C-GMTQ landed without incident at 1917Z. No impact on ATC operations.

On final for Runway 24, a Nolinor Aviation Boeing 737-33A (C-GNLQ/NRL943) from Val-d’Or, QC (CYVO) to Montreal/Mirabel, QC (CYMX) reported a bright light mid-runway on the south side that it thought was possibly a vehicle. After verification, a vehicle (MA6) confirmed that it was a runway edge light that was stuck and defective but was at full intensity. The airport duty manager (ADM) was notified. Impact: All subsequent arrivals had to be advised.

Incident at BORDEN ON (Heli) (CYBN)

A Government of Canada, Department of National Defence Bell 412 (GRFN405) from Gore Bay-Manitoulin, ON (CYZE) to Borden, ON (CYBN) went overdue. There was no pilot cell number on the flight plan. The Borden duty officer was called, who investigated further. The Joint Rescue Coordination Centre (JRCC) Trenton was also advised at the time of the information request (INREQ) and assisted in finding the status of the aircraft. It was determined the aircraft had a similar flight yesterday, however, no flight had taken place today, and so the flight plan had been filed in error.

Incident at PETERBOROUGH ON (CYPQ) (FOD (foreign object debris))

Aviation Incident Report #17508: A Seneca College of Applied Arts and Technology Cessna 172S (C-GSEY) on a flight from Peterborough, ON (CYPQ) rejected its take-off on Runway 13 due to a bird strike, and reported returning to the apron. C-GSEY called CYPQ universal communications (UNICOM) advising they were on the apron and confirming the bird strike. The Duty Manager and Safety Advisor attended Runway 13 in a vehicle (Mobile 102), and recovered the remains of a robin. Runway 13 remained open, although a mobile vehicle entered and remained on the runway for approximately 4 minutes for inspection and recovery of remains. A Safety Management Systems (SMS) report was filed in the CYPQ SMS.

Incident at YELLOWKNIFE NT (CYZF)

At 2055Z, a WestJet Encore de Havilland DHC-8-402 (WEN3358) from Yellowknife, NT (CYZF) to Calgary, AB (CYYC) was taxiing to H for departure Runway 16. The aircraft requested to return to the gate due to an intoxicated passenger not complying, and for police to meet them at the gate. The airport duty manager was on frequency in another vehicle and made aware and said they would make the necessary calls. The aircraft returned to the gate and were met by police.

The Airport Duty Manager (ADM) found a swallow carcass between the runway threshold and B6. A Nolinor Aviation Boeing 737-200 (NRL950) was delayed for takeoff and a Cargojet Boeing 757-200 (CJT951) slowed down on the approach. No other operational impact.

A Top Aces Inc. McDonnell Douglas TA-4J (C-FGWT/FD99) from Montreal/Mirabel, QC (CYMX) to Montreal/Mirabel, QC (CYMX) reported mechanical difficulties and requested the presence of emergency vehicles. The aircraft did not declare an emergency, and landed safely on Runway 29 at 1837Z. Although the runway was closed, the landing was cleared by the airport duty manager (ADM) and the runway was inspected beforehand. The aircraft had one person on board (SOB) and 3,000 pounds of fuel. The National Operations Centre (NOC) and the Joint Rescue Coordination Centre (JRCC) Trenton were notified. No impact on operations.

Delayed entry as a result of NAV CANADA safety and evaluation quality assurance (QA) review : After landing on Runway 24, an Académie Aéronautique Cessna 152 (C-GPFX) from Montreal/Mirabel, QC (CYMX) to Montreal/Mirabel, QC (CYMX) reported having a flat tire and was unable to clear the runway past the lights on B4. Three aircraft were in the circuit (CCT) and conducted low approaches while the airport duty manager (ADM) cleared the aircraft off the runway and inspected the runway. The runway was available at 0040Z. B4 remained closed while the tire was changed. No other impact on operations.

Aviation Incident Report #17451: A passenger on a WestJet aircraft (WJA711) from Toronto/Lester B. Pearson, ON (CYYZ) to Vancouver, BC (CYVR) was running up and down the aisle, kicking the back of another passengers seat, and running into a passengers elbow, resulting in two passengers almost having a fist fight. Flight attendants requested police meet the aircraft. CYVR Duty Manager spoke with all parties involved. Flight attendants were not pressing charges.

At approximately 1920Z, an Anderson Air Ltd. Dassault Falcon 900EX (C-GBDY) on a flight from Vancouver, BC (CYVR) declared "PAN PAN" and a loss of airspeed indicators while in the climb through 8200ft. Flight returned to CYVR with an uneventful landing.
UPDATE: TSB Report #A21P0119: C-GBDY, an Anderson Air Ltd. Dassault Falcon 900 EX, was conducting an IFR flight from Vancouver Intl, BC (CYVR) to Las Vegas/McCarran Intl, NV (KLAS) with 3 crew members and 4 passengers on board. While in the climb, through approximately 8200 feet above sea level, the crew observed erroneous air speed indications on the number 1 side with associated "MISCOMPARE" and "PITOT HEAT L FAIL" crew alerting system (CAS) messages. With the left side pitot heat failure message, the crew reverted to the right system, as per the checklist. The right seat pilot assumed flying duties, the failure was reported to air traffic control (ATC) with a PAN PAN urgency call. The aircraft returned to CYVR without incident. On inspection, company maintenance confirmed the number 1 pitot tube was not heating up. The system was reset and the pitot heat system was ground tested. No fault was found and the aeroplane was returned to service.

A WestJet Boeing 737-8CT (C-FUSM/WJA656) from Calgary, AB (CYYC) to Toronto/Lester B. Pearson, ON (CYYZ) reported that the first officer was in medical distress and advised diverting to Winnipeg/James Armstrong Richardson, MB (CYWG). They requested that an ambulance meet the flight at the gate.
UPDATE: TSB Report #A21C0120: C-FUSM, a Boeing B737-800 operated by WestJet, was conducting flight WJA656 from Calgary Intl. (CYYC), AB to Toronto/Lester B. Pearson Intl (CYYZ), ON. While in cruise approximately 40 NM northeast of Kenora (CYQK), ON, the first officer became ill. A deadheading WestJet captain, who was on board the aircraft as a passenger, assumed the duties of the incapacitated first officer. After speaking with the operators ground based medical support, the flight diverted to Winnipeg/James Armstrong Richardson Intl (CYWG), MB where an uneventful landing was conducted. The first officer was transferred to a local hospital for assessment.

Pearson Intl, ON (CYYZ) to Dublin Intl, Ireland (EIDW) when a flight attendant became incapacitated and was unable to continue with their duties. The flight crew requested a return to the gate and for EMS to meet the aircraft.

Accident at 35NM N KENORA ON (CYQK)

A Vortex Helicopters Inc. Bell 212 (C-FVHS) on a flight from Kenora, ON (CYQK) crashed 55 miles north of the CYQK airport.
UPDATE: TSB Report #A21C0075: C-FVHS, a Bell 212 helicopter operated by Vortex Helicopters Inc., departed Kenora, ON (CYQK) to conduct forest fire fighting duties approximately 35NM north of Kenora, equipped with a 150 foot long line and water bucket attached. The helicopter completed a cycle of bucketing and was returning to a staging area for fuel. During the return flight, the pilot noticed the bucket was twisted. The pilot decided to stabilize the bucket by adding water from a nearby lake. During the process, the pilot lost sight of the bucket and the helicopter began to drift to the right. The pilot then felt a tug on the helicopter and, seconds later, the helicopter impacted the water. The pilot, who was seated in the left seat, exited the helicopter, swam to shore and was picked up by a passing helicopter and transported to the staging area. The pilot was then transported to the local hospital with minor injuries. The helicopter was substantially damaged. The visibility in the area was reported as being reduced to 2 to 3 miles in smoke, and the winds were reported as calm.

The Airport Duty Manager (ADM) came out and chased away a skunk. A Durham Flight Centre Cessna 152 (C-FFYA) from Toronto/Oshawa, ON (CYOO) to Toronto/Oshawa, ON (CYOO), waiting to depart, was delayed by a few minutes. No other impact to operations.

TSB Report #A21F0252: C-FYMQ, a Sunwing Airlines Inc. B737-800 conducting flight SWG451 from Punta Cana International, Dominica Republic (MDPC) to Toronto Pearson International, ON (CYYZ). Shortly after take-off, a flight attendant became incapacitated for the flight, however, was able to resume all duties prior to landing. The flight landed in CYYZ without further incident.

TSB Report #A21F0250: C-GFCP, an Air Canada Rouge Airbus A320-214 was conducting flight RV1621 from Fort Lauderdale/Hollywood Intl, FL (KFLL) to Toronto/Lester B. Pearson Intl, ON (CYYZ). During cruise, approximately 70NM south of Pittsburg Intl, PA (KPIT), the Captain became incapacitated. The First Officer assumed Pilot Flying (PF) duties and the Captain was able to continue duties as Pilot Monitoring (PM). No emergency was declared. The aircraft landed at its destination without further incident.

UPDATE: TSB Report #A21W0106: C-GXAX, a WestJet Airlines Ltd. Boeing 737 MAX 8, was operating as flight WS1590 from Calgary International, AB (CYYC) to Atlanta/Hartsfield-Jackson, GA (KATL). After departure, at the top of the climb, the captain became medically incapacitated. The first officer initiated a return to CYYC and a dead-heading captain took over the operating captains duties. The flight landed in CYYC without further incident.

TSB Report #A21W0036: C-GNGV, an Air Canada Airbus A220-300, was operating as flight ACA1186 from Kelowna, BC (CYLW) to Calgary, AB (CYYC) with 5 crew and 29 passengers on board. While descending through FL190 into the Calgary area, the aircraft encountered moderate turbulence for 5 to 10 seconds. Three flight attendants, who were moving about the cabin, sustained minor injuries but were able to continue with their duties. All passengers were seated with their seatbelts fastened; there were no reported injuries amongst the passengers. The flight arrived without further incident and EMS met the aircraft at the gate. Maintenance follow up: The aircraft underwent a Phase 1 inspection; no faults found and the aircraft was returned to service.

Incident at LA RONGE (BARBER FIELD) SK (CYVC) (Other operational incident, ATM - other)

At 2105Z, a Saskatchewan Public Safety Agency Canadair CL-215-6B11 (C-FAFQ/TKR219) from La Ronge (Barber Field), SK (CYVC) to La Ronge (Barber Field), SK (CYVC) reported a fire 2NM southeast (SE) of the airport. CYVC airspace was restricted. At 2215Z, the provincial duty officer advised that the fire was extinguished. The airspace returned to normal conditions.
