sky Regional Airlines Embraer ERJ 170-200 LR (SKV7682) from Chicago/OHare, IL (KORD) to Toronto/Lester B. Pearson, ON (CYYZ) to taxi via Bravo to hold short of Victor. A United Airlines Boeing 737-800 (UAL459) from Toronto/Lester B. Pearson, ON (CYYZ) to San Francisco, CA (KSFO) called outbound at AL for taxi and was issued taxi via Tango, cross Runway 33R and hold short of Echo. No traffic was passed to either aircraft. UAL459 had to brake to avoid the conflict with SKV7682. Action taken with controller. No further impact to operations.

Incident at OTTAWA / MACDONALD-CARTIER INTL ON (CYOW) (Aerodrome - runway or taxiway surface condition, Aerodrome, runway or taxiway shutdown, ATM - other)

At the Ottawa/Macdonald-Cartier, ON (CYOW) aerodrome, during a controller change, the controller who was replaced did not inform their colleague that Taxiway Echo had been closed by NOTAM due to water that had pooled on it. The ground controller cleared an Air Canada Airbus A320-211 (C-GQCA/ACA1406) from Ottawa/Macdonald-Cartier, ON (CYOW) to Punta Cana, Dominican Republic (MDPC) to taxi on Echo. ACA1406, which indicated that it could cross the flooded zone, was escorted by a vehicle so that it could do so.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Incursion - manoeuvring area)

TSB Report #A19O0006: C-FEJB, an Embraer ERJ 170-200 SU, (operated by Sky Regional Airlines, flight 7665) was departing from runway 06L at Toronto/LBPIA (CYYZ) during snowy weather conditions. At the same time, a convoy of snow plows /sweeper vehicles were operating north of runway 06L and proceeding southbound on taxiway Echo. ATC instructed the vehicles to turn left on to taxiway Charlie (east), however the vehicles turned left on Charlie 2 (C2) towards Runway 06L and continued past the hold short position. Ground control instructed the vehicles to STOP, and they came to rest just prior to 06L. The tower controller instructed C-FEJB to abort; the flight crew rejected the take-off and the aircraft came to rest just prior to C2.

At the Toronto/Billy Bishop, ON (CYTZ) aerodrome, all airport lighting was turned off by electricians at 0359Z without notice. A Pascan Pilatus PC-12/47E (C-GJVM/PSC495) from Saint Hubert, QC (CYHU) to Toronto/Billy Bishop, ON (CYTZ) was exiting Runway 26 onto Taxiway Echo at the time. Visibility was 1/2 statue mile (SM) in -RA and mist. A staff vehicle had to come out and escort the aircraft. No impact to operations.

An Air Canada Airbus A320 (ACA673) from Halifax/Stanfield, NS (CYHZ) to Calgary, AB (CYYC) was given a taxi routing of CYYC Taxiway Juliet to hold short of Juliet Romeo, which was correctly readback by the pilot. However, ACA673 taxied via Echo. After scanning, the controller noticed the deviation and routed ACA673 onto Juliet via Juliet Quebec. No impact to operations.

A WestJet Encore de Havilland DHC-8-402 (C-GENU/WEN3441) from Moncton/Romeo LeBlanc, NB (CYQM) to Toronto/Lester B. Pearson, ON (CYYZ) was given taxi instruction to taxi via Echo, Runway 24 to hold short of Runway 11 and the aircraft read back the same. Incident at HALIFAX / STANFIELD INTL NS (CYHZ)

An Air Canada Airbus A320-211 (C-FDRP/ACA611) from Halifax/Stanfield, NS (CYHZ) to Toronto/Pearson, ON (CYYZ) was given line up instruction for Runway 23 at Taxiway Echo and started to backtrack on Runway 23 from Taxiway Echo. The tower questioned the pilot and they advised they were turning around and were ready to depart.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT

A Moncton Flight College Diamond DA 20-C1 (C-FNDC) from Moncton/Romeo LeBlanc, NB (CYQM) to Moncton/Romeo LeBlanc, NB (CYQM) was told to taxi in Echo to hold short of Runway 24 and read back the instruction. The aircraft then proceeded to cross Runway 24. No impact on operation other than a slightly delayed landing clearance for another Diamond DA20 (DV20) on final.

Incident at GREATER MONCTON ROMEO LEBLANC INTERNATIONAL AIRPORT

A Kalitta Charters II Boeing 747-4HQF (N782CK/CKS962) from Chicago/OHare, IL (KORD) to Moncton, NB (CYQM) struck its left side outboard engine on a snow bank while taxiing on the main apron. No impact on operations.
Update TSB Report #A19A0014: N782CK, a Boeing 747-400 cargo aircraft operated by Kalitta Air, was conducting flight CKS962 from Chicago/O’Hare Intl (KORD), IL to Greater Moncton Romeo LeBlanc Intl (CYQM), NB with 7 crew members on board. After the landing on Runway 24 at CYQM, the aircraft taxied onto Taxiway Echo to the apron. As the aircraft entered the apron, the flight crew deviated off the taxi lane during the left turn onto the apron. The number one engine then contacted a snow bank, which caused damage to the engine. There were no injuries and no impact to airport operations. The subsequent flight was cancelled. Maintenance personnel inspected the aircraft, and the operator elected to ferry the aircraft on 3 engines the following day.

A Jazz Bombardier CL-600-2B19 (series 100) (C-GMJA/JZA119) from Baltimore-Washington, MD (KBWI) to Montreal/Pierre Elliott Trudeau, QC (CYUL) on final for Runway 06L, conducted a go-around at the towers request, because the previous arrival, an Air Creebec de Havilland DHC-8-106 (C-GXCN/CRQ928) from Chibougamau/Chapais, QC (CYMT) to Montreal/Pierre Elliott Trudeau, QC (CYUL), had started to exit via Echo (which was closed).

Incident at MONTRÉAL INTL (MIRABEL) QC (CYMX) (Incursion - runway - vehicle)

An Atlas Air Boeing 767-200 (N657GT/GTI906) from Montreal/Mirabel, QC (CYMX) to Covington/Cincinnati, KY (KCVG) headed from Apron Cargo to the de-icing station via F, H, I, and E and crossed J. A vehicle (MV6+E), which was cleared to taxi on all taxiways, was informed of the route that GTI906 had planned to take, and the vehicle confirmed that it had yielded to the aircraft, as planned in the agreement with Aéroports de Montréal (ADM). While GTI906 was preparing to turn onto Echo, a snow-removal vehicle crossed in front of the aircraft on J. GTI906 indicated that it was forced to stop.
