The aircraft came to a stop and advised the tower that it had inadvertently crossed the hold line. No other traffic was involved and there was no impact on operations.

Incident at 244NM NE TORONTO / LESTER B. PEARSON INTL ON (CYYZ)

TSB Report #A21O0003: C-GJZS, a Bombardier CL-600-2D24 (Series 900) operated by Jazz, was conducting flight JZA8918 from Toronto/Lester B. Pearson, ON (CYYZ) to Moncton/Romeo LeBlanc, NB (CYQM) with 4 crew members and 16 passengers onboard. Shortly after takeoff, the flight crew noticed a small engine number 1 fan vibration of 1.4 Mils, which is still within acceptable limits. As the aircraft continued climbing, the vibration slowly increased until the aircraft levelled off at FL390, where the vibration reached the yellow arc, with an indication from 3.7 to a maximum of 4.0 Mils. The crew actioned the QRH and conducted the applicable trouble shooting. The captain requested a lower altitude from ATC to reduce power and attempt to stabilize the vibration. As the indication was still around 3.3 Mils, the crew declared a "PAN PAN", turned back towards CYYZ and shut down the number 1 engine, as directed by the QRH. The aircraft landed safely in CYYZ, where emergency vehicles were standing by. Engine number 2 fan vibration remained steady, around .04 and 0.5 Mils. Company maintenance conducted an inspection, with no defects found. The number one fan blade retaining pins were replaced, and the engine tested. Results were within acceptable limits.

C-GFOL, a Beech 200 operated by Keewatin Air as flight KEW210, was conducting a flight from Thompson, MB (CYTH) to CYWG and was cleared for the right-hand visual approach for Runway 31. C-FSNV was instructed to make a left turn of 270 degrees and then join the left base for Runway 31. C-FSNV was observed making a right hand turn. KEW210 was on the approach for Runway 31 and was instructed to turn right to 090. Both aircraft had confirmed that they had each other in sight. KEW210 was instructed to maintain 2000 ASL and turn right for the downwind for Runway 31. C-GWCY, a Boeing 737-6CT operated by WestJet as WJA475, was conducting a flight from Toronto/Lester B. Pearson, ON (CYYZ) to CYWG. WJA475 was cleared for the approach to Runway 31 and a loss of separation occurred between KEW210 and WJA475 when WJA475 was on the approach. Frequency 122.0 MHz of Sydney/Douglas McCurdy, NS (CYQY) failed while a Jazz Bombardier CL-600-2D24 (JZA8210) from Toronto/Lester B. Pearson, ON (CYYZ) to Sydney/Douglas McCurdy, NS (CYQY) was on the approach. Unable to contact the aircraft once it was past the final approach fix (FAF). The frequency resumed once the aircraft was on the ground. The technical operations coordinator (TOC) was advised, and the TOC advised the Charlottetown (YYG) flight service station (FSS) that the CYQY 122.0 MHz modem was showing an error condition. CYQY 122.0 MHz was NOTAMed as unserviceable (U/S).

UPDATE: TSB Report #A21A0001: C-FUJZ, a Bombardier CL-600-2D24 (CRJ900) aircraft operated by Jazz Aviation LP, was conducting flight JZA8219 from Sydney/J.A. Douglas McCurdy, NS (CYQY) to Toronto/Lester B. Pearson, ON (CYYZ). On climb out of CYQY, the flight crew received an "AFT LAV SMOKE" warning message. The cabin crew reported a visible haze and strong odour in the vicinity of the aft lavatory, but no smoke. The flight crew carried out the required checklist items, including pulling circuit breakers to remove electrical power from the aft lavatory fan motor. The haze and odour was reported as dissipating, and the flight crew requested a diversion to Moncton/Romeo LeBlanc, NB (CYQM) and declared an emergency as a precaution, to ensure that emergency services were standing by. The aircraft landed at CYQM without further incident. Maintenance personnel subsequently confirmed and replaced a faulty aft lavatory fan motor.

Incident at 13NM MONTRÉAL / PIERRE ELLIOTT TRUDEAU INTL QC (CYUL)

An Airbus A220-300 operated by Air Canada (C-GJYA/ACA404) from Toronto/Lester B. Pearson Intl, ON (CYYZ) from Montreal/Pierre Elliott Trudeau Intl, QC (CYUL) initiated an overshoot due to wind shear. The Montreal (YUL) tower instructed the aircraft to climb the runway heading to 4000 feet while coordinating with the terminal control unit (TCU). The aircraft was observed in a left turn at 1444Z and continued its climb to 8700 feet without coordination. No impact to operations.

Pearson Intl, ON (CYYZ) to Quebec/Jean Lesage Intl (CYYZ), ON, was immobilized on Runway 24 due to an emergency door open alarm whilst a Gouvernement du Quebec Canadair CL-600-2B16 (C-GQBQ/QUE20) from Baie-Comeau, QC (CYBC) to Quebec/Jean Lesage Intl, QC (CYQB) was at 3NM final. QUE20 conducted an overshoot.

A Government of Quebec Canadair CL600-2B16 (Series 601-3A) (C-GQBQ/QUE20) from Baie-Comeau, QC (CYBC) to Quebec/Jean Lesage, QC (CYQB) received instructions to conduct a missed approach for Runway 24 due to the presence of a Jazz Bombardier CL-600-2D24 (Series 900) (C-FBJZ/JZA8210) from Toronto/Lester B. Pearson, ON (CYYZ) to Quebec/Jean Lesage, QC (CYQB) which was immobilized on the runway due to a technical problem. The aircraft landed at 0419Z. No other impact on operations.
