Incident at VAL-D'OR QC (CYVO)

A Propair Beech 100 (PRO4600) from Rouyn-Noranda, QC (CYUY) to Val-DOr, QC (CYVO) mentioned a problem with the landing gear indicator and requested to conduct a low approach so that the flight service station (FSS) on duty could verify the situation. The aircraft then landed without incident. No impact on operations.

At the Montreal/Mirabel, QC (CYMX) aerodrome, there was no control over the runway centreline lights. One out of two lights stayed lit permanently on level 3. The airport duty manager (ADM) was informed. The problem was resolved by an electrician at 0238Z.

Incident at 10NM RIVIÈRE-DU-LOUP QC VOR (YRI)

An Air Canada Boeing 787-9 (C-FKSV/ACA875) from Frankfurt, Germany (EDDF) to Montreal/Pierre Elliott Trudeau, QC (CYUL) reported having an issue with their hydraulic system on the right side. They requested emergency vehicles at CYUL airport upon arrival. Runway 24R was closed by NOTAM, therefore it was re-opened temporarily by the Aéroports de Montréal (ADM). ACA875 landed safely at 1508Z.

Update TSB #A19F0146: C-FKSV, a Boeing 787-9 aircraft operated by Air Canada, was conducting flight ACA875 from Frankfurt/Rhein-Main (EDDF), Germany to Montreal/Pierre Elliot Trudeau Intl (CYUL), QC with 10 crew members and 243 passengers on board. During cruise flight at FL380 in the vicinity of N57/W025, the aircraft experienced a lost of the right hydraulic system pressure. The flight crew initially declared a PAN PAN as they were preparing to divert to London/Heathrow (EGLL), UK. However, it was subsequently determined that the issue was not affecting the safety of the flight or the ETOPS operation, as the system pressure and quantity was operating normally with the Electric Driven Pump on. The PAN PAN was cancelled, and the flight continued to CYUL without further incident. The operators maintenance found that the Engine Driven Pump (EDP) was leaking, and proceeded to replace it.

The airport duty manager (ADM) was notified.

A Bombardier Inc. Bombardier BD-500-1A11 (CS300) (BBA530) from Montreal/Mirabel, QC (CYMX) to Montreal/Mirabel, QC (CYMX) exited via Taxiway T and reported the presence of a foreign object (a plastic bag). The aircraft came to a stop and informed air traffic services (ATS). The airport duty manager (ADM) was notified. BBA530 started taxiing again approximately 10 minutes later, once the foreign object was removed.

TSB Report #A19P0029: C-FZRM, a Boeing 737-800 aircraft operated by WestJet, was conducting flight WJA2152 from Vancouver Intl (CYVR), BC to Puerto Vallarta/Licenciado Gustavo Díaz Ordaz Intl (MMPR), Mexico with 6 crew members and 128 passengers on board. Approximately 2 hour and 50 minutes into the flight, a cabin crew member became ill and was unable to continue duties. The affected cabin crew member’s duties were reassigned to the remainder of the cabin crew for the rest of the flight. Upon arrival, the affected cabin crew member was transported to hospital by EMS.

During cruise flight at FL390 in the vicinity of Kamloops (CYKA), BC, one of the flight crew members became incapacitated and was unable to perform flying duties. A cabin crew member assisted the pilot to the rear of the aircraft, while MedLink was consulted. The flight continued to CYEG where it landed without further incident. The aircraft was met at the gate by Emergency Medical Services (EMS). The pilot was assessed, and released shortly thereafter.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ) (Aerodrome, runway or taxiway shutdown)

An Air Georgian Bombardier CL-600-2B19 (series 100) (C-GZJA/GGN7209) from Toronto/Lester B. Pearson, ON (CYYZ) to Savannah Intl, GA (KSAV) advised Cleveland Air Route Traffic Control Center (ZOB) they were requesting return to CYYZ due to an equipment issue. On first contact it was determined that GGN7209 was experiencing an unspecified hydraulic issue, no emergency declared. YYZ Tower initiated a 1C for their arrival with Aircraft Rescue and Fire Fighting (ARFF) in attendance. GGN7209 landed safely at 1721Z on a non-duty runway. The runway was inspected and returned to service (RTS) at 1740Z. No impact on operations.

A privately registered Piper PA-23-250 from Delhi, ON (CDH6) to Toronto/Billy Bishop, ON (CYTZ), while on its VFR flight enroute to CYTZ, came within close proximity of a Panorama Fixed Wings Ltd. Pilatus PC-12/45 (PNO782) from Hamilton, ON (CYHM) to Montreal/Trudeau, QC (CYUL) while the IFR flight was departing from CYHM. The Piper PA-23-250 transited the CYHM zone and conflicted with the departing PNO782. No further impact to operations. Controller removed from duty.

Incident at SUDBURY ON (CYSB)

A privately registered Cessna 180H landed on Moose Lake, and when the pilot called at 0054Z to give their arrival report, they reported having to descend when entering the Sudbury, ON (CYSB) control zone (CZ) as there was opposite direction traffic already in the CZ at 2200. The pilot stated that they caught part of a traffic broadcast on 122.8 which seemed to correspond to the traffic they were referring to, but did not get a call sign or response on 122.8. The Cessna 180H landed without further incident. No traffic was observed by the specialist on duty (either visually or on radar), and no contact was made with the Sudbury (YSB) flight service station (FSS) by the offending aircraft.

Incident at TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

A Flair Airlines Boeing 737-400 series (FLE133) from Toronto/Lester B. Pearson, ON (CYYZ) to Winnipeg/James Armstrong Richardson, MB (CYWG) aborted takeoff of Runway 06L. The aircraft rolled long and exited the runway at Taxiway D5. The South Ground taxied FLE133 along Taxiway D, at which point the aircraft waited for approximately 10 minutes and then asked if aircraft rescue and fire fighting (ARFF) would check their brakes. The aircraft did not declare an emergency. The Airline Operations Control Centre (AOCC) and Greater Toronto Airports Authority (GTAA) Duty Manager were informed and vehicles RED21 and RED5 attended the aircraft. FLE133 began taxiing back to the gate at 0043Z with both ARFF vehicles following.
UPDATE from Airworthiness: A Civil Aviation Safety Inspector (CASI) looked into the occurrence. The take-off was aborted for a master caution light. There were no other indications. The following actions were taken by maintenance: Log 120064: Hight energy stop/heat damage inspection performed in accordance with (IAW) Aircraft Maintenance Manual (AMM) 05-51-07. No faults found. First office (F/O) and Captain master caution lights relamped and cleared IAW AMM 33-15-00. Checked serviceable, quick access recorder (QAR) data sent and evaluated, no issues found as per Maintenance Control Centre (MCC) Calgary. There has been no recurrence.

Shortly after the departure from LGKR, a cabin crew member became sick. The flight crew contacted Medlink, and a nurse came forward to assist. The medical condition of the cabin crew member improved through the flight, however duties could not be performed. Within the next 10 minutes, the flight crew member started to feel ok and was able to resume duties of Pilot Monitoring (PM). The aircraft landed at MUCC without further incident. In case of a re-occurrence during the return portion (SWG159) from MUCC to CYFC, it was decided that the affected flight crew member would carry out the same duties (PM). The affected flight crew member felt unwell again for a brief period during the return flight, which resulted in a state of fatigue. Controlled rest was used, in conjunction with careful hydration. Prior to the arrival at CYFC, the affected flight crew member assumed PM duties, and the flight landed without further incident.

TSB Report #A19F0051: C-GXAX, a Boeing 737-8 MAX aircraft operated by WestJet, was conducting flight WJA1118 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Las Vegas/McCarran Intl (KLAS), NV with 6 crew members and 160 passengers on board. During cruise flight, a cabin crew member sustained a medical incident and was unable to continue duties. Duties were distributed to the remaining cabin crew members for the landing.

TSB Report #A19F0045: C-GHLV, a Boeing 767-300 aircraft operated by Air Canada rouge, was conducting flight ROU1658 from Toronto/Lester B. Pearson Intl (CYYZ), ON to Tampa Intl (KTPA), FL with 8 crew members and 261 passengers on board. As the flight was preparing to begin the descent into KTPA, a cabin crew member became incapacitated due to illness. A qualified employee travelling as a passenger on the flight was assigned the duties for the landing, and the aircraft landed at KTPA without further incident.

TSB Report #A19F0039: C-GUBD, an Airbus 330-200 aircraft operated by Air Transat, was conducting flight TSC560 from Montreal/Pierre Elliott Trudeau Intl (CYUL), QC to Cancun Intl (MMUN), Mexico with 11 crew members and 334 passengers on board. Approximately 90 minutes into the flight, a cabin crew member became ill and was unable to continue duties. The affected cabin crew member’s duties were reassigned to the remainder of the cabin crew for the rest of the flight.

Incident at 938NM NW Bahias de Huatulco Intl (MMBT)

TSB Report #A19F0033: C-FUJR, a Boeing 737-800 aircraft operated by WestJet, was conducting flight WJA2050 from Vancouver Intl (CYVR), BC to Huatulco/Bahías de Huatulco Intl (MMBT), México with 6 crew members and 158 passengers on board. During cruise flight approximately 2 hours before the arrival at MMBT, a flight crew member became ill and was incapacitated. The other flight crew member contacted Medlink who suggested that the sick flight crew member take a specific medication. Just before the top of descent, the flight crew member was able to resume normal duties for the rest of the flight.

TSB Report #A19F0002: C-FIYA, a Boeing 767-300 aircraft operated by Air Canada rouge, was conducting flight ROU1980 from Toronto/Lester B. Pearson Intl (CYYZ), ON to México City/Benito Juárez Intl (MMMX), México. Prior to the landing at MMMX, the flight crew was instructed to enter a hold in the vicinity of enroute waypoint PIVOL, approximately 90 nm north east of MMMX. During the hold, a flight crew member became ill and was unable to continue flight duties. The remaining flight crew member continued to operate the aircraft, and landed at destination without further incident.

TSB Report #A19F0014: C-GLAT, an Airbus 310-300 aircraft operated by Air Transat, was conducting flight TSC801 from Samaná/El Catey Intl (MDCY), Dominican Republic, to Toronto/Lester B. Pearson Intl (CYYZ), ON. During cruise flight, a cabin crew member became ill and was not able to perform assigned duties. These duties were redistributed to the rest of the cabin crew for the remainder of the flight.
